Fever is the primary reason for consultation and admission to pediatric emergency departments. The aim of this study was to describe the clinical, etiological and prognostic aspects of acute fever in children aged 0 - 15 years. Patients and Methods: This was a prospective cross-sectional study from April 1, 2021 to March 31, 2022 and affected all children aged 0 to 15 years old with a temperature of 39˚C or higher (≥), having less than five days and hospitalized in the pediatric department of the Gabriel Touré University hospital. Results: During the study period, 150 children were included, the sex-ratio was 0.85. Children aged 0 - 5 years represented 71.4% of the sample. References represented more than half of the sample, or 58% of cases. Body temperature was above 40˚C in 18.7% of cases. Functional signs were dominated by respiratory signs in 28% and digestive signs in 18%. Clinical pallor was found in 55.3% of children associated with signs of respiratory struggle in 70% of cases and tachycardia in 40% of patients. Anemia was present in 65.3% of patients. Hyperleucocytosis and leukopenia were found in 45.3% and 43.3% of patients respectively. The C-reactive protein was positive in 44.7% of patients and Cytobacteriological Examination and Chemistry of Cerebrospinal Fluid was positive in 8.7% of cases. The thick drop was positive in 44.7% of patients and blood cultures grew in 6 patients. Etiology was dominated by severe malaria 54 cases (36%), pneumonia (19.3%), meningitis (12.6%), and in those under one month it was bacterial neonatal infection (8.6%). In our study, slightly more than one patient out of three died during hospitalization, 36% (54 cases/150) and among these 70% (38 cases/54) were under 5 years old. The most lethal pathologies were severe malaria (36%), bacterial pneumonia (19.3%), meningitis (12.6%), bacterial neonatal infections (8.6%) and measles complicated by pneumonia (5.3%). A Case Report of Congenital Dermal Sinus Tract with Dermoid Cyst and Lipoma

Congenital dermal sinus tract (CDST) is a rare entity of spinal dysraphism with an incidence of 1 in 2500 live births. The presumed etiology relates to a focal failure of disjunction resulting in a persistent adhesion between the neural and cutaneous ectoderm. CDST is commonly associated with other pathologies such as myelomeningocele, split cord malformation, tethered cord and inclusion tumors. The authors report a case of a 2-year-old girl discharging lumbosacral CDST. The preoperative MRI showed the dermal sinus tract with 2 intradural lesions from L1 to L3. The tract was surgically excised followed by complete excision of the cyst and lipoma. The dermal sinus tract was attached to the medullary cone and was cut there, allowing “en bloc” removal of the dermal sinus tract. There were two distinct types of tumor. One cystic is located posterior to the medullary cone and the other is located laterally to the medullary cone, whose appearance suggests a lipoma. Both tumors were removed completely. The dura was closed in a tight manner. The pathological examination confirmed the lipoma and Dermoid Cyst. In the postoperative course, the child had a motor deficit in the lower limbs, which recovered completely after 3 months of physiotherapy.

Conjunctival Granuloma Caused by an Unknown Metallic Foreign Body

Introduction: Conjunctival granulomas are tumor pathologies of various origins. A foreign body, whatever its nature, is often involved in its genesis. To date, there is very little data on foreign body conjunctival granulomas, the prevalence would be approximately 10% and their underestimation is probably linked to the inconsistency of their clinical presentation. Patient and Observations: We report a case of conjunctival granuloma caused by a metallic foreign body that went unnoticed in a young boy, the interest of rigor on primary protection measures for high-risk occupations, and of the strict application of the diagnostic protocol relating to ocular trauma. Discussion: foreign body granulomas, despite their usually harmless nature, can jeopardize the anatomical and functional prognosis of the affected eye. Only a detailed interview and a meticulous clinical examination allow the diagnosis to be made and the distinction to be made with conjunctival granulomas of different causes. Conclusion: the possibility of the involvement of a foreign body in the genesis of conjunctival granulomas must impose the rigorous application of diagnostic and therapeutic protocols.

