Mortality Rates of Ventricular Septal Defect for Children in Kazakhstan: Spatio-Temporal Epidemiological Appraisal

Objective: The aim is to study the trends in ventricular septal defect (VSD) mortality in children in ***: The retrospective study was done for the period 2011–2020. Descriptive and analytical methods of epidemiologywere applied. The universally acknowledged methodology used in sanitary statistics is used to calculatethe extensive, crude, and age-specific mortality rates. Results: Kazakhstan is thought to be seeing an increase inmortality from VSDs in children. As a result, this study for the years 2011 to 2020 was conducted to retrospectivelyassess data from the central registration of the Bureau of National Statistics that was available throughoutthe nation. Age-standardized mortality data were obtained and compared between age categories. It was shownthat 507 children died from this condition throughout the time period under study. The average annual standardizedmortality rate was 1.88 per 100,000 population and tended to decrease over time. The peak of mortality wasnoted at the age of up to 1 year, namely the neonatal period. Cartogram mortality rates were calculated usingstandardized indicators. Additionally, age-sex variations were taken into account when performing all ***: In recent years, the death rate from VSD has declined from 1.5 to 0.6 per 100,000 people, withthe trend remaining constant (T = 1.4%, R^(2) = 0.5825). The analysis of mortality trends related to VSD is crucial inboth theoretical and practical aspects, as it enables early detection and treatment of VSDs. The findings of thisstudy will be valuable to public health authorities in developing a strategy to treat VSDs effectively.

Surgical Versus Percutaneous Stenting Treatment of Isolated Aortic Coarctation: Long-Term Follow-Up

Background:In recent decades,aortic stenting has become a promising alternative to surgery for both native aortic coarctation and re-stenosis in children and ***,comparative long-term outcomes have poorly been ***:We included 212 patients with previous aortic repair(19±8.7 years)divided into 3 groups:139 with single-time surgical repair(CoA-S group);18 with single-time percutaneous stenting(CoA-PS group);and 55 hybrid patients with multiple aortic procedures because of re-coarctation occurrence(CoA-H group).All patients underwent 24-hour ambulatory blood pressure monitoring and trans-thoracic ***:After a median follow-up of 17 years after aortic repair,antihypertensive therapy was recorded in a significantly higher proportion of patients(83%)in CoA-PS group compared to 65%and 46%of CoA-H and CoA-S groups,respectively(p=0.002).Pulse pressure values were higher in CoA-PS patients compared to the others(p<0.001).Echocardiogram showed significant residual aortic gradient in 50%of CoA-PS and 73%of CoA-H patients compared to 33%of CoA-S patients(p<0.0001).Indeed,stenting was associated to higher incidence of re-coartaction(p<0.0001).At multivariate regression Cox analysis adjusted for age at repair and need for antihypertensive therapy,percutaneous stenting was an independent predictor of echocardiographic evidence of re-coarctation(p≤0.001).Conclusion:Aortic coarctation stenting was independently associated with re-coartaction occurrence during long-term follow up when compared to surgical ***,patients with aortic stenting had lower blood pressure control at 24-hour ambulatory blood pressure monitoring and higher need for antihypertensive therapy.

Atrial tachyarrhythmia in adult congenital heart disease

The adult congenital heart disease (ACHD) population continues to grow and most cardiologists, emergency room physicians and family doctors will intermittently come into contact with these patients. Oftentimes this may be in the setting of a presentation with atrial tachyarrhythmia; one of the commonest late complications of ACHD and problem with potentially serious implications. Providing appropriate initial care and ongoing management of atrial tachyarrhythmia in ACHD patients requires a degree of specialist knowledge and an awareness of certain key issues. In ACHD, atrial tachyarrhythmia is usually related to the abnormal anatomy of the underlying heart defect and often occurs as a result of surgical scar or a consequence of residual hemodynamic or electrical disturbances. Arrhythmias significantly increase mortality and morbidity in ACHD and are the most frequent reason for ACHD hospitalization. Intra-atrial reentrant tachycardia and atrial fibrillation are the most prevalent type of arrhythmia in this patient group. In hemodynamically unstable patients, urgent cardioversion is required. Acute management of the stable patient includes anticoagulation, rate control, and electrical or pharmacological cardioversion. In ACHD, rhythm control is the preferred management strategy and can often be achieved. However, in the long-term, medication side-effects can prove problematic. Electrophysiology studies and catheter ablation are important treatments modalities and in certain cases, surgical or percutaneous treatment of the underlying cardiac defect has a role. ACHD patients, especially those with complex CHD, are at increased risk of thromboembolic events and anticoagulation is usually required. Female ACHD patients of child bearing age may wish to pursue pregnancies. The risk of atrial arrhythmias is increased during pregnancy and management of atrial tachyarrhythmia during pregnancy needs specific consideration.

