PK-tailored tertiary prophylaxis in patients with severe hemophilia A at beijing Children’s Hospital

Importance:Tertiary prophylaxis using a low-dose regimen is usually insufficient to prevent recurrent joint bleeding and deterioration in joint diseases in children with severe hemophilia ***(PK)dosing is a useful approach to increase the precision and efficiency of ***:To explore the efficacy of PK-tailored tertiary prophylaxis in children with severe hemophilia ***:We implemented a PK-tailored tertiary prophylaxis program for 15 boys with severe hemophilia A aged 5-16 years at Beijing Children’s *** PK testing and a 6-month evaluation period(phase I),15 patients were divided in two groups according to individual PK data and actual bleeding:(1)a PK-tailored group[modified prophylaxis regimen according to PK data for the next 6 months(phase II);n=8]and(2)a maintenance group(continued the original regimen for the next 6 months;n=7).We compared the bleeding rate,infusion frequency,and factor VIII(FVIII)consumption between the two ***:In the PK-tailored group,the median annual joint bleeding rate was reduced from 7.8 in phase I to 1.4 in phase II,mean annual total factor consumption increased from 1619.0 IU/kg in phase I to 2401.9 IU/kg in phase II,and median infusion frequency for prophylaxis increased from 104 times/year in phase I to 156 times/year in phase II(P<0.05).Although the FVIII consumption increased,it remained at approximately half of the standard ***:PK-tailored prophylaxis may represent a more efficient approach to individual prophylaxis in China,but further studies are required to verify this.

Hidden in the hands-the special inner Bagua acupoints for pediatric massage

pediatric massage is one of the external treatment methods that can improve children'sconstitution and immunity by regulating the viscera (the basic organs in traditonal Chinesemedicine theory), dredging the meridians, regulating Qi and blood, and balancing Yin and *** one of the specific acupoints of pediatric massage, inner Bagua acupoint (one of the clinicallyused acupoints in children's massage that locate in the palm of the hand and the periphery ofthe palm) play a key role. Inner Bagua acupoints are the commonly used acupoints in pediatricmassage, which have the function of regulating Qi and eliminating phlegm (a kind pathologicfluid in TCM theory) and improving digestion. Inner Bagua acupoints have a unique curativeeffect on many diseases of digestive and respiratory system in children. Therefore, this articleattempts to elaborate and summarize the origin, location, operation and clinical application ofthe inner Bagua acupoints. This article hopes to provide a basis information for advocating anddisseminating pediatric massage, which is favorable for wide-range popularization anddevelopment.

MLS (Maroteaux-Lamy syndrome) or MPS VI (mucopolysaccharidosis VI) is an autosomal recessive pathology in which there is absence or low activity of the enzyme N-Acetylgalactosamine-4-Sulfatase, which hydrolyzes GAGs (glycosaminoglycans) in the body (mainly dermatan sulfate). Consequently, there occurs lysosomal deposition of GAGs in connective tissue multisystemic. Myocardium and heart valves are frequently affected structures, presenting a direct correlation with the reports of complications and deaths. Case report: RGS, male, 3 years and 2 months, diagnosed with MPS VI from the first month of life, in weekly ERT (enzyme replacement therapy) since 4 months of age (inconstant). At physical examination: normotensive, with holosystolic heart murmur 3+/6+ in mitral focus. Complementary tests: normal electrocardiogram, echocardiogram with pronounced mitral regurgitation, concentric left ventricular hypertrophy of moderate degree and mild aortic insufficiency. Discussion: Mitral valve disease is common in patients with MLS. Conditions such as cardiomyopathy, fibroelastosis, aneurysm and pulmonary hypertension may occur in these patients, indicative of morbidity and mortality. Early and constant ERT may be useful in slowing a progression of heart disease. Conclusions: follow-up with a cardiologist is important to evaluate the progression of cardiac complications in MPS VI. Constant and early ERT provides better prognosis for these patients.

Background:The purpose of this study was to evaluate the effectiveness of limited-resource hospitals in managing mild and moderate hospitalized cases of COVID-19 with comorbidities and in preventing their progression to severe ***:Data were obtained from 88 moderate COVID-19 patients with comorbidities who were admitted to limitedresource *** data were classified into several parts:comorbidities,chronic medication before hospital admission,symptoms of COVID-19 before and during hospitalization,clinical features,laboratory findings on hospital admission,complications during hospitalization,as well as worst laboratory values during hospitalization,hospital stay,and *** clinical features,laboratory results,type of oxygen therapy used,and the final treatment outcome were all evaluated to assess for any potential ***:All patients were alive upon *** admission,the majority of patients(60.2%)received COVID-19 treatment,and the average hospital stay was 12 *** most common symptoms were fever(88.7%),cough(95.5%),shortness of breath(90.9%),myalgia(84.1%),confusion(63.6%),headache(62.5%),sore throat(88.7%),rhinorrhea(17%),chest pain(58%),diarrhea(19.3%),nausea and vomiting(38.6%),anosmia(62.5%),as well as dysgeusia(64.8%).Based on chest radiograph or computed tomography(CT)scan,9.1%of the patients had unilateral pneumonia,90.9% had bilateral pneumonia,and 96.6% had multiple mottling and ground-glass *** was found associated with a significant increase in headache(p=0.005),rhinorrhea(p=0.013),chest pain(p=0.007),and the need for positive airway pressure(p=0.008).Between pre-and post-hospital admissions,there was a significant increase in lactate dehydrogenase and ferritin but a decrease in platelet,D-dimer,hemoglobin,lymphocytes,neutrophils,and total leucocyte count(p<0.001).There was a significant association between hospital stay and D-dimer level(p=0.05).Conclusion:Limited-resource hospitals in Egypt w