Introduction: The objective of this work was to study maternal deaths noted on arrival in the Gynecology and Obstetrics department at Fousseyni Daou hospital in Kayes over a period of 10 years. Materials and Methods: This was a cross-sectional, descriptive study with data collection over a period of 10 years;The data collection was retrospective over nine years from January 1, 2013 to December 31, 2021 and prospective over one year from January 1, 2022 to December 31, 2022. This study focused on all patients whose death was noted on arrival during pregnancy, labor or in the postpartum period in the Gynecology-Obstetrics department of Fousseyni Daou hospital. Confidentiality and anonymity were respected. The processing and analysis of statistical data were carried out using SPSS 20.0 software. Results: During the study period, we recorded 93 cases of death noted on arrival out of a total of 606 maternal deaths, i.e., a frequency of 15.34%. The average age was 27 years with the extremes of 20 years and 34 years. They came mainly from rural areas at 74%, were married at 82%, uneducated at 51.6%, housewives at 87.1%. The profession of the spouses is worker at 37.6%. In our sample, evacuated patients were the most represented with 75.3%. Postpartum hemorrhage was the most frequent reason for admission with 22.6%. The deceased patients had no medical history at 86%. In our series, 59.5% of the deceased patients had not had antenatal consultations (CPN). Patients who died on arrival and who had given birth at home were the most represented with 54.8%. Deaths from immediate postpartum hemorrhage complicated by shock were the most frequent with 25.8% followed by severe anemia 8.6%. Deaths were mainly due to direct obstetric causes at 76.3%. In these deaths observed on arrival, the 2nd delay was identified at 48.4%. Conclusion: Maternal deaths observed on arrival remain frequent in the Kayes region. BACKGROUND Children with critical acute abdominal conditions often undergo intestinal stoma *** To explore the impact of a visual mobile terminal-based extended care model on caregiver competence for children with *** One hundred twenty children with enterostomies and their caregivers in a children's hospital in Beijing were divided into a control group and a study *** control group(60 cases)received traditional telephone follow-up for continuity of care,while the study group(60 cases)used a visualization mobile terminal-based care *** incidence of stoma-related complications,caregiver burden scale,and competence scores of children with stoma were compared between the two *** The primary caregiver burden score in the study group(37.22±3.17)was significantly lower than that in the control group(80.00±4.47),and the difference was statistically significant(P<0.05).Additionally,the caregiving ability score of the study group(172.08±3.49)was significantly higher than that of the control group(117.55±4.28;P<0.05).The total incidence of complications in the study group(11.7%,7/60)was significantly lower compared to the control group(33.3%,20/60;χ2=8.086,P=0.004).CONCLUSION The visual mobile terminal-based care model reduces caregiver burden,improves home care ability,lowers the incidence of complications and readmission rates,and supports successful second-stage reduction surgery for children with enterostomies.

Objective: To examine the effectiveness of Chinese medicine(CM) Lianhua Qingwen Granule(LHQW) and Jingyin Gubiao Prescription(JYGB) in asymptomatic or mild patients with Omicron infection in the shelter hospital. Methods: This single-center retrospective cohort study was conducted in the largest shelter hospital in Shanghai, China, from April 10, 2022 to May 30, 2022. A total of 56,244 asymptomatic and mild Omicron cases were included and divided into 4 groups, i.e., non-administration group(23,702 cases), LHQW group(11,576 cases), JYGB group(12,112 cases), and dual combination of LHQW and JYGB group(8,854 cases). The length of stay(LOS) in the hospital was used to assess the effectiveness of LHQW and JYGB treatment on Omicron infection. Results: Patients aged 41–60 years, with nadir cycle threshold(CT) value of N gene <25, or those fully vaccinated preferred to receive CM therapy. Before or after propensity score matching(PSM), the multiple linear regression showed that LHQW and JYGB treatment were independent influence factors of LOS(both P<0.001). After PSM, there were significant differences in LOS between the LHQW/JYGB combination and the other groups(P<0.01). The results of factorial design ANOVA proved that the LHQW/JYGB combination therapy synergistically shortened LOS(P=0.032). Conclusions: Patients with a nadir CT value <25were more likely to accept CM. The LHQW/JYGB combination therapy could shorten the LOS of Omicron-infected individuals in an isolated environment.

Autologous cord blood vs individualized supplements in autistic spectrum disorder:CORDUS study results

BACKGROUND Diffuse panbronchiolitis(DPB)is a rare,chronic inflammatory lung disease mar-ked by chronic cough,breathlessness,and preceding *** often persist for years and can be misdiagnosed as asthma,particularly in *** report describes a DPB case resolved with long-term azithromycin therapy,em-phasizing the need for a timely and accurate *** SUMMARY A 12-year-old girl,diagnosed with asthma at age five and managed with inhaled corticosteroids and long-acting beta-2 agonists,developed a history of chronic productive cough and chronic sinusitis for a *** examination,she exhibited wheezing and coarse *** receiving treatment for an asthma exacer-bation,her symptoms did not improve.A chest X-ray revealed reticulonodular infiltration in both lower lungs,prompting further evaluation with high-resolu-tion computed tomography(HRCT).The HRCT confirmed centrilobular nodule opacities,a'tree-in-bud'pattern,and non-tapering bronchi,suggesting *** cold hemagglutinin titers at 128 further supported the *** cough and sinusitis resolved within a month after starting azithromycin therapy,chosen for its anti-inflammatory and immunomodulatory ***-up HRCT scans after 1 year of continuous treatment showed complete *** This case highlights the importance of early diagnosis and prompt treatment in achieving favorable outcomes for DPB.
