Knowledge and Tube Feeding Practices for Older Adult Patients among Japanese Registered Nurses

The aim of this study was to examine nurses’ thinking regarding the indications for tube feeding in older adults with eating difficulties and to determine whether the team approach can improve their knowledge and tube feeding practices. We sent self-administered questionnaires to 436 nurses and collected them from November to December 2010 (response rate, 70.0%). The results indicated that approximately 30% of the Japanese nurses thought that older patients with dementia should be on tube feeding and more than half of the nurses answered that tube feeding is indicated for aspiration-prone, frail, older adults. Moreover, our findings showed that nurses who organize multidisciplinary teams performed more interventions for dysphagia before introducing tube feeding than the reference group as analyzed by multivariate adjustment (odds ratio, 2.1-6.6). In conclusion, a multidisciplinary team approach is expected to make better decisions on the treatment and care of older patients with difficulty eating, including the need for tube

Complications and its risk factors of percutaneous subclavian vein catheters in pediatric patients:enhancing the outcomes of a university hospital in a low-income and middle-income country

Objective Assessing central venous catheter-related complications with regular feedback and investigating risk factors are mandatory to enhance *** aim of this study is to assess our experience in the management of pediatric subclavian vein catheters(SVCs)and to investigate the main risk factors for *** In this prospective observational study,we included children aged 3 months to 14 years who underwent infraclavicular subclavian vein catheterization consecutively using the anatomic landmark *** were divided into two groups:group 1 included complicated catheters and group 2 included noncomplicated *** management protocol was standardized for all *** comparing the two groups,univariate and multivariate logistic regression were used to investigate the risk factors for *** In this study,we included 134 pediatric *** rate of complications was 32.8%.The main complications were central line-associated bloodstream infection(63.6%),bleeding and/or hematoma(22.7%),mechanical complications(13.6%),and vein thrombosis(13.6%).After adjustment for confounding factors,predictors of catheter-related complications were difficult insertion procedure(adjusted odds ratio(aOR)=9.4;95%confidential interval(CI):2.32 to 38.4),thrombocytopenia(aOR=4.43;95%CI:1.16 to 16.86),comorbidities(aOR=2.93;95%CI:0.58 to 14.7),and neutropenia(aOR=5.45;95%CI:2.29 to 13.0).Conclusions High rates of complications were associated with difficult catheter placement and patients with comorbidities and severe *** reduce catheter-related morbidity,we suggest an ultrasoundguided approach,a multidisciplinary teaching program to improve nursing skills,and the use of less invasive devices for patients with cancer.

Verbal Abuse, Physical Assault, and Harassment in pediatric Medical Settings: A Nationwide Survey in Japan

Background: Verbal abuse, physical assault, and harassment of pediatric healthcare workers by patients and their families have become increasingly serious. These incidents lead to various problems, including the deterioration of staff members’ mental health and employee turnover. Methods: This study was conducted as the first Japanese nationwide survey to assess the abuse perpetrated by patients and families and experienced by medical staff in pediatric medical settings in 2018, countermeasures employed, and staff member evaluations of the countermeasures. Results: Responses from 526 facilities were received, clinics accounted for 79.8% of these facilities;88.1% of the respondents were females. Further, 10.5% of the facility staff reported having experienced verbal abuse, violence, or harassment from patients or family members. Of 1103 cases of victimizations, there were 719, 136, and 248 cases of verbal abuse, violence, and harassment, respectively. Manuals and guidelines had been created and training and drill performances were conducted as countermeasures in 10.7% and 9.0% of facilities, respectively;7.3% of staff members felt that the countermeasures were effective, corresponding to 45.6% of the affected staff. Conclusion: The findings suggest the necessity of thoroughly familiarizing staff with existing facility countermeasures as well as introducing new countermeasures such as victimization simulations.

Examining the Digital Story Created by an Adolescent with Cancer: Insights and Ideas from a Case Story

Digital Storytelling (DST) has been recognized as a holistic and innovative storytelling avenue. Survival rates for pediatric oncology patients are improved, making it necessary to investigate the psychosocial perspectives of these children. This research began following a comprehensive review of the scientific literature and identification of a gap as it pertains to using DST as an avenue for acquiring data about this aggregate of patients. The purpose of this project was to explore the use of DST among pediatric cancer patients and their perspectives regarding diagnosis. We employed a qualitative case study design with two oncology patients between the ages of 11 - 17 years. The narrative, digital story, and field notes comprise the qualitative data. DST narratives and field notes were analyzed using qualitative content analysis to extract themes. Findings will provide the foundation for future research to explore the use of DST as both an intervention and data collection method in this population.

