Symptoms of spinal dysfunction mimicking systemic diseases(ogundana Syndrome):a review of 35 cases treated with manual therapy over 30 years

objective and method:Symptoms manifesting in other parts of the body signaling the existence of spinal dysfunction(ogundana Syndrome)in patients(N=35,age range 20-56yrs;mean age 36yrs)diagnosed to have systemic diseases but treated with manual therapy were identified after a 30 year case note ***:Incipient parkinsonism(general edema and expressionless mask face),stroke(weakness of one side of the body),tinnitus(headache with noise in one or both ears),breast cancer(breast pain)and dyspnea(difficulty in breathing)were some physician diagnoses and clinical symptoms *** percent rate occurrence was noted affecting significantly more females(74%)than males(26%,Polved people with family history of back pain(66%)engaged in low income occupations(farming 40%,trading 25%,artisans 15% and civil servants 20%).Symptoms got better after vertical oscillatory pressure therapy to appropriate spinal *** of treatment was influenced by duration of spinal dysfunction prior to therapy and age of younger ***:one percent of patients with spinal dysfunction misdiagnosed to have systemic diseases due to misleading symptoms are finally referred for physiotherapy of manifesting *** therapy to the appropriate vertebro-geographic region(s)relieves the symptom(s)without prejudice to existing diagnosis.

Relationship between Serum Leptin and Adiponectin Levels in obese Nigerians with Type 2 Diabetes Subjects

Background: obesity is rising globally, independent of ethnicity, race and age, and is associated with increased risk of cardiovascular mortality and morbidity especially in persons living with diabetes. The effect of adipokines such as leptin, resistin and adiponectin which are secreted by adipose tissue factors has been linked to the increased risk of the cardiovascular mortality and morbidity. Aim: This study aims to assess the levels of serum leptin and serum adiponectin in obese type 2 diabetes subjects, and their relationship with cardio-metabolic component (using lipid profile). method: This is a cross-sectional comparative hospital-based study in which one hundred and fifty participants grouped into 60 obese, 60 non-obese none diabetic and 30 non-obese non-diabetic adults with similar age from the endocrinology outpatient’s clinic, General outpatient department (GoPD) and staff clinic of obafemi Awolowo University teaching hospitals complex (oauTHC). Anthropometric parameters and other relevant clinical details of all subjects were obtained. Fasting venous blood samples were taken from all subjects for the determination of fasting plasma glucose (FPG), fasting lipid profile, glycosylated haemoglobin levels (HbA1C), fasting serum insulin, leptin, and adiponectin. Data was analysed using the Statistical Package of Social Sciences (SPSS) version 23.0. Results: Twenty-eight (46.7%)) subjects were males while 32 subjects (53.3%) were females (in obese groups). There was no statistical difference between these two groups for both sexes (p = 1.000). The age range for all subjects was 34 to 64 years with their mean age being 52 ± 7.3 years, and for type 2 subjects and the obese non-diabetic 50.7 ± 7.3 years respectively. The mean body mass index of the non-obese and non-diabetic was 23.7 ± 4 Kg/m2 while the anthropometric measurements of the obese two groups were similar. Pancreatic ductal adenocarcinoma (PDAC) has become a significant health concern with increasing incidence and mortality rates over the past few decades. Researchers have turned their attention to cutting-edge mass spectrometry (mS) technology due to its high-throughput and accurate detection capacity, which plays a vital role in understanding the mechanisms and discovering biomarkers for pancreatic diseases. In this review, we comprehensively investigate various methodologies of quantitative and qualitative proteomics mS technologies, alongside bioinformatical platforms employed in pancreatic cancer research. The integration of these optimized approaches provides novel insights into the molecular mechanisms underlying tumorigenesis and disease progression, ultimately facilitating the discovery of potential diagnostic, prognostic biomarkers, and therapeutic targets. The robust mS-based strategy shows promise in paving the way for early diagnosis and personalized medicine for pancreatic cancer patients.

The“terminal line”:a novel sign for the identification of distal mesorectum end during Tme for rectal cancer

Background:Although the clinical importance of complete,intact total mesorectal excision(Tme)is the widely accepted standard for decreasing local recurrence of rectal cancer,the residual mesorectum still represents a significant component of resection margin *** study aimed to use a visible intraoperative sign to detect the distal mesorectal end to ensure complete inclusion of the mesorectum and avoid unnecessary ***:The distal mesorectum end was investigated retrospectively through a review of 124 operative videos at the Union Hospital of Fujian medical University(Fujian,China)and Cleveland clinic(ohio,USA)by two independent surgeons who were blinded to each ***,28 cadavers and 44 post-operative specimens were prospectively examined by hematoxylin and eosin(H&e)staining and masson’s staining to validate and confirm the findings of the retrospective *** and multivariate analyses were carried out to detect the independent factors that can affect the visualization of the distal mesorectal ***:The terminal line(TL)is the distal mesorectal end of the transabdominal and transanal Tme(taTme)and appears as a remarkable pearly white fascial structure extending posteriorly from 2 to 10 o’*** examination revealed that the fascia propria of the rectum merges with the presacral fascia at the TL,beyond which the mesorectum ends,with no further downward *** the retrospective observation,the TL was seen in 56.6%of transabdominal Tme and 56.0%of taTme *** approach and tumor distance from the anal verge were the independent variables that directly influenced the detection of the TL(P=0.03 and P=0.01).Conclusion: The TL is a visible sign where the transabdominal Tme should end and the taTme should begin. Recognitionof the mesorectal end may impact the certainty of complete mesorectum inclusion. Further clinical trials are needed toconfirm the preliminary findings.

