This study aimed to determine the relationship between the different factors of analgesic therapy and the compliance of chronic pain inpatients. We prospectively investigated 100 consecutive inpatients with non- cancer chronic pain who were hospitalized to receive oral analgesic treatment in the Pain Department of West China Hospital from May 2013 to October 2013. Patients who completed the treatment plan were recorded as good compliance, whereas patients who partly completed or even refused the treatment were recorded as moderate or non-compliance, respectively. A total of 73 （73.7%）, 17 （17.1%）, and 9 （9.2%） patients showed good, moderate, and non-compliance, respectively. Univariate analyses showed significantly better compliance among farmers, patients educated in college or above, with family income of 〈 3000 CNY, and with severe or moderate pain than those employed and unemployed （P= 0.02）, patients educated below college （P= 0.013）, with family income of ≥ 3000 CNY （P = 0.025）, and with mild pain （P 〈 0.001）, respectively. Logistic regression analysis showed that the family income of≥ 3000 CNY （OR： 2.50, 95%CI： 1.65-4.51, P= 0.021） and mild pain （OR： 1.27, 95%CI： 1.03-3.31, P= 0.016） were associated with moderate or non-compliance with oral analgesic treatment. In conclusion, the low compliance with oral treatment of analgesics was found in Chinese inpatients with chronic pain and compliance was negatively associated with family income and degree of pain of patients.

Chemoresistance has long been the bottleneck of ovarian cancer(OC)*** has been shown that mitochondria play a crucial role in cell response to chemotherapy and that dysregulated mitochondrial dynamics is intricately linked with diseases like OC,but the underlying mechanisms remain ***,we demonstrate a new mechanism where CRL4^(CUL4A/DDB1)manipulates OC cell chemoresistance by regulating mitochondrial dynamics and ***4^(CUL4A/DDB1)depletion enhanced mitochondrial fission by upregulating AMPKα^(Thr172)and MFF^(Ser172/Ser146)phosphorylation,which in turn recruited DRP1 to ***4^(CUL4A/DDB1)loss stimulated mitophagy through the Parkin-PINK1 pathway to degrade the dysfunctional and fragmented ***,CRL4^(CUL4A/DDB1)loss inhibited OC cell proliferation,whereas inhibiting autophagy partially reversed this *** findings provide novel insight into the multifaceted function of the CRL4 E3 ubiquitin ligase complex in regulating mitochondrial fission,mitophagy,and OC *** of CRL4^(CUL4A/DDB1)and mitophagy may be a promising therapeutic strategy to overcome chemoresistance in OC.

The Value of Blood Tumor Markers in the Prognosis of Patients with Unresectable Locally Advanced or Metastatic Gastric Cancer before First-line Chemotherapy

Objective:To study the significance of CEA,CA199,CA72.4 and CA125 in the peripheral blood of patients with advanced or metastatic gastric ***:The clinical data of 109 patients with non-operative local advanced or metastatic gastric cancer who received first-line chemotherapy in our center from July 2013 to May 2015,and the detection results of CEA,CA199,CA72.4 and CA125 tumor markers before chemotherapy were retrospectively collected,and their correlation with the clinicopathological characteristics and prognosis of the patients were ***:The positive rates of CEA,CA199,CA72.4 and CA125 were 46.8%,40.2%,53.5%and 35.0%respectively in 109 cases of gastric cancer,and the positive rates of combined detection of four markers were 87.2%.CEA positive was significantly correlated with liver metastasis(P=0.014),CA125 positive with peritoneal metastasis(P=0.005).In univariate analysis,median PFS(6.3 months vs 11.1 months,P=0.020)and median OS(9.9 months vs 16.9 months,P=0.007)of CA72.4 positive subjects were significantly shorter than those of negative subjects,and median PFS(6.2 months vs 7.8 months,P=0.002)and median OS(9.8 months vs 15.6 months,P=0.009)of CA125 positive subjects were significantly shorter than those of negative subjects.95%CI 1.016-29.601).Of 109 patients,49(45.0%)were low-risk group(0 or 1 risk factor),60(55.0%)were high-risk group(2 to 4 risk factors).The median survival time of low-risk and high-risk groups was 18.5 months and 9.9 months,respectively(P=0.001).Conclusion:CA72.4 and CA125 are related to the prognosis of patients with local advanced or metastatic gastric cancer,and the prognosis model is helpful to stratify the risk of patients and provide the best treatment plan for each patient.

