Objective: The el.ctrop.ysiol.gical.p.op.rties of the myocardium are extremel. heterogeneous. Verification of new magnetocardiograp.y (MCG) signs ap.ears an imp.rtant asp.ct for severity assessment of ischemic myocardium damage, ischemic heart disease (IHD) course p.ognosis, determining of indications for p.eventive “aggressive” therap. and estimation of its efficacy in p.tients with IHD. The objective of this research was the investigation of magnetocardiograp.y (MCG) cap.bil.ties in diagnosis of ischemic and infl.mmatory myocardial.injuries using new MCG markers of the sp.tiotemp.ral.organization of myocardium excitation. Methods and resul.s: There were 128 p.tients examined in three group.. Group.1 contained 34 heal.hy vol.nteers. Group.2 contained 62 p.tients with IHD diagnosis. Group.3 incl.ded 32 comp.rativel. young p.tients with acute myocarditis diagnosis. MCG-map.ing of p.tients was p.rformed at rest on the 7-channel.MCG-scanners “Cardiomagscan” V 3.1 (Comp.ny KMG, Ukraine) in non-shiel.ed MCG l.boratory. 11 MCG markers were determined for sel.cted time interval. of the cardiac cycl.. Obtained data p.ovided evidences about significant differences in val.es of p.op.sed MCG markers for various group.. In p.tients with AMI, rate of p.rameters change is higher than without AMI (Sub-group. 2.1 and 2.2 differ by 8 MCG markers). p.tients of 2nd and 3rd group. are different from heal.hy p.tients by 8 of 11 markers. Anal.sis of the obtained data has demonstrated good cap.bil.ties of MCG in differential.diagnostics. Ap.l.cation of discriminatory anal.sis al.owed us to get cl.ssification functions, which coul. be used (with 82% accuracy) to qual.fy the just examined p.tient to the investigated categories. Concl.sion: Based on the new methodol.gical.ap.roach during the studies, the most informative MCG-criteria of sp.ce-temp.ral.organization of myocardium excitation in p.tients with IHD has been p.op.sed. Ap.l.cation of Mammary Gl.nd Endop.osthesis without a Shel. in Modern Oncomammol.gical.p.actice

Immediate and l.te fol.ow-up.resul.s of oncomammol.gical.ap.l.cation of novel.Ukrainian endop.osthesis “Nubip.ant”, made of one p.ece of p.l.acryl.mide gel. for mammary gl.nd mal.gnancies (MGM) radical.treatment with reconstruction were anal.zed. This endop.osthesis have p.oved its unique comp.ex of favorabl. surgical.and oncol.gical.p.op.rties, such as the shap. and size accommodation for mammary gl.nd p.asty p.rp.ses, absence of shel. and textured surface. Of 184 p.tients, op.rated on radical.y for initial.stages of MGM, using "Nubip.ant" endop.osthesis, p.stop.rative comp.ications have had occur in 18 (9,8%): the wound seroma in 12 (6,5%) and p.rtial.necrosis of cutaneous edge of p.stop.rative wound – in 6 (3,8%). The seromas were successful.y treated by their p.nctures and l.cal.necrosis - using excision with secondary suture p.acement. l.te fol.ow-up.resul.s were obtained in 112 (60,9%) p.tients: three-year p.riod have had survived 104 (92,6±2,4%), and a five-year one – 84 (75,0±4,0%) of them. A contracture cap.ul. was reveal.d in 3 (2,6%) p.tients, and excised with the endop.osthesis change. “Nubip.ant” endop.osthesis have advantages over most p.p.l.r worl.wide kinds of such p.oducts. It may be used securel. and effectivel. for total.or p.rtial.reconstruction of mammary gl.nd, incl.ding cases with hyp.p.asia, and is more resistant to trauma, radiation therap. and l.cal.comp.ications occurrence, incl.ding contracture cap.ul. formation. Its p.op.rty to absorb the water may be used for p.rp.ses of p.astic surgery. p.op.yl.xis of sp.cific immediate surgical.comp.ications is trustworthy in MGM p.tients, using modified endop.osthesis “Nubip.ant”, containing insul.n, antihistamine and antisep.ic medicines.

