Nestin in gastrointestinal and other cancers: Effects on cells and tumor angiogenesis

Nestin is a class Ⅵ intermediate filament protein that was originally described as a neuronal stem cell marker during central nervous system (CNS) development, and is currently widely used in that capacity. Nestin is also expressed in non-neuronal immature or progenitor cells in normal tissues. Under pathological conditions, nestin is expressed in repair processes in the CNS, muscle, liver, and infarcted myocardium. Furthermore, increased nestin expression has been reported in various tumor cells, including CNS tumors, gastrointestinal stromal tumors, pancreatic cancer, prostate cancer, breast cancer, malignant melanoma, dermatofibrosarcoma protuberances, and thyroid tumors. Nestin is reported to correlate with aggressive growth, metastasis, and poor prognosis in some tumors; however, the roles of nestin in cancer cells have not been well characterized. Furthermore, nestin is more specifically expressed in proliferating small-sized tumor vessels in glioblastoma and gastric, colorectal, and prostate cancers than are other tumor vessel markers. These findings indicate that nestin may be a marker for newly synthesized tumor vessels and a therapeutic target for tumor angiogenesis. It has received a lot of attention recently as a cancer stem cell marker in various cancer cells including brain tumors, malignant rhabdoid tumors, and uterine, cervical, prostate, bladder, head and neck, ovarian, testicular, and pancreatic cancers. The purpose of this review is to clarify the roles of nestin in cancer cells and in tumor angiogenesis, and to examine the association between nestin and cancer stem cells. Nestin has the potential to serve as a molecular target for cancers with nestin-positive cancer cells and nestin-positive tumor vasculature.

miRNAs in precancerous lesions of the gastrointestinal tract

In spite of the well-established understanding of the phenotypic lesions occurring in the shift from native epithelia to invasive (adeno) carcinoma, the molecular typing of the precancerous changes in the gastrointestinal tract remains unreliable. In recent years, no biomarkers have aroused as much interest as the miRNAs,a class of non-coding RNA molecules that function as endogenous silencers of numerous target genes. Aberrant miRNA expression is a hallmark of human disease,including cancer. Unlike most mRNAs, miRNAs are both long-living in vivo and very stable in vitro . Such characteristics allow their testing in paraffin-embedded tissue samples, which is essential in the biological profiling of small (phenotypically characterized) preneoplastic lesions of the gastrointestinal tract (as well as in other fields of human pathology). The upcoming challenge lies in the reliable identification of disease-specific targets of dysregulated miRNAs, to enable miRNA testing in the clinical management of the secondary prevention of gastrointestinal cancer.

Early gastric cancer(EGC)is an invasive carcinoma involving only the stomach mucosa or submucosa,independently of lymph node *** represents over 50%of cases in Japan and in South Korea,whereas it accounts only for approximately 20%of all newly diagnosed gastric cancers in Western *** main classification systems of EGC are the Vienna histopathologic classification and the Paris endoscopic classification of polypoid and non-polypoid lesions.A careful endoscopic assessment is fundamental to establish the best treatment of ***,EGCs are curable if the lesion is completely removed by endoscopic resection or *** types of EGC can be resected endoscopically;for others the most appropriate treatment is surgical resection and D2 lymphadenectomy,especially in Western *** favorable oncological prognosis,the extended lymphadenectomy and the reconstruction of the intestinal continuity that excludes the duodenum make the prophylactic cholecystectomy mandatory to avoid the onset of biliary complications.

Quality of life in patients with esophageal stenting for the palliation of malignant dysphagia

Incidence of esophageal cancer(EC) is rising more rapidly in the Western world than that of any other cancer. Despite advances in therapy,more than 50% of patients have incurable disease at the time of *** precludes curative treatment and makes palliative treatment a more realistic option for most of these patients. Dysphagia is the predominant symptom in more than 70% of patients with EC and although several management options have been developed in recent years to palliate this symptom,the optimum management is not ***-expanding metal stents(SEMS) are a well-established palliation modality for dysphagia in such ***-related quality of life(HRQoL) is becoming a major issue in the evaluation of any therapeuticor palliative *** date,only a few published studies can be found on Medline examining HRQoL in patients with advanced EC treated with SEMS *** aim of this study was to review the impact on HRQoL of SEMS implantation as palliative treatment in patients with *** Medline articles regarding HRQoL in patients with advanced EC,particularly those related to SEMS,were *** most studies,relief of dysphagia was the only aspect of HRQoL being measured and SEMS implantation was compared with other palliative treatments such as brachytherapy and laser therapy. SEMS insertion provides a swift palliation of dysphagia compared to brachytherapy and no evidence was found to suggest that stent implantation is different to laser treatment in terms of improving dysphagia,recurrent dysphagia and better HRQoL,although SEMS insertion has a better technical success rate and also reduces the number of repeat interventions.