Pleural and peritoneal metastasis accompanied by malignant pleural effusion(MPE)or malignant ascites(MA)is frequent in patients with advanced solid tumors that originate from the lung,breast,gastrointestinal tract and *** delivery of CAR-T cells represents a new strategy to control tumor dissemination in serous ***,malignant effusions constitute an immune-suppressive environment that potentially induces CAR-T cell ***,we demonstrated that the anti-tumor cytotoxicity of conventional 2nd-generation CAR-T cells was significantly inhibited by both the cellular and non-cellular components of MPE/MA,which was primarily attributed to impaired CAR-T cell proliferation and cytokine production in MPE/MA ***,we found that PD-L1 was widely expressed on freshly-isolated MPE/MA *** on this feature,a novel PD-L1-targeting chimeric switch receptor(*** CSR)was designed,which can bind to PD-L1,switching the inhibitory signal into an additional 4-1BB *** co-expressed with a 2nd-generation CAR,*** CSR-modified CAR-T cells displayed superior fitness and enhanced functions in both culture medium and MPE/MA environment,causing rapid and durable eradication of pleural and peritoneal metastatic tumors in xenograft *** investigations revealed elevated expressions of T-cell activation,proliferation,and cytotoxicity-related genes,and we confirmed that PD-L1 scFv and 4-1BB intracellular domain,the two important components of *** CSR,were both necessary for the functional improvements of CAR-T ***,our study shed light on the clinical application of *** CSR-modified dual-targeting CAR-T *** on this study,a phase I clinical trial was initiated in patients with pleural or peritoneal metastasis(NCT04684459).

Objective: To investigate the protective effects of modified Linggui Zhugan Decoction(MLZD), a traditional Chinese medicine formula, on obese type 2 diabetes mellitus(T2 DM) rats. Methods:Fifty Sprague-Dawley rats were randomly divided into 5 groups by a random number table, including normal, obese T2 DM(ob-T2 DM), MLZD low-dose [MLDZ-L, 4.625 g/(kg·d)], MLZD middle-dose [MLD-M,9.25 g/(kg·d) ] and MLZD high-dose [MLD-H, 18.5 g/(kg·d)] groups,10 rats in each group. After 4-week intervention, blood samples and liver, pancreas, muscle tissues were collected to assess the insulin resistance(IR), blood lipid, adipokines and inflammation cytokines. The alteration of phosphatidylinositol 3 kinase(PI3 K)-protein kinase B(PKB or Akt)/the mammalian target of rapamycin(mTOR)-ribosome protein subunit 6 kinase 1(S6 K1)/AMP-activated protein kinase(AMPK)-peroxisome proliferator-activated receptor gamma coactivator 1 alpha(PGC-1α) pathways were also studied. Results: MLZD dose-dependently reduced fasting blood glucose,fasting insulin, homeostasis model of assessment for IR index and increased insulin sensitive index compared with ob-T2 DM rats(P2 DM rats after 4-week treatment(P<0.05 or P<0.01).Improvements in adipokines and inflammatory cytokines were observed with a raised level of adiponectin and a reduced level of leptin, resistin, tumor necrosis factor-α and interleukin-6(P<0.05 or P<0.01). MLZD regulated the PI3 K-Akt/mTOR-S6 K1/AMPK-PGC-1α pathways and restored the tissue structure of liver and pancreas(P<0.05 or P<0.01). Conclusions: MLZD ameliorated glycolipid metabolism and inflammation, which may be attributed to the regulation of PI3 K-Akt/mTOR-S6 K1/AMPK-PGC-1α pathways.

