Congenital Malaria in the borgou/alibori regional university teaching hospital: Myth or Reality?

Introduction: The occurrence of malaria is classically described as uncommon or exceptional in the neonatal period. This work aims to describe the epidemiological, diagnostic, therapeutic, clinical features and outcomes of congenital malaria in the borgou/alibori regional university teaching hospital (CHUD B/A) located in Parakou. Patients and Methods: This research is a cross-sectional and descriptive study that was conducted in the neonatal intensive care unit of the CHUD B/A from January to December 2017. It included and involved all newborns less than eight (08) days old with positive thick smears. Comorbidities were taken into account to establish the diagnosis of congenital malaria with accuracy. Findings: A total of 170 newborns out of 892 newborns admitted tested positive with Plasmodium falciparum thick smears, i.e., a hospital-based frequency of 19%. The sex ratio was 1.1. The mean age was 22.44 ± 2.72 hours. There was malaria-bacterial infection comorbidity in 39.3% of cases. Among the 91 cases of thick smears testing positive without stigma of bacterial infection, G6PD deficiency and fetal-maternal Rhesus incompatibility, the main reasons for admission were prematurity (51.6%), low birth weight (44%), neonatal distress (31.8%) and respiratory distress (15.3%). In addition, the main signs found during the physical examination were neurological disorders or impairments (43.9%), respiratory distress (30%), hyperthermia (12.1%) and jaundice (4.4%). The mean or median parasitemia was 490 parasites/μl, with extremes ranging from 100 to 6500 parasites/μl. Treatment was based on artemisinin derivatives. The patient condition improved in 88.7% of cases. Overall mortality was estimated at 11.9%, and the specific mortality was 13.2%. Conclusion: Congenital malaria is a reality that actually prevails in the CHUDB/A. Audits of Death in HIV-Infected Children and Adolescents Followed up in the pediatric Department of the regional teaching hospital of borgou/alibori from 2005 to 2020

Introduction: Human immunodeficiency virus (HIV) is a major public health problem with high morbidity and mortality among children. The objective of this work was to audit the deaths of children and adolescents with HIV infection followed up in the pediatric department of the regional teaching hospital of borgou/alibori (CHUDB/A) the from 2005 to 2020. Patients and Method: This was a retrospective and descriptive study conducted in the pediatric department of CHUD/B-A in Parakou. All children with HIV infection who died from January 1, 2005 to August 31, 2020 were included. Data collection was carried out in three stages: a phase of medical records processing, a phase of community survey and a phase of death audits. The variables studied were sociodemographic, clinical, biological, therapeutic and evolutionary. Results: Over the study period, the data of 464 infected children were recorded, including 92 deaths, representing a case fatality rate of 19.83%. Severe acute malnutrition (69.23%), gastro-intestinal tract infections (43.58%) and serious opportunistic pulmonary infections (24.36% pulmonary tuberculosis and 19.23% pneumocystis) were the main causes of death. The main dysfunctions found were: the delayed diagnosis of HIV infection (79.35%), the absence or delay in consultation when the child’s clinical condition deteriorates (32.61% and 47.83%), delayed initiation of antiretroviral treatment (42.39%) and non-adherence to treatment (38.04%). Non-adherence to treatment was predominant in adolescents (90.49%). Conclusion: Specific interventions for early detection, adequate nutritional care, psychosocial support for adolescents and mothers of children are necessary to reduce mortality due to HIV among children and adolescents.

<i>Helicobacter pylori</i>Infection (<i>Hp</i>) among Children in the Northern Benin in 2018

