目的　探讨BCNU诱导ERCC2表达在胶质瘤细胞耐药中的作用。方法　选用ERCC2基础表达在蛋白水平相同 ,而mRNA水平不同的 2株人脑胶质瘤细胞T98 G和SK MG 4(在mRNA水平 ,T98 G比SK MG 4高 2 5倍 ) ,在体外培养时用BCNU处理 ,随后采用WesternBlot测定肿瘤细胞的ERCC2蛋白水平变化。结果　对BCNU耐药的T98 G在BCNU处理后 6h ,ERCC2蛋白水平上升 6倍 ,随后逐渐下降 ,48h回复到基础水平。而对BCNU敏感的SK MG 4,BCNU处理后ERCC2蛋白水平没有增加。结论　本研究提示 ,mRNA基础水平与胶质瘤耐药密切相关 ,在mRNA基础水平较高的肿瘤细胞 ,BCNU能诱导ERCC2蛋白表达 ,并直接与肿瘤耐药相关。

An 8-year-old boy born prematurely at 26 weeks’ gestation presented with a reticulate scar on the left cheek. This started as an eroded area on day 3 postpartum,followedbycrustingand scarring. There were also linear scars on the left forearm, left lower back and abdomen. The clinical picture fits the entity of congenital erosive and vesicular dermatosis. This retrospective case report delineates the outcome with diagnosis only evident after an 8-year time period.

Rationale:Cryptococcal infections commonly occur in immunosuppressed patients and are uncommon in immunocompetent *** concerns:A 32 year old lady,active smoker presented with right chest pain,dry cough and loss of *** chest radiograph showed a lobulated lung mass in the right lower *** developed headache and right cranial nerve palsy during *** investigations were done including lumbar puncture,brain and chest ***:Cryptococcal meningitis with pulmonary ***:She received five months of effective antifungal treatment;however,the patient did not respond ***,removal of pulmonary cryptococcoma was ***:Her condition improved and she no longer had any ***:Disseminated cryptococcosis is rare in immunocompetent *** case highlights the importance of high index of suspicion and we postulate that lobectomy helped in reducing the cryptococcal burden in her body,thus facilitating better response to antifungal therapy.

Valgus osteotomy in delayed presentation of femoral neck fractures using fixed angle simple dynamic hip screw and plate

Purpose: Reduction and fixation in femoral neck fracture in young patients have a problem of nonunion, requiring additional procedures like valgus osteotomy;but fixation devices are technically difficult for in experienced surgeons. We aims to assess the results of valgus osteotomy in femoral neck fracture in our setup. Methods: We reported a series of 20 patients of higher Pauwel's angled fracture of femoral neck fracture presenting late wherein valgus osteotomy was added to reduction fixation which was secured with a commonly available 135 dynamic hip screw and plate. Results: Femoral neck fractures united in 16 patients (80%). Excellent to good results (Harris hip score >80) were seen in 70% patients. Angle of correction of preoperative Pauwels has been changed from 68.3 to 34.3. Conclusion: The 135 dynamic hip screw and plate provides rigid internal fixation after valgus osteotomy and being a more familiar fixation device simplifies the procedure with good results.

The coronavirus disease 2019(COVID-19)pandemic has devastated global *** key host factors essential for SARS-CoV-2 RNA replication is expected to unravel cellular targets for the development of broad-spectrum antiviral drugs which have been quested for the preparedness of future viral ***,we have identified host proteins that associate with nonstructural protein 12(nsp12),the RNA-dependent RNA polymerase(RdRp)of SARS-CoV-2 using a mass spectrometry(MS)-based proteomic *** the candidate factors,CDK2(Cyclin-dependent kinase 2),a member of cyclin-dependent kinases,interacts with nsp12 and causes its phosphorylation at T20,thus facilitating the assembly of the RdRp complex consisting of nsp12,nsp7 and nsp8 and promoting efficient synthesis of viral *** crucial role of CDK2 in viral RdRp function is further supported by our observation that CDK2 inhibitors potently impair viral RNA synthesis and SARS-CoV-2 *** together,we have discovered CDK2 as a key host factor of SARS-CoV-2 RdRp complex,thus serving a promising target for the development of SARS-CoV-2 RdRp inhibitors.