Pulmonary sequestration is a rare congenital cardiovascular malformation.[1,2]Open surgical excision is the usual treatment,although thorascopic intervention and transcatheter embolization have been ***,only a limited number of articles involving percutaneous closure the rare pulmonary malformations have been published in the literature.[3]What’s more,previous studies involving percutaneous pulmonary sequestration closure with repeated respiratory symptoms at the very young age suggested that the optimal therapeutic strategy has yet to be determined.[4]We hereby describe a particular case of percutaneous closure of a large intralobar pulmonary sequestration at elderly age.

Background:This study aims to explore the efficacy of selective unifocalization(UF)for major aortopulmonary collateral arteries(MAPCAs)unifocalization in children with pulmonary atresia with ventricular septal defect(PA/VSD).Methods:A retrospective analysis of 13 patients with PA/VSD/MAPCAs who underwent surgery from June 2017 to December ***,age,preoperative cardiovascular angiography test results and McGoon ratio were *** properties of the collateral arteries were evaluated by angiography,and selective UF for the“dendritic”MAPCAs and ligation of MAPCAs demonstrating distortion and ***:A total of 13 severe patients underwent one-stage repair,of which 1 case underwent ventricular septal fenestration and died after 2 weeks of ECMO *** median age was 37 months,and a median weight was 14.0 kg.A right ventricular to aortic systolic pressure ratio(pRV/pAo)of no more than 0.5 was achieved in 12 living ***:Selective unifocalization based on MAPCAs morphology can achieved a good outcome at the early *** surgical concept might be provided a novel insight into treatment for some of the subgroups presenting with this complex form of PA/VSD/MAPCAs.

Combined Echocardiography and Lung Ultrasound for Extubation Outcome Prediction in Children after cardiac surgery

Background:Children are at risk of extubation failure after congenital heart disease *** cases should be identified to avoid possible adverse consequences of failed *** study aimed to identify ultrasound predictors of successful extubation in children who underwent cardiac ***:Children aged 3 months to 6 years who underwent cardiac surgery(if they were intubated for>6 h and underwent a spontaneous breathing trial)were included in this ***:We included 83 children who underwent surgery for congenital heart *** echocardiography and lung ultrasound were performed immediately before spontaneous breathing *** spontaneous breathing trial completion,respiratory parameters,including arterial blood gas analysis and frequency-to-tidal volume ratio,were similarly *** outcome assessment,all children were followed up for 48 h after *** successfully extubated 57 children(68.7%).These children were significantly older and weighed more but had shorter aortic cross-clamp and cardiopulmonary bypass *** who could not be weaned or extubated had prolonged total mechanical ventilation and pediatric intensive care unit *** the multivariate regression analysis,a lung ultrasound score≥12 and ejection fraction≥40%immediately before spontaneous breathing trials were the only independent predictors of successful *** combined,the lung ultrasound score and an ejection fraction≥40%showed a better diagnostic performance than every other isolated variable(lung ultrasound,N-terminal-pro-B-type natriuretic peptide,and frequency-to-tidal volume ratio).Conclusions:The combination of lung ultrasound and transthoracic echocardiography immediately before the spontaneous breathing trial effectively predicts extubation outcomes in children after cardiac surgery.

Coarctation of the aorta(CoA)is an uncommon congenital cardiovascular malformation,which is rare associated with several other cardiac and vascular anomalies,such as bicuspid aortic valve,ventricular septal defect(VSD),patent ductus arteriosus and aortic arch hypoplasia.[1,2]Open surgical repair is the usual treatment for neonatal CoA,whereas balloon angioplasty and stent placement are the treatment of choice for older children and adults.[3]

Should we consider patent foramen ovale and secundum atrial septal defect as different steps of a single anatomo-clinical continuum？

1 Introduction Isolated atrial septal defects （ASD） represent 7% of all cardiac anomalies and can present at any age. Adolescents and adults with simple congenital heart disease such as isolated atrial septal defects are more likely to reach adult age without being diagnosed. Secundum ASD （sASD） is by far the most common type, occurring in 1/1500 live births, with 65% to 75% involving females, On the other hand, patent foramen ovale （PFO） represents an endemic variant in the normal population with a prevalence of 25%-27%. These two entities appear so different that is difficult to fred a conjunction ring： nevertheless we use the same philosophy for the treatment, Indeed, device-based closure has been proved to be effective in both settings.