The interaction of stem cells and vascularity in peripheral nerve regeneration

The degree of nerve regeneration after peripheral nerve injury can be altered by the microenvironment at the site of injury. Stem cells and vascularity are postulated to be a part of a complex pathway that enhances peripheral nerve regeneration;however, their interaction remains unexplored. This review aims to summarize current knowledge on this interaction, including various mechanisms through which trophic factors are promoted by stem cells and angiogenesis. Angiogenesis after nerve injury is stimulated by hypoxia, mediated by vascular endothelial growth factor, resulting in the growth of preexisting vessels into new areas. modulation of distinct signaling pathways in stem cells can promote angiogenesis by the secretion of various angiogenic factors. Simultaneously, the importance of stem cells in peripheral nerve regeneration relies on their ability to promote myelin formation and their capacity to be influenced by the microenvironment to differentiate into Schwann-like cells. Stem cells can be acquired through various sources that correlate to their differentiation potential, including embryonic stem cells, neural stem cells, and mesenchymal stem cells. each source of stem cells serves its particular differentiation potential and properties associated with the promotion of revascularization and nerve regeneration. exosomes are a subtype of extracellular vesicles released from cell types and play an important role in cell-to-cell communication. exosomes hold promise for future transplantation applications, as these vesicles contain fewer membrane-bound proteins, resulting in lower immunogenicity. This review presents pre-clinical and clinical studies that focus on selecting the ideal type of stem cell and optimizing stem cell delivery methods for potential translation to clinical practice. Future studies integrating stem cell-based therapies with the promotion of angiogenesis may elucidate the synergistic pathways and ultimately enhance nerve regeneration.

ong>objective:ong> The aim of this piece of work is to study the contribution of pelvic ultrasound while exploring pelvic pathologies at Hubert Koutoukou maga teaching University Hospital “CNHU-HKm” and at the teaching University Hospital of mother and Child “CHU-meL” of Cotonou. ong>Patients and methods:ong> A descriptive, cross-sectional and analytical study with prospective collec-tion was carried out at the University clinic of Gynecology and obstetrics “CUGo” at the CNHU-HKm and in the gynecology department of the CHU-meL, over a period of 4 months (may to August 2018). We included in this study women over 12 years old who performed a pelvic ultrasound and were followed in the aforementioned health centers. ong>Results:ong> The population of the study consisted of 181 patients. The pelvic ultrasound request frequency was 18.84% compared to all ultrasound examinations. The age group of 25 - 29 years old was the most represented (22.10%). Pelvic pain was the first mo-tive for pelvic ultrasound request (39.8%). The abnormalities found by ultra-sound were uterine and ovarian respectively in 55.4% and 38% of cases. Uter-ine pathology was dominated by uterine.m.oma (44%). ovarian cyst (13%) and ovarian dystrophy (14%) were the main ovarian pathologies. There was a statistically significant relationship between certain reasons for consultation, mainly metrorrhagia, dysmenorrhoea, amenorrhoea, leucorrhoea (p e ultrasound result. ong>Conclusion:ong> The ultrasound remains the first choice examination and is of a great value in exploring pelvic pathologies. In most cases, it helps to find the responsible lesion and to make the appropriate ther-apeutic decisions.

Implicit Hypotheses Are Hidden Power Droppers in Family-Based Association Studies of Secondary outcomes

Family-based tests of association between a genetic marker and a disease constitute a common design to dissect the genetic architecture of complex traits. The FBAT software is one of the most popular tools to perform such studies. However, researchers are also often interested in the genetic contribution to a more specific manifestation of the phenotype (e.g. severe vs. non-severe form) known as a secondary outcome. Here, what we demonstrate is the limited power of the classical formulation of the FBAT statistic to detect the effect of genetic variants that influence a secondary outcome, in particular when these variants also impact on the onset of the disease, the primary outcome. We prove that this loss of power is driven by an implicit hypothesis, and we propose a derivation of the original FBAT statistic, free from this implicit hypothesis. Finally, we demonstrate analytically that our new statistic is robust and more powerful than FBAT for the detection of association between a genetic variant and a secondary outcome.
