Mpox disease is caused by a double-stranded DNA virus, genus Orthopoxvirus of the family Poxviridae. The incubation period is usually 6 to 13 days but can range from 5 to 21 days while symptoms and signs may persist for 2 to 5 weeks. Although, the clinical features are usually less severe when compared to the deadly smallpox, the disease can be fatal with case fatality rate between 1% and 10%. In Imo State, Nigeria, there has been a changing epidemiology of the disease in the last 6 years and the frequency and geographic distribution of cases have progressively increased. This study aims to conduct a review of the disease epidemiology between 2017 and 2023 and implications for surveillance in Imo State. Surveillance data from the Surveillance Outbreak Response and Management System (SORMAS) was extracted between January 2017 and December 2023 across the 27 Local Government Areas (LGAs) of Imo State. A line list of 231 suspected cases was downloaded into an excel template and analyzed using SPSS® version 20 software. Analysis was done using descriptive statistics and associations were tested using Fischer’s exact at 0.05 level of significance. Of the 231 suspected cases, 57.1% (132) were males, 42.9% (99) were females and the modal age group was between the ages of 0 - 4 (32.5%). Eight (8) LGAs (districts) accounted for 71% (n = 164) of all the suspected cases. 21.2% (49) were confirmed positive, 27 males (55.1%) and 22 females (44.9%) (p > 0.05). Modal age group was 20 - 24 (22.4%, n = 11), 18% (9) were children under 14 years, p > 0.05. Case fatality rate was 8% (n = 4). There was no significant association between mortality and age group. Five (5) LGAs accounted for about 60% (29) of all confirmed cases. These LGAs contribute only 20% to the total population in the State. Only 5.6% and 4% of suspected and confirmed cases, respectively, had knowledge of contact with an infectious source. The study described the epidemiology of Mpox outbreaks between 2017

Objectives: Atlantoaxial dislocation remains a rare and serious condition with a high preoperative and postoperative morbidity and mortality. Its successful surgical management is still challenging and gratifying for neurosurgeons. Several technics have been described such as wiring, trans articular screwing, C1C2 screwing with plate and screw introduced by Goel et al., and modified by insertion of polyaxially screw and rod many years later by Harms. Unavailability and expensiveness of upper cervical spine instrumentation device led us to C1C2 Wiring resulting in a good outcome. Finally, a quadriplegic patient with a more comfortable financial condition had ordered devices from abroad and benefit for Goel and Harms screwing technique and improved dramatically from ASIA A to ASIA E. Material and methods: This is a retrospective study of patients managed in our department by a same neurosurgeon from January 2019 to April 2024. Results: We defined 6 men and 1 woman with an average age of 33 years. Unrestrained driver in a rollover motor vehicle accident was most common. Only one patient was neurologically intact on admission. Neurovegetative disorders were noticed in one patient. Dislocation was associated to a fracture of the dens in two patients. Three patients have been successfully operated with remarkable outcome, mostly from ASIA A to E. Conclusion: C1C2 dislocation is a serious condition and C1C2 Wiring represents an effective and cheaper technic. Therefore, this technic should deserve consideration above all in low incomes countries when screwing devices are not available. Seatbelt should be demanded for motor vehicle drivers and passengers.

Disaster Risk Governance as Assemblage: The Chilean Framework of the 1985 San Antonio Earthquake

The purpose of this article is to analyze disaster risk governance through assemblage theory, identifying how-during the altered political context of a military regime with a centralized disaster risk management as in the case of Chile in 1985-new actors emerge during the disaster response phase as a de/reterritorialization effect that is influenced by their agencies and relationships, disfiguring the edges of the assemblage. Based on this conceptualization, it is possible to investigate the interactions between the different actors, their power relations, and their reconfigurations in the governance exercise. For this purpose, we reviewed the response phase of the 1985 San Antonio earthquake that affected the central zone of Chile, where strategic functions, institutions, and forms of power are *** describe and visualize the actors during the response phase in the disaster risk governance framework, a map of actors was developed that identifies the existing relationships and their different weights. The central scale proved to be dominant and occupied a political space that was transfigured by its overrepresentation-enforced by allies such as the banking system and business associations-enhancing a neoliberal agenda. The leaps in scale from the central scale to the local scale cancel agency of the last, destabilizing its capacity to deal with the effects of the earthquake and isolating it from the decision-making processes. Consequently,delays in providing aid demonstrate that authoritarian governments do not provide better management in the disaster response phase.

