Prednisolone therapy accelerates recovery of severe drug-induced liver injury: A prospective, randomized controlled study

Background and aims:Drug-induced liver injury(DILI)is one of the most serious adverse drug reactions and its incidence has been increasing *** evidence suggests that immune activation and systemic inflammatory responses are very important in the progression of *** are often used in DILI,but their clinical usefulness remains *** therefore conducted a prospective,randomized controlled study to investigate whether corticosteroid therapy can accelerate recovery and reduce mortality in severe DILI(SDILI).Methods:SDILI patients with total bilirubin?171μmol/L who presented to the Fifth Medical center of chinese pla General Hospital,Beijing from 2016 to 2019 were randomly allocated to prednisolone and control *** endpoints were resolution of SDILI,defined as a decrease in total bilirubin of at least 35μmol/L to<171μmol/L,and overall survival at 6 *** in the prednisolone group received prednisolone 60 mg/day therapy for the first 7 *** with a decrease in total bilirubin of more than 35μmol/L on day 8 continued on tapering doses of prednisolone;otherwise,prednisolone was ***:On day 8,50.75%(34/67)and 26.47%(18/68)of the participants in the prednisolone and control groups,respectively,achieved the primary endpoint(p¼0.002).However,there was no significant difference in overall survival at 6 months:95.52%(64/67)vs.91.18%(62/68)in the prednisolone and control groups,respectively(p¼0.3).All deaths in both groups occurred in patients who failed to achieve SDILI resolution on day ***:Prednisolone therapy may accelerate the recovery of SDILI.

The unconditional security of quantum key distribution(QKD) can be guaranteed by the nature of quantum *** with the traditional two-dimensional BB84 QKD protocol, high-dimensional quantum key distribution(HDQKD) can be applied to generate much more secret ***, practical imperfections in realistic systems can be exploited by the third party to eavesdrop the secret *** practical beam splitter has a correlation with wavelength,where different wavelengths have different coupling *** this property, we propose a wavelength-dependent attack towards time-bin high-dimensional QKD *** is more, we demonstrate that this attacking protocol can be applied to arbitrary d-dimensional QKD system, and higher-dimensional QKD system is more vulnerable to this attacking strategy.

Reduction in nerve root compression by the nucleus pulposus after Feng's Spinal Manipulation

Ninety-four patients with lumbar intervertebral disc herniation were enrolled in this study. Of these, 48 were treated with Feng＇s Spinal Manipulation, hot fomentation, and bed rest （treatment group）. The remaining 46 patients were treated with hot fomentation and bed rest only （control group）. After 3 weeks of treatment, clinical parameters including the angle of straight-leg raising, visual analogue scale pain score, and Japanese Orthopaedic Association score for low back pain were improved. The treatment group had significantly better improvement in scores than the control group. Magnetic resonance myelography three-dimensional reconstruction imaging of the vertebral canal demonstrated that filling of the compressed nerve root sleeve with cerebrospinal fluid increased significantly in the treatment group. The diameter of the nerve root sleeve was significantly larger in the treatment group than in the control group. However, the sagittal diameter index of the herniated nucleus pulposus and the angle between the nerve root sleeve and the thecal sac did not change significantly in either the treatment or control groups. The effectiveness of Feng＇s Spinal Manipulation for the treatment of symptoms associated with lumbar intervertebral disc herniation may be attributable to the relief of nerve root compression, without affecting the herniated nucleus pulposus or changing the morphology or position of the nerve root.

Enterovirus 71 infections are common and frequently asymptomatic or benign but can manifest the critical symptom of severe inflammation of the central nervous system with subsequent pulmonary edema and cardiorespiratory distress. A hallmark of EV71 infection is marked elevation of neutrophil level, yet the exact contribution of neutrophils to severe/fatal immunopathology remains *** this study of human EV71 infection cases and an EV71 infection mouse model, we demonstrate functional contribution of the host innate immune system to the life-threatening state of this infectious disease. Specifically, humans and mice with active EV71 infection showed significantly increased neutrophils in central nervous system(cerebral spinal fluid of humans and brain of mice). Depletion of neutrophils from the EV71-infected mice, both by antibody inhibition and gene knockout, led to a marked enhancement of survival;in contrast, depletion of macrophages had no affect on survival, even though elevated macrophage count in brain accompanied the EV71 infection. Chemokine [C-Cmotif] ligand(CCL)-3 and CCL-2 were up-regulated in both patients and mice with active EV71 infection, but only CCL-3 contributed to the accumulation and activation of neutrophils in the disease focus and to the survival rate of the mice. The mechanism underlying the immunopathogenic role of CCL-3 in mice may be mediated by CCR1/3 in humans and CCR5 in mice, according to the accompanying selective elevations observed. Molecular studies showed that the CCL-3 expression was regulated at the transcriptional level at least partly through the PI3 K and NF-k B pathway. Deep sequencing of neutrophils from both human and mouse CNS tissues identified 5 miRNAs with EV71 infection-related differential expression that target CCL-3 and may regulate it at the post-transcriptional level. Diabetic wounds significantly affect patient quality of *** are a promising treatment to accelerate wound healing owing to their high drug-loading capacity and efficient drug delivery;however,few studies to date have comprehensively reviewed microneedles for diabetic wound *** up-to-date review summarizes the research progress in microneedles for diabetic wound healing,including manufacturing materials and techniques,structures,designs,release mechanisms,delivery substances,and their specific *** study showed that most microneedles designed for diabetic wounds are made of synthetic polymers and/or natural materials using polydimethylsiloxane *** geometric structure and design directly influence penetration ability and drug delivery *** can deliver antibiotics,hypoglycemic agents,traditional chinese medicines,metal ions,growth factors,exosomes,stem cells,and microorganisms,thus promoting diabetic wound healing through diverse mechanisms,such as antibacterial,anti-inflammatory,antioxidant,hypoglycemic,and angiogenic activities,at different stages of the healing *** conclusion,microneedles are promising drug delivery systems for the treatment of diabetic wounds.
