BACKGROUND Islet amyloid deposition and reducedβ-cell mass are pathological hallmarks in type 2 diabetes mellitus *** date,the pathological features of the islets in diabetes secondary to pancreatic ductal adenocarcinoma(PDAC)have not been specifically *** To provide further insight into the relationship between islet amyloid deposition of the residual pancreas in PDAC patients and to explore whether regional differences(proximal vs distal residual pancreas)are associated with islet amyloid *** We retrospectively collected clinical information and pancreatic tissue removed from tumors of 45 PDAC patients,including 14 patients with normal glucose tolerance(NGT),16 patients with prediabetes and 15 new-onset diabetes(NOD)patients diagnosed before surgery by an oral glucose tolerance test at West China Hospital from July 2017 to June *** volume was calculated by multiplying the estimated area of pancreatic tissue on each image slice by the interval between slices based on abdominal computer tomography *** sections of paraffin-embedded pancreas specimens from both the proximal and/or distal regions remote from the tumor were stained as follows:(1)Hematoxylin and eosin for general histological appearance;(2)hematoxylin and insulin for the determination of fractionalβ-cell area(immunohistochemistry);and(3)quadruple insulin,glucagon,thioflavin T and DAPI staining for the determination ofβ-cell area,α-cell area and amyloid *** Screening for pancreatic histologic features revealed that duct obstruction with islet amyloid deposition,fibrosis and marked acinar atrophy were robust in the distal pancreatic regions but much less robust in the proximal regions,especially in the prediabetes and NOD *** with this finding,the remnant pancreatic volume was markedly decreased in the NOD group by nearly one-half compared with that in the NGT group(37.35±12.16 cm^(3) vs 69.79±18.17 cm^(3),P<0.001).As expecte

Background: Accurate estimates of the baseline incidence of childhood intussusception could help safety monitoring after introduction of rotavirus vaccines. Therefore, we studied the incidence of intussusception in the state ofQatar. Methods: We retrospectively reviewed cases of intussusception in children younger than 14 years. Children were treated between 2002 and 2012 at a single hospital inQatar. We analyzed information according to age, sex, clinical signs, diagnostic- and treatment-related characteristics, and length of stay. We calculated the national incidence of intussusception using census data. Results: A total of 106 children were diagnosed and treated for intussusception over 11 years. The male to female ratio of intussusception was 1.4:1. The most common symptom was bleeding per rectum (37%). One hundred andfour children with intussusception underwent diagnostic and therapeutic contrast enema, and 31.7% of patients underwent operative management. The length of stay was longer in patients who underwent operative reduction thanthat in those who did not. Conclusions: This is the first study to estimate the incidence of childhood intussusception prior to introduction of rotavirus vaccination in Qatar. A prospective surveillance system using a standardized case definition would be useful for examining the occurrence of intussusception in theArabicGulfarea and developing countries.

Background:Ataxia telangiectasia (AT) is a genetically based multisystemic *** aimed to make a comprehensive evaluation of multisystem involvement in AT by describing clinical features and outcome of 91 ***:Medical records of the patients who were diagnosed and followed by a multidisciplinary approach during a 27-year period (1988-2015) were reviewed ***:Forty six female and 45 male patients with a mean follow-up period of 39.13±4.28 months were *** mean age at the time of symptom onset and diagnosis were 15.4±1.09 months and 73.6144.11 months,*** abnormalities were progressive truncal ataxia,nystagmus,dysarthria,oculomotor apraxia and *** one patients (34.1%) became dependent on wheelchair at a mean age of 12.1±2.8 *** patients (12.1%) became bedridden by a mean age of 14.7±1.8 *** magnetic resonance imaging revealed pathological findings in 47/66 *** immunological parameters were determined in 51/91 patients:immunoglobulin (Ig)A deficiency (n=38),lymphopenia (n=30),IgG (n=15) and IgG2 (n=11) *** of recurrent sinopulmonary infections (n=45) and bronchiectasis (n=22) were found to be more common in patients with impaired immunological parameters (P=0.029and P=0.023,respectively).Malignancy developed in 5 patients,being mostly lymphoreticular in origin and resulted in death of 4 ***:AT is a long lasting disease with muitisystem involvement necessitating multidisciplinary follow up,as described in our *** diagnosis of malignancy and supportive treatments regarding pulmonary and neurological health may prolong survival and increase the quality of life.

AIM: To test protracted irinotecan infusion plus a low-dose cisplatin in this Phase Ⅱ trial to decrease its toxic-ity. METHODS: The eligibility criteria were: (1) histologi-cally proven measurable gastric cancer; (2) performance status of 0 or 1; (3) no prior chemotherapy or comple-tion of prior therapy at least 4 wk before enrollment; (4) adequate function of major organs; (5) no other active malignancy; and (6) written informed consent. The regi-men consisted of irinotecan (60 mg/m2) on d 1 and 15 by 24-h infusion and cisplatin (10 mg/m2) on d 1, 2, 3, 15, 16, and 17. Treatment was repeated every 4 wk. RESULTS: Thirty-one patients were registered between April 2000 and January 2001. The response rate for all 31 patients, 20 patients without prior chemotherapy, and 11 patients with prior chemotherapy was 52% (16/31), 60% (12/20), and 36% (4/11), respectively. The median survival time was 378 d. The median number of courses given to all patients was 2. Grade 4 neutropenia oc-curred in 11 (35%) patients, while grade 3 to 4 diarrhea or nausea occurred in 1 (3%) and 3 (10%) patients, respectively. Fatigue was minimal as grade 1 fatigue was found only in 3 (10%) patients. Other adverse events were mild and no treatment-related deaths ***: This regimen showed a high level of ac-tivity and acceptable toxicity in patients with metastatic gastric cancer.

A 79-year-old previously healthy man presented with acute acalculous cholecystitis with obstruction of the biliary tract. He was successfully treated with antibiotics and percutaneous transhepatic gallbladder drainage, but returned to the hospital two days after discharge with a rare complication of this technique, biliopleural fistula. A thoracostomy tube was inserted to drain the pleural effusion, and the patient’s previous antibiotics reinstated. After two weeks of drainage and antibiotics, the fistula healed spontaneously without the need for further intervention.
