Immediate and Long-Term Results of Transcatheter Closure of Patent Ductus Arteriosus—Comparison of Two Decades before and after Change in Antibiotic Infective Endocarditis Prophylaxis Guidelines

Objectives:To determine immediate and long-term follow-up of transcatheter closure of patent ductus arteriosus(PDA)in children.Background:National antibiotic prophylaxis(AP)guideline for infective endocarditis changed after 2009,the effect on practice of PDA closure is unknown.Methods:Observational single center study analyzing follow-up of PDA closure comparing two time periods before(2002–2009)and after(2010–2019)changes in AP guideline.Results:332 patients(68.1%female),median(interquartile range)age 3.0 years(1.5–5.7)and body weight 14.0 kg(10.0–19.3),were enrolled.PDA morphology was conical type A(50.3%),window type B(1.2%),tubular type C(40.1%),complex type D(2.1%),elongated type E(0.9%)and other(5.4%).Minimal PDA diameter and length were 1.9 mm(1.3–2.5)and 8.0 mm(6.2–10.2).PDA was closed using coils(56.3%),Amplatzer Duct Occluders(41.9%)and others(1.8%).Complete closure rate was 61.1%at catheter intervention,72.3%on day 1,87.7%after 6 months and 98.4%at last follow-up on echocardiography.Moderate complication rate(severity level 3)was 4.2%and major complication rate(severity level 4)0.3%,with no catastrophic complications(severity level 5).Annual PDA closure rate declined in the second time period(22.6/year vs.15.5/year,p=0.018),PDA size increased(1.6 mm vs.2.0 mm,p=0.002)and proportion of coils decreased(72.4%vs.37.1%,p98%)and only a small number of complications leading to reintervention or surgery.Change in AP guidelines changed indication for and practice of PDA closure.

Importance A nasogastric tube is used commonly to decompress the stomach and provide enteral feeding in surgical and medical practice.Sometimes this safe and innocent‐looking tube may lead to unexpected complications.We focus here on the possibility of spontaneous‘lariat loop’knotting of the nasogastric tube when some resistance is felt on tube retrieval and describe a method of safe tube removal.Case presentation We present a case of self‐knotting of a nasogastric tube that was placed to decompress the stomach during the postoperative period after surgical repair of anorectal malformation in a 4‐month‐old boy.Conclusion Self‐knotting of the distal end of nasogastric tube is an unusual complication with catastrophic sequelae if not addressed properly.If any resistance is felt during nasogastric tube retrieval,self‐knotting of the tube must be suspected.

BACKGROUND Congenital enteric duplication cysts are tubular or cystic structures that normally lie alongside the gastrointestinal(GI)tract.Enteric duplication cysts are typically solitary lesions that occur anywhere near the GI tract from the neck to the rectum,but having multiple duplication cysts is rare,and presentation within the pancreas is extremely rare.CASE SUMMARY We herein demonstrate a case of esophageal,gastric,and gastric-type duplication cyst of the pancreas in a seventeen-month-old girl who presented with failure to thrive,abdominal pain,vomiting,hematemesis,and melena since the age of three months.The cysts were excised by thoracoscopy and laparoscopy in the same setting.To our knowledge,no such case has been published.CONCLUSION Enteric duplications can occur throughout the entire alimentary tract.When they occur in the pancreas,they present a formidable challenge in both diagnosis and treatment.Due to the risk of complications and malignant transformation,surgical removal is the recommended treatment of all duplication cysts.

BACKGROUND A one-stage laparoscopic operation has recently been considered a favorable option for the management of patients with Hirschsprung's disease(HD)due to its superior cosmetic results.One-stage transanal endorectal pull-through for the treatment of rectosigmoid HD has been widely used in newborns without complications.However,enterostomy is required in some HD cases for enterocolitis and dilated colon.Our transumbilical enterostomy(TUE)and twostage laparoscopy-assisted anorectoplasty were effective and achieved a similar cosmetic effect to one-stage laparoscopy on the abdominal wall in patients with anorectal malformation,but the effect in patients with HD is unclear.AIM To evaluate the safety,efficacy and cosmetic results of TUE in two-stage laparoscopy-assisted pull-through for HD.METHODS From June 2013 to June 2018,53 patients(40 boys,13 girls;mean age at enterostomy:5.5±2.2 mo)who underwent enterostomy and two-stage laparoscopy-assisted pull-through for HD with stoma closure were reviewed at our institution.Two enterostomy approaches were used:TUE in 24 patients,and conventional abdominal enterostomy(CAE)in 29 patients.Eleven patients with rectosigmoid HD had severe preoperative enterocolitis or a dilated colon.26 patients had long-segment HD,and 16 patients had total colonic aganglionosis(TCA).The patients with left-sided HD underwent the two-stage laparoscopic Soave procedure,and the patients with right-sided HD and TCA underwent the laparoscopic Duhamel procedure.Demographics,enterostomy operative time,complications and cosmetic results were respectively evaluated.RESULTS There were no differences between the groups with respect to gender,age at enterostomy,weight and clinical type(P>0.05).No conversion to open technique was required.Two patients experienced episodes of stomal mucosal prolapse in the TUE group and 1 patient in the CAE group(8.33%vs 3.45%,P>0.05).No parastomal hernia was observed in either of the two groups.Wound infection at the stoma was seen in 1 case in the TUE group,and 2 case

