Clinical Supervision of midwifery Students at the University Teaching Hospital school of nursing and midwifery in Lusaka, Zambia

Introduction: Clinical supervision plays a significant role in the acquisition of psychomotor skills by student midwives during training leading to the provision of high quality midwifery care. However, the acquisition of the psychomotor skills required for successful practice and learning can only be acquired if student midwives are supported and guided by clinical supervisors in their learning environments. Main Objective: The main objective of the study was to determine factors influencing clinical supervision of Student Midwives in Lusaka Urban District. Methodology: A cross-sectional study design that employed both quantitative and qualitative methods was used. The study population comprised 124 Nurses and Midwives working in Lusaka urban District. Self-administered questionnaires were used to collect data from the Nurses and midwives and a focus group discussion guide was used to guide discussions with the student midwives at Lusaka College of nursing and midwifery and a total of three focus group discussions were conducted. Quantitative data were entered into by Statistical Package Social Sciences (SPSS) version 22 for windows. Chi-square test was used to test associations among variables. The confidence interval was set at 95% with a significance level of 0.05. Qualitative data were analysed using content analysis to classify words into themes and categories by looking for repeated ideas or patterns of thoughts. Results: The findings showed that a high percentage of the clinical supervisors (89.5%) had not been trained in clinical supervision and most respondents had moderate knowledge on clinical supervision of students. Majority of the respondents (63%) exhibited inadequate supervision skills. The attitudes of all the supervisors towards supervision were positive. Use of ginger versus stretching exercises for the treatment of primary dysmenorrhea: a randomized controlled trial

BACKGROUND： Dysmenorrhea is a common gynecologic problem. In some cases, non-medical treatments are considered to be more effective, with fewer side effects. Ginger and exercise are alternative treatments for dysmenorrhea, but in the present study they were not combined. OBJECTIVE： In this study, the effects of ginger and exercise on primary dysmenorrhea were compared. DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS： This randomized controlled tdal was performed in Mazandaran University of Medical Sciences, Iran. Two groups of female students were recruited by simple random allocation. In each group, 61 students with moderate to severe primary dysmenorrhea with regular menstrual cycles and without a history of regular exercise were assessed. The ginger group received 250 mg ginger capsules from the onset of menstruation. In the exercise group, belly and pelvic stretching exercises were performed for 10 min, 3 times per week. MAIN OUTCOME MEASURES： Intensity of pain was assessed according to a visual analogue scale after the first and the second month. RESULTS： Exercise was significantly more effective than ginger for pain relief （31.57± 16.03 vs 38.19 ± 20.47, P = 0.02）, severity of dysmenorrhea （63.9% vs 44.3% mild dysmenorrhea, P = 0.02） and decrease in menstrual duration （6.08 ± 1.22 vs 6.67± 1.24, P = 0.006）, in the second cycle. CONCLUSION： Stretching exercises, as a safe and low-cost treatment, are more effective than ginger for pain relief in primary dysmenorrhea.

Drug calculation ability of qualified paramedics:A pilot study

BACKGROUND: The inability of paramedics to perform accurate calculations may result in a compromise of patient safety which may result from under or over dosing of drugs, incorrect joules for defibrillation, or a major adverse event such as death. The objective of this study was to identify the drug calculation and mathematical ability of qualified operational ***: The study used a cross-sectional design with a paper-based calculation questionnaire. Twenty paramedics enrolled in an intensive care paramedic course were eligible to participate in the study. The questionnaire consisted of demographic, drug calculation(seven questions), and mathematical(five) questions. Students were given no notice of the impending study and use of a calculator was not ***: All eligible students participated in the study. The average time employed as a paramedic was 7.25 years, SD 2.5 years, range four years to twelve years. Four(20%) students got all 12 questions correct, and five(41.6%) got 50% or less. The average score was 8.6(71.7%) correct, SD 2.8 correct, range 3 to 12 correct questions. There were eight(40%) conceptual errors, 12(60%) arithmetical errors, and five(25%) computational ***: The results from this study supports similar international studies where paramedic's ability to undertake mathematical and drug calculations without a calculator varies, with some results highlighting the paramedics mathematical skills as a potential risk to patient safety. These results highlight the need for regular continuing mathematical and drug calculation practice and education to ensure a lower error rate.

