Closed External Subtalar Dislocation: A Case Report

Subtalar dislocation is described as the simultaneous dislocation of the talo-calcaneal and talo-navicular joints without any tibio-talar or talar neck associated fractures. This injury is rare, and represents 1% of dislocations. We report the case of 28-year-old male patient, without any medical history, who had been involved in a sport accident causing a rare closed external subtalar dislocation without any bone fracture. An open reduction was indicated after failure of closed reduction and the X-ray control showed good joint congruence;the fixation was maintained for 6 weeks and then the rehabilitation was started. The evaluation was favorable, and we noted good functional results after a decline of 12 months.

Recurrence of Cortical Venous Haemangioma in the Tibia: Interest of Sclerotherapy

We report on a rare case of an intra-osseous haemangioma involving the medial portion of tibia in a 29-year-old man. The aim is to emphasize on diagnosis difficulties, possible recurrence and interest of sclerotherapy. In radiographs, the lesion was a well-defined osteolytic lesion having 3 mm of diameter with perimeter’s sclerosis. The computed tomography images demonstrated an intra-medullar gap’s image of the tibia with cortical participation. An osteoid osteoma was evocated but eliminated by the histhological examination. The osseous scintigraphy didn’t find any anomaly. The outcome was marked by the recurrence of pain. T1-weighted Magnetic resonance imaging showed a hypointense lesion, while T2-weighted images revealed hyperintense areas, with internal, hypointense septa. Gadolinium-enhanced T1-weighted images showed lattice-like enhancement of the lesion. These images signed a recurrence of the intra-osseous venous haemangioma. The sclerotherapy improved the pain in three days and prevented another recurrence.

Quality of life of elderly nursing home residents and its correlates in Kayseri. A descriptive-analytical design: A cross-sectional study

Purpose: To define the level of quality of life in an elderly population and to investigate the effects of selected variables, such as anemia, fatigue, depression and sleep disorders, on the quality of life. Design and Methods: The study was conducted in Gazio?lu Nursing Home, located in the city center of Kayseri, on 136 subjects ≥65 in the year 2008-2009. Data were collected using the Geriatric Depression Scale (GDS), Fatigue Severity Scale (FSS), Pittsburgh Sleep Quality Index (PSQI) and the World Health Organization Quality of Life-OLD (WHOQOL- OLD) Module. In the statistical analysis mean ± standard deviation, student’s t-test, ANOVA and Spearman correlation analysis were used. Re- sults: The mean total score of quality of life was 43.45 ± 10.30. Of the residents 47.0% had a poor quality of life. Autonomy had the lowest (35.70 ± 19.96) and intimacy had the highest (48.75 ± 17.96) subdomain scores. Fatigue significantly decreased the total and autonomy, social par- ticipation and death and dying subdomain scores. Anemia had a significant adverse effect on intimacy, depression on autonomy and intimacy and sleep disorder on death and dying. There were negative correlations between fatigue with past-present-future activities and social participation, depression with social participation, intimacy, death and dying and glucose levels with social participation and intimacy. Implications: About half of the subjects had a poor quality of life. Fatigue was the sole factor to negatively affect the total score in WHOQOL-OLD. Depression, anemia and sleep disorder adversely affected the autonomy, social participation, intimacy, death and dying subdomain scores but not in all.

The management of symptomatic lumbar synovial cysts is still a challenge and a matter of debate with no existing strong consensus. There are different treatment techniques and strategies, the most debated matter is to fuse or not to fuse, efficiency of less invasive techniques. One of our purposes was to provide an updated review of a literature about treatment of juxta facet synovial cysts and as standard surgical strategies are inconsistent, we present a small series of patients treated using different approaches, most of them were treated with MISS techniques, endoscopy, providing good results.

Adamantinoma is a primary low grade malignant bone tumor that is predominantly located in the mid-portion of the tibia. The tumor is of interest for two reasons： first, there still exists considerable dispute as to the origin of the lesion and recent reports reveal that the condition is more malignant than had previously been supposed. Although cases of adamanti- noma located to the axial skeleton have been reported, this is the first case of adamantinoma located to pelvic bone in Iran. Here we present the clinical, radiological ＆ histopathological features of a 19 year-old male with painful lesion located to the right pelvic bone which was morphologically and immunohistochemically diagnosed as adamantinoma. In general, metastasis is seen in 15%-20% of patients. The spread can occur to regional nodes, lung and infrequently to skeleton, liver and brain . Several weeks after surgery, our patient＇s condition gradually worsened. A CT-scan of abdomen revealed widespread liver metastasis and the patient died due to acute liver failure. This case demonstrates that the mortality rate from adamantinoma is not always low.
