Laparoscopic surgery:A qualified systematic review

AIM: To review current applications of the laparoscopic surgery while highlighting the standard procedures across different ***: A comprehensive search was undertaken using the Pub Med Advanced Search Builder. A total of 321 articles were found in this search. The following criteria had to be met for the publication to be selected: Review article, randomized controlled trials, or metaanalyses discussing the subject of laparoscopic surgery. In addition, publications were hand-searched in the Cochrane database and the high-impact journals. A total of 82 of the findings were included according to matching the inclusion criteria. Overall, 403 full-text articles were reviewed. Of these, 218 were excluded due to not matching the inclusion criteria. RESULTS: A total of 185 relevant articles were identified matching the search criteria for an overview of the current literature on the laparoscopic surgery. Articles covered the period from the first laparoscopic application through its tremendous advancement over the last several years. Overall, the biggest advantage of the procedure has been minimizing trauma to the abdominal wall compared with open surgery. In the case of cholecystectomy, fundoplication, and adrenalectomy, the procedure has become the gold standard without being proven as a superior technique over the open surgery in randomized controlled trials. Faster recovery, reduced hospital stay, and a quicker return to normal activities are the most evident advantages of the laparoscopic surgery. Positive outcomes, efficiency, a lower rate of wound infections, and reduction in the perioperative morbidity of minimally invasive procedures have been shown in most indications. CONCLUSION: Improvements in surgical training and developments in instruments, imaging, and surgical techniques have greatly increased safety and feasibility of the laparoscopic surgical procedures.

A 46-year-old Chinese woman presented with nausea, recurrent vomiting, and abdominal pain. Gastroduodenal endoscopic examination revealed an oval-shaped submucosal tumor at the prepyloric area on the posterior wall of the stomach. A degenerated gastrointestinal stromal tumor was suspected. Distal gastrectomy was performed and a histological diagnosis of heterotopic pancreas （HPs） was confirmed. The patient had an uneventful postoperative course and was discharged 7 d a^er operation. The patient remains healthy and symptom-free in the follow-up of 6 mo. This is a report of a case of gastric outlet obstruction resulting from pancreatic heterotopia in the gastric antrum in an adult woman.

背景与目的2018年,胃肠道(gastrointestinal,GI)癌症占全球癌症病例的26.3%和癌症死亡人数的35.4%。本研究旨在分析全球6种主要胃肠道癌症:食道癌(esophageal cancer,EC)、胃癌(gastric cancer,GC)、肝癌(liver cancer,LC)、胰腺癌(pancreatic cancer,PC)、结肠癌(colon cancer,CC)和直肠癌(rectal cancer,RC)的发病率、死亡率、患病率和致病风险因素。方法我们使用全球癌症观察站(Global Cancer Observatory,GCO)和全球卫生观察站(Global Health Observatory,GHO)数据库,回顾当前的GI癌症发病率、患病率和死亡率,分析GI癌症患病率与国家的人类发展指数(human development indices,HDI)的相关性,确定致病风险因素,估计不同年龄和性别人群的发病率和死亡率的发展趋势。结果2020年,GI癌症年龄标化发病率趋势与死亡率密切相关,LC、EC和GC的发病率在亚洲最高,结直肠癌(colorectal cancer,CRC)和PC的发病率在欧洲较高。发病率和死亡率呈正相关,而死亡发病比(mortalityto-incidence ratio,MIR)与国家HDI水平呈负相关。发展中国家的高MIR可能反映了预防战略和有效治疗的缺乏。GI癌症在欧洲较常见,与HDI和生活方式相关风险因素,如饮酒、吸烟、肥胖、运动不足和高血胆固醇水平等呈正相关,但与高血压和糖尿病呈负相关。在大多数国家/地区,EC的发病率一直在下降,GC的发病率也多呈下降趋势,而CRC的发病率却在上升,特别是在年轻人群中。除年轻男性外,LC和PC的发病率在所有年龄和性别的人群中也都在上升。在大多数国家/地区,EC、GC和CRC的死亡率都在下降,在PC和LC中观察到了年龄特定趋势:在年轻人口中减少,在老年人口中增加。结论在全球范围内,较高的GI癌症负担在很大程度上伴随着所谓的西方生活方式相关因素,这些因素反映了高和非常高的国家人类发展指数水平。在HDI非常高的国家/地区,患者生存时间更长,但随着国家HDI水平的提高,GI癌症患者也在增加。因此,我们建议优化GI癌症预防策略和改进治疗方案,尤其是对合并代谢性疾病的患者。

Significance of multivisceral resections in oncologic surgery: A systematic review of the literature

BACKGROUND Multivisceral resections (MVR) are often necessary to reach clear resections margins but are associated with relevant morbidity and mortality. Factors associated with favorable oncologic outcomes and elevated morbidity rates are not clearly defined. AIM To systematically review the literature on oncologic long-term outcomes and morbidity and mortality in cancer surgery a systematic review of the literature was performed. METHODS PubMed was searched for relevant articles (published from 2000 to 2018). Retrieved abstracts were independently screened for relevance and data were extracted from selected studies by two researchers. RESULTS Included were 37 studies with 3112 patients receiving MVR for colorectal cancer (1095 for colon cancer, 1357 for rectal cancer, and in 660 patients origin was not specified). The most common resected organs were the small intestine, bladder and reproductive organs. Median postoperative morbidity rate was 37.9%(range: 7% to 76.6%) and median postoperative mortality rate was 1.3%(range: 0% to 10%). The median conversion rate for laparoscopic MVR was 7.9%(range: 4.5% to 33%). The median blood loss was lower after laparoscopic MVR compared to the open approach (60 mL vs 638 mL). Lymph-node harvest after laparoscopic MVR was comparable. Report on survival rates was heterogeneous, but the 5-year overall-survival rate ranged from 36.7% to 90%, being worst in recurrent rectalcancer patients with a median 5-year overall survival of 23%. R0 -resection, primary disease setting and no lymph-node or lymphovascular involvement were the strongest predictors for long-term survival. The presence of true malignant adhesions was not exclusively associated with poorer prognosis. Included were 16 studies with 1.600 patients receiving MVR for gastric cancer. The rate of morbidity ranged from 11.8% to 59.8%, and the main postoperative complications were pancreatic fistulas and pancreatitis, anastomotic leakage, cardiopulmonary events and post-opera

To the Editor： A 13-year-old girl had experienced abdominal pain and nausea for about 1 year, along with epigastric discomfort. An endoscopic intervention revealed a mass that protruded into the stomach lumen [Figure 1a]. A biopsy revealed nonnal gastric intlcosa. Computed tomography （CT） indicated a mass of about 2 cm between the pancreatic tail, spleen, and stomach. CT angiography showed a 27 mm aneurysm dilatation in the first 3^rd of the middle splenic artery [Figure lb].