Extended activated sludge model no. 1 (ASM1) for simulating biodegradation process using bacterial technology

Phosphorus is one of the most important nutrients required to support various kinds of biodegradation processes. As this particular nutrient is not included in the activated sludge model no. 1 （ASM1）, this study extended this model in order to determine the fate of phosphorus during the biodegradation processes. When some of the kinetics parameters are modified using observed data from the restoration project of the Xuxi River in Wuxi City, China, from August 25 to 31 in 2009, the extended model shows excellent results. In order to obtain optimum values of coefficients of nitrogen and phosphorus, the mass fraction method was used to ensure that the final results were reasonable and practically relevant. The temporal distribution of the data calculated with the extended ASM1 approximates that of the observed data.

OBJECTIVE:The interleukin(IL)-12 cytokine family is closely related to the development of T helper cells,which are responsible for autoimmune disease enhancement or ***-12 family members are generally heterodimers and share threeα-subunits(p35,p19,and p28)and twoβ-subunits(p40 and EBI3).However,aβ-sheet p40 homodimer has been shown to exist and antagonize IL-12 and IL-23 signaling ***,we assumed the existence of a p40-EBI3 heterodimer in nature and sought to investigate its role in immune ***:The presence of the p40-EBI3 heterodimer was confirmed by ELISA,immunoprecipitation,and western blotting.A p40-EBI3 vector and p40-EBI3-Fc protein were synthesized to confirm the immunological role of this protein in mice with collageninduced arthritis(CIA).The anti-inflammatory effects of p40-EBI3 were analyzed with regard to clinical,histological,and immune cell-regulating features in mice with ***:Clinical arthritis scores and the expression levels of proinflammatory cytokines(e.g.,IL-17,IL-1β,IL-6,and TNF-α)were significantly attenuated in p40-EBI3-overexpressing and p40-EBI3-Fc-treated mice with CIA compared to vehicle-treated mice with *** joint damage and vessel formation-related gene expression were also reduced by p40-EBI3 heterodimer *** vitro,the p40-EBI3-Fc protein significantly suppressed the differentiation of Th17 cells and reciprocally induced CD4+CD25+Foxp3+(regulatory T)cells.p40-EBI3 also inhibited osteoclast formation in a concentrationdependent ***:In this study,p40-EBI3 ameliorated proinflammatory conditions both in vivo and in *** propose that p40-EBI3 is a novel anti-inflammatory cytokine involved in suppressing the immune response through the expansion of Treg cells and suppression of Th17 cells and osteoclastogenesis.

Management of the enteroatmospheric fistula:A case report

BACKGROUND Enteroatmospheric fistula(EAF)is a catastrophic complication that can occur after open *** cause severe body fluid loss,hypercatabolism,and wound complications,leading to adverse clinical *** SUMMARY A 72-year-old female patient underwent ventral hernia *** days after the surgery,she exhibited severe abdominal pain with septic *** laparotomy revealed extensive intestinal adhesions and severe intraperitoneal *** the patient was hemodynamically unstable,a salvage operation rather than definite surgery was needed,and three surgical open drains were inserted into the peritoneal *** EAFs developed,and it was almost impossible to isolate and reduce the fistula output despite the use of vacuum-assisted closure dressings and endoscopic stent ***,we anastomosed two vascular grafts to the openings of each EAF to restore enteric *** inserted vascular grafts showed acceptable patency,and the patient could receive optimal nutritional support with elemental enteral *** underwent EAF resection 76 d after graft *** Control of the enteric effluent are key elements in achieving favorable clinical conditions which should precede definite surgery for EAFs.

Background There are different definitions to identify/classify children with medical complexity(CMC).We aimed to investigate and describe the definitions used to classify CMC in epidemiological *** PubMed,SciELO,LILACS,and EMBASE were searched from 2015 to 2020(last updated September 15th,2020)for original studies that presented the definition used to classify/identify CMC in the scientific research *** applied the Preferred Reporting Items for Systematic Reviews and Meta-Analyses *** the included studies,the following were identified:first author,year of publication,design,population,study period,the definition of CMC used,limitations,and *** Nine hundred and sixty-seven records were identified in the searched databases,and 42 met the inclusion *** the 42 studies included,the four most frequent definitions used in the articles included in this review were classification of CMC into nine diagnostic categories based on the International Classification of Diseases,Ninth Revision(ICD-9)(35.7%,15 articles);update of the previous classification for ICD-10 codes with the inclusion of other conditions in the definition(21.4%,nine articles);definition based on a medical complexity algorithm for classification(16.7%,seven articles);and a risk rating system(7.1%,three articles).Conclusions CMC definitions using diagnostic codes were more ***,several limitations were found in its *** research highlighted the need to improve health information systems to accurately characterize the CMC population and promote the provision of comprehensive care.
