Minimally Invasive Total Arterial Coronary Artery Bypass Grafting in Left Main Stem Disease

Objective: The aim was to show that Minimally Invasive total arterial revascularization for left main stem coronary artery disease, via a left anterior Mini thoracotomy using bilateral internal thoracic arteries is feasible, reproducible and safe. Further, there has been no exclusive data or experience with minimally invasive coronary artery bypass grafting in left main stem disease. Methods: From April 2019 to March 2024, 41 patients with left main stem stenosis, left main equivalent disease and unprotected left main with triple vessel disease underwent off pump minimally invasive multivessel coronary artery bypass grafting using either in situ pedicled Bilateral Internal Thoracic arteries or Left and Right Internal Thoracic artery Y composite conduits at three centers. Bilateral Internal Thoracic arteries were harvested under direct vision. All patients had an Intra-Aortic Balloon Pump inserted via the femoral artery prior to induction of anesthesia, to prevent any hemodynamic instability, arrhythmias, and was removed following completion of the procedure in the operating room without any complications. Efficacy and outcomes were evaluated by i) Primary (MACCE)-Major Adverse Cardiac and Cardiovascular events and ii) Secondary outcome measures including total length of stay, return to full physical activity and quality of life. Mean follow-up was 1.4 years (Maximum was 2.5 years). Results: 41 patients with left main stem coronary artery stenosis, underwent total arterial revascularization using bilateral internal thoracic arteries. Left main stem stenosis was present in 29 patients, Unprotected left main stem stenosis with triple vessel disease in 7 and left main equivalence in 5 patients. In this cohort, 29 patients with only left main stem stenosis had 2 grafts each, 7 patients with left main and triple vessel disease had 3 grafts and 5 patients with left main equivalent disease had 2 grafts respectively. This work presents a study of the Paleogene sandstones of the Manika plateau in Kolwezi, dr.Congo. These sandstones belong to the “Grès polymorphes” group, which together with the overlying “Sables ocre” makes up the Kalahari Supergroup. Sedimentological and geochemical analyses have enabled us to characterize these sandstones and determine their origin, the conditions of their formation and the tectonic context in which they were developed. The results show that the sandstones are quartz arenites with a high level of mineralogical, textural and chemical maturity. They are recycled sandstones, formed in an intracratonic sedimentary basin, in the context of a passive continental margin, after a long fluvial transport of sediments. These sandstones initially come from intense alteration of magmatic rocks with felsic composition, mainly tonalite-trondhjemite-granodiorite (TTG) complexes, in hot, humid palaeoclimatic conditions and oxidizing environments.

The vortex generator(VG)and its well-known effect in flow optimization are widely studied and employed across different engineering ***,while the same working principles of VG may be well suited for the applications on surface-cleaning technologies,such promising potential is hardly,if any,explored in the published ***,in the present study,the influence on flow-induced particle resuspension brought by a rectangular vG in a channel flow is investigated with the help of highfidelity computational fluid dynamics *** increases of particle removal forces and resuspension rates are discovered in long,strip-like regions with reduced boundary-layer thickness resulted from the vG-induced vortices,and the enhancement effect is especially significant for configurations with the VG installed at a greater angle of *** is also shown that while the resuspension enhancements on the lower and the upper surfaces of the channel exhibit distinct statistical characteristics,having a VG in the channel improves the overall particle-removing capability of the channel flow by introducing higher surface-averaged removal forces and particle resuspension *** but not least,the increase of resuspension rate is especially significant for the smaller,micron-scale particles which are otherwise hardly disturbed by a VG-less channel flow,and such resuspension-enhancement effectgenerally subsides with increasing particle size.

Climate change, mainly caused by the use of non-renewable fuels, has raised global concerns and led to the search for less polluting energy sources, making hydr.gen a promising energy alternative with the potential to contribute to changes in the energy mix of various countries through the use of technologies that enable its production and use with low or zero carbon emissions. In this context, Brazil has aroused great interest from other countries in exploring its renewable resources for the production of hydr.gen (green hydr.gen). In this sense, the use of natural gas pipelines and the use of hydr.gen in mixtures with natural gas have become the subject of studies due to their economically viable alternative for the immediate use of this energy vector. However, there are still technical and regulatory challenges regarding the integration of hydr.gen into the existing natural gas pipeline network. In this context, the present study aims to addr.ss the effects of hydr.gen interaction with the structure of natural gas pipeline steel and the regulatory barriers to the use of this network for the transportation of green hydr.gen, particularly in the state of Ceará/Brazil. After extensive analysis of literature and regulatory documents, it was concluded that: 1) Ceará/Brazil has strong potential to meet the demand for green hydr.gen through the use of solar and wind energy sources;2) there is feasibility for the adaptation or conversion of natural gas infrastructure for the transportation of green hydr.gen;3) discussions regarding the regulatory competence of green hydr.gen transportation and distribution through the natural gas network in Brazil are still incipient;4) the current regulation of the natural gas industry can serve as a subsidy for the regulation of green hydr.gen and natural gas transportation.