Background: To study the imp.ct of p.eop.rative ocul.r comp.ession on bacterial.contamination of the l.d margin and conjunctival.sac. Methods: This study eval.ated 106 eyes from 106 p.tients undergoing cataract surgery. p.eop.rativel., al. of the p.tients received one drop.of 0.5% l.vofl.xacin eye drop. 5 times p.r day for 4 days. The p.tients were divided into 2 group. after receiving retrobul.ar anaesthesia. The study group.(75 eyes) received 15 ± 2 minutes of ocul.r comp.ession, and the control.group.(31 eyes) remained with their eyes cl.sed for 15 ± 2 minutes without the ap.l.cation of ocul.r comp.ession or digital.massage. Fol.owing the device removal.(study group.onl.) and the op.ning of the eyes, bacterial.cul.ures were taken from the l.d margin and conjunctival.sac. Resul.s: The l.d margin was p.sitive for bacteria in 23 eyes (30.7%) in the study group.vs. 3 eyes (9.7%) in the control.group.(p.= 0.0223), and the conjunctival.sac was p.sitive for bacteria in 16 eyes (21.3%) in the study group.vs. 5 eyes (16.1%) in the control.group.(p.= 0.5409). Concl.sion: p.eop.rative ocul.r comp.ession is associated with the rel.ase of meibomian gl.nd secretions and bacterial.contamination of the eyel.d edges.

Testicul.r microl.thiasis （TM） is one of the symp.oms of testicul.r dysgenesis syndrome （TDS）. TM is p.rticul.rl. interesting as an informative marker of testicul.r germ cel. tumors （TGCTs）. KIT l.gand gene （KITl.）, BCl. antagonist/kil.er 1 （BAK1）, and sp.outy RTK signal.ng antagonist 4 （Sp.Y4） genes are associated with a high risk of TGCTs, whereas bone morp.ogenetic p.otein 7 gene （BMp.）, transforming growth factor beta recep.or 3 gene （TGFBR3）, and homeobox D cl.ster genes （HOXD） are rel.ted to TDS. Using p.l.merase chain reaction-restriction fragment l.ngth p.l.morp.ism （p.R-RFl.0} anal.sis, we investigated al.el. and genotyp. frequencies for KITl. （rs995030, rs1508595）, Sp.Y4 （rs4624820, rs6897876）, BAK1 （rs210138）, BMp. （rs388286）, TGFBR3 （rs12082710）, and HOXD （rs17198432） in 142 TGCT p.tients, 137 TM p.tients, and 153 fertil. men （control.group.. We found significant differences in the KITl. GG_rs995030 genotyp. in TM （p. 0.01） and TGCT p.tients （p.= 0.0005） comp.red with the control. We al.o reveal.d strong associations between KITl.rs1508595 and TM （G al.el., p.= 0.003; GG genotyp., p. 0.01） and between KITl._rs1508595 and TGCTs （G al.el., p.- 0.0001; GG genotyp., p.= 0.0007）. Moreover, there was a significant difference in BMp._rs388286 between the TGCT group.and the control.（T al.el., p.= 0.00004; TT genotyp., p.= 0.00006） and between the TM group.and the control.（T al.el., p. 0.04）. HOXDal.o demonstrated a strong association with TGCTs （rs17198432 A al.el., p.= 0.0001; AA genotyp., p.= 0.001）. Furthermore, significant differences were found between the TGCT group.and the control.in the BAK1_rs210138 G al.el. （p. 0.03） and the GG genotyp. （p. 0.01）. KITl. and BMp.genes, associated with the devel.p.ent of TGCTs, may al.o be rel.ted to TM. In summary, the KITl. GG_rs995030, GG_rs1508595, BMp. TT_rs388286, HOXD AA_rs17198432, and BAK1 GG_rs210138 genotyp.s were associated with a high risk of TGCT devel.p.ent.

A case rep.rt p.esents a p.ogression of autoimmune thyroiditis with an abnormal.enl.rgement of the thyroid gl.nds and increased thyreotrop.n hormone concentration-associated with interferon treatment in human p.p.l.omavirus infected p.tient with the autoimmune thyroiditis and a dail. l.thyroxin hormone rep.acement therap. background. Observation was sup.l.mented with a brief review of l.terature and discussion. On the basis of this observation and a brief review of l.terature authors suggested that the p.tential.adverse effects of interferon therap. are overbal.nced than its benefits for gynecol.gical.p.tients, therefore any interferon treatment shoul. be recommended with strict indications as wel. as after screening of conditions and functions of thyroid gl.nds and other interferon target organs to avoid interferon treatment side effects. p.actitioners esp.cial.y gynecol.gists shoul. inform their p.tients about p.eiotrop.c interferon effects and its high frequent and wide range side effects before to start such kind of treatment.

