Objective:To investigate the importance of immunization in preventing measles infection and to determine the most useful laboratory tests for confirmation of ***:This study included pediatric cases evaluated with a presumed diagnosis of measles between December 2022 and June 2023,at Marmara University Pendik Training and Research *** effects of vaccination status and underlying disease on the clinical course,treatments,and complications were ***:In total,117 patients were enrolled in the study with a median age of 80 months(IQR:32.5-125.0).Twelve patients with contact history were asymptomatic and had an underlying disorder,and intravenous immunoglobulin was given to them for post-exposure ***-one patients had confirmed measles *** treatment was given to three patients(a newborn,a girl with rhabdomyosarcoma,and a healthy boy)with respiratory ***-eight percent of confirmed measles cases were unvaccinated,and all hospitalized cases were unvaccinated or *** full-vaccinated children had confirmed measles *** PCR from nasopharyngeal swabs was negative in all of them,and their diagnosis was established with anti-measles IgM ***:The measles vaccine is the most effective way to protect from measles and measles-related *** measles can also occur in fully vaccinated patients,the disease is milder than in unvaccinated *** ELISA and RT-PCR tests together may be beneficial in patients with high clinical suspicion for early diagnosis.

Purpose Various complications are associatedwith percutaneous endoscopic gastrostomy(PEG)procedures in *** push technique is being increasingly used,but its complications are insufficiently *** aimed to assess all complications related to PEG procedures and compare the safety of the pull and push *** Retrospective review of consecutive pediatric patients who underwent PEG between 2002 and *** In total,216 children underwent 217 PEG *** push technique was used in 138(64%)cases,and the pull technique in 79(36%)*** median follow-up time was 6.1(0.1-18.3)*** complication rate was high(57%)and patients experienced complications years after the ***,51%and 67%of patients experienced complications in the push and pull groups,*** rates of minor and major complications were higher in the pull group than in the push group(63%vs 48%,p=0.028;and 11%vs 6%,p=0.140,respectively).Reoperation was also more common in the pull group(17%vs 7%,p=0.020).Conclusions The overall complication rate of PEG procedures is ***,most complications are mild and do not require *** increasing push technique appears to be safer than the traditional pull *** long-term morbidity is related to gastrostomies in children.

Challenges and pitfalls of youth-onset type 2 diabetes

Background:The aim of this prospective randomized study was to evaluate the impact of visual guidelines(picture book)and parents tutoring on pediatric urological surgery on parent's stress and anxiety,the number of postoperative contacts,and complications.Materials and methods:Following institutional ethical committee approval,a special picture book reflecting different stages of the con-valescent period following multiple types of pediatric urological surgery was *** were randomly divided into 2 groups in which 33 parents in Group 1 received the picture book in addition to routine instructions prior to the surgery and 31 in Group 2 received only routine postoperative *** parents were asked to answer a questionnaire(Amsterdam Preoperative Anxiety and Infor-mation Scale)regarding the level of anxiety before surgery and immediately after surgery in the recovery *** number of postop-erative parent's calls,nonplaned emergency room visits,and complications were recorded.Results:No statistically significant difference in perioperative parental anxiety was found(p=0.88).The visual tutoring group had a sig-nificantly lower rate of emergency room admissions(6.6％vs.18.6％,p=0.0433),however parents from this group made a higher num-ber of postoperative calls(9.9％vs.3.1％,p=0.38).Two(6.6％)from the tutoring group expressed their desire to omit visual counseling in future surgical preparation and 4(13.2％)did not have an *** satisfaction with regards to the preoperative counseling and information and the number of postoperative complications was similar in both groups.Conclusions:Visual tutoring does not add any value to parental anxiety but seems helpful in reducing postoperative emergency room *** parents preferred to exclude visual information from future preoperative counseling.

The incidence and prevalence of youth-onset type 2 diabetes mellitus(T2DM)are *** rise in frequency and severity of childhood obesity,inclination to sedentary lifestyle,and epigenetic risks related to prenatal hyperglycemia exposure are important drivers of the youth-onset T2DM epidemic and might as well be responsible for the early onset of diabetes ***,youth-onset T2DM has a more extreme metabolic phenotype than adult-onset T2DM,with greater insulin resistance and more rapid deterioration of beta cell ***,intermediate complications such as microalbuminuria develop in late childhood or early adulthood,while end-stage complications develop in *** to the lack of efficacy and safety data,several drugs available for the treatment of adults with T2DM have not been approved in youth,reducing the pharmacological treatment *** this mini review,we will try to address the present challenges and pitfalls related to youth-onset T2DM and summarize the available interventions to mitigate the risk of microvascular and macrovascular complications.

Background:Repair of proximal hypospadias remains a challenge with no consensus on the best surgical *** reports have shown recurrence of chordee after plate-preserving *** this report,we present our experience in managing cases who presented with persistent/recurrence of chordee after tubularized incised plate(TIP)repair for proximal ***:Between 2015 and 2023,the study included eight patients who presented with ventral penile curvature after previous hypospadias repair in infancy(TIP urethroplasty).Reoperation to correct persistent ventral curvature was performed several years after the primary TIP repair(3-15 years;mean 7.9 years;median 7.5 years).The reoperation was a two-stage procedure:the first stage comprised transection of the urethra and grafting(lower lip mucosal graft);the second stage(6 months later)was a Thiersch-Duplay ***:Complications included poor taking of the graft after the first stage in one case(12.5%),which was regrafted using buccal(cheek)*** after second stage included urethro-cutaneous fistulae in five(62.5%)that were successfully surgically treated at a later ***-up after second stage ranged between 6 months and 7 years(mean 2.4 years;median 1 year).Satisfaction with reoperation to correct penile curvature was noted by parents as well as older children(adolescents)who appreciated better cosmesis by increasing ventral penile length and improved ***:Preservation of the urethral plate in proximal hypospadias may result in shortened penile length,mostly on the ventral aspect.A two-stage reoperation to increase the length of the penile urethra can successfully reverse this complication.

Background:Nonalcoholic fatty liver disease(NAFLD)is associated with impaired renal function,and both diseases often occur alongside other metabolic ***,the prevalence and risk factors for impaired renal function in patients with NAFLD remain *** objective of this study was to identify the prevalence and risk factors for renal impairment in NAFLD ***:All adults aged 18-70 years with ultrasound-diagnosed NAFLD and transient elastography examination from eight Asian centers were enrolled in this prospective *** fibrosis and cirrhosis were assessed by FibroScan-aspartate aminotransferase(FAST),Agile 3+and Agile 4 *** renal function and chronic kidney disease(CKD)were defined by an estimated glomerular filtration rate(eGFR)with value of<90 mL/min/1.73 m^(2) and<60 mL/min/1.73 m^(2),respectively,as estimated by the CKD-Epidemiology Collaboration(CKD-EPI)***:Among 529 included NAFLD patients,the prevalence rates of impaired renal function and CKD were 37.4%and 4.9%,*** multivariate analysis,a moderate-high risk of advanced liver fibrosis and cirrhosis according to Agile 3+and Agile 4 scores were independent risk factors for CKD(P<0.05).Furthermore,increased fasting plasma glucose(FPG)and blood pressure were significantly associated with impaired renal function after controlling for the other components of metabolic syndrome(P<0.05).Compared with patients with normoglycemia,those with prediabetes[FPG≥5.6 mmol/L or hemoglobin A1c(HbA1c)≥5.7%]were more likely to have impaired renal function(P<0.05).Conclusions:Agile 3+and Agile 4 are reliable for identifying NAFLD patients with high risk of *** glycemic control in the prediabetic stage might have a potential renoprotective role in these patients.
