myeloproliferative neoplasms(mPNs)are defined as clonal disorders of the hematopoietic stem cell in which an exaggerated production of terminally differentiated myeloid cells ***,Philadelphia-negative mPNs,i.e.,polycythemia vera,essential thrombocythemia and primary myelofibrosis,exhibit a propensity towards the development of thrombotic complications that can occur in unusual sites,e.g.,portal,splanchnic or hepatic veins,the placenta or cerebral *** pathogenesis of thrombotic events in mPNs is complex and requires an intricate mechanism involving endothelial injury,stasis,elevated leukocyte adhesion,integrins,neutrophil extracellular traps,somatic mutations(e.g.,the V617F point mutation in the JAK2 gene),microparticles,circulating endothelial cells,and other factors,to name a ***,we review the available data on Budd-Chiari syndrome in Philadelphia-negative mPNs,with a particular focus on its epidemiology,pathogenesis,histopathology,risk factors,classification,clinical presentation,diagnosis,and management.

Background: The prevalence of heart failure with preserved ejection fraction may be changing as a result of changes in population demographics and in the prevalence and treatment of risk factors for heart failure. Changes in the prevalence of heart failure with preserved ejection fraction may contribute to changes in the natural history of heart failure. We performed a study to define secular trends in the prevalence of heart failure with preserved ejection fraction among patients at a single institution over a 15-year period. methods: We studied all consecutive patients hospitalized with decompensated heart failure at mayo clinic hospitals in olmsted County, minnesota, from 1987 through 2001. We classified patients as having either preserved or reduced ejection fraction. The patients were also classified as community patients(olmsted County residents) or referral patients. Secular trends in the type of heart failure, associated cardiovascular disease, and survival were defined. Results: A total of 6076 patients with heart failure were discharged over the 15-year period; data on ejection fraction were available for 4596 of these patients(76 percent). of these, 53 percent had a reduced ejection fraction and 47 percent had a preserved ejection fraction. The proportion of patients with the diagnosis of heart failure with preserved ejection fraction increased over time and was significantly higher among community patients than among referral patients(55 percent vs. 45 percent). The prevalence rates of hypertension, atrial fibrillation, and diabetes among patients with heart failure increased significantly over time. Survival was slightly better among patients with preserved ejection fraction(adjusted hazard ratio for death, 0.96; P=0.01). Survival improved over time for those with reduced ejection fraction but not for those with preserved ejection fraction. A complex network precisely regulates the cell cycle through the G1,S,G2,and m phases and is the basis for cell division under physiological and pathological *** the one hand,the transition from one phase to another as well as the progression within each phase is driven by the specific cyclin-dependent kinases(CDKs;e.g.,CDK1,CDK2,CDK4,CDK6,and CDK7),together with their exclusive partner cyclins(e.g.,cyclin A1,B1,D1–3,and e1).on the other hand,these phases are negatively regulated by endogenous CDK inhibitors such as p16^(ink4a),p18^(ink4c),p19^(ink4d),p21^(cip1),and p27^(kip1).In addition,several checkpoints control the commitment of cells to replicate DNA and undergo mitosis,thereby avoiding the passage of genomic errors to daughter *** are often constitutively activated in cancer,which is characterized by the uncontrolled proliferation of transformed cells,due to genetic and epigenetic abnormalities in the genes involved in the cell ***,several oncogenes and defective tumor suppressors promote malignant changes by stimulating cell cycle entry and progression or disrupting DNA damage responses,including the cell cycle checkpoints,DNA repair mechanisms,and ***,genes or proteins related to cell cycle regulation remain the main targets of interest in the treatment of various cancer types,including hematologic *** this context,advances in the understanding of the cell cycle regulatory machinery provide a basis for the development of novel therapeutic *** present article summarizes the pathways as well as their genetic and epigenetic alterations that regulate the cell cycle;moreover,it discusses the various approved or potential therapeutic targets associated with the cell cycle,focusing on hematologic malignancies.

