Sacrospinous fixation (SSF) or Richter’s intervention (RI) aims to treat genital prolapse by securing the posterior vaginal wall to the small sacrospinous ligament. It is performed by low approach and includes a dissection of the pararectal space, visual exposure of the sacrospinous ligament and a needle with strait needle holder with nonabsorbable threads. It is often associated with a more complex corrective procedure, including cystocele cure, vaginal hysterectomy and posterior myorrhaphy. The objective of this study is to report the results of SSF in the gynecology department of Hopital du Mali. A descriptive study was conducted from September 2014 to September 2015 concerning 37 patients operated on for uterine prolapse (UP). All patients with grade III UP were included in our study in whom a unilateral hysterectomy (UH) and sacrospinous fixation (SSF) were performed. All the patients were scheduled. Preoperatively they had benefited from an assessment and a pre-anesthetic consultation. Hospitalization of at least 24 hours prior to the operation was required. Postoperative follow-up was two years with a physical examination at 3 months, 9 months and 15 months, and phone calls between physical consultations. During the study period, we performed 37 RIs. The mean age of the patients was 48 years with extremes of 41 to 73 years. The large multiparity was found out in 35 cases (94.59%), the pauciparous were two with 3 deliveries for each. Long labor of more than 18 hours was found out in 9 patients (24.32%) and home delivery in 13 cases (35.13%). The duration of the occurrence of prolapse was at least two years and 35 patients were going through menopause. The type of anesthesia used for the surgery was spinal anesthesia for all patients. Objective: To learn the epidemiological, clinical, paraclinical, therapeutical aspects and prognosis of haemorrhage of the third trimester during pregnancy. Methodology: It is about a transversal and descriptive study, realized at the department of obstetrics and gynaecology from January 1st to June 30th 2020. Evaluation of the Practice of Childbirth in Cases of Antecedent Uterine Scarring at the Maternity Ward of the Community Hospital Centre in Bangui

Objective: Pregnancy in a scarred uterus is considered to be high-risk, especially during parturition. Nevertheless, the literature favours vaginal delivery as long as obstetric conditions are favourable. The aim of our study was to contribute to the improvement of delivery management for women with scar uteri at the CHU Communautaire. Methodology: This was a cross-sectional study conducted over an 18-month period from 01 January 2020 to 30 June 2021 on women with scar uterus who came to give birth at the CHU Communautaire maternity unit. The sample was exhaustive;any parturient with a history of scarred uterus of more than one year with a clinically normal pelvis and whose fetus was in cephalic presentation. The following were excluded from the study: pregnant women with a uterine scar of less than one year, fetuses in a non-cephalic vertex presentation, pregnant women with antecedent complicated childbirth at the previous cesarean section, large fetuses informed consent had been obtained from the pregnant women. Anomynia was respected throughout the study. Results: During this period, we recorded 205 parturients with a scar uterus. The incidence of delivery with a scar uterus was 9.5%. Uterine testing was attempted in 85.9% of cases, with a success rate of 75.6%. Factors predictive of a successful uterine test was: maternal age less than 35 years (P = 0.0027), entry into labour at the reference maternity hospital (P Conclusion: Delivery with a scarred uterus is an increasingly frequent occurrence at the Maternity Unit of the CHU Communautaire in Bangui. Uterine testing is the first option if conditions allow, but careful selection of candidates during antenatal care or at the very beginning of labour is necessary for this type of delivery.

Objective: To study the epidemiological, clinical, therapeutical, anatomopathological aspects and prognosis of the ectopic pregnancy. Methodology: It was a transversal and descriptive study from January 1st to December 31st 2018 at the department of obstetrics and gynecology of the Teaching Hospital Sourô Sanou of Bobo-Dioulasso. Results: During the study period, we registered 4706 deliveries with 1272 cases of cesareans and 79 cases of ectopic pregnancy with a frequency of 1 ectopic pregnancy for 60 deliveries and 6 ectopic pregnancies for 100 caesareans. The average age of the patients in our study was 28.66 years old (19 - 45 years) and the average parity of 1.96 [0 - 6]. Married women represented 83.54 % of the workforce. The risk factors were dominated by sexual transmitted diseases (25.31%) and abortions (20.25%). Clinical signs were dominated by pelvic pain (100% of cases), metrorrhagea (86.5% of cases) and amenorrhea (64.6% of cases). Culdocentesis brought lysed incoagulable blood in 83.7% cases. Acupuncture Enhances Chances of Pregnancy in Unexplained Infertile Patients Who Undergo A Blastocyst Transfer in A Fresh-Cycle

Objective: To analyze the effects of acupuncture on in vitro fertilization patients with unexplained ***: We retrospectively analyzed the charts of a total of 302 cycles performed in 273 patients attending Assisted Reproductive Technology(ART) Centre from August 2013 to August *** the study period,embryo transfer with acupuncture(Acupuncture group,46 cases) and without acupuncture(Control group,42 cases) were *** to embryo transfer,the following points were used in the acupuncture group: Neiguan(CX 6),Diji(SP 8),Taichong(Liv 3),Baihui(Gv 20),and Guilai(S 29).These sessions were carried out two times before and after embryo transfer in a single *** addition,auricular acupuncture was also performed at ear points,including ear point 55(Shenmen),ear point 58(Zhigong),ear point 22(Neifenmi)and ear point 34(Naodian).The biochemical pregnancy rate and clinical pregnancy rate after transplantation were compared between two ***: The clinical pregnancy rate in the acupuncture group was higher than that in the control group [60.9%(28/46) vs.33.3%(14/42),respectively,P<0.05].Likewise,the live birth rate in the acupuncture group was also higher than that in the control group [71.7%(33/46) vs.31.0%(12/42),P<0.01).Conclusion: Administration of acupuncture on the day of embryo transfer dramatically improved fertility results in women who underwent in vitro fertilization/intra-cytoplasm sperm injection for reproduction.