背景:食管替代术应用于儿童仅限于很少的情况,包括其他治疗方式无效的长段型食管闭锁和腐蚀性物质摄入导致的食管狭窄。本研究旨在分享三家意大利医学中心胃移位术的应用经验。方法:这是一项回顾性研究,数据提取自一项前瞻性数据库。我们纳入了该数据库中过去15年所有行胃移位术的儿童患者。结果:15年期间,8例婴幼儿(男性3例,女性5例)接受了胃移位术。长段型食管闭锁6例,腐蚀性食管狭窄2例。无一例患儿死亡。3例患者出现了早期并发症:2例于术后3周出现自限性涎腺瘘,1例空肠造瘘者出现空肠穿孔,予再次手术治疗。1例患儿出现晚期并发症,即吻合口狭窄,进行了2次内镜扩张。中位随访时间60月(18~144月)。在最后一次临床随访中,6例患儿饮食正常,2例进食困难(原位空肠造瘘);所有患儿经发音矫正治疗后语言表达良好。所有患儿术后体重和身高均正常增长。结论:本研究报道了三家意大利中心胃移位术的治疗经验,无论从外科技术(简单、可重复、可接受的并发症)还是从长期随访(良好的经口进食、正常的社会生活、正常的生长曲线)来看,该术式均显示出满意的治疗效果。

Urethrocutaneous fistula may complicate hypospadias repair.We noticed that double-layered preputial dartos flaps added to tubularized incised plate urethroplasty can reduce the risk of urethrocutaneous fistula.The aim of this study was to compare the outcomes of tubularized incised plate urethroplasty with double-layered preputial dartos flaps to with single-layered local fascial flaps in preventing urethrocutaneous fistula.A retrospective cohort study was conducted between January 2017 and December 2020 at Jordan University Hospital(Amman,Jordan).Boys who were aged between 6 months and 5 years,diagnosed with distal hypospadias,and not circumcised were included.The primary outcome was the occurrence of urethrocutaneous fistula in patients who underwent tubularized incised plate urethroplasty with a double-layered fascial flap.The results showed a total of 163 boys with distal hypospadias;among them,116 patients underwent tubularized incised plate urethroplasty with a single-layered fascial flap,and 47 underwent tubularized incised plate urethroplasty with a double-layered fascial flap.The development of urethrocutaneous fistula was higher in the group receiving tubularized incised plate urethroplasty with a single-layered fascial flap than in the group receiving tubularized incised plate urethroplasty with a double-layered fascial flap after 1 month,6 months,and 12 months(6.9%vs 0,10.3%vs 0,and 5.2%vs 0,respectively),and the difference after 6 months was statistically significant(P=0.02).

INTRODUCTION Retroperitoneal malignant teratoma is a rare congenital tumor,representing 1%-10%of retroperitoneal tumors in children.12 A pathological migration of primitive germ cells to the genital ridges can stop at various levels,which explains the localizations of these tumors.3 The diagnosis is done either prena-tally,or later in front of an abdominal mass.45 We reported a case treated in our department and reviewed the diagnostic and therapeutic aspects of its management.

background Postoperative adhesions(POAs)causing mechanical bowel obstruction is a challenging problem for children who underwent prior abdominal surgeries.Unlike in adults,the role of conservative management remains controversial and children tend to require re-operation.However,despite the longer lifetime risk inherent to the pediatric population,studies are still lacking to come up with guidelines on diagnosis and management.This study aimed to review the profiles and outcomes of patients who underwent surgery for POA and to identify significant risk factors.Methods This was a retrospective review of 172 patients admitted at the Philippine General Hospital from 2010 to 2018 with the diagnosis of POA.Patients who did not undergo surgery and patients who had a different cause of bowel obstruction with the adhesions being only incidental were excluded from the study.results Ninety-one patients were included in the study.The mean age at surgery was 92.32 months and the male:female ratio was 2.8:1.The most common primary diagnoses were appendicitis,intussusception,and abdominal trauma.POA occurred within the first postoperative year in 63%.The 30-day morbidity and mortality rates were 9.89%and 7.69%,respectively.The most common complications were fascial and anastomotic dehiscences,and sepsis was the most common cause of death.Stoma creation during the index operation,and the presence of bowel gangrene,intestinal resection,and stoma creation during the surgery for POA were statistically significant risk factors for morbidity and mortality.Conclusion The findings in this study support the experience in literature that timely operative intervention can prevent bowel strangulation in cases of mechanical bowel obstruction from POA.However,more data and studies are recommended to arrive at an evidence-based guideline in the management of these children.
