AIM:To evaluate the role of endoscopic retrograde cholangiopancreatography (ERCP) for choledocholithiasis in patients with sickle cell anemia (SCA) in the era of laparoscopic cholecystectomy (LC). METHODS:Two hundred and twenty four patients (144 male,80 female; mean age,22.4 years; range,5-70 years) with SCA underwent ERCP as part of their evaluation for cholestatic jaundice (CJ). The indications for ERCP were:CJ only in 97,CJ and dilated bile ducts on ultrasound in 103,and CJ and common bile duct (CBD) stones on ultrasound in 42. RESULTS:In total,CBD stones were found in 88 (39.3%) patients and there was evidence of recent stone passage in 16. Fifteen were post-LC patients. These had endoscopic sphincterotomy and stone extraction. The remaining 73 had endoscopic sphincterotomy and stone extraction followed by LC without an intraoperative ***:In patients with SCA and cholelithiasis,ERCP is valuable whether preoperative or postoperative,and in none was there a need to perform intraoperative cholangiography. Sequential endoscopic sphincterotomy and stone extraction followed by LC is beneficial in these patients. Endoscopic sphincterotomy may also prove to be useful in these patients as it may prevent the future development of biliary sludge and bile duct stones.

High birth weight is a risk factor of dental caries increment during adolescence in Sweden

This study aimed to assess whether birth weight is associated with dental caries during the teenage period. In this register-based cohort study, all children of 13 yrs of age (n = 18,142) who resided in the county of Stockholm, Sweden, in 2000 were included. The cohort was followed until individuals were 19 yrs of age. Information regarding dental caries was collected from the Public Health Care Administration in Stockholm. Data concerning prenatal and perinatal factors and parental socio-demographic determinants were collected from the Swedish Medical Birth Register and National Registers at Statistics Sweden. The final logistic regression model showed that birth weight (≥4000 g), adjusted for potential confounders, was significantly associated with caries increment (DMFT ≥ 1) between 13 and 19 yrs of age (OR: 1.29, 95% CI = 1.13 - 1.48). The relatively enhanced risk OR was further increased from 1.29 to 1.52 in sub- jects with birth weight (≥4600 g). On the contrary, subjects with birth weight (<2500 g) exhibited a sig- nificantly lower risk (OR: 0.67, 95% CI = 0.50 - 0.89) for exhibiting caries experience (DMFT ≥4) at 19 yrs of age. In conclusion, birth weight can be regarded as a predictor for dental caries and birth weight (≥4000 g) is especially a risk factor for caries increment during adolescence.

BACKGROUND:The prognosis and clinical management of patients with chronic liver diseases are closely related to the severity of liver *** biopsy is considered the gold standard for the staging of liver ***,it is an invasive test sometimes related to *** study aimed to assess the diagnostic value of enhanced liver fibrosis(ELF) test to predict liver fibrosis in patients with chronic hepatitis ***:This study included 162 patients with liver disease and 67 healthy *** acid,tissue inhibitor of matrix metalloproteinase type 1,and amino-terminal propeptide type III procollagen were measured by enzymelinked immunosorbent assay with the ELF test ADVIA Centaur(Siemens Healthcare Diagnostics Inc.).Fibrosis stage was determined using the Metavir scoring ***:In our study,for the diagnosis of significant fibrosis(Metavir F≥2) a cut-off value >7.72 provides a sensitivity of 93.0% and a specificity of 83.0%.The areas under the receiver operator characteristic curve,sensitivity,specificity,and positive and negative predictive values were 0.94,93.3%,81.0%,93.3%,and 81.0%,respectively(P9.3 provides a sensitivity of 93.0% and a specificity of 86.0%.The areas under the receiver operator characteristic curve,sensitivity,specificity,and positive and negative predictive values were 0.94,79.1%,90.8%,75.6%,and 92.3%,respectively(P<0.001).CONCLUSIONS:The ELF test is a promising non-invasive method for assessing liver fibrosis in patients with chronic hepatitis *** is effective in the diagnosis of both fibrosis and cirrhosis.

Current Advances in Transcatheter Intervention for Children Born with Congenital Heart Defects: A Review of Literature

