Cost-analysis of inpatient and outpatient parenteral antimicrobial therapy in orthopaedics: A systematic literature review

BACKGROUND Increasing numbers of total joint arthroplasties and consecutive revision surgery are associated with the risk of periprosthetic joint infections (PPJI). Treatment of PPJI is complex and associated with immense socio-economic burden. One treatment aspect is parenteral antiinfective therapy, which usually requires an inpatient setting [Inpatient parenteral antibiotic therapy (IPAT)]. An alternative is outpatient parenteral treatment [outpatient parenteral antibiotic therapy (OPAT)]. To conduct a health economic cost-benefit analysis of OPAT, a detailed cost analysis of IPAT and OPAT is required. So far, there is a lack of knowledge on the health economic effects of IPAT and OPAT for PPJI. AIM To review an economic comparison of IPAT and OPAT. METHODS A systematic literature review was performed through Medline following the PRISMA guidelines. RESULTS Of 619 identified studies, 174 included information of interest and 21 studies were included for quantitative analysis of OPAT and IPAT costs. Except for one study, all showed relevant cost savings for OPAT compared to IPAT. Costs for IPAT were between 1.10 to 17.34 times higher than those for OPAT. CONCLUSION There are only few reports on OPAT for PPJI. Detailed analyses to support economic or clinical guidelines are therefore limited. There is good clinical evidence supporting economic benefits of OPAT, but more high quality studies are needed for PPJI.

Effects of COVID-19 on outpatient Visitation of Japanese Parkinson’s Disease and Parkinsonism Patients Receiving Rehabilitation

Background: Rehabilitation is effective for preventing progression of Parkinson’s disease (PD) and parkinsonism, but access to services during the COVID-19 pandemic was difficult. The Japanese already practice high levels of social distancing and personal restraint and are keenly aware of dangers related to infectious disease. We therefore presume that many patients discontinued rehabilitation due to behavioral restraint. Objectives: We investigated whether PD patients and parkinsonism patients attending outpatient rehabilitation facilities in Japan initiated rehabilitation interruptions and their associated reasons before and during the COVID-19 pandemic. Methods: Subjects were PD and parkinsonism patients attending a neurology clinic outpatient rehabilitation department in Japan before and during the first year of the COVID-19 pandemic (February 1, 2019, to January 31, 2021). Patients lost to follow-up were investigated as to how long they attended rehabilitation until interruption. We classified factors causing rehabilitation interruption into COVID-19-related factors, disease factors and social reasons. Results: Among 259 subjects (125 men, 134 women, mean age: 76.6 ± 8.5 years), 133 patients (51.4%) initiated interruption of rehabilitation therapy. Cumulative incidence of interruption events was significantly higher during than before the COVID-19 pandemic (p Conclusions: Cumulative incidence of interruption events increased in Japanese PD and parkinsonian patients attending an outpatient rehabilitation facility during the COVID-19 pandemic, primarily due to COVID-19-related factors. Patients and their families need education and guidance to facilitate continuous rehabilitation.

Comparison of aerobic conjunctival bacterial flora in pregnant, reproductive-aged and postmenopausal women

AIM: To evaluate the effect of hormonal status on aerobic conjunctival flora in women. METHODS: One hundred fifty-eight women [reproductive-aged (n=55), pregnant (n=51), and postmenopausal (n=52)] who admitted to outpatient clinic of Obstetrics and Gynecology Department of Denizli State Hospital were enrolled. Age, body-mass index (BMI), obstetric history, cigarette smoking, drug usage, presence of systemic disease, and intraocular pressure (IOP) were recorded for each patient. The samples were taken from the lower fornix with two culture swabs and directly incubated in culture containing 5% sheep blood, eosin-methylene blue and chocolate agar. The other swab specimen was Gram stained. All growths and microscopic results were analyzed. RESULTS: The coagulase-negative Staphylococcus was the predominant organism isolated in the conjunctival samples in both three groups. The aerobic microorganism growth rate for all isolated aerobic organisms revealed no significant change in the three groups (P>0.05). The conjunctival culture positivity rates were similar in the three groups (49% in reproductive-aged, 57% in pregnant and 58% in postmenopausal women) (P>0.05). Age, IOP, BMI, gravidity, parity, cigarette smoking, drug usage, and presence of systemic diseases did not have an effect on culture positivity in three groups. CONCLUSION: Results of this study showed that conjunctival aerobic flora and bacterial colonization did not differ between reproductive-aged, pregnant and postmenopausal women.

Impact of Rotavirus Vaccination in Severe Rotavirus Gastroenteritis outpatient Visits at Three Pediatric Primary Care clinics in Shibata City, Niigata Prefecture, Japan

The impact of rotavirus (RV) vaccination in reducing severe rotavirus gastroenteritis (RVGE) in outpatient settings was prospectively surveyed in three pediatric clinics in Shibata City. In children younger than 3 years of age, the occurrence of severe RVGE among all acute gastroenteritis (AGE) was found to be significantly lower in three seasons after introduction of RV vaccines, compared to that in 2011, before introduction of RV vaccines. The incidence rates of severe RVGE among children younger than 3 years of age were found to be reduced by 71.2%, 47.7%, and 81.1% for 2012, 2013, and 2014, respectively, compared to that in 2011. These results suggest that the RV vaccination is effective for the prevention of severe RVGE in Japanese voluntary RV vaccination settings with estimated coverage rates of 32.5%, 40.5% and 47.1% for 2012, 2013 and 2014, respectively. It is expected that the reducing effect on severe RVGE would be persistently established by increasing the vaccine coverage rates.

Is There Any Threshold for Vitamin D That Elevates Parathyroid Hormone

Objective: To evaluate vitamin D status in relation to serum levels for parathyroid hormone (PTH) and corrected calcium among endocrine outpatients. Methods: A total of 760 patients (mean(SD) age: 40.0(12.6) years, 94.6% were females) admitted to our endocrinology outpatient clinic were included and evaluated with respect to patient demographics, serum levels for 25-hydroxyvitamin D (25(OH) D, ng/mL), PTH (pg/mL) and corrected calcium (mg/dL) as well as the vitamin D status. Results: Vitamin D deficiency was determined in 65.0% and elevated PTH levels in 20.3% of patients. iPTH levels were significantly higher in females than in males (58.9(40.4) vs. 45.1(26.2) pg/mL, p = 0.031) and in summer than in winter cases (63.9(47.7) vs. 54.2(32.8) pg/mL, p = 0.002), whereas no difference was found in serum levels for 25(OH) D, corrected calcium and phosphate with respect to gender and season. Significantly higher levels for iPTH were noted in vitamin D deficient patients (60.7(43.9) pg/mL) than in normal (51.1(33.4) pg/mL) and vitamin D insufficient (57.1(26.0) pg/mL) cases (p = 0.03). iPTH levels were correlated positively with age (r = 0.116, p = 0.001) and negatively with corrected calcium (r = −0.097, p = 0.008), P (r = −0.224, p = 0.000) and 25(OH) D (r = −0.134, p = 0.000), whereas no correlation was noted between 25(OH) D and corrected calcium levels. Conclusion: Our findings indicated that vitamin D deficiency in 65.0%, whereas PTH elevation only in 20.3% of endocrine outpatients, despite the significantly negative correlation of PTH to 25(OH) D and significantly higher levels of PTH among vitamin D deficient cases than in vitamin D insufficient and sufficient cases. Gender’s and seasonal differences had influence on serum levels for PTH but not on either 25(OH) D or corrected calcium, and no correlation was evident between 25(OH) D and corrected calcium levels.