Background: Hp infection is the most common chronic bacterial infection in developing countries and is contracted especially in childhood where it remains silent. Because of its involvement in the genesis of certain cancers, the WHO has classified Hp in the category of carcinogen class I. The aim of this study was to determine the prevalence of Hp infection among children from a District in Northern Benin, and to identify the factors associated with it. Patients and methods: This scientific investigation is a cross-sectional, descriptive, and analytical study based on a prospective collection of data carried out from July to September 2018. Recruitment was probabilistic;it was based on the WHO cluster sampling technique implemented among children aged 3 to 10 years without recent history of antibiotic treatment and proton pump inhibitors. Hp infection was diagnosed during the search for bacteria antigen in the feces through a quick Elisa test which proved positive. Results: We included 250 children in the study. Among them, 151 (60.4%) had Hp infection. 79 (52.3%) out of the 151 infected children were female, i.e. a sex ratio of 0.91. Mean age for those infected children was 6 ± 2.3 years. Among the 151 children infected with Hp, 98 (64.9%) were asymptomatic. Factors associated with that infection were: children’s age > 5 years (p = 0.0461), use of contaminated drinking water (p = 0.0001), meals away from home (p = 0.0039), mothers’ low educational status (p = 0.0137) and low monthly income (p = 0.0116) as well as household size > 3 (p = 0.0002). Conclusion: Hp infection is common among children aged 3 to 10 years in Northern Benin. Often asymptomatic, it is facilitated and exacerbated by unsanitary conditions and low socioeconomic status.

Relationship between Early Breastfeeding, Exclusive Breastfeeding and Continuation of Breastfeeding until 24 Months in Parakou in 2016

Introduction: Early breastfeeding is recommended by the WHO as one of the essential practices for child nutrition. Objective: To identify the relationship between early breastfeeding, exclusive breastfeeding (EBF) and continuation of breastfeeding up to 24 months of age. Patients and Methods: This research is a cross-sectional, descriptive and analytical study conducted in the District of Parakou (Benin) in November 2016. Sampling was probabilistic and used the WHO cluster sampling technique. Children aged 6 to 24 months and their mothers were included. The main variables investigated during the study were related to practices of early breastfeeding, EBF and continuation of breastfeeding until 24 months of age. The data collected through direct interview were entered and processed using the Epi-info 7.2 software. Chi-squared test was used to compare proportions;the differences existing between the proportions were considered as significant if p Findings: Among the 420 children included, the prevalence of EBF was 26.67%. EBF and continuation of breastfeeding up to 24 months were statistically related to early breastfeeding with p values estimated at 0.014 and 0.047, respectively. Conclusion: Improving and enhancing the performance of breastfeeding practice should be possible by promoting early breastfeeding.

Progression of Corneal Astigmatism after Phaco-Alternative at the university hospital Center of the Oueme-Plateau Department in Porto-Novo

Objective: To study the postoperative evolution of corneal astigmatism following phaco-alternative at the university hospital Center of Ouémé-Plateau (UHC-OP) in Porto-Novo. Method: This was a prospective, cross-sectional study with a descriptive and analytical purpose conducted from April 19 to September 20, 2021;a period of 6 months. The study included all patients presenting with senile or non-senile cataracts without a history of corneal trauma and who underwent phaco-alternative during the study period. Results: A total of 62 eyes underwent phaco-alternative. The average age was 63 ± 12 years with a male predominance of 59.7%. Phaco-alternative was performed with 74.2% linear incision and 25.8% smile incision. Perioperative incidents were mainly capsular ruptures with vitreous loss occurring in 8.1% of cases. The mean preoperative astigmatism was 1.29 ± 1.5 D with an axis of 180˚ ± 20˚, indicating with-the-rule astigmatism. The mean postoperative astigmatism was 2.2 ± 1.5 D with an axis of 90˚ ± 20˚, indicating against-the-rule astigmatism. The mean induced astigmatism at Day 30 was 1.4 ± 1.2 D for smile incision and 1.8 ± 1.2 D for linear incision. Conclusion: Phaco-alternative yields good results with few complications but remains astigmatogenic. The smile incision appears to be less astigmatogenic.

Systemic scleroderma is a rare disease in which visceral manifestations occur, particularly peripheral vascular, digestive, cardiopulmonary and renal. It is pathology with a predilection for women. The present clinical case is that of a man with the renal complications of scleroderma and the difficulties of the treatment even in the developed countries like France. In the present case, the management of this disease required a high dose of corticosteroid therapy and extra-renal purification. Early detection of complications through a minimal clinical examination supplemented with paraclinic tests has proved necessary.