背景：与裸金属支架相比，药物洗脱支架可降低非复杂病变的再狭窄发生率。尚未在复杂狭窄病变中评价药物洗脱支架的应用情况。目的：在比既往试验更复杂的患者人群中研究多聚缓释紫杉醇洗脱支架的安全性和效果。设计、地点及参试者：于2003年2月至2004年3月在66家大学和社区医疗机构进行前瞻性、安慰剂对照、双盲、多中心试验。1 156例患者接受了单支冠状动脉狭窄（血管直径，2．25～4．0 mm；病变长度，10～46 mm）支架植入治疗，包括664例（57．4％）患有复杂或既往未曾研究的病变（需要2．25 mm、4．0mm和／或多个支架），并于9个月时进行临床和血管造影随访。干预：患者随机分配接受1个或多个裸金属支架（n=579）或者外观相同的紫杉醇洗脱支架（n=577）。主要观测指标：9个月时因缺血而需要靶血管血运重建。结果：基线特征匹配良好。糖尿病患者占31％。平均（SD）参照血管直径为2．69（0．57）mm，参照病变长度为17．2（9．2）mm，B：／C型病变占78％。每个病变平均植入1．38（0．58）个支架（总平均长度[SD]，28．4[13．1]mm）；需要多个支架的病变占33％。使用直径2．25 mm和4．0 mm支架的病变分别为18％和17％。与裸金属支架相比，紫杉醇洗脱支架可使9个月靶病变血运重建率从15．7％降至8．6％（P＜0．001），靶血管血运重建率从17．3％降至12．1％（P=0．02）。心源性死亡或心肌梗死（裸金属支架5．5％，紫杉醇洗脱支架5．7％）和支架血栓形成发生率两者类似（两组均为0．7％）。在整个研究队列，血管造影再狭窄从33．9％降至18．9％（P＜0．001），包括采用2．25 mm支架（49．4％比31．2％；P=0．01）、4．0mm支架（14．4％比3．5％；P=0．02）和多个支架（57．8％比27．2％；P＜0．001）的患者。结论：与裸金属支架相比，在有复杂病变的患者人群中植入紫杉醇洗脱支架可有效降低临床和血管造影再狭窄的发生率。

Context: The American College of Cardiology/American Heart Association(ACC/AHA ) guidelines for the management of non-ST-segment elevation acute coronary syn dromes(NSTe ACS) recommend early invasive management for high-risk patients, gi ven the benefits with this approach demonstrated in randomized clinical trials. objectives: To determine the use and predictors of early invasive management str ategies(cardiac catheterization ours following presentation) in high-risk patients with NSTe ACS and to examine the association of early invasive management with mortality. Design, Set ting, and Patients: The CRUSADe(Can Rapid Risk Stratification of Unstable Angina Patients Suppress Adverse outcomes With early Implementation of the ACC/AHA Gui delines) Quality Improvement Initiative evaluated care patterns and outcomes for 17926 high-risk NSTe ACS patients (positive cardiac markers and/or ischemie el ectrocardiographic changes) based on ACC/AHA guidelines recommendations at 248 U S hospitals with catheterization and revascularization facilities between march 2000 and September 2002. main outcome measures: Use of early invasive management within 48 hours of presentation, predictors of early invasive management, and i n-hospital mortality. Results: of the 17926 patients analyzed, 8037 (44.8%) un derwent early cardiac catheterization less than 48 hours following presentation. Predictors of early invasive management included cardiology care, younger age, lack of prior or current congestive heart failure, lack of renal insufficiency, ischemic electrocardiographic changes, positive cardiac markers, white race, and male sex. Patients treated with early invasive management were more likely to b e treated with medications and interventions recommended by the ACC/AHA guidelin es and had a lower risk of in-hospital mortality after adjusting for difference s in clinical characteristics and after comparing propensity-matched pairs (2.5 %vs 3.7%, Ponclusions: An early invas

蛛网膜下腔出血（subarachnoidhemorrhage，SAH）是一种可对中枢神经系统造成毁灭性打击并对其他多个器官产生显著影响的急性脑血管病。SAH患者被常规收入重症监护病房并由多学科团队进行治疗。由于高质量研究证据的缺乏，使得治疗方法各异，可供选择的指导意见不多。现有的指南主要强调危险因素、预防、自然史以及再出血的预防，很少涉及SAH患者的重症监护问题。美国神经危重症监护学会组织了一次国际性多学科共识会议来探讨SAH的重症监护处理。根据发表的文章和研究领域，此次会议召集了来自欧洲和北美地区的神经重症监护科、神经外科、神经内科、介入神经放射科和神经麻醉科方面的专家。根据临床经验和制定实践指南方面的经验，选择4名经验丰富的神经重症监护专家组成评判委员会。应用推荐分级的评估、制定与评价（GradesofRecommendationsAssessment，Developmentandevaluation，GRADe）系统进行文献回顾，结合参会者的经验、评判委员会的审评以及文献讨论产生推荐意见。应用GRADe系统制定推荐意见，其制定原则不仅强调研究资料的质量，而且还重视利弊权衡与实践转化。对SAH患者日常处理过程中面临的所有问题均提供指导和推荐意见，即使缺乏高质量的证据。