This review aims to sum up the improvements witnessed in the field of interventional cardiology during recent times. The last decade has witnessed significant technical advances in the field of radiological imaging and also in interventional cardiology which has helped to offer more non-invasive solutions for the management of congenital heart defects. This has resulted from the use of advanced 3-dimensional fusion imaging instead of conventional 2-dimensional angiography, applying interactive real-time enhancement and using computed tomography and Magnetic Resonance Imaging for interventional procedures. Similarly the introduction of next generation devices, have not only improved the final outcome of the procedure but also has helped in reducing the challenges that were faced before and with the initial generation of devices. These advances have helped not only in reducing the radiation exposure, the use of contrast medium dose but also have resulted in improved early survival. The field of neonatal cardiology has advanced at an unprecedented pace. The transcatheter closure of patent ductus arteriosus has evolved over time and now it has been made possible at much lower body weight. Similarly, early use of stents for aortic coarctation has been found effective in some patients, especially when surgical intervention had been denied. The application of the hybrid approach for the management of complex congenital heart defects has also been effectively applied. More recently transcatheter placement of the pulmonary valve has been introduced for severely stenotic and/or regurgitant pulmonary valve in adolescents and adults. It is anticipated that in near future, this procedure would be available for relatively younger patients. In conclusion: last 2 decades have improvised pediatric interventional cardiology to incorporate less invasive solutions for CHD. Background This study aimed to develop an expert consensus regarding the epidemiology,diagnosis,and management of cow’s milk protein allergy(CMPA)in the Middle *** A three-step modified Delphi method was utilized to develop the *** specialized pediatricians participated in the development of this *** statement was considered a consensus if it achieved an agreement level of>80%.Results The experts agreed that the double-blind placebo-controlled oral challenge test(OCT)should be performed for 2-A weeks using an amino acid formula(AAF)in formula-fed infants or children with suspected ***-fed infants with confirmed CMPA should be offered a therapeutic *** panel stated that an extensively hydrolyzed formula(eHF)is indicated in the absence of red flag *** the same time,the AAF is offered for infants with red flag signs,such as severe anaphylactic *** panel agreed that infants on an eHF with resolved symptoms within 2-4 weeks should continue the eHF with particular attention to the growth and nutritional *** the other hand,an AAF should be considered for infants with persistent symptoms;the AAF should be continued if the symptoms resolve within 2-4 weeks,with particular attention to the growth and nutritional *** cases with no symptomatic improvements after the introduction of an *** measures should be *** panel developed a management algorithm,which achieved an agreement level of 90.9%.Conclusion This consensus document combined the best available evidence and clinical experience to optimize the management of CMPA in the Middle East.

Neonatal Kawasaki Disease: A Case Report

Objective: To report a case of Kawasaki disease (KD) with unusual age at presentation. Case Report: A 26-day-old Saudi boy presented with fever, irritability and poor feeding for 3 days. There was right cervical lymph node enlargement. There was mild throat congestion and bilateral congested tympanic membrane. The liver was palpable. Otherwise, there were no more abnormal physical findings. Laboratory findings revealed high erythrocyte sedimentation rate and positive C-reactive protein. The patient was admitted into an isolation room and antibiotics were started. On the 5th day, the condition of the patient deteriorated. Ampicillin was discontinued and vancomycin was started till getting the results of the culture sensitivity. On the 7th day, he developed diaper rash and fever spikes continued. On the 8th day, the patient developed swelling, redness and hotness of the right hand. Possibility of sepsis was considered and the administered antibiotics were imipenem and vancomycin, while cefotaxime was discontinued. On the 12th day, the patient developed anemia and leukocytosis. Gamma glutamyl transpeptidase was high. On the 15th day, the patient started to develop maculopapular rash on the trunk and desquamation of both hands and feet with swelling, cracked lips, bilateral conjunctival injection and unilateral neck lymph node swelling. So, the infant’s condition was clinically diagnosed as KD. Aspirin was started and 2 doses of intravenous immunoglobulins were given. On the 18th day, the condition of patient improved and the fever subsided. Conclusions: The diagnosis of KD among neonates is a clinical challenge. Diagnosis is based on clinical criteria after the exclusion of other diseases presenting with high persistent fever. Early treatment by administering intravenous immunoglobulins and aspirin can help prevent cardiac complications. There is a pressing need to raise awareness among pediatricians about this disease.

Clinical Demographic Patterns of Type 1 Diabetes in Saudi Children in Tabuk City, 2000-2010

We aimed to identify the variation in the clinical background of children diagnosed with type 1 diabetes mellitus (T1DM) at King Salman Military Hospital (KSMH), Tabuk City, Kingdom of Saudi Arabia, from 2000 to 2010. Methods: This retrospective observational study was based on the clinical records of pediatric diabetes outpatients at KSMH. All children aged Results: Of 313 patients recruited, female patients were predominant (p = 0.002). The mean age of onset was 6.46 years (standard deviation, 3.02). One-third of the newly diagnosed patients were overweight (35.5%). Diabetic ketoacidosis (DKA) was the presenting feature in 38.0% of patients, wherein female patients and those aged 0 - 3 years exhibited the highest likelihood of developing DKA (odds ratio, 1.7 and 2.9, respectively). Moreover, underweight children had a greater DKA incidence than healthy, overweight, or obese children (p = 0.02). Conclusion: This study provides additional data on T1DM in the population of the Kingdom of Saudi Arabia. In particular, we found a female predominance at presentation as well as 2 peaks for age at onset. Moreover, the BMI was lower in younger age groups overall, but was greater in older boys. Furthermore, the DKA rates were high in younger children. Thus, our data confirm the presence of variable clinical patterns in the Kingdom of Saudi Arabia, which requires further epidemiological analysis using national registry data.