Factors Associated with Overweight and Obesity in the Schools of Parakou in 2017

Introduction: The dietary changes and the new lifestyles which occurred in low-income countries like Benin resulted in a nutritional transition with its burden of overweight and obesity among the child and adolescent population who is still suffering from undernutrition. This study aimed to identify the factors associated with overweight and obesity in the schools of Parakou in 2017. Patients and Methods: This research work is a cross-sectional descriptive and analytic study carried out from May to June 2017 in the state-owned and private high/secondary schools of Parakou. It focused on a representative sample of 730 high school students randomly selected according to the WHO two-stage cluster sampling technique. The weight and size of each of the students included in the study were measured;and then their body mass index (BMI) was calculated. The data related to respondents’ dietary pattern and lifestyle were collected. Those data were processed using the software Epi Info version 7.2.1. The prevalence ratio (PR) was used to measure associations with their confidence interval and the difference was considered significant when p < 05. Findings: In total, 730 students were involved in the study, including 377 male i.e. a ratio of 1.1. Prevalence of overweight/obesity was estimated at 16.2% including 12.5% of overweight and 3.7% of obesity. The factors associated with overweight and obesity were female sex, excessive intake of sugar-sweetened beverages and drinks, low level of physical activity and family history of obesity. Conclusion: A behavior change regarding the different contributing factors is necessary to reverse the trends in overweight and obesity prevalence in the high/secondary schools in the District of Parakou.

Variability of anti-human transglutaminase testing in celiac disease across Mediterranean countries

To verify the precision and accuracy of transglutaminase antibodies (TGA) assays across Mediterranean countries. METHODSThis study involved 8 referral centres for celiac disease (CD) in 7 Mediterranean countries. A central laboratory prepared 8 kits of 7 blinded and randomized serum samples, with a titrated amount of Human TGA IgA. Each sample was analysed three times on three different days, with each centre running a total of 21 tests. The results were included in a blindly coded report form, which was sent to the coordinator centre. The coordinator estimated the mean coefficient of Variation (CoVar = σ/μ), the mean accuracy (Accur = Vobserved - Vreal) and the mean percent variation (Var% = [(Vobserved - Vreal)/Vreal] × 100). RESULTSThe analysis showed that 79.17% of the mean variation fell between -25% and +25% of the expected value, with the accuracy and precision progressively increasing with higher titres of TGA. From values 1.25 times greater than the normal cut-off, the measurements were highly reliable. CONCLUSIONTGA estimation is a crucial step for the diagnosis of CD; given its accuracy and precision, clinicians could be confident in establishing a diagnosis.

Hemodialysis patients’ satisfaction and associated factors in National teaching hospital Hemodialysis Center, Cotonou (Benin)

Background: Patients’ satisfaction is a relevant component in assessing healthcare services. The objective of this study was to study the level of satisfaction of chronic hemodialysis patients and the associated factors in Cotonou CNHU-HKM dialysis center. Patients and Methods: It was a cross-sectional, descriptive and analytical study conducted by administering a questionnaire on days off dialysis. It was carried out from 1st November 2015 to 1st January 2016 in CNHU-HKM Nephrology university Clinic of Cotonou. Hemodialysis patients aged 18 years and above having given their informed consent were included in the study. Patients’ recruitment was comprehensive. Likert scale was used in assessing the level of satisfaction with 4 as “very satisfied” and 1 for “very dissatisfied”. Satisfaction was evaluated on the basis of Ware dimensions. The threshold for satisfaction was 50. Outcomes: Overall, 377 patients were included in the study. Respondents’ mean age was 51.5 ± 13.3 years with 1.37 as sex ratio. Arteriovenous fistula was used for 80.1% of hemodialysis patients. 77.7% of the patients underwent dialysis twice a week while 45.9% were administered a four-hour dialysis. The overall average proportion of “satisfied” was 76.5%. The level of satisfaction was 52% for healthcare environment, 61% for service delivery, 73.9% for healthcare accessibility, 76.1% for healthcare structure, 77.5% for healthcare management, 90.3% for quality of healthcare, 88.3% for interpersonal relationship and 93.2% for efficiency and continuum of healthcare. Factors associated with satisfaction included age (p = 0.02), vascular access (p = 0.04) and urea reduction ratio (p = 0.01). In addition, the degree of satisfaction of hemodialysis patients was not statistically associated with sex, marital status, educational level, history of the disease, age of dialysis, number of sessions per week, and duration per session.