Prognostic significance of lymphovascular infiltration in overall survival of gastric cancer patients after surgery with curative intent

Objective: Lymphovascular infiltration(LVI) is frequently detected in gastric cancer(GC) specimens. Studies have revealed that GC patients with LVI have a poorer prognosis than those without ***: In total, 1,007 patients with curatively resected GC at Department of Gastric Cancer, Tianjin Medical University Cancer Institute and Hospital were retrospectively enrolled. The patients were categorized into two groups based on the LVI status: a positive group(PG;presence of LVI) and a negative group(NG;absence of LVI). The clinicopathological factors corrected with LVI and prognostic variables were analyzed. Additionally, a pathological lymphovascular-node(lvN) classification system was proposed to evaluate the superiority of its prognostic prediction of GC patients compared with that of the eighth edition of the N staging ***: Two hundred twenty-four patients(22.2%) had LVI. The depth of invasion and lymph node metastasis were independently associated with the presence of LVI. GC patients with LVI demonstrated a significantly lower overall survival(OS) rate than those without LVI(42.8% vs. 68.9%, respectively;P2. The lvN classification was found to be more appropriate to predict the OS of GC patients after curative surgery than the pN staging system. The-2 log-likelihood of lvN classification(4,746.922) was smaller than the value of pN(4,765.196), and the difference was statistically significant(χ^2=18.434, P2. LVI should be incorporated into the pN staging system to enhance the accuracy of the prognostic prediction of GC patients.

Aromatic Essential Oil Acupressure Combined with Music Therapy in Cancer Pain Management

Objective: To explore the effect of aroma oil acupressure combined with music therapy in pain intervention for tumor patients. Methods: 60 patients with malignant tumors accompanied by cancer pain who were hospitalized in the oncology department of a tertiary-level hospital in Jingzhou City, Hubei Province, from January 2022 to December 2022 were selected as the study subjects, and were divided into 30 cases in the control group and 30 cases in the intervention group according to the stratified sampling method. The control group was divided into 30 cases of control group and 30 cases of intervention group according to the stratified sampling method. The control group was given medicine according to the conventional step analgesia, and the intervention group was given medicine according to the conventional step analgesia with the addition of aromatic oil acupressure combined with music therapy, and the effect of the intervention was valued by the NRS, the SAS and the SDS. Results: The NRS, SAS and SDS of patients in the intervention group were lower than those in the control group (p Conclusion: Aromatic oil acupressure combined with music therapy can effectively improve the pain symptoms of tumor patients, relieve anxiety and improve the quality of sleep of patients. Combined with pharmacological methods of pain relief, it can reduce the dose of pain medication and the frequency of administration of medication, and find a safe, low-cost, non-pharmacological pain complementary alternative therapy for tumor patients with cancer pain.

The Values of the Prognostic Nutrition Index and the Patient Generated-Subjective Global Assessment during the Nutritional Assessment of Patients with Gastrointestinal Tumors

Objective To compare the value and consistency among the Patient Generated-Subjective Global Assessment(PG-SGA)and the Prognostic Nutrition Index(PNI)for assessing nutritional status in gastrointestinal tumor *** 251 patients from gastric cancer surgical ward from January 2019 to January 2020 were recruited through convenience sampling in this respective *** screening and assessment were conducted for 251 gastrointestinal tumor patients using the nutritional risk screening 2002(NRS 2002)PG-SGA,and the *** was calculated using the serum albumin level and the total lymphocyte count obtained from the patients’routine laboratory examination when they were admitted to the *** receiver operating characteristic(ROC)of the PG SGA and the PNI were plotted with the NRS 2002 used as the gold standard,and the diagnostic value of the PG-SGA and PNI was reflected by the area under the curve(AUC),sensitivity,specificity and Youden *** then determined the optimal cut-off for the PNI and tested the consistency of the PG-SGA and *** The optimal cut-off point for the PNI was calculated to be *** AUC of the PG-SGA was 0.908(95%CI 0.871-0.944).The sensitivity was 89.9%,specificity was 76.2%and the Youden index was *** AUC of the PNI was 0.594(95%CI 0.516-0.572).The sensitivity was 73.8%,specificity was 44.3%and the Youden index was *** the consistency test,the kappa value was 0.838(P<0.001).Conclusion The PNI is of limited value for assessing malnutrition,although it did have good consistency with the *** combination of the PNI and PG-SGA can be used for diagnosing assessing malnutrition in clinical practice.

A Review of Grief Counselling Practices for Family Members of oncology Patients in Hospice Care

In cancer patients, especially terminal patients, the family members of the patients will develop more serious sadness and find it difficult to face death rationally, which affects the quality of life and activities. Because of this, in the clinical treatment of oncology patients, strengthening hospice care for family members, doing a good job of death education, assisting them to face and participate in the clinical treatment of oncology in a positive way, and avoiding excessive grief can simultaneously improve the quality of life of patients and their families.
