To identify the etiology of non-A,non-B,non-C hepatitis,23 such patients succes-sively admitted in our hospital during 1988～1992 were investigated using an enzyme immunoas-say kit with two recombinant hepatitis E virus(HEV)antigens from Genelabs(Redwood City,***).Anti-HEV-IgG was detected in 18 cases of *** disease was diagnosed as hep-atitis *** 18 patients were Guangzhou *** is one of the first reports on the pres-ence of sporadic hepatitis E in a metropolis beyond the endemic *** present observationshowed that sporadic hepatitis E occurred out of season and was unlike the epidemic form in *** of 18 patients aged from 20 to 50 years,suggesting that it usually prevailed amongthe young-to-middle-aged *** ratio of male to female was 14 to *** ratio of icterictype to anicteric type was 16 to *** jaundice of 16 icteric patients disappeared in 24.6 days(range,14～60 days).The average alanine aminotransferase level was 806 IU/L(range,74～1676 IU/L),which declined to normal in 30.4 days(range,19～77 days).All patients recov-ered completely in 4.7 *** report also indicated that the course of sporadic hepatitis E,as of epidemic form,is self-limiting and has no chronic sequelae.

Cirrhosis is the main complication of chronic liver disease, leads to progressive liver function impairment and is the main risk factor for the development of liver cancer. Liver failure at endstage cirrhosis is associated with increased mortality with liver transplantation as the only possible treatment at this stage. The pathogenesis of liver cirrhosis is not completely elucidated. Although the common factors leading to liver injury, such as viral hepatitis, alcohol consume or fatty liver disease can be identified in the majority of patients a small percentage of patients have no apparent risk factors. Moreover given the same risk factors, some patients progress to cirrhosis whereas others have a benign course, the reason remains unclear. In order to developnew diagnostic and therapeutic tools, it is s essential to understand the pathogenesis of cirrhosis. The identification of genetic risk factors associated with cirrhosis is one of the possible approach to achieve these goal. In the past years several studies have supported the role of telomere shortening and cirrhosis. In the recent year several studies on the relation between several single nucleotide polymorphism(SNPs) and cirrhosis have been published; it has been proposed also a cirrhosis risk score based on seven SNPs. Also epidemiological studies on identical twins and in different ethnic groups have been supporting the importance of the role of genetic risk factors. Finally in the very recent years it has been suggested that telomere shortening may represent a genetic risk factor for the development of cirrhosis.

In order to study the relationship between replicative status and human leucocyte antigens(HLA),HLA class Ⅱ antigen on hepatocyte membrane was analyzed in liver biopsies from 49 pa-tients with chronic hepatitis B virus *** results revealed that expression of HLA classⅡ antigen on hepatocyte membrane was observed in 57% (13/23) of hepatitis B e antigen (HBeAg)positive,only in 15% (4/26) of anti-HBe positive *** display of HLA class Ⅱ antigen onhepatocyte membrane yeas found in 65% (11/17) hepatitis B core antigen (HBcAg) positive and in19% (6/32) HBcAg negative patients with chronic hepatitis B virus *** was nosignificant difference in the expression of HLA class Ⅱ antigen on hepatocyte membrane between mi-nor hepatic diseases and active liver *** findings suggested that display of HLA classⅡ antigen on hepatocyte membrane might be associated with viral replication in the chronic hepati-tis B virus carriers.

Objective: To study the relationship of the mutation of HBV preS/S gene in HBsAg carrying pregnant women and intrauterine transmission. Methods: Polymerase chain reaction (PCR) was used to amplify HBV preS/S gene from sera of 8 HBsAg carrying pregnant women, 4 women's neonates infected with HBV, and the other's neonates non-infected with. The PCR products were cloned and 5 clones were chosen from every woman for DNA sequencing. Results: Heterogeneity of HBV preS/S gene in HBsAg carrying pregnant women having intrauterine transmission was much higher than that from having not intrauterine transmission, and the divergence rate of nucleotide sequences also higher strikingly. Conclusion: High heterogeneity of HBV preS/S gene of HBsAg positive pregnant women may be relative to high rate of intrauterine transmis-sion

hepatitis C virus(HCV)infection is an excellent immunological model for understanding the mechanisms developed by non-cytopathic viruses and tumors to evade the adaptative immune *** antigen-specific cytotoxic T cell response is essential for keeping HCV under control,but during persistent infection,these cells become exhausted or even *** exhaustion process is progressive and depends on the infection duration and level of *** high antigenic load and long duration of infection,T cells become extremely exhausted and ultimately disappear due to *** development of exhaustion involves the impairment of positive co-stimulation induced by regulatory cytokines,such as transforming growth factor beta *** cytokine downregulates tumor necrosis factor receptor(TNFR)-associated factor 1(TRAF1),the signal transducer of the T cell co-stimulatory molecule TNFR superfamily member 9(known as 4-1BB).This impairment correlates with the low reactivity of T cells and an exhaustion *** with interleukin-7 in vitro restores TRAF1 expression and rescues T cell effector *** process of TRAF1 loss and its in vitro recovery is hierarchical,and more affected by severe disease *** conclusion,TRAF1 dynamics on T cells define a new pathogenic model that describes some aspects of the natural history of HCV,and sheds light on novel immunotherapy strategies for chronic viral infections and cancer.
