Superior rectus/levator complex in acquired anophthalmic socket repaired with spheric implant—a computed tomography scan and topographic study

AIM:To determine whether the levator palpebrae superioris(LPS)/superior rectus(SR)muscle complex,can influence the position of the upper lid and fornix in acquired anophthalmic ***:This comparative non-randomized and non-interventional study included retrospective data of 21 patients with unilateral acquired anophthalmic sockets repaired with spheric ***-resolution computed tomography(CT)measurements of the LPM/SR muscle complex and clinical topographic position of the upper lid,superior and inferior fornix depth in primary gaze position were *** data were presented as frequency and percentage proportions and quantitative variables comparing the socket measurements with the normal contralateral orbit was statistically analyzed using non-parametric tests considering P0.05,all comparisons).The superior fornix depth was similar in the anophthalmic socket and the contralateral normal orbit(P=0.192)as well the inferior fornix depth(P=0.351).CONCLUSION:Acquired anophthalmic sockets repaired with spheric implants have shorter LPS,thicker SR,and more lagophthalmos than normal *** relationship of the LPS and SR with other orbital structures,associated with passive or active forces acting in the final position of the lids and external ocular prosthesis should be further investigated.

Aim: To report severe visual loss caused by optic nerve avulsion (ONA) in children with door-handle trauma. Methods: Clinical records at a tertiary eye care hospital, of 14 children who sustained severe visual loss as a result of door-handle injuries, were reviewed. The data were analysed for location, presenting symptoms and signs, diagnostic studies, intervention, and the cause of visual loss. Results: There were 11 males and three females with an average age of 8 years and an average height of 125 cm. The place of trauma was home in 11 and school in three children. Presenting visual acuity (VA) was light perception (LP) in five patients and no light perception (NLP) in nine. All the 14 children had evidence of ONA and four patients had ruptured eye globes that required initial repair. The diagnosis of ONA was made clinically or by imaging studies and confirmed histopathologically in eyes that were enucleated. Average follow up was 28.8 months (range 4 months to 8 years). Final VA was LP in one patient and NLP in 13 patients, eight eyes required enucleation for painful blind eye or to achieve optimal cosmesis. Conclusion: ONA was the common cause of visual loss in children who sustained ocular trauma caused by door-handles.

Background:Bowen’s disease (BD), also known as squamous intraepidermal carcinoma, is a malignant skin tumor with a potential to progress to invasive carcinoma. Objective:We sought to report a large series of patients with BD treated with Mohs micrographic surgery (MMS). Methods:This prospective, multicenter, case series included all patients in Australia treated with MMS for BD, who were monitored by the Skin and Cancer Foundation between 1993 and 2002. Results:There were 270 cases; the majority (93.4%) were located in the head and neck area. In 50.7%of cases it was a recurrent tumor. In 20%the tumor was initially misdiagnosed as basal cell carcinoma or squamous cell carcinoma. No cases with perineural invasion were diagnosed. There were 6 cases of recurrence (6.3%) of 95 patients who completed a 5-year follow-up period after MMS. Conclusion:The low 5-year recurrence rate of BD with MMS emphasizes the importance of margin-controlled excision, especially in the head and neck area where tissue preservation is essential.

Objectives: To report a patient with ischemic orbital compartment syndrome as a complication of spinal surgery in the prone position. Design: Interventional case report. Methods: An 80-year-oldman underwent a prolonged lumbar decompression laminectomy for spinal stenosis, under general anesthesia in the prone position. Several hours later, the patient complained of left periocular pain and reduced vision. Examination revealed significant facial edema, left proptosis, and a tight orbit, as well as no light perception and elevated intraocular pressure in the left eye, with complete internal and external ophthalmoplegia. Main Outcome Measures: Clinical course, imaging findings, management, and final outcome. Results: Magnetic resonance imaging confirmed the clinical diagnosis of a compartment syndrome with elevated intraorbital tension. A lateral canthotomy and cantholysis were performed, and high-dose IV steroids were started. The proptosis and facial swelling subsided gradually, but no improvement was noted in left visual acuity or left ocular movements. Conclusion: It is important to be familiar with this rare complication after prolonged surgery in the prone position. Although the prognosis seems to be poor, it is essential to monitor these patients perioperatively and to intervene surgically and medically once the diagnosis of orbital compartment syndrome is established.

Objectives: To present the first reported case of eyelid involvement in pyodermatitis-pyostomatitis vegetans (PDPSV) leading to the diagnosis of ulcerative colitis, and to reviewthe literature. Design: Interventional case report. Methods: A 29- year-old man presented with a 4- week history of severe bilateral upper and lower eyelid margin ulceration and pustules unresponsive to topical and systemic broad-spectrum antibiotic treatment. Further questioning revealed the existence of skin and oral lesions. Main Outcome Measures: Clinical course, histological findings, and response to treatment. Results: The histological and immunofluorescence studies were suggestive of PDPSV. Colonoscopy showed significant chronic active ulcerative colitis. Treatment with systemic steroids and sulfasalazine resulted in complete resolution of eyelid, oral, and skin lesions. However, stopping the steroids resulted in recurrence of eyelid and oral lesions and required recommencement of treatment. Conclusion: It is important to be familiar with this pustular skin condition, as correct diagnosis may lead to the diagnosis of inflammatory bowel disease. Although periocular involvement is probably rare, the combined typical skin and oral lesions and the characteristic histological and immunofluorescence tests should suggest the correct diagnosis.

