Satisfaction of Patients Treated in physical medicine and rehabilitation Department in CNHU-HKM of Cotonou

Satisfaction of patients is an important dimension of the hospital management and the evaluation of the quality of care and services offered to patients. Objective: To estimate the degree of satisfaction of patients admitted in physical medicine and rehabilitation Department in the National Hospital and University Center (CNHU-HKM) in Cotonou. Method: Cross-sectional study with descriptive and analytical aim. It was led from June 15th to October 15th 2013, on 158 patients or guides of children or not communicating patients having ambulatory care in the Department of physical medicine and rehabilitation and who agree to participate to the study. The level of global satisfaction of the subjects was estimated with a digital scale from 0 to 10. It was secondarily transformed into a discreet qualitative variable. Result: Rate of participation was 63.2%. Patients had deficiencies of rheumatilogical origine (41.1%), neurological (27.2%), traumatological (24.1%). Their pathologies were evolving since 1 month to 10 years. It was about their first attendance of the rehabilitation Department of CNHU-HKM in 72.2%. Patients’ degree of satisfaction was good for the deadliness of expectation, reception (80%), cleanliness of the building, respect of patients’ intimacy. Global satisfaction of patients was good in 72%. It was influenced by the deadline of evolution of the pathologies. Discussion-Conclusion: The level of satisfaction of patients admitted in the rehabilitation Department in CNHU-HKM was acceptable. However, they are parameters that remain to be improved.

Knowledge towards physical medicine and rehabilitation among Training Doctors and Medical Students at the Mohammed VI University Hospital of Marrakech

The aim of our study is to evaluate the knowledge regarding physical medicine and rehabilitation among physicians in training and medical students at the Mohammed VI University Hospital in Marrakech, to approach the knowledge, attitudes, and practices of doctors regarding physical medicine and rehabilitation and evaluate the knowledge in theoretical training related to PRM among the students. We conducted a monocentric cross-sectional analytical study, using a web-based anonymous survey, carried out among 558 undergraduate student and training doctors, randomly selected in the form of a survey on the knowledge towards physical medicine and rehabilitation. We received 558 survey duly completed by students of the Faculty of medicine and Pharmacy of Marrakech (62.4%) and training doctors (37.6%). The mean age of the participants was 24.53 ± 3.9 years, with extremes ranging from 17 to 39 years. 52.7% of the participants were from the former educational reform, The predominance of participation was remarkable among pediatricians 23%, The population who knew PRM was the majority (79.3%), 40.7% of the participants were unaware of the availability of a PRM department at Mohamed VI University Hospital, 0.5% of all training doctors and medical students questioned strongly agreed with the sufficiency of their training in disability management were belonging to the new reform, 84.1% of participants had never attended or referred a patient to the PRM department. 23.2% of training doctors affirmed the referral of patients to PRM for further management. Despite the essential role of PRM in the management of diseases, it remains little known by training doctors and medical students. This lack of knowledge of PRM reflects the lack of the undergraduate and postgraduate of the medical education in the field of rehabilitation.

Epidemiological data on acupuncture and physical and rehabilitation medicine in the European Union

OBJETIVE: To know the prevalence of acupuncturists within physical and rehabilitation medicine specialists in Europe and their characteristics,as well as to analyze the pathologies treated and therapeutic techniques ***: This is a cross-sectional descriptive study based on a structured and closed questionnaire. The study was carried out from October 2011 to May ***: A total of 115 specialists from 19 countries completed the questionnaire. The mean characteristics of the physical and rehabilitation medicine(PRM) specialists who are also acupuncturists in Europe were: women(62.6 %); mean age of47.91 years old; 4.20 years of physical and rehabilitation medicine school and 2.00 years of acupuncture training; 15.03 years of mean time of clinical practice as physical and rehabilitation medicine specialists and 10.42 years of experience as acupuncturists; 35.65 % had a doctorate(Ph D) degree,obtained at a mean age of 33.55 years old; working in a public hospital(80%); treating mainly neurological conditions(48.1%) and using mainly techniques such as kinesiotherapy(11.81%),orthosis(10.40%) and electrotherapy(9.92%). Acupuncture was applied by 32.2% of the participants mainly in musculoskeletal conditions(27.38%). A mean number of more than 20 patients a day were treated by47.04% of the specialists,being 41.8% of the patients(adults and children). Significant differences were found between the variables,acupuncture and gender(Pearson's correlation Index = 0.007).However,no differences were observed in relation to acupuncture and age,doctorate degree,years of clinical practice and workplace(all with P values >0.05).CONCLUSION: In the European area,physical and rehabilitation medicine specialists who are also acupuncturists are mainly female,middle-aged,with15 years of professional experience,Ph D holders,working in public hospitals,treating mainly neurological conditions with kinesiotherapy and acupuncture,and treating over 20 patients a day(adults and child

