There are basically four stages of a person＇s thinking. The first is a person＇s quest for meaning. It is a time for questions. Socrates said that an unexamined life is not worth living. Yet in America, living an examined life is rare. The challenge is to live an examined life in a culture of unexamined lives. The second stage is a time for answers. We cannot avoid the realism of evil, yet today, few have seriously examined absolute evil. The third stage is a time for confirmation and assurances to our answers of evil and pain. The fourth stage is the time for commitment and beginning of the journey. For the simple person who has all the answers, there is no further journey. At the other extreme is the intellectual who never has all the answers. There is always a question for another day and there is never any really settled resolution on what he or she believes or why. There are two basic philosophical systems that have influenced our thinking about morals and about law. The first is naturalism, which began in the 6th century B.C., in the Greek Ionian culture with a certain set of cosmologies, most of which were naturalistic in nature where reality was expressed in terms of water, fire, earth, or air. Naturalism has experienced great growth since in the Enlightenment. However, the notion at the heart of morality and law is idealism. Indeed, for much of the history of legal thought, the connection between morality and law was found in idealism. It is within the philosophy of idealism that we find the history of natural law theory. This matters because how one thinks about the nature and purpose of law, what forms the foundation of law and how it is made, either legislatively or through the courts is one of the most elusive and persistent problems in the entire range of human thought. This article explores some of that history.

Objective The management of hypertrophic obstructive cardiomyopathy is not well-defined in the *** therapy with 3-blockers and calcium-channel blockers are the mainstay therapy for symptomatic *** is usually reserved for patients who fail medical therapy. Alcohol septal ablation has been recently introduced as an alternative therapy. Patients and Methods Ninety-five patients older than 65 years of age were included. All patients have completed one year of follow-up. The mean age was 72 ± 5 years, 47 patients were females, 10 patients with history of hypertension. Results The mean rise in CK post alcohol ablation was 1052 ± 430IU. The mean NYHA class decreased from 2.9 ± 0.6 to 1.2 ± 0.5 ( P ＜ 0.001 ). The exercise duration on treadmill testing increased from 328 ± 260 s to 349 ± 39 s. The mean resting left ventricular outflow tract gradient decreased from 65 ± 37 mmHg to 16 ± 29 mmHg at one year. one patient died in the hospital after coronary artery bypass grafting that was done subsequent to spiral dissection of the left anterior descending artery during ablation. Thirteen patients developed complete heart block immediately after ablation requiring pacing therapy. Conclusions Alcohol septal ablation seems to be an effective alternative therapeutic option for elderly patients with hypertrophic obstructive cardiomyopathy. Larger studies with longer follow-up are needed.

富集在沉积物中的重金属是长期影响城市河流地表水生态的潜在威胁。通过采集位于重庆主城区的典型山地城市河流——清水溪干流河底沉积物,监测各点位沉积物样品中的重金属含量以及附近地表水环境指标,并利用多种评价方法和统计学手段来评判清水溪干流沉积物重金属的生态风险和来源解析。结果表明,清水溪干流沉积物中As、Cd、Cr、Cu、Hg、Ni、Pb和Zn均值含量分别为7.55±8.25、0.238±0.056、86.43±5.14、78.21±18.22、2.49±2.00、33.86±5.29、42.99±9.21和198.71±43.55 mg·kg^(-1)。地累积指数(I_(geo))评价体系中,Hg达到了重度污染水平,Cu、Zn、Cd和Pb也有轻度污染。大部分点位沉积物重金属含量超过现行的农用地土壤污染风险管控标准,建议清淤淤泥需谨慎用于农用地。沉积物潜在生态风险指数(RI)均值为1984.54±1515.92,Hg对RI的贡献度达到了90.78%,清水溪干流沉积物存在着极大的生态风险。重金属外源输入的主要区域集中于河流源头,加之上游地表水较高的pH,共同导致重金属(除As和Ni外)含量从上游至下游逐渐减小的趋势。由于山地河流源头较高的比降,在源头输入的外源污染对干流中上游沉积物中的重金属含量均造成一定影响。通过主成分分析和聚类分析表明,沉积物中Cr、Cu、Hg、Pb和Zn主要源于源头工业点源污染,并存在3种细分类型,应源于不同的行业类型;As和Ni主要源于污水处理站/城市面源污染;上述两种来源均对Cd有显著贡献。

Array-based comparative genomic hybridization (array CGH),a method used to detect gains or losses of genetic material,has recently been applied to prenatal diagnosis of genomic imbalance in the clinical laboratory setting. This new and exciting diagnostic tool represents a major technological step forward in cytogenetic testing and addresses many of the limitations of current cytogenetic methods. Conventional chromosome analysis,the current gold standard in prenatal diagnosis,focuses primarily on the detection of common aneuploidies and is limited by its capacity to detect only those copy number changes that are large enough to be microscopically visible (typically 5-6 Mb in size at the 500 band level). In contrast,array CGH analysis simultaneously evaluates regions across the entire genome and allows for detection of unbalanced structural and numerical chromosome abnormalities of less than one hundred kb. Array CGH analysis also overcomes some of the limitations of chromosome analysis,such as the requirement for cell culture and longer reporting time,by using direct uncultured fetal specimens. With many diagnostic laboratories now embracing this technology,the past year has seen tremendous growth in the use of array CGH analysis for prenatal diagnosis. This review aims to summarize array CGH methodo-logy and its current applications in prenatal diagnosis.
