Two subtelomeric chromosomal deletions in forty-six children with idiopathic mental retardation

Mental retardation (MR) affects approximately 3% of the population. In infancy and early childhood, mental retardation often presents as developmental delay. Unexplained MR is responsible for more than half of all cases. There is some evidence that subtle chromosomal rearrangements involved in subtelomeric regions ( gene-rich regions) , resulting in segmental aneuploidy and gene-dosage imbalance, are significant unrecognized causes of idiopathic mental retardation (IMR). Human chromosome terminals are gene rich, with specialized telomeric structures, including the simple tandem repeat ( TTAGGG )_n, and additional complex subtelomeric repeats. Rearrangements involving telomere-adjacent DNA are more likely to have phenotypic consequences than rearrangements in many other parts of the genome. A number of candidate genes implicated in recognizable syndromes are known to be present in the telomeric region. We analyzed 46 patients with IMR or developmental delay by fluorescence in situ hybridization ( FIsH ) using subtelomeric probes to explore whether cryptic subtelomeric rearrangements have a role in the pathogenesis of unexplained MR and to determine the value of FIsH in the detection of microdeletions associated with MR.

Introduction To improve compliance with voiding diaries in children with primary monosymptomatic nocturnal enuresis(PMNE),a new modified 3-day weekend frequency-volume chart(FVC)was designed,and the compliance and validity of this modified FVC was evaluated by comparing with the International children's Continence society(ICCs)recommended voiding *** A total of 1200 patients with PMNE were enrolled in the study from 13 centers in China and were randomly assigned to record this modified FVC or the ICCs-recommended voiding *** primary outcome measure was the compliance,assessed by comparing the completing index and the quality score of diaries between two *** secondary outcome measure was the validity,evaluated by comparing the constituent of subtypes,micturition parameters and response rate to *** Among the 1200 participants enrolled in the study,447 patients completed the ICCs-recommended voiding diary and 469 completed the modified *** diurnal completing index and the quality score of the modified FVC group were better than those of the ICCs *** addition,there was no significant difference between these two groups in the subtype classification,or in the response rate to *** The modified FVC could be applied to obtain the voiding characteristics of children with PMNE as the ICCs-recommended voiding diary does and offers a reasonable and better choice for children with PMNE from the unselected population in the future.

To understand the epidemiological and clinical features of the symptomatic and asymptomatic pediatric cases of COVID-19,we carried out a prospective study in shanghai during the period of January 19 to April 30,2020.A total of 49 children(mean age 11.5±5.12 years)confirmed with sARs-CoV-2 infection were enrolled in the study,including 11(22.4%)domestic cases and 38(77.6%)imported ***(81.8%)local cases and 12(31.6%)imported cases had a definitive epidemiological ***-eight(57.1%)were symptomatic and 21(42.9%)were *** asymptomatic nor symptomatic cases progressed to severe *** mean duration of viral shedding for sARs-CoV-2 in upper respiratory tract was 14.1±6.4 days in asymptomatic cases and 14.8±8.4 days in symptomatic cases(P>0.05).Forty-five(91.8%)cases had viral RNA detected in *** mean duration of viral shedding in stool was 28.1±13.3 days in asymptomatic cases and 30.8±18.6 days in symptomatic participants(P>0.05).childrens shed viral RNA in stool for a longer duration than school-aged children(Pses had seropositivity for antibodies against sARs-CoV-2 within 1–3 weeks after confirmation with *** conclusion,asymptomatic sARs-CoV-2 infection may be common in children in the community during the COVID-19 pandemic *** cases shed viral RNA in a similar pattern as symptomatic cases *** is of particular concern that asymptomatic individuals are potentially seed transmission of sARs-CoV-2 and pose a challenge to disease control.

Interventricular septal Hematoma after Congenital Cardiac Defects Repair at a single Institution