Evidence-based Practice of Chinese medicine in physical rehabilitation Science

Chinese medicine is among other traditional medical systems practiced either as a coadjutant intervention to Greek medicine or as the unique therapeutic intervention for illness prevention, treatment or rehabilitation. The complete spectrum from that traditional system includes acupuncture and moxibustion, herbal and food therapy, massage therapy （tuina）, physical exercises （taijiquan）, and breathing exercises （qigong）. In this article, it is presented several randomized controlled trials and systematic reviews on the application of all therapeutic modalities from Chinese medicine in the physical rehabilitation scenario. The discussed studies encompasses both ＂positive＂ and ＂negative＂ results of Chinese medicine intervention for disabilities due to illnesses of the nervous, musculoskeletal or cardiovascular systems. Additionally, the importance of the personalized approach for Chinese medicineand rehabilitation is emphasized together with the need for reproducible methods for pattern differentiation and intervention selection. Chinese medicine resources are recognized as promising methods for therapeutic rehabilitation and can be incorporated into the rehabilitation science. The wide variety of therapeutic resources explains why Chinese medicine is currently a multidisciplinary practice for health protection and promotion, early diagnosis and treatment as well as rehabilitation with roles in the public health care system.

Status of the Specialty of physical and rehabilitation medicine

The objectives of this paper are to help specialists in physical and rehabilitation medicine plan their careers, influence the future of the PRM specialty and advocate effectively for those with limitations of functioning. The experiences of the authors are the sources of its observations and suggestions. The senior author has had extensive national and international experience in the specialty as a clinician, teacher, researcher, manager, leader and advocate. The junior author adds the insights of one who only recently has become immersed in PRM. The following are suggestions and observations the authors believe could contribute to the future success of individual PRM specialists and the PRM specialty. ①Review and reinforce the significant accomplishments of PRM over the last 70 years. It has significantly expanded the focus of health care to include the importance of functioning. ②Identify the specialty′s current strengths, weaknesses, threats and opportunities. As a clinical specialty, it has expanded nationally and internationally, yet its research contributions remain limited. The absence of an evidence base for much of its activity risks reduced support from payers, yet the population needing its services continues to expand. ③Develop strategies to increase the ability of PRM specialists and PRM to improve the lives of people with or likely to have limitations of functioning. These strategies could include sharing best practices, supporting effectiveness research and developing expert consensus guidelines. The need for physicians who are experts in the strategies of improving the lives of those with complex problems of functioning will continue to expand. This need will assure the continuation of the specialty of PRM as long as it improves the abilities of its members to help those with problems of functioning, and it advocates for those whom its members serve.

Distinct brain remodeling has been found after different nerve reconstruction strategies,including motor representation of the affected ***,differences among reconstruction strategies at the brain network level have not been *** study aimed to explore intranetwork changes related to altered peripheral neural pathways after different nerve reconstruction surgeries,including nerve repair,endto-end nerve transfer,and end-to-side nerve ***–Dawley rats underwent complete left brachial plexus transection and were divided into four equal groups of eight:no nerve repair,grafted nerve repair,phrenic nerve end-to-end transfer,and end-to-side transfer with a graft sutured to the anterior upper ***-state brain functional magnetic resonance imaging was obtained 7 months after *** independent component analysis algorithm was utilized to identify group-level network components of interest and extract resting-state functional connectivity values of each voxel within the *** in intra-network resting-state functional connectivity were compared among the *** muscle reinnervation was assessed by behavioral observation(elbow flexion)and *** results showed that alterations in the sensorimotor and interoception networks were mostly related to changes in the peripheral neural *** repair was related to enhanced connectivity within the sensorimotor network,while end-to-side nerve transfer might be more beneficial for restoring control over the affected limb by the original motor *** thalamic-cortical pathway was enhanced within the interoception network after nerve repair and end-to-end nerve *** areas related to cognition and emotion were enhanced after end-to-side nerve *** study revealed important brain networks related to different nerve *** networks may be potential targets for enhancing motor recovery.