Uterine Rupture: Epidemiological Aspects, Etiologies and Maternal-Fetal Prognosis in the Obstetric gynecology department of the Donka CHU Conakry National Hospital, Guinea

Objectives: The objectives of this work were to calculate the frequency of the uterine rupture, to describe the epidemiological profile, to identify the etiologies and to establish the maternal prognosis and foetal. Methodology: It was about a descriptive survey with compilation of the data in two phases: a retrospective spreading on one period of 6 years and the other forecasting of 1 one year achieved to the service of Obstetric gynecology of the hospital National Donka, Fallen from Conakry, Guinea. Results: We recorded 24.030 childbirths of which 188 cases of uterine rupture either a frequency of 0.78, which represents an uterine rupture for 128 childbirths. The epidemiological profile was the one of a woman of 24 to 28 years (31.91%), housewives (69.14%), without prenatal follow-up (47.87%), big multipare (37.76%) and évacuées (78.78%). The motives of consultation have been dominated by the hémorragie (95.74%). The rupture was of transverse type in the majority of the case (63.82%). The hysterorraphy was the most performed surgical procedure which is 85.10% followed by the total sub hysterectomy in 10.63%. The newborns of birth weight superior or equal to 4000 g represent 25.53%. The maternal morbidity has been dominated by the anemia of the postpartum (60%). We recorded a rate of maternal létalité of 12.76%. The maternal deaths were due to the hemorrhage in 78.57%. The living newborns endured a respiratory distress in 9.57% and those stillborn represent 87.23%. The etiologies of uterine rupture were dominated by fetal-pelvic disproportions 48.93% followed by an iatrogenic uterine rupture 22.33%. Conclusion: The reduction of this uterine rupture rate would pass by the recentered prenatal consultation offered, the one of obstetric cares and complete néonataux of emergency, the discount to level of the beneficiaries of the basic structures so that they can discover the cases in time susceptible to drag a rupture to evacuate better in time and the promptness in the

Conjunctival Granuloma Caused by an Unknown Metallic Foreign Body

Introduction: Conjunctival granulomas are tumor pathologies of various origins. A foreign body, whatever its nature, is often involved in its genesis. To date, there is very little data on foreign body conjunctival granulomas, the prevalence would be approximately 10% and their underestimation is probably linked to the inconsistency of their clinical presentation. Patient and Observations: We report a case of conjunctival granuloma caused by a metallic foreign body that went unnoticed in a young boy, the interest of rigor on primary protection measures for high-risk occupations, and of the strict application of the diagnostic protocol relating to ocular trauma. Discussion: foreign body granulomas, despite their usually harmless nature, can jeopardize the anatomical and functional prognosis of the affected eye. Only a detailed interview and a meticulous clinical examination allow the diagnosis to be made and the distinction to be made with conjunctival granulomas of different causes. Conclusion: the possibility of the involvement of a foreign body in the genesis of conjunctival granulomas must impose the rigorous application of diagnostic and therapeutic protocols.

GGN repeat length of the androgen receptor gene is associated with antral follicle count in Chinese women undergoing controlled ovarian stimulation

Objective To evaluate the association of GGN repeat polymorphism of androgen receptor(AR)with ovarian reserve and ovarian response in controlled ovarian stimulation(COS).Methods This genetic association study was conducted among a total of 361 women aged≤40 years with basal FSH≤12 U/L undergoing the GnRH-agonist long protocol for COS in a university affiliated IVF *** repeat in the AR gene was analyzed with Sanger *** primary endpoint was the number of antral follicle counts(AFCs),and the secondary endpoints were stimulation days,total dose of gonadotropin(Gn)used,total number of retrieved oocytes,ovarian sensitivity index,and follicular output *** The GGN repeat in exon 1 of the AR gene ranged from 13 to 24,and the median repeat length was *** on the genotypes(S for GGN repeats0.05).Generalized regression analysis indicated no significant differences in ovarian stimulation parameters among the 3 groups,either before or after adjusting for confounding factors(P>0.05).Conclusion GGN repeat length on the AR gene is associated with AFC but not with ovarian response in Chinese women,indicating that AR gene polymorphisms may affect ovarian reserve.

Dear Editor,Alpha-thalassemia (α-tha) is an inherited hemolytic disease with an increasing prevalence worldwide that is caused by a complete or partial absence of α-globin chain synthesis due to α-globin chain synthesis disorders (Farashi and Harteveld, 2018). Mild α-tha is generally considered untreatable, whereas transfusion-dependent and severe forms require lifelong transfusions, which can lead to chronic iron overload necessitating chelation therapy (Horvei et al., 2021; Baird et al., 2022). Recombinant lentiviral vectors(LVs) have been shown to be effective in transducing nondividing hematopoietic stem cells (HSCs)(Chandrakasan and Malik, 2014). Kan and colleagues reported that direct in-utero delivery of a lentiviral-based human α-globin gene to a mice model of α-tha initially led to high,erythroid-specific expression, which decreased to low levels over time (Han et al.,2007). While numerous gene therapy strategies have been developed for β-tha,there is limited research on the use of LVs carrying the α-globin gene for treating α-tha, indicating a major unmet medical need for this patient population.