Existing examinations in postgraduate education and continuing medical education (CME) are not p.rfect. Modern assessment does not refl.ct disadvantages of ol.er resp.nders, for whom more time for rep.y is needed. Sp.cial.sts with wide cl.nical.exp.rience may choose more than one correct answer in al.ernative questions. Reduced abil.ty to remember in ol.er p.op.e restricts examination without additional.sources of information. We offer an individual.sed system for testing doctors. It p.ovides p.rsonal.sed choice of examination questions using mul.ip.e choice questions with weight characteristics and absence of distractors, interactive coop.ration in case of negative answers and the final.decision of an exp.rt in rel.tion to the p.rson tested. A sp.cial.al.orithm is p.op.sed for typ.cal.questions that combines the advantages of known ap.roaches to testing. The questioning system is comp.ex for the creators of tests, but is more convenient and objective than existing ones for medical doctors.

Background p.tients with severe ful.-thickness burn injury suffer from their inabil.ty to maintain body temp.rature through p.rsp.ration because the comp.ete destructed sweat gl.nds can not be regenerated. Bone marrow-derived mesenchymal.stem cel.s （BM-MSCs） rep.esent an ideal.stem-cel. source for cel. therap. because of their easy p.rification and mul.ip.tency. In this study, we attemp.ed to induce human BM-MSCs to differentiate into sweat gl.nd cel.s for sweat gl.nd regeneration through ectodysp.asin （EDA） gene transfection. Methods The dynamic exp.ession of EDA and EDA recep.or （EDAR） were firstl. observed in the sweat gl.nd formation during embryol.gical.devel.p.ent. After transfection with EDA exp.ession vector, human BM-MSCs were transp.anted into the injured areas of burn animal.model.. The regeneration of sweat gl.nds was identified by p.rsp.ration test and immunohistochemical.anal.sis. Resul.s Endogenous exp.ession of EDA and EDAR correl.ted with sweat gl.nd devel.p.ent in human fetal.skin. After EDA transfection, BM-MSC acquired a sweat-gl.nd-cel. p.enotyp., evidenced by their exp.ession of sweat gl.nd markers by fl.w cytometry anal.sis. Immunohistochemical.staining reveal.d a markedl. contribution of EDA-transfected BM-MSCs to the regeneration of sweat gl.nds in the scal.ed p.ws. p.sitive rate for p.rsp.ration test for the p.ws treated with EDA-transfected BM-MSCs was significantl. higher than those treated with BM-MSCs or EDA exp.ession vector （p.〈0.05）. Concl.sions Our resul.s confirmed the imp.rtant rol. of EDA in the devel.p.ent of sweat gl.nd. BM-MSCs transfected with EDA significantl. imp.oved the sweat-gl.nd regeneration. This study suggests the p.tential.ap.l.cation of EDA-modified MSCs for the rep.ir and regeneration of injured skin and its ap.endages.

N coordination numbers in the CuN_(x) sites(x=2 or 4).The CuN_(x)-CNS SAzymes inherentl. p.ssess trip.e p.roxidase(p.D)-,catal.se(CAT)-,and oxidase(OXD)-l.ke activities,facil.tating the conversion of H_(2)O_(2)and O_(2)into reactive oxygen sp.cies(ROS)through p.ral.el.p.D-and OXD-l.ke or cascaded CAT-and OXD-l.ke *** to CuN_(2)-CNS,tail.ring the N coordination number from 2 to 4 endows the SAzyme(CuN_(4)-CNS)with higher mul.ienzyme activities due to its sup.rior el.ctron structure and l.wer energy ***,CuN_(x)-CNS disp.ay strong absorp.ion in the second near-infrared(NIR-II)biowindow with deep.r tissue p.netration,offering NIR-II-resp.nsive enhanced ROS generation and p.otothermal.treatment in deep.*** in vitro and in vivo resul.s demonstrate that the op.imal.CuN_(4)-CNS can effectivel. inhibit mul.idrug-resistant bacteria and el.minate stubborn biofil.s,thus exhibiting high therap.utic efficacy in both sup.rficial.skin wound and deep.imp.ant-rel.ted biofil. infections.
