AIM: Fatty acid-CoA ligase 4 (FACL4) is an arachidonatepreferring enzyme which has been shown to be up-regulated in human colon cancer tissues and implicated in the colon tumorigenesis. The purpose of this study was to investigatethe role of FACL4 in the human hepatocellular carcinoma (HCC) tumorigenesis and the specific signal pathways involved in this ***: We investigated the expression and regulation of FACL4 in HCC, adjacent non-tumorous liver tissues, and cell ***: In HCC patients, we demonstrated that FACL4 gene expression was markedly elevated in the cancerous tissues than in the adjacent non-cancerous liver *** addition, several human hepatoma cell lines, including Hep3B and HepG2, expressed high levels of FACL4. Stable overex-pression of FACL4 knockdown plasmids (small interfering RNA, siRNA) to Hep3B cells significantly decreased FACL4 expression and subsequently limited the cell proliferation. Treatment of Hep3B cells with 8bromo-cAMP and SB203508 (p38 MAPK inhibitor) significantly suppressed the FACL4 expression. CONCLUSION: FACL4 is involved in the HCC tumorigenesis and both cAMP and p38 MAPK pathways are associated with the regulation of FACL4 in HCC.

Background: A diverse range of pathologies affect the sinonasal cavities, because of the nonspecific nature of the symptoms, most patients with sinonasal masses are diagnosed late when the disease is at an advanced stage. The diagnostic challenge of identifying the nature of sinonasal masses before treatment would guide therapeutic strategies. Our objective was to evaluate the accuracy of computed tomography in the preoperative diagnosis of sinus masses at the Central Hospital of Yaoundé. Method: A retrospective chart review was conducted;CT scan reports and pathology reports of patients operated for sinonasal masses between January 2012 and November 2022 were compared. Results: Forty-five patients met the inclusion criteria for the study. The average age was 39.98 ± 18.34 years, with extremes of 15 and 87 years. The sex ratio H/F is 0.76 with a female predominance. The median time to consultation was 14 [12 - 18] months, with extremes of 11 and 36 months. The most frequently encountered histological type was benign tumors with sinonasal polyposis (PNS) in the lead, i.e. 34.1% of cases. The types adenocarcinoma, adenoid cystic carcinoma and non-Hodgkin’s lymphoma were represented equally in 6.8% of cases. Management was dominated by Caldwell-Luc surgery, i.e. 59.1% of cases. The frequency of disease recurrence after surgical treatment was 18.2%. As for the mortality rate, it was 11.4%. The concordance rate of the character of malignancy on imaging compared to histology was 90.9%. This level of concordance was statistically significant according to the Gamma test (p Conclusion: Preoperative CT scans correlate with histology and have a prognostic role in surgically treated sinonasal masses.

Slow spinal compressions are due to the development of an expansive process in the spinal canal. It is a very common pathology, the diagnosis of which is mainly clinical. However, magnetic resonance imaging occupies an essential place in the site diagnosis and etiological research in the management. Non-traumatic spinal cord compression is a diagnostic and therapeutic emergency, requiring early and appropriate management. MRI is the benchmark imaging examination for this pathology. No similar previous MRI study in Mali. We undertook this work with the aim to determine the place of MRI in the diagnosis of spinal cord compressions in Mali hospital. Method and Patients: This was a descriptive retrospective study, carried out at the hospital’s medical imaging department from January 1, 2017 to December 31, 2018 (02 years). It involved all patients, regardless of sex and age, sent for an MRI examination of the spine, and in whom spinal cord compression was diagnosed. We used a 0.35T low-field MRI machine with solid-state antennas. Results: We collected 179 cases of spinal cord compression MRI out of 585 spinal MRI performed, (frequency of 30.59%). The average age was 53.5 years with a male predominance (sex ratio 3.7). Motor disorders were the most common reason for examination (41%). We used the T1 T2 sagittal and T2 axial sequences. IV injection of gadolinium was performed in 48% of patients. The topographic lesions were: cervical (54.7%), thoracic (31.3%) and several segments (9.5%). The lesions concerned the compartments: extradural (79.3%), intradural (4.5%), and intramedullary (16.2%). The processes were degenerative (57.5%). tumorous (29.6%), infectious (12.3%) and vascular (0.6%). Conclusion: MRI is the benchmark imaging test for the management of non-traumatic spinal cord injury. Myelo-CT can be an alternative in the absence or in case of MRI contraindication.