BACKGROUND:Gallstone disease is common,and complications that are frequently encountered include acute cholecystitis and acute pancreatitis,but rarely gallbladder perforation. METHOD:Data were retrospectively collected from clinical case notes and a literature review is presented. RESULTS:A 72-year-old lady presented with spontaneous gallbladder perforation,pericholecystic abscess and cholecystoduodenal fistula as the first manifestations of gallstone *** was previously well and had no abdominal *** condition was successfully managed with initial antibiotic therapy followed by interval cholecystectomy and fistula repair. CONCLUSIONS:our case highlighted some uncommon but severe complications which occurred simultaneously as the first manifestations of previously asymptomatic gallstone *** complications need to be considered in patients suspected of intra-abdominal sepsis,even when there are no characteristic symptoms.

A 64-year-old woman presented with a 2-week history of an asymptomatic mass involving the lower lip. Histopathological examination showed a well-circumscribed tumour composed of many lobules separated by thin, fibrous connective tissue. Individual lobules were composed of round or polyhedral tumour cells, which had a characteristic finely granular and vacuolated cytoplasm and eccentric hyperchromatic nuclei. Positive staining was observed with Periodic acid-Shiff,and immunohistochemistry for cytokeratin, α-1 antitrypsin, and S-100 protein resulting in a final diagnosis of acinic cell carcinoma. Acinic cell carcinoma represents a well-established, although uncommon, entity in the classification of neoplasms of salivary gland origin. The parotid salivary gland is the most frequent site of acinic cell carcinoma, whereas the lip is a particularly unusual site. The unusual presentation of this tumour may lead to confusion with a mucocele. Given these findings, we suggest that acinic cell carcinoma should be considered in the differential diagnosis of any mucocele-like mass on the lower lip.

We describe three cases of inadvertent placement of the urinary catheter into the *** 85-year-old gentleman on long-term indwelling catheter(IDC)for neurogenic bladder presented with fever and right flank *** of abdomen and pelvis demonstrated the tip of the IDC to be located within the right vesicoureteric junction with acute right hydronephrosis and acute pyelonephritis.A 74-year-old woman,on long-term IDC for neurogenic bladder was found to have hydronephrosis on ultrasound ***-enhanced CT intravenous pyelography done subsequently showed the IDC was in the right distal ureter.A 47-year-old lady,on IDC for urinary retention and voiding dysfunction likely secondary to schizophrenia and anti-psychotic medications,presented with raised creatinine.A non-enhanced CT of her abdomen and pelvis was done and showed that the tip of the urethral IDC was located up to the left vesicoureteric *** all patients,the hydronephrosis resolved after changing the catheter and they were well on *** also review the literature to identify the incidence,outcomes and possible risk *** our knowledge,only 20 cases have been reported thus far in the English *** serious complications can occur,the incidence is very *** risk factor that has been identified is long-term catheterization in patients with neurogenic *** do not recommend routine imaging after catheterization in this group of ***,we should still be mindful of the possibility of this occurrence and evaluate and treat as necessary when clinical suspicion arises.

Background/Purpose: Isolated splenic abscesses are rare in pediatric patients. The recommended treatment in the literature has been in favor of splenectomy, although conservative treatment with splenic preservation is being increasingly reported. We report successful management of 4 pediatric patients with splenic abscess by needle aspirations and antibiotics. Materials and Methods: Four children (aged 7-11 years; male-female, 3:1) were admitted in our institution with history of high-grade fever with chills, anorexia, left hypochondrial pain, and splenomegaly. One child was a known case of thalassemia, and one had a history of typhoid fever. The others did not have any predisposing condition. Ultrasonography (USG) and computed tomographic scan of the abdomen showed a solitary abscess in the spleen in 2 patients and multiple abscesses in the other 2. Ultrasonography-guided needle aspiration in 3 cases revealed purulent fluid, which, on culture, grew Escherichia coli in 1 case, Salmonella paratyphi A in 1 case, but sterile in 1 case. Blood culture was sterile in all the cases, but Widal’s test was positive in 2 patients. Treatment protocol included USG-guided needle aspiration of pus along with intravenous ceftriaxone, metronidazole, and amikacin for 3 to 12 weeks. Results: All 4 patients showed a good response to conservative treatment. Serial USG showed gradual resolution of abscess, and none was subjected to splenectomy. Conclusion: Isolated splenic abscess in children can be successfully treated with needle aspirations and intravenous antibiotics, thereby avoiding splenectomy.
