Evidence-based practice profiles of nurses: a transcultural study

Objective: To compare(i) the evidence-based practice(EBP) profiles and(ii) socio-demographic characteristics of nurses in 4 selected low-middle income countries(LMICs).Methods: A cross-sectional survey was conducted among 677 nurses from 4 hospitals in 4 countries(Afghanistan, Ghana, Iran, and Uganda).Results: The findings revealed that there are significant differences between the 3 EBP subscales(that is, practice/use, attitude, and knowledge/skill) across the 4 countries(P < 0.05). No significant EBP differences were identified across age groups and gender in the subscales of practice, knowledge, and attitude. Practice and knowledge subscales were significantly associated with working schedules(that is, par t-time and full-time)(P < 0.05). Analyses also revealed significant differences between attitude and knowledge subscales and working experience(P < 0.05).Conclusions: Nurses in the 4 countries have moderate practice, attitude, and knowledge/skil s of EBP. Nurses in LMICs require continuous professional development programs and suppor t to enhance their practice and knowledge regarding EBP in clinical settings.

Objective:The study of effective factors on the quality of life(QoL),as an important criterion,in the outcome of new therapeutic and preventive strategies in human immunodeficiency virus(HIV)has been considered by researchers.To study the relationship between spiritual well-being and Health-Related Quality of Life(HRQoL)and some related factors in patients with Acquired Immunodeficiency Syndrome([AIDS]/HIV).Methods:In this cross-sectional study,224 people were selected as the study population(112 were male and 112 were female)from patients with AIDS/HIV at the Behavioral Counseling Center of Shiraz with available sampling method.The research tool was Palutzian&Ellison well-being scale and QoL Questionnaire(SF-36).Data were analyzed using Pearson correlation test.Results:There was a significant direct relationship between physical function dimensions(P=0.003).There was a significant relationship between general well-being,mental well-being,and total spiritual well-being(P<0.001).There was a statistically significant relationship between the dimensions of QoL and spiritual well-being with some age section of the patients,and their occupation and education(P<0.001).Conclusions:Spirituality has an impact on the QoL dimensions.Therefore,HIV positive people’s quality of life could be improved by the possibility of the spiritual well-being promotion as a method of adaptation with the disease.

Background:It is obvious that hemodialysis is a stressful process,leading to numerous social and psychological problems,and may cause fatigue and helplessness.Objective:This study aimed to explain the experiences of hemodialysis clients regarding the effect of psychological problems on fatigue.Methods:This article was extracted from a qualitative study conducted on clients undergoing hemodialysis in Khorramabad.To this end,a total of 15 participants were purposefully selected using the snowball sampling method and the required data was collected using individual,face to face,detailed and semi-structured interviews.There were 12 client interviewees and one doctor,one nurse and one hemodialysis client's spouse.Content analysis was used to analyze the data.Results:Continuous analysis of data and documents obtained from the interviews on the hemodialysis clients'experiences and comments regarding the main causes of fatigue revealed two main categories:1)Psychological effects with subcategories of psychological stress and exhaustion;2)Needs and constraints with subcategories of individual and social needs and constraints.Conclusion:According to the clients'speech and expression analysis,psychological impacts and disease-induced needs and constraints were the most important cause of fatigue and its continuation among these clients.

Objective:In cardiovascular disease,a patient’s anxiety and depression can increase cardiac rehabilitation duration and recovery.Lavender aromatherapy as a non-pharmacological intervention effective in other contexts may be an efficient intervention to alleviating anxiety and depression in patients with Acute Coronary Syndrome(ACS).Methods:In this study,110 ACS patients were randomly assigned to two intervention and control groups.Inhalation of the lavender fragrance was prescribed for the intervention group and the drop of aromatic almond for the control group for 3 days.Results:The first-day anxiety and depression were significantly different in the two groups at 1 h and 9 h after the intervention.The’morning’s difference before the intervention was not significant,but it was substantial 1 h after the intervention.On the third morning of the intervention,this difference was confirmed.Conclusions:This study confirmed the effectiveness of lavender aromatherapy in reducing anxiety and depression in ACS patients.This’study’s results enable intensive care nurses to use aromatherapy with lavender oil as a non-pharmacological and cost-effective intervention to reduce their psychological tensions and increase patient satisfaction during hospitalization in the cardiac care units(CCU).

Drug calculation ability of qualified paramedics:A pilot study

BACKGROUND: The inability of paramedics to perform accurate calculations may result in a compromise of patient safety which may result from under or over dosing of drugs, incorrect joules for defibrillation, or a major adverse event such as death. The objective of this study was to identify the drug calculation and mathematical ability of qualified operational paramedics.METHODS: The study used a cross-sectional design with a paper-based calculation questionnaire. Twenty paramedics enrolled in an intensive care paramedic course were eligible to participate in the study. The questionnaire consisted of demographic, drug calculation(seven questions), and mathematical(five) questions. Students were given no notice of the impending study and use of a calculator was not permitted.RESULTS: All eligible students participated in the study. The average time employed as a paramedic was 7.25 years, SD 2.5 years, range four years to twelve years. Four(20%) students got all 12 questions correct, and five(41.6%) got 50% or less. The average score was 8.6(71.7%) correct, SD 2.8 correct, range 3 to 12 correct questions. There were eight(40%) conceptual errors, 12(60%) arithmetical errors, and five(25%) computational errors.CONCLUSION: The results from this study supports similar international studies where paramedic's ability to undertake mathematical and drug calculations without a calculator varies, with some results highlighting the paramedics mathematical skills as a potential risk to patient safety. These results highlight the need for regular continuing mathematical and drug calculation practice and education to ensure a lower error rate.