Objective: The aim of this piece of work is to study the contribution of pelvic ultrasound while exploring pelvic pathologies at Hubert Koutoukou Maga Teaching University Hospital “CNHU-HKM” and at the Teaching University Hospital of Mother and Child “chu-MEL” of Cotonou. Patients and Methods: A descriptive, cross-sectional and analytical study with prospective collec-tion was carried out at the University Clinic of Gynecology and Obstetrics “CUGO” at the CNHU-HKM and in the gynecology department of the chu-MEL, over a period of 4 months (May to August 2018). We included in this study women over 12 years old who performed a pelvic ultrasound and were followed in the aforementioned health centers. Results: The population of the study consisted of 181 patients. The pelvic ultrasound request frequency was 18.84% compared to all ultrasound examinations. The age group of 25 - 29 years old was the most represented (22.10%). Pelvic pain was the first mo-tive for pelvic ultrasound request (39.8%). The abnormalities found by ultra-sound were uterine and ovarian respectively in 55.4% and 38% of cases. Uter-ine pathology was dominated by uterine myoma (44%). Ovarian cyst (13%) and ovarian dystrophy (14%) were the main ovarian pathologies. There was a statistically significant relationship between certain reasons for consultation, mainly metrorrhagia, dysmenorrhoea, amenorrhoea, leucorrhoea (p Conclusion: The ultrasound remains the first choice examination and is of a great value in exploring pelvic pathologies. In most cases, it helps to find the responsible lesion and to make the appropriate ther-apeutic decisions.

Introduction: Breast cancer is the leading cancer and the leading cause of cancer death in women worldwide. The objective of our study was to describe the epidemiological, clinical and therapeutic aspects of breast cancer at the General Hospital of Loandjili in Pointe Noire. Patients and Methods: This was a descriptive and cross-sectional study that took place in the Cancer department of the General Hospital of Loandjili in Pointe Noire during the period from January 1st, 2012 to December 31st, 2016 for duration of 5 years. 54 records of patients over 18 years of age with histological evidence were collected. The variables studied were: age, level of education, time to presentation, telltale sign, stage of extension, histological type, and histological grade. Chi-square test was used to compare variables and groups. Results: At the end of this study, we collected 54 files of breast cancer patients from 150 patients hospitalized in the Cancer department. The frequency of breast cancer was 36%. The average age was 50 ± 13.6 years;the extremes were 27 years and 79 years. The most represented age group was from 37 to 40 years old with a percentage of 28%. The patient’s level of study was primary in the majority of cases (57%). The most represented histological type was invasive ductal carcinoma, (100%). The majority of patients arrived at advanced stages 44.4% and metastatic stages 26%. Patients were treated in most of cases by chemotherapy (82%), surgery (42%) and radiotherapy (10%). There was a correlation between time of presentation and stage of extension. Conclusion: Breast cancer is the first cancer of the woman in Pointe Noire. Patients arrive at advanced stages;the most common histological type is invasive ductal carcinoma. The presentation time is correlated with the extension stage. These results suggest the establishment of a cancer control program in developing countries.

AIM: To investigate the clinical and pathologic fi ndings, and to discuss the pathophysiology of hepatocellular carcinoma with cardiac metastasis. METHODS: Eight hepatoma patients with cardiac metastasis, who were treated by surgical excision from 1993 to 2006, were retrospectively studied. Detailed clinical parameters were analyzed. RESULTS: Of those eight patients, two (25%) were women and six (75%) were men, with the mean age of 50 years (range, 40-70 years). The presentations included: asymptomatic (75%), heart failure (25%), and pulmonary embolism (12.5%). All lesions involved the right atrium, and extended to the lung (12.5%), inferior vena cava (25%), and left atrium (12.5%). The level of tumor marker, alpha-fetal protein, was not correlated with the severity of metastasis or disease prognosis. Moreover, the availably estimated doubling time was less than 3 mo. The pathological fi ndings included variablehemorrhage and necrosis. The survival time following surgery also varied from one month to more than 30 mo. CONCLUSION: Hepatoma metastasis to the heart was detected in all eight patients. This study demonstrates that surgery might help the outcome in such cases.
