BACKGROUND Picky eating is a commonly observed behavior among children globally,negatively impacting their physical and mental *** common characteristics distinguish peaky eaters,including food selectivity,food neophobia,and food avoidance,there is no clear definition to assess this *** to the unavailability of data regarding picky eating,it wasn’t easy to estimate its *** To develop a regional protocol to help healthcare professionals identify and manage mild and moderate picky eating *** A virtual roundtable discussion was held in April 2021 to gather the opinions of seven pediatricians and two pediatric dietitians from eight Middle Eastern countries who had great experience in the management of picky *** discussion covered different topics,including clearly defining mild and moderate picky eating,identifying the role of diet fortification in these cases,and the possibility of developing a systematic approach to diet *** The panel identified picky eating as consuming an inadequate amount and variety of foods by rejecting familiar and unfamiliar *** of the time,moderate picky eating cases had micronutrient deficiencies with over-or undernutrition;the mild cases only showed inadequate food consumption and/or poor diet *** attention to the organic red flags like growth faltering and development delay and behavioral red flags,including food fixation and anticipatory gagging,will help healthcare professionals evaluate the picky eaters and the caregivers to care for their *** dietary supplementation and commercial food fortification play an important role in picky eating,they were no benefit in the Middle *** The panel agreed that food fortification through a food-first approach and oral nutritional supplements would be the best for Middle Eastern *** recommendations would facilitate identifying and managing picky-eating children in the Middle East.

background While diffuse atherosclerotic disease affecting the posterior circulation has been described extensively,the prevalence,natural history and angiographic characteristics of isolated symptomatic basilar artery stenosis(ISBAS)remain *** We reviewed our prospective institutional database to identify patients with≥50% symptomatic basilar artery(BA)stenosis without significant atherosclerotic burden in the vertebral or posterior cerebral *** mechanism,collateral circulation,and degree and length of stenosis were *** primary outcome was time from index event to new transient ischaemic attack(TIA),acute ischaemic stroke(AIS)or *** outcome variables included modified Rankin Scale(mRS)score on discharge and last *** Of 6369 patients with AIS/TIA,91(1.43%)had ***-three(80.2%)patients presented with AIS and 18(19.8%)with ***-nine(31.9%)were women and the median age was 66.8±13.6 *** mean follow-up time was 2.7 *** most common stroke mechanism was artery-to artery thromboembolism(45.2%),followed by perforator occlusion(28.7%)and flow-dependent/hypoperfusion(15.1%).The percentage of stenosis was lower in patients who had favourable outcome compared with those with mRS 3-6 on discharge(78.3±14.3 vs 86.9±14.5,p=0.007).Kaplan-Meier curves showed higher recurrence/death rates in patients with≥80% stenosis,mid-basilar location and poor collateral *** 13% of patients with ISBAS presented with complete BA *** ISBAS is an uncommon(1.43%)cause of TIA and *** in their 60s are mostly affected,and artery-to artery embolism is the most common stroke ***-basilar location,≥80% stenosis and poor collateral circulation are important factors associated with worse prognosis.

Liver transplantation for non-resectable colorectal liver metastases

At present,there are no absolute practice guidelines for liver transplants in colorectal liver metastases(CRLM),and treatment protocols for unresectable CRLM are institution-specific(1).One of the first prospective studies evaluating the safety of liver transplants for CRLM was the SECA-I trial(1,2).A 5-year overall survival rate of 60% was observed in this trial,which far outperformed survival with chemotherapy alone(3).In the SECA-Ⅱ trial,enrollment was increased,and patients with better prognostic factors,including at least a 10% response to chemotherapy before the transplant,were selected.

Bacterial endocarditis is a rare complication amongst solid organ transplant recipients and is often linked to bacteremia. Majority of these recipients do not have underlying valvular heart disease or congenital valvular abnormalities. Staphylococoocusaureus and Enterococcus species are the most commonly isolated organisms. There are very few reports of gram-negative bacteria causing endocarditis in liver transplant recipients. We report a 51-yearold male, a liver transplant recipient, who developed bacterial endocarditis of the mitral valve due to extended spectrum of betalactamase producing strain of Escherichia coli and was managed successfully with antibiotics.

