The importance of the safety of healthy living liver donors is widely recognized during donor hepatectomywhich is associated with blood loss, transfusion, and subsequent post-operative morbidity. Although the low central venous pressure(CVP) technique can still be effective, it may not be advantageous concerning the safety of healthy donors undergoing hepatectomy. Emerging evidence suggests that stroke volume variation(SVV), a simple and useful index for fluid responsiveness and preload status in various clinical situations, can be applied as a guide for fluid management to reduce blood loss during living donor hepatectomy. Synthetic colloid solutions are also associated with serious adverse events such as the use of renal replacement therapy and transfusion in critically ill or septic patients. However, it is uncertain whether the intra-operative use of colloid solution is associated with similarly adverse effects in patients undergoing living donor hepatectomy. In this review article we discuss the recent issues regarding the low CVP technique and the high SVV method, i.e., maintaining 10%-20% of SVV, for fluid management in order to reduce blood loss during living donor hepatectomy. In addition, we briefly discuss the effects of intra-operative colloid or crystalloid administration for surgical rather than septic or critically ill patients.

Background:Red cell distribution width(RDW)and serum albumin concentration are associated with postoperative ***,the usefulness of the RDW/albumin ratio in burn surgery remains ***,we evaluated the association between RDW/albumin ratio and 90-day mortality after burn ***:Between 2013 and 2020,a retrospective review of patients in a burn intensive care unit(ICU)was *** operating characteristic curve,multivariate Cox logistic regression,multivariate logistic regression and Kaplan-Meier analyses were conducted to evaluate the associ-ation between RDW/albumin ratio and 90-day mortality after burn ***,prolonged ICU stay rate(>60 days)and ICU stay were ***:Ninety-day mortality was 22.5%(210/934)in burn *** factors for 90-day mortality were RDW/albumin ratio at postoperative day 1,age,American Society of Anesthesiologists physical status,diabetes mellitus,inhalation injury,total body surface area burned,hypotensive event and red blood cell transfusion *** area under the curve of the RDW/albumin ratio at postoperative day 1 to predict 90-day mortality,after adjusting for age and total body surface area burned,was 0.875(cut-off value,6.8).The 90-day mortality was significantly higher in patients with RDW/albumin ratio>6.8 than in those with RDW/albumin ratio≤6.8(49.2%vs 12.3%,p6.8 than in those with RDW/albumin ratio≤6.8(34.5%vs 26.5%;21[11-38]vs 18[7-32]days).Conclusion:RDW/albumin ratio>6.8 on postoperative day 1 was associated with higher 90-day mortality,higher prolonged ICU stay rate and longer ICU stay after burn surgery.

BACKGROUND Trigeminal neuralgia(TN) is a severe type of neuropathic pain which is often inadequately managed using conventional therapies. In this report, we present the first case of TN treated with gasserian ganglion nerve coblation(NC).CASE SUMMARY A 58-year-old man presented with right facial pain, mostly localized in the right zygomatic zone, alveolar region, and jaws. Similar to acupuncture and shock pain, the pain lasted about five seconds after each attack before resolving unaided. A diagnosis of TN was made, after which treatment with acupuncture therapy and oral carbamazepine was given. However, the pain was not satisfactorily controlled. Subsequently, gasserian ganglion NC of the right trigeminal nerve guided by computed tomography(CT) was performed on the patient. Following this procedure, the right zygomatic, alveolar, submandibular,and cheek pain disappeared completely. The right zygomatic and alveolar areas experienced mild numbness(level II). At 1-, 2-, 3-, and 6-mo follow-ups after surgery, the patient was painless and the numbness score was level *** CT-guided gasserian ganglion(NC) is an effective treatment for TN and is associated with less or no postoperative numbness or hypoesthesia in comparison with current standard-of-care approaches.

Ovarian endometrioma（OE）, also known as “chocolate cysts,” is a cystic mass that develops in the ovaries due to endometriosis and is a common gynecological condition characterized by the growth of endometrial tissue outside the uterus, leading to symptoms such as dysmenorrhea, pelvic pain, and infertility. However, the precise molecular and cellular mechanisms driving this pathophysiology remain largely unknown, posing challenges for diagnosis and treatment. Here, we employed integrated single-cell transcriptomic profiling of over 52,000 individual cells from endometrial tissues of OE patients and healthy donors and identified twelve major cell populations. We identified notable alterations in cell type-specific proportions and molecular signatures associated with OE. Notably, the activation of IGFBP5+macrophages with pro-inflammatory properties, NK cell exhaustion, and aberrant proliferation of IQCG+and KLF2+epithelium are key features and may be the potential mechanisms underlying the pathogenesis of OE. Collectively, our data contribute to a better understanding of OE at the single cell level and may pave the way for the development of novel therapeutic strategies.

Standardized Treatment of Chinese medicine Decoction for Cancer pain Patients with Opioid-Induced Constipation:A Multi-center Prospective Randomized Controlled Study

Objective： To observe the efficacy and the influence on quality of life （QOL） of syndrome differentiation treatment with Chinese medicine （CM） for opioid-induced constipation as well as the safety and influence on analgesic effect of opioids. Methods： Totally 406 cases enrolled from 53 collaborating medical centers were randomly assigned to a CM group and a control group. The CM group were treated with CM decoction based on syndrome differentiation, and the control group were treated with Phenolphthalein Tablet. Both groups were treated for 14 days. Cleveland constipation score （CCS）, numedcal rating scale （NRS） of pain and Chinese version of European Organisation for Research and Treatment of Cancer, Quality of Life Questionnaire-C30 V3.0 （EORTC QLQ-C30 V3.0） were used to evaluate the efficacy, pain controlled and QOL status. Results： The comparisons of CCS score reduction and QOL between the two groups after treatment suggested that the improvements of constipation and QOL in the CM group were better than that in the control group （P〈0.05）. The total efficiency of the CM group was better than the control group （93.5% vs. 86.4%, P〈0.05）. There was no significant difference in NRS scores between before and after treatment in both groups. There was no serious drug-related adverse event during the course of study. Conclusion： CM decoction could effectively treat opioid-induced constipation and improve patients＇ QOL at the same time. It is safe and doesnt affect the analgesic effect of opioids when treating constipation.