Objectives: To present a large series of patients with orbital invasion by per iocular basal cell carcinoma (BCC). Design: Retrospective, noncomparative, inter ventional case series. Patients: All cases diagnosed with orbital invasion by pe riocular BCC between January 1985 and July 2004 in 3 Orbital Units in Australia. Methods: The clinical records of all patients were reviewed. Main Outcome Measu res: Patients’demographics, clinical presentation, histologic subtypes, treatme ntmodalities, recurrence rate, and tumor-related death. Results: There were 64 patients (49 males) with a mean age of 70±13 years. Most tumors (84.4%) were r ecurrent or previously incompletely excised, and the medial canthus was most fre quently involved (56.2%). Signs suggestive of orbital involvement included a ma ss with bone fixation (35.7%), limitation of ocular motility (30.4%), and glob e displacement (17.6%). There were no signs suggestive of orbital invasion in 3 5.7%. Most patients (51.6%)-had infiltrative histologic findings, and perineu ral invasion was present in 19.3%. Treatment modalities were mainly exenteratio n alone or combined with radiotherapy. During a mean follow-up period of 3.6 ye ars, 3 cases of recurrence (4.7%)were diagnosed. Only 1 patient (1.6%) died fr om tumor-related causes. Conclusions: Orbital invasion by periocular BCC is an uncommon event that may be associated with significant ocular morbidity and, rar ely, death. Because orbital invasion may often be clinically silent, clinicians need to be alert to the possibility in high-risk tumors and consider appropriat e imaging. Surgical treatment with exenteration or excision, with or without rad iotherapy, results in a low recurrence and mortality rate.

Objectives: To report a patient with a rare case of orbital eosinophilic angiocentric fibrosis (EAF) and to review the literature. Design: Interventional case report. Methods: A 61-yearold man presented with a 6-week history of right periorbital edema and painless proptosis. Examination revealed a nonaxial proptosis, lateral globe displacement, and mild limitation in right eye adduction. Main OutcomeMeasures: Clinical course and radiological and histological findings. Results: Orbital imaging revealed a right medial orbital mass with involvement of middle ethmoidal air cells. An orbital biopsy of the mass demonstrated an inflammatory infiltrate with a marked eosinophilic component, onion skinning of vessels, and surrounding fibrosis. The diagnosis of orbital EAF was made. There was no response to a 3-month treatment course with systemic steroids, but the patient did not want any further surgical interventions. Conclusion: Although orbital EAF is rare, ophthalmologists need to be aware of this entity, as it may invade the orbit from the sinonasal tract or present as a localized orbital mass. The presence of even minimal sinus involvement and the characteristic histopathology are useful in establishing the correct diagnosis.

Objective: To report severe ocular injuries caused by pointed door handles in children. Design: Noncomparative, interventional, retrospective case series. Methods: Clinical records of 15 patients who sustained severe ocular injuries from door handles were reviewed for place of trauma, presenting symptoms and signs, surgical procedures performed, and final outcome. Main Outcome Measures: Trauma caused by complications from the door handle injury, such as loss of vision and ocular and adnexal injuries, and treatment rendered. Results: There were 12 boys and 3 girls with an average age of 7.8 years (range, 6-10 years). Place of trauma was at home in 12 children and in school in 3 children. Presenting visual acuity (VA) was 4/200 in 1 patient, light perception (LP) in 5 patients, and no light perception (NLP) in 9 patients. There were 9 eyelid or canalicular laceration s, 4 ruptured globes, and 14 optic nerve avulsions. All ruptured globes were repaired initially and eyelid and canalicular lacerations were repaired primarily within 24 hours of presentation. Average follow-up was 21 months (range, 1 month-8 years). Final VA was 20/30 in 1 patient, LP in 1 patient, and NLP in 13 patients. Eight eyes required enucleation for painful blind eye or to achieve optimal cosmesis. Conclusions: Pointed door handles installed at current heights may pose a significant risk of ocular and periocular injuries among young children.

Objectives: To present the surgical outcomes and postoperative complications in a series of patients who underwent upper lid surgery using autogenous hard palate grafts (HPGs) or free tarsal grafts (FTGs) as posterior lamella replacement material. Design: Retrospective, comparative, interventional case series. Patients: Thirty-one consecutive patients who were operated in 2 oculoplastics centers between July 2000 and January 2005. Methods: All patients’ clinical records were reviewed. Main Outcome Measures: Postoperative upper eyelid contour and viability, ocular discomfort, keratopathy, and corneal edema, as well as assessment for donor site complications and final graft dimensions. Results: There were 31 patients who underwent upper lid surgery (15 HPGs, 16 FTGs). The complications in the HPG group included corneal edema or transient keratopathy (13.3% ), partial graft dehiscence (13.3% ), upper lid retraction (13.3% ), and necrosis of the overlying skin flap (6.7% ). There were no significant postoperative complications in the FTG group during a mean follow-up period of 13.5± 5 months. Donor site complications included 2 cases of mild upper lid retraction and central peaking. There were an average of 17% decrease in FTG vertical height and a 24% decrease in HPG vertical height during the follow-up period. Conclusion: Hard palate grafts may be associated with a higher rate of complications in upper lid surgery relative to FTGs, although most complications are temporary. Graft contraction could be reduced by oversizing.
