Background: Incorporation of HPV tests into cervical cancer screening programs may be advantageous over conventional cytology, especially in developing nations, where the largest burden of cervical cancer is observed. Objectives: To conduct an evaluation of commercially available molecular HPV tests in Brazilian women. Study design: Two groups were recruited: group A was composed of 511 women referred to the clinics because of a previous abnormal Pap test while group B consisted of 2464 subjects under routine screening. Cervical samples were collected using SurePath liquid cytology (LBC) device, and split into aliquots which were submitted to molecular testing by Hybrid Capture and cobas HPV. Colposcopy and biopsies were made according to the standard guidelines, directed by cytological diagnosis. Results: Prevalence of HSIL was 5.97% and 0.7% in Group A and B respectively. High-Risk HPV DNA was found in about 9% of group B women, while in group A this frequency was 24%. Having CIN3+ as the study end-point, the negative predictive values for molecular methods were above 99.8%. All “in-situ” and invasive cervical carcinomas were detected by both HPV nucleic acid assays. Conclusion: Use of HPV DNA testing was feasible and highly sensitive in cancer screening settings of Brazil.

目的:研究自制改造餐具的使用对脑卒中患者单侧忽略致进食残留的影响。方法:首次发病的脑卒中并发单侧忽略患者8例,病程2～6周,治疗的第1周选取奇数3d,使用一定规格的固定轻质钢盘进食;第2周的奇数3d,使用同规格旋转音乐餐盘食用同样的食物,均固定在中餐时段。统计每餐后残余食物重量(g)。结果:第1周残余食物重量明显重于第2周(67.14±12.46g与18.74±11.65g,P<0.05)。结论:自制改造餐具的使用对脑卒中患者单侧忽略所致进食残留的改善有较好临床疗效。

自2019年12月新型冠状病毒肺炎(Coronavirus Disease 2019, COVID-19)暴发以来,已有大量相关临床研究完成注册或已启动。然而,不同研究间临床结局指标存在显著的异质性,同时一些不关键的指标会造成研究资源的浪费。本研究旨在构建COVID-19临床试验核心结局指标集(core outcome set, COS),为相关临床研究方案设计、开展和证据转化提供参考。本研究的实施参照《有效性试验核心结局指标(COMET)手册》(1.0版),研究组成员包括呼吸系统和危重医学、中医学、循证医学、临床药理学、统计学专家以及医学杂志编辑等。通过检索临床试验注册网站(***和***)中COVID-19相关临床研究方案,提取方案中的结局指标,形成指标池。78个COVID-19临床试验方案纳入研究,收集结局指标259个,合并规范后确定132个结局指标,涉及7个指标域。经遴选,形成了包括58个结局指标在内的初始指标清单。经过两轮德尔菲调查和一轮共识会议,最终确立针对COVID-19不同临床分型的核心指标集(COS-COVID)。COS-COVID包括1个轻型指标(病毒核酸检测转阴时间)、4个普通型指标(住院时间、复合事件发生率、临床症状积分和病毒核酸检测转阴时间)、5个重型指标(复合事件发生率、住院时间、PaO2/FiO2、机械通气时间和病毒核酸检测转阴时间)、1个危重型指标(全因死亡率)及1个康复期指标(肺功能)。COS-COVID作为目前临床评价COVID-19干预效果最具临床价值及操作性的指标集合,对证据评估和决策具有重要意义。随着对疾病认知的不断深入和COS-COVID应用的反馈,课题组将对本核心指标集进行进一步完善并更新。

Most cancers occur in older people and the burden in this age group is *** the past two decades the evidence on how best to treat this population has increased ***,implementation of new best practices has been slow and needs involvement of *** perspective paper explains why older people with cancer have different needs than the wider *** overview is given of the recommended approach for older people with cancer and its benefits on clinical outcomes and *** older patients,the geriatric assessment(GA)is the gold standard to measure level of fitness and to determine treatment *** GA,with multiple domains of physical health,functional status,psychological health and socio-environmental factors,prevents initiation of inappropriate oncologic treatment and recommends geriatric interventions to optimize the patient’s general health and thus resilience for receiving *** studies have proven its benefits such as reduced toxicity,better quality of life,better patient-centred communication and lower healthcare *** GA might require investment of time and resources,this is relatively small compared to the improved outcomes,possible cost-savings and compared to the large cost of oncologic treatments as a whole.

The global trend toward aging populations has resulted in an increase in the occurrence of Alzheimer's disease(AD)and associated socioeconomic *** metabolism of amyloid-β(Aβ)has been proposed as a significant pathomechanism in AD,supported by results of recent clinical trials using anti-Aβ***,the cognitive benefits of the current treatments are *** etiology of AD is multifactorial,encompassing Aβand tau accumulation,neuroinflammation,demyelination,vascular dysfunction,and comorbidities,which collectively lead to widespread neurodegeneration in the brain and cognitive ***,solely removing Aβfrom the brain may be insufficient to combat neurodegeneration and preserve *** attain effective treatment for AD,it is necessary to(1)conduct extensive research on various mechanisms that cause neurodegeneration,including advances in neuroimaging techniques for earlier detection and a more precise characterization of molecular events at scales ranging from cellular to the full system level;(2)identify neuroprotective intervention targets against different neurodegeneration mechanisms;and(3)discover novel and optimal combinations of neuroprotective intervention strategies to maintain cognitive function in AD *** Alzheimer's Disease Neuroprotection Research Initiative's objective is to facilitate coordinated,multidisciplinary efforts to develop systemic neuroprotective strategies to combat *** aim is to achieve mitigation of the full spectrum of pathological processes underlying AD,with the goal of halting or even reversing cognitive decline.