Chinese family care patterns of childhood rheumatic diseases:A cluster analysis

Objectives:The purpose is to distinguish family care(FC)patterns of childhood rheumatic diseases in Chinese families and to determine the predictors of FC ***:This secondary analysis contained two cross-section surveys with a convenient sample of totally 398 caregivers who have a child with rheumatic diseases from four pediatric *** were required to completed Family Management Measure *** analysis was used to distinguish patterns and multinomial logistic regression analysis was used to find ***:Four patterns were identified:the normal-perspective and collaborative(28.4%),the effortless and contradictory(24.6%),the chaotic and strenuous(18.3%),and the confident and concerning(28.7%).Disease category(x2=21.23,P=0.002),geographic location(x2=8.41,P=0,038),maternal educational level(x2=12.69,P=0.048)and family monthly income(x2=33.21,P<0.001)predicted different ***:FC patterns were different among ***-related and family-related factors were vital predictors to distinguish patterns consistent with the Family Management Style *** result assisted that clinicians recognize FC patterns and predictors effectively to provide tailored advice in time.

The Application of Multimedia Messaging Services via Mobile Phones to Support Outpatients:Home nursing Guidance for pediatric Intestinal Colostomy Complications

Objective: To present a model of extended home nursing support provided through WeChat for pediatric colostomy patients between the first and second surgery to support parents and to assist them in nursing possible complications at home, to reduce colostomy complications caused by improper care, to improve the quality of life of patients at home and to ensure the success of the recovery and second surgery. Methods: A combination of relevant clinical information, health care guidelines, and training as well as the establishment of a specific monitoring protocol was provided to the parents during the patient’ s first hospitalization. A support system for nursing the colostomy was established with regular follow-up via WeChat after the patient’ s first discharge from the hospital using a predetermined protocol. During the 3 to 6 months of home care in-between the colostomy operations, the charge nurse initiated the phone follow-up and provided specific, individual guidance and feedback. If necessary, extended serv-ice at predetermined intervals was provided in the 1st week, the 1st month, the 2nd month and the 3rd month. Results: Extended assistance for the home care of patients with pediatric colostomy complications im-proved the quality of life of the patients and caregivers and was accepted by all parties involved upon in-troduction. The assistance provided support for the parents at all hours, efficiently reduced the level of complications for this type of patient and facilitated fast referrals to hospital care in cases of emergen-cies. Furthermore it paved the way to successful second stage surgeries, provided direct feedback to the charge nurse and improved the work satisfaction and sense of achievement of the nurses involved. Conclusions: All 80 cases supported by WeChat successfully underwent two-stage pediatric colostomy operations. The response of the parents and the medical staff involved was very positive. Changes in Nurses after Online Workshops Using the pediatric nursing Care Model

This study aimed to clarify the changes in nurses’ cognition about the rate and likelihood of implementation of daily ethical nursing items used in pediatric care before and after attending online workshops using the pediatric nursing Care Model (PNCM). A 1.5-hour online workshop was held twice over two weeks. A total of nine nurses participated in the program. These online workshops using PNCM helped achieve the original goals and the results revealed a positive outcome of the workshops. We should devise an educational program to arouse participants’ interest and comprehension to make the most of online competency.

The effects of EMLA cream on pain responses of preschoolers

The aim of present study was to assess the effects of a eutectic mixture of topical anesthetics (EMLA cream) on pain responses of preschoolers during venipuncture. The pain responses were reported by children, nurses, and the children’s mothers. The pre- and post-test-designed study, which included a non-equivalent control group, was carried out in a pediatric unit at E University Hospital in South Korea. Pain responses during venipuncture were measured by the self-reporting of the children using the Face Pain Rating Scale (FPRS), the Procedure Behavior Checklist by nurses (PBCL), the Visual Analog Scale (VAS) by mothers, in addition to measurements of the children’s pulse rate and level of oxygen saturation. EMLA cream was effective in decreasing the pain responses of the children as assessed by FPRS, PBCL, and VAS scores. However, pulse rate and level of oxygen saturation of the children during venipuncture were not significantly different between the two groups. EMLA cream could be a useful option for reducing pain in preschool children during invasive procedures.

This study aimed to describe structure, functionality and development of pediatric heart transplant families, from the understanding of the experienced chronic condition. A descriptive, exploratory study using a qualitative approach was developed in a hospital situated in a city of Northeast Brazil, in 2012. The study was conducted with three families, using the Calgary Family Assessment Model (CFAM). In addition to the genogram, an analysis of the children’s/adolescents’ family members was conducted at the time of interview. Two families were binuclear families and one was mononuclear. From the evaluation, it was possible to determine the care and support network of children and adolescents with heart transplants, as well as some difficulties faced by the investigated families. Nurses are important in this family context since the visualization of the family genogram can generate a change in the nursing care to each family.