Use of ginger versus stretching exercises for the treatment of primary dysmenorrhea: a randomized controlled trial

BACKGROUND： Dysmenorrhea is a common gynecologic problem. In some cases, non-medical treatments are considered to be more effective, with fewer side effects. Ginger and exercise are alternative treatments for dysmenorrhea, but in the present study they were not combined. OBJECTIVE： In this study, the effects of ginger and exercise on primary dysmenorrhea were compared. DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS： This randomized controlled tdal was performed in Mazandaran University of Medical Sciences, Iran. Two groups of female students were recruited by simple random allocation. In each group, 61 students with moderate to severe primary dysmenorrhea with regular menstrual cycles and without a history of regular exercise were assessed. The ginger group received 250 mg ginger capsules from the onset of menstruation. In the exercise group, belly and pelvic stretching exercises were performed for 10 min, 3 times per week. MAIN OUTCOME MEASURES： Intensity of pain was assessed according to a visual analogue scale after the first and the second month. RESULTS： Exercise was significantly more effective than ginger for pain relief （31.57± 16.03 vs 38.19 ± 20.47, P = 0.02）, severity of dysmenorrhea （63.9% vs 44.3% mild dysmenorrhea, P = 0.02） and decrease in menstrual duration （6.08 ± 1.22 vs 6.67± 1.24, P = 0.006）, in the second cycle. CONCLUSION： Stretching exercises, as a safe and low-cost treatment, are more effective than ginger for pain relief in primary dysmenorrhea.

目的系统评价座椅辅助阻力带运动(chair-based resistance band exercise,CRBE)对长期护理机构老年人身体功能、睡眠质量和抑郁的影响。方法对AgeLine,CINAHL,PubMed,Embase,Cochrane Library,Scopus和Web of Science等数据库进行检索。检索时间为建库至2022年3月,纳入对慢性疾病老年人进行CRBE的随机对照试验研究。采用物理治疗证据数据库量表(the Physiotherapy Evidence Database,PEDro)对纳入文献进行质量评价,随机效应模型和固定效应模型用于生成集合效应量。结果共9篇文章符合纳入标准。结果显示,CRBE可促进日常生活活动(6项研究;SMD=0.30,P=0.001),肺活量(3项研究;MD=40.35,P<0.001),握力(5项研究;MD=2.17,P<0.001),上肢肌肉耐力(5项研究;MD=2.23,P=0.012),下肢肌肉耐力(4项研究;MD=1.32,P<0.001),上肢柔韧性(4项研究;MD=3.06,P=0.022),下体柔韧性(4项研究;MD=5.34,P<0.001),动态平衡(3项研究;MD=-0.35,P=0.011)、睡眠质量(2项研究;MD=-1.71,P<0.001),并减轻抑郁状况(2项研究;SMD=-0.33,P=0.035)。结论证据表明CRBE改善了长期慢性疾病老年人的身体功能参数、睡眠质量,并减轻抑郁状况。这项研究可用以敦促长期护理机构让行动不便的老年人参与体育活动。

Pediatric palliative care for children with cancer:a concept analysis using Rodgers’evolutionary approach

Background Providing access to pediatric palliative care(PPC)for children living with a serious illness,such as cancer,is of critical importance,although this specialized intervention,as a novel concept,still seems vague and complicated.The present study analyzed the concept of PPC for children with cancer.Methods Rodgers'evolutionary method was employed for the concept analysis.Articles on PPC,particularly those for children suffering from cancer,published between 2010 and 2021 were searched in valid academic research databases.The inclusion criteria for the full-text articles were based on the characteristics,antecedents,and consequences of PPC for children with cancer.Results In total,19 relevant articles were selected and then reviewed and analyzed after applying the inclusion criteria and the final sampling.The analysis of the concept of PPC for children affected with cancer revealed four characteristics,including"PPC as holistic and integrated care","PPC as patient-and family-centered care","PPC as early-start continuous care",and"PPC as interdisciplinary and team-based care".Some effective factors could also act as antecedents for this concept,i.e.,health care providers'training and expertise as well as human resources and financing.Moreover,improved quality of life,symptom reduction,and coordination between patient care and family support were among the PPC consequences for such children.Conclusions The study results demonstrated that delivering PPC to children with cancer demanded a comprehensive view of its various dimensions.Furthermore,numerous factors need to be delineated for its accurate and complete implementation.