Background: Interventricular septal hematoma is a rare complication after congenital cardiac repair. The managementvaries according to the literature. We present our experience with this rare complication. Methods:Echocardiography database were reviewed with the term ‘‘hematoma’’ or “hypoechoic mass” for patients whounderwent congenital heart surgery from January 2018 to December 2021 at our institution to identify potentialinterventricular septal hematoma cases. Relevant data of the patients identified were collected. Focus was put onthe presentation, management, outcomes according to patent medical charts and serial echocardiographic reportdata. Results: In total, there were 5 patients included. The mean age and weight at surgery were 5.5 ± 3.6 monthsand 5.5 ± 1.4 kg, respectively. Four patients were diagnosed with ventricular septal defect and the other one beingdouble outlet of the right ventricle. While all patients had intraoperative transesophageal echocardiography, 80%(4 of 5) of Interventricular septal hematoma were revealed intraoperatively. Only one patient received hematomadrainage intraoperatively while the other 3 identified in the operating room were only closely observed. One afterventricular septal defect repair presented continuous dysfunction of the left ventricle at the last follow-up, whilethe others were doing well. All hematomas resolved completely with a mean time to interventricular septal hematomaresolution of 35.8 ± 16.9 days. Conclusion: Infants seem to be at a higher risk for Interventricular septalhematoma following congenital heart surgery. While the majority of interventricular septal hematoma has abenign postoperative course, some may result in ventricular dysfunction. Management strategies may be chosenon a case-by-case basis.

Beyond the Pediatric end-stage liver disease system: solutions for infants with biliary atresia requiring liver transplant

Biliary atresia(BA), a chronic progressive cholestatic disease of infants, is the leading cause for liver transplant in children, especially in patients under two years of age. BA can be successfully treated with the Kasai portoenterostomy; however most patients still require a liver transplant, with up to one half of BA children needing a transplant by age two. In the current pediatric end-stage liver disease system, children with BA face the risk of not receiving a liver in a safe and timely manner. In this review, we discuss a number of possible solutions to help these children. We focus on two general approaches:(1) preventing/delaying need for transplantation, by optimizing the success of the Kasai operation; and(2) expediting transplantation when needed, by performing techniques other than the standard deceased-donor, whole, ABO-matched organ transplant.

Genome-wide analysis of differential DNA methylation in silver-Russell syndrome

silver-Russell syndrome(sRs) is clinically heterogeneous disorder characterized by low birth weight, postnatal growth restriction, and variable dysmorphic features. Current evidence strongly implicates imprinted genes as an important etiology of sRs. Although almost half of the patients showed DNA hypomethylation at the H19/IGF2 imprinted domain, and approximately7%–10% of sRs patients have maternal uniparental disomy of chromosome 7(UPD(7) mat); the rest of the sRs patients shows unknown etiology. In this study, we investigate whether there are further DNA methylation defects in sRs patients. We measured DNA methylation in seven sRs patients and five controls at more than 485,000 CpG sites using DNA methylation microarrays. We analyzed methylation changes genome-wide and identified the differentially methylated regions(DMRs) using bisulfite sequencing and digital PCR. Our analysis identifies epimutations at the previously characterized domains of H19/IGF2,providing proof of principle that our methodology can detect the changes in DNA methylation at imprinted loci. In addition,our results showed a novel sRs associated imprinted gene OsBPL5 located on chromosome 11p14 with the probe cg25963939,which is hypomethylated in 4/7 patients(P=0.023, β=.0.243). We also report DMRs in other genes including TGFβ3, HsF1,GAP43, NOTCH4 and MYH14. These DMRs were found to be associated with sRs using GO pathway analysis. In this study,we identified the probe cg25963939, located at the 5′UTR of imprinted gene OsBPL5, as a novel DMR that is associated with sRs. This finding provides new insights into the mechanism of sRs etiology and aid the further stratification of sRs patients by molecular phenotypes.

Objective： To provide an objective reference for the syndrome types of Chinese medicine（CM） associated with pediatric primary nephrotic syndrome（PNs）.Methods： A cross-sectional study was *** on clinical symptoms,CM syndrome types,biochemical indices,and medications used were collected from 98 children with ***,the correlation between CM syndromes and biochemical indices,as well as medications used,was ***： The four most common symptoms in children with PNs were brown urine,red tongue,excessive sweating,and swelling of the face and *** syndromes of qi deficiency of Fei（Lung） and shen（Kidney）（FsQD） and yin deficiency of Gan（Liver） and shen（GsYD） were the most common main CM syndrome *** syndrome score correlated significantly with the total cholesterol level,urine protein/creatinine ratio,and urine Ig G and albumin levels（P〈0.01 or P〈0.05）.The use of maintenance glucocorticoids combined with immunosuppressive agents correlated with FsQD syndrome,and the use of maintenance glucocorticoids alone correlated with GsYD syndrome（P〈0.05）.Conclusion： Two of the most common CM syndrome types were FsQD and GsYD *** syndrome may be caused by some factors related to lipid levels,protein loss,and the use of immunosuppressive *** use of maintenance glucocorticoids may cause GsYD syndrome.