Effects of aromatherapy with Rosa damascena on nulliparous women's pain and anxiety of labor during first stage of labor

Background： Reducing labor pain and anxiety is one of the most important goals of maternity care. Objective： This study aimed to assess the effects of aromatherapy with Rosa damascena on pain and anxiety in the first stage of labor among nulliparous women. Design, setting, participants and interventions： This was a randomized clinical trial of 110 nulliparous women. The eligible participants were randomly assigned to two groups of aromatherapy and control in an Iranian maternity hospital. The participants received 0.08 mL of Rosa damascena essence in the aro- matherapy group and 0.08 mL of normal saline in the control group, every 30 min. Pain was measured 3 times, once each at three stages of cervical dilation （4-5, 6-7, and 8-10 cm）. Anxiety was measured twice, once each at two stages of cervical dilation （4-7 and 8-10 cm）. The tools for data collection were the Spielberger anxiety questionnaire, numerical pain rating scale, demographic and obstetric questionnaire, and an observational checklist. Data analyses included the t-test, Mann-Whitney U test and Chi-square test. Main outcome measures： Severity of labor pain and severity of anxiety were used as primary outcome measures. Labor and delivery characteristics （including number of contractions, duration of contractions in second stage, Bishop score, augmentation by oxytocin, Apgar score, and mode of delivery）, demographic characteristics, and fertility information were used as secondary outcome measures. Results： Pain severity in the group receiving aromatherapy with R. damoscena was significantly lower than in the control group after treatment at each pain assessment （cervical dilation of 4-5, 6-7, and 8-10 cm; P 〈 0.05）. Anxiety levels were also significantly lower in the treatment group than in the control group after treatment at each time of measurement （cervical dilation of 4-7 and 8-10 cm; P 〈 0.05）. Evidence-based practice profiles of nurses: a transcultural study

Objective: To compare(i) the evidence-based practice(EBP) profiles and(ii) socio-demographic characteristics of nurses in 4 selected low-middle income countries(LMICs).Methods: A cross-sectional survey was conducted among 677 nurses from 4 hospitals in 4 countries(Afghanistan, Ghana, Iran, and Uganda).Results: The findings revealed that there are significant differences between the 3 EBP subscales(that is, practice/use, attitude, and knowledge/skill) across the 4 countries(P < 0.05). No significant EBP differences were identified across age groups and gender in the subscales of practice, knowledge, and attitude. Practice and knowledge subscales were significantly associated with working schedules(that is, par t-time and full-time)(P < 0.05). Analyses also revealed significant differences between attitude and knowledge subscales and working experience(P < 0.05).Conclusions: Nurses in the 4 countries have moderate practice, attitude, and knowledge/skil s of EBP. Nurses in LMICs require continuous professional development programs and suppor t to enhance their practice and knowledge regarding EBP in clinical settings.

Relationship between spiritual well-being and Health-Related Quality of Life and some related factors in patients with AIDS/HIV

Objective:The study of effective factors on the quality of life(QoL),as an important criterion,in the outcome of new therapeutic and preventive strategies in human immunodeficiency virus(HIV)has been considered by *** study the relationship between spiritual well-being and Health-Related Quality of Life(HRQoL)and some related factors in patients with Acquired Immunodeficiency Syndrome([AIDS]/HIV).Methods:In this cross-sectional study,224 people were selected as the study population(112 were male and 112 were female)from patients with AIDS/HIV at the Behavioral Counseling Center of Shiraz with available sampling *** research tool was Palutzian&Ellison well-being scale and QoL Questionnaire(SF-36).Data were analyzed using Pearson correlation ***:There was a significant direct relationship between physical function dimensions(P=0.003).There was a significant relationship between general well-being,mental well-being,and total spiritual well-being(P<0.001).There was a statistically significant relationship between the dimensions of QoL and spiritual well-being with some age section of the patients,and their occupation and education(P<0.001).Conclusions:Spirituality has an impact on the QoL ***,HIV positive people’s quality of life could be improved by the possibility of the spiritual well-being promotion as a method of adaptation with the disease.

Predictors of caring behaviors of mothers of premature infants based on the health belief model

Objective:To investigate predictors of caring behaviors of mothers of premature infants based on the health belief ***:This cross-sectional study was conducted by using the structural equation modeling on 168 mothers of premature infants,who were selected by convenience sampling method from October 2017 to February 2018 in *** were collected by using a standard *** and reliability of all data collection tools were *** were analyzed by using SPSS V.16 and Mplus6 ***:The structural equation modeling of the initial health belief model did not have a good fit,but the fitness of model 2 obtaining from the modified initial model was confirmed by changes in locations of *** of constructs of model 2 had a significant positive association with the caring behavior of mothers of premature infants and only 2.8%of variance of caring behaviors in mothers could be predicted by the sum of variables of demographic characteristics and the modified health belief model ***:Given that the findings do not approve the use of the health belief model in predicting determinants of caring behavior of mothers of premature infants,it is suggested to apply this model to investigate the effect of educational intervention based on the health belief model on the caring behavior of mothers.