Real-world evidence (RWE) is clinical evidence on a medical product’s safety and efficacy that is generated using real-world data (RWD) resulting from routine healthcare delivery. This study evaluates the clinical efficacy of amoxicillin + clavulanic acid in childr.n with pharyngitis, acute otitis, or acute rhinosinusitis with suspected bacterial origin under normal office and home conditions. Methods: This was a real-life, prospective, observational, pharmacovigilance study. It included childr.n of both sexes between 2 and 12 years old, with a diagnosis of Rhinopharyngitis (tonsillitis), Acute Otitis Media and Rhinosinusitis. The main effectiveness variable evaluated was reduction and time to resolution of symptoms. All patients received Amoxicillin/Clavulanic Acid suspension 600 mg/42.9 mg/5 mL at a dose of 90 mg/Kg/day in two doses, every 12 hours for 7 days. The evaluations were carried out at the beginning, at 72 hours (3rd day) and at 7 days. All patients underwent culture and antibiogram. Results: The majority of cultures were negative for pathogenic germs, suspecting unidentifiable germs, or viral etiology despite the rigorous selection of subjects following validated scores. The most frequently isolated germ was Staphylococcus aureus;growth of gram-negative bacteria was reported in 33.33% of the cultures. There was a significant improvement in symptoms in childr.n with tonsillitis and rhinosinusitis from the first 72 hours of treatment, persisting until the 7 days. In the otitis media group, returning to normal by the tenth day. During the conduction of this investigation, no adverse effects associated with the prescribed therapy were reported.

The dynamic transformation of land use and land cover has emerged as a crucial aspect in the effective management of natural resources and the continual monitoring of environmental shifts. This study focused on the land use and land cover (LULC) changes within the catchment area of the Godavari River, assessing the repercussions of land and water resource exploitation. Utilizing LANDSAT satellite images from 2009, 2014, and 2019, this research employed supervised classification through the Quantum Geographic Information System (QGIS) software’s SCP plugin. Maximum likelihood classification algorithm was used for the assessment of supervised land use classification. Seven distinct LULC classes—forest, irrigated cropland, agricultural land (fallow), barren land, shrub land, water, and urban land—are delineated for classification purposes. The study revealed substantial changes in the Godavari basin’s land use patterns over the ten-year period from 2009 to 2019. Spatial and temporal dynamics of land use/cover changes (2009-2019) were quantified using three Satellite/Landsat images, a supervised classification algorithm and the post classification change detection technique in GIS. The total study area of the Godavari basin in Maharashtra encompasses 5138175.48 hectares. Notably, the built-up area increased from 0.14% in 2009 to 1.94% in 2019. The proportion of irrigated cropland, which was 62.32% in 2009, declined to 41.52% in 2019. Shrub land witnessed a noteworthy increase from 0.05% to 2.05% over the last decade. The key findings underscored significant declines in barren land, agricultural land, and irrigated cropland, juxtaposed with an expansion in forest land, shrub land, and urban land. The classification methodology achieved an overall accuracy of 80%, with a Kappa Statistic of 71.9% for the satellite images. Introduction: A traumatic brain injury (TBI) is caused by a forceful bump, blow, or jolt to the head or body, or by an object that pierces the skull and interrupts the normal function of the brain. Severe TBI is estimated at 73 cases per 100,000 people. The mortality of severe TBI can be reduced if a timely diagnosis and treatment of the injuries are made through prognostic factors. Objective: To determine the prognostic factors related to mortality in severe traumatic brain injury at the Hospital General de Zona No. 46. Material and Methods: Retrospective, cross-sectional and descriptive study in beneficiaries admitted to the Hospital General de Zona (HGZ) No. 46 of the Mexican Institute of Social Security (IMSS by its acronym in Spanish), with a diagnosis of severe TBI;the possible prognostic factors related to mortality of severe TBI were obtained from their records. Measures of central tendency and chi square were used for data analysis. Results: The study sample consisted of 60 subjects diagnosed with severe traumatic brain injury, of which 5 (8%) were women and 55 (92%) were men, and all 60 (100%) patients died. The average age of the sample was 26 with a standard deviation of 9 years. The variables that had a p value less than or equal to 0.05 were: Mydr.asis, seizures, Hyperglycemia, Normoglycemia, Hypothermia and Hypotension. This means that these variables were associated with mortality. Conclusion: Statistical significance is demonstrated in prognostic factors of mortality in severe traumatic brain injury with p dr.asis, seizures, hyperglycemia, normoglycemia, hypothermia and hypotension.

Introduction: COVID-19 was an emerging disease putting all public health systems in countries around the world in a state of emergency. To be able to prevent its spread and morbidity and mortality, several appropriate strategies were necessary, such as vaccination. The latter has been the subject of controversy. The objective of the present study is therefore to evaluate the factors associated with the acceptance of this vaccine within the population of the Kasenga State Health Area. A result which will shed light on future strategies to be put in place for possible new vaccines. Methodology: Is a prospective and analytical cross-sectional study conducted over a period of approximately 1 month from January 5 to February 5, 2024. A survey questionnaire in Kobotoolbox was useful for collecting data. STATA software was very important for us in analyzing the data collected. Results: Prevalence of vaccination against COVID-19 among the population of the Kasenga State Health Area is 37.5% (28.4 - 45.6). The study revealed that reluctance is observed among most of the population for different reasons, including, first and foremost, the deliberate aspect of not wanting to take the vaccine (46.6%) and rumors that this antigen is dangerous and harmful (32.9%). 72.5% of respondents believe that the COVID-19 vaccine is a fabrication, unhealthy and that the disease itself never existed. The study proved that there was a statistical relationship between age (p = 0.001) and adherence to vaccination. And the refusal of respondents to recommend the vaccine to loved ones was a factor associated with non-adherence to vaccination (OR = 7.901, 95% IC [3.028 - 20.615], p = 0.000). Conclusion: Vaccination against COVID-19 was not well accepted by the population of the study site. Raising public awareness and involving community leaders and political-administrative authorities, which has not been done well, would play an important role in the good perception of the disease, of the vaccine