While the incidence of gastric cancer(GC)in general has decreased worldwide in recent decades,the incidence of diffuse cancer historically comprising poorly cohesive cells-GC(PCC-GC)and including signet ring cell cancer is *** concerning PCC-GC is scarce and unclear,mostly due to a large variety of historically used definitions and *** to other histological subtypes of GC,PCC-GC is nevertheless characterized by a distinct set of epidemiological,histological and clinical features which require a specific diagnostic and therapeutic *** aim of this review was to provide an update on the definition,classification and therapeutic strategies of *** focus on the updated histological definition of PCC-GC,along with its implications on future treatment strategies and study ***,specific considerations in the diagnostic management are ***,the impact of some recent developments in the therapeutic management of GC in general such as the recently validated taxane-based regimens(5-Fluorouracil,leucovorin,oxaliplatin and docetaxel),the use of hyperthermic intraperitoneal chemotherapy as well as pressurized intraperitoneal aerosol chemotherapy and targeted therapy have been reviewed in depth for their relative importance for PCC-GC in particular.

BACKGROUND The overall risk of de novo malignancies in kidney transplant recipients(KTRs)is higher than that in the general *** is associated with long-lasting exposure to immunosuppressive agents and impaired oncological vigilance due to chronic kidney *** cancer(CRC),frequently diagnosed in an advanced stage,is one of the most common malignancies in this cohort and is associated with poor ***,because of the scarcity of data concerning adjuvant chemotherapy in this group,there are no clear guidelines for the specific management of the CRCs in *** present a patient who lost her transplanted kidney shortly after initiation of adjuvant chemotherapy for colon *** SUMMARY A 36-year-old woman with a medical history of kidney transplantation(2005)because of end-stage kidney disease,secondary to chronic glomerular nephritis,and long-term immunosuppression was diagnosed with locally advanced pT_(4A)N_(1B) M_(0)(clinical stage Ⅲ)colon adenocarcinoma *** right hemicolectomy,the patient was qualified to receive adjuvant chemotherapy that consisted of oxaliplatin,leucovorin and 5-fluorouracil(FOLFOX-4).The deterioration of kidney graft function after two cycles caused chemotherapy cessation and initiation ofhemodialysis therapy after a few months. Shortly after that, the patient started palliativechemotherapy because of cancer recurrence with intraperitoneal *** of adjuvant chemotherapy for colon cancer increases the risk of rapid kidney graft lossdriven also by under-immunosuppression。

Botulinum toxin associated with fissurectomy and anoplasty for hypertonic chronic anal fissure: A case-control study

BACKGROUND Lateral internal sphincterotomy is still the approach of choice for the treatment of chronic anal fissure(CAF)with internal anal sphincter(IAS)hypertonia,but it is burdened by high-risk postoperative faecal incontinence(FI).Sphincter saving procedures have recently been reconsidered as treatments to overcome this *** most employed procedure is fissurectomy with anoplasty,eventually associated with pharmacological *** To evaluate whether fissurectomy and anoplasty with botulinum toxin injection improves the results of fissurectomy and anoplasty *** We conducted a case-control study involving 30 male patients affected by CAF with hypertonic IAS who underwent fissurectomy and anoplasty with V-Y cutaneous flap *** patients were divided into two groups:Those in group I underwent surgery alone,and those in group II underwent surgery and a botulinum toxin injection directly into the *** were followed up for at least 2 *** goals were to achieve complete healing of the patient and to assess the FI and recurrence rate along with manometry *** The intensity and duration of post-defecatory pain decreased significantly in both groups of patients starting with the first defecation,and this reduction was higher in group *** days after surgery,we achieved complete wound healing in all the patients in group II but only in 80%of the patients in group I(Psurgery alone in patients affected by CAF with IAS hypertonia.

T-cell proliferation is inhibited by the induction of indoleamine 2,3-dioxygenase in spleen-derived dendritic cells in rat

Background Increasing evidence suggests that, by the production of indoleamine 2, 3-dioxygenase （IDO）, dendritic cells （DC） may reduce the activity of T lymphocytes and inhibit T lymphocyte proliferation-induced immune *** promising way is inspired by increasing IDO expression in DG cells for immune tolerance after transplantation. The aim of this work was to examine the effect of interferon-y （IFN-γ） on the expression of IDO by *** Spleen-derived rat DCs were cultured and induced by cytokines, and the expression of OX62 and surface molecules CD80 and CD86 were measured with flow cytometry. After the DCs were induced by IFN-γ at different concentrations （0, 100, 300, 500 U/ml）, the expression levels of IDO mRNA were measured with real-time PCR, and the expression levels of IDO protein in DCs were measured with Western blotting. The allogeneic mixed lymphocyte reaction （MLR） was used to test the effects of DCs incubated with different concentrations of IFN-γ on allogeneic T lymphocyte *** Under the microscope, the DCs induced by IFN-γ showed a typical dendritic morphology. The expression rate of OX62 was above 80% and the positive expression rates of CD80 and CD86 were both about 80%. The expressions of IDO mRNA and IDO protein increased gradually with the increase of IFN-γ concentration, showing statistical significance in the differences between the groups （P 〈0.05）. Compared with the control DC, the DC incubated with IFN-γ had a notable decrease in allostimulatory activity （P 〈0.05）. With the increasing IFN-γ concentration, the T lymphocyte proliferation decreased, and the difference between the groups was also statistically significant （P 〈0.05）.Conclusions The highly purified spleen derived rat DCs can be successfully acquired through the improved adhesion in-vitro method. IFN-γ can induce increased expression of IDO in spleen-derived rat DCs and reduce the spleen DCs＇ capacity to stimulate the proliferation of allogeneic T cel