Trends in the rates of hospitalizations for acute stroke among patients over 90 years of age with atrial fibrillation in the United States： from 2005 to 2014

Background Acute stroke （AS） rates in patients over 90 years of age （very elderly） with atrial fibrillation （AF） in the United States（US） are not known. We assessed trends in hospitalizations for AS among very elderly with AF in the US from 2005 to 2014. Methods Weused the nationwide inpatient sample （NIS） from the USA; 2005-2014. AF and AS diagnoses were abstracted using international classifica-tion of diseases, 9th Revision, clinical modification （ICD-9-CM） codes. Results From 2005 to 2014, 3,606,073 hospitalizations of veryelderly with AF were reported. Of these, 188,948 hospitalizations （141,822 hospitalizations in women and 47,126 hospitalizations in men）had AS as the primary diagnosis. Age adjusted AS hospitalizations increased in the total cohort （3217/million in 2005 to 3871/million in2014）, in women （3540/million in 2005 to 4487/million in 2014） and in men （2490/million in 2005 to 3173/million in 2014）（P 〈 0.001）.Anticoagnlation rates increased in women （8% in 2005 to 19.9% in 2014） and in men （8.9% in 2005 to 21.6% in 2014）. AS rates, thoughnumerically lower than the total cohort, showed an increasing trend in anticoagnlated patients as well （all anticoagulated patients： 212/millionin 2005 to 513/million in 2014; anticoagulated women： 224/million in 2005 to 529/million in 2014, anticoagnlated men： 184/million in 2005to 518/million in 2014）. Conclusions There is an increasing trend in AS hospitalizations among nonagenarians with AF in the US despiteimproving utilization of anticoagulants in this patient population. The etiologies driving this alarming trend are unclear and require fur-ther study.

Current situation and influencing factors of palliative care practice ability among oncology nurses:A multicenter cross-sectional study

Objectives:The study aimed to survey the current situation and explore the factors that influence the ability of palliative care practice among oncology ***:A multicenter cross-sectional study was conducted using stratified random sampling to select 26 tertiary hospitals’oncology departments in Hubei Province,China.A total of 1,198 nurses were included and finished the questionnaire consisting of social demographic characteristics,Palliative Care Self-Report Practice Scale(PCPS),End-of-life Professional Caregiver Survey(EPCS),and Self-Perceived Pain Assessment Knowledge and Confidence Scale(Self-PAC)through the online *** were analyzed using t-test,one-way ANOVA,Pearson correlation analysis,and multiple linear regression analysis in SPSS ***:The total score for PCPS was 67.17±12.57,the three dimensions’scores were:physical symptom care(32.50±6.10),spiritual and psychological care(23.35±4.97),communication(11.58±2.48).There are significant positive correlations between the palliative care practice ability and core competence(r=0.77,P<0.01),as well as pain assessment ability(r=0.56,P<0.01).Multiple regression analysis identified female,with high education background(bachelor’s degree and master’s degree or above),interest in palliative care,pain assessment ability,and core competence were positive predictors of palliative care practice ability(Adjusted R^(2)=0.668,P<0.05).Conclusions:The overall ability of the oncology nurses to practice palliative care was relatively high,but the palliative nurses reported suboptimal performance in the communication dimension of palliative nursing practice *** comprehensively improve oncology nurses’palliative care practice ability,managers must consider the gender structure,educational background,enthusiasm for palliative care work,core competence,and pain assessment ability.

Pediatric burn resuscitation:past, present, and future

Burn injury is a leading cause of unintentional death and injury in children, with the majority being minor (less than 10%). However, a significant number of children sustain burns greater than 15%total body surface area (TBSA), leading to the initiation of the systemic inflammatory response syndrome. These patients require IV fluid resuscitation to prevent burn shock and death. Prompt resuscitation is critical in pediatric patients due to their small circulating blood volumes. Delays in resuscitation can result in increased complications and increased mortality. The basic principles of resuscitation are the same in adults and children, with several key differences. The unique physiologic needs of children must be adequately addressed during resuscitation to optimize outcomes. In this review, we will discuss the history of fluid resuscitation, current resuscitation practices, and future directions of resuscitation for the pediatric burn population.
