肝细胞癌患者的症状和症状群及常用工具的整合性综述

目的综合现有知识,识别肝细胞癌(hepatocellular carcinoma,HCC)患者的症状和症状群,以及相关评估工具,以最大限度地改善症状管理。方法采用Whittemore和Knafl提出的整合性综述方法框架,在PubMed..ScienceDirect、Scopus、CINAHL和ThaiJO等数据库中对文献进行系统检索。检索文献发表时间限定为2005年一2022年,仅纳人经过同行评审及有英文摘要的文献。由2名研究员独立进行文献的筛选、提取和分析。结果共纳入14篇文献。分为3个主题:HCC患者的症状.症状评估和症状群。疲劳、乏力、胃痛或腹痛/腹胀、食欲不振、味觉改变、睡眠障碍、苦恼和悲伤是HCC患者最常见的症状。患者症状与肿瘤分期和治疗方法有关。常用的症状评估工具包括5种类型:HCC特异性症状评估癌症一般症状评估、非症状构念测量、特定症状(如疲劳、睡眠障碍、焦虑和抑郁)测量及结合临床检查的症状评估等5种。HCC患者的症状群分为5类:疼痛相关症状、胃肠道症状、神经心理症状和感觉症状.肝功能异常相关症状及其他。结论该综述对HCC患者症状、症状评估和症状群相关知识进行了综合。尽管目前有多种评估工具,但没有一种能涵盖HCC患者的所有症状。建议今后的研究纳入更大的同质化的样本,以便更准确地评估测量工具的效能,避免在将症状分类为症状群时出现歧义,提高症状管理的有效性。

目的通过对自我和身份的反思,探索年轻乳腺癌患者为回归家庭需要履行的妻子角色期望,为医务工作者制订癌症家庭康复的相关干预措施提供参考依据。方法采用描述性现象学方法和目的取样法。于2023年3月—4月在江苏省徐州市某三级甲等医院的乳腺外科和肿瘤内科选取年轻乳腺癌患者及其配偶进行半结构化的面对面访谈。访谈内容被逐字转录,采用科莱兹现象学方法进行分析。结果 16例乳腺癌患者和6名配偶参与了访谈,平均年龄分别为(35.95±3.36)岁和(37.67±5.28)岁。在治疗和康复期间,关于年轻乳腺癌患者妻子角色期望内容共提炼出3个主题,8个子主题:自尊自爱(保障自己的生命、坦然面对疾病、重拾女性角色的自信);夫妻关系的调适(和谐有效的夫妻沟通、为婚姻和爱情给予支持、共创美好的夫妻生活);协助家庭康复(减轻配偶的照护与经济负担、家庭日常的管理)。结论年轻乳腺癌患者及其配偶对妻子角色的期望主要包括自我、夫妻和家庭三个方面。自尊和自爱是最基本的期望,而调整夫妻关系和协助家庭康复则是更高的期望。

目的哮喘-慢性阻塞性肺疾病肺重叠症(Asthma-chronic obstructive pulmonary disease overlap,ACO)患者的生活质量较低,肺功能较差,病情经常加重。由于暴露于污染环境会加重其症状,增加发病率和病死率,环境管理对其至关重要。旨在通过系统性综述,寻找证据,以指导制订护理干预措施,促进ACO患者对空气质量的自我管理。方法 2023年3月,系统检索CINAHL、Medline、Academic Search Complete、Scopus、Web of Science、Cochrane图书馆、乔安娜布里格斯研究所(Joanna Briggs Institute,JBI)循证实践数据库,以及Google Scholar。对纳入研究进行数据提取和综合,并对护理干预措施进行分类。采用JBI研究质量评估工具进行方法学质量评估。结果共纳入5篇来自不同国家的研究。促进ACO患者自我监控空气质量的干预措施包括:警觉性干预措施(医务人员定期巡诊、进行症状评估),监测干预措施(监测室内外诱发因素),教育干预措施(空气质量警报、避免接触过敏原),以及相关政策的影响,如无烟政策和改善空气质量的综合战略。尽管这些研究报告了不同的干预措施,方法质量也存在差异,但这些干预措施显示了预防患者病情恶化、尽量减少急诊就诊和住院治疗的潜力。这些关键点可作为护理干预措施的关键组成部分。结论有必要采取全面措施,让护士参与促进患者自我监控空气质量的多学科团队、提高认识、制订和实施护理干预,从而使ACO患者受益。同时,需要进一步开展研究,以扩大证据基础并完善这些干预措施。

目的从医护人员的视角评价上海市已故肿瘤患者的善终质量现状,为提高安宁疗护服务水平提供科学依据。方法采用横断面调查法,于2023年4月-7月,通过方便抽样邀请上海市2所三级医院、2所二级医院以及4所社区卫生服务中心的261名有照顾临终肿瘤患者经历的医护人员参与调查。采用一般资料问卷及基于健康照护视角的优逝量表(The Good Death Scale for Patients in China, GDS-PCN)对其进行调查。GDS-PCN包括专业照护、家人陪伴、有备无憾、平静离世、保持尊严、维持控制、躯体安适7个维度,共36个条目。每名参与调查的医护人员使用问卷评价其最近照顾的1例已故肿瘤患者的死亡情况,共评价261例已故肿瘤患者的离世情况。结果 GDS-PCN量表总分为(144.11±17.86)分,其中专业照护维度平均分最高为(4.21±0.58)分,有备无憾维度和维持控制维度得分较低,分别为(3.75±0.70)分和(3.86±0.70)分。不同医疗机构等级、病房类型、住院时间、与医护人员之间沟通(病情、治疗方案和死亡相关话题)情况、治疗方案决策类型的患者量表得分的比较,差异有统计学意义(P<0.05)。曾在社区卫生服务中心住院的患者的得分高于三级医院的患者(P<0.05);安宁疗护病房的患者得分高于非安宁疗护病房的患者(P<0.05);住院15天以上患者的得分高于住院少于3天的患者(P<0.05);与医护人员沟通部分或全部病情的患者得分高于从未沟通过的患者(P<0.05);与医护人员讨论部分、大部分或更多的治疗计划的患者得分分别高于从未讨论过治疗计划的患者(P<0.05)和仅有限沟通过治疗计划的患者(P<0.05)。与医护人员部分谈论过死亡相关话题的患者的得分高于从未谈论过的患者(P<0.05);而与家庭成员共同参与治疗决策的患者的得分高于仅家庭成员参与决策的患者(P<0.05)。结论上海市肿瘤患者的善终情况总体处于中高等水平。提高安宁疗护资源利用、加强医护人员与患者沟通临终事宜是提高患者善终水平的有效途径。未来需开展更多的研究,从多角度探究中国患者的善终质量。

This paper introduced the content, compilation process, reliability and validity, scoring method of the evaluation tool for patients’ medication compliance at home and abroad, and reviewed the research progress of the tool. The evaluation method, dimension, scoring method, evaluation content and application scope of the tool were compared, so as to provide reference for nurses to comprehensively and accurately evaluate patients’ medication status.

Objective:To determine the temporal trend and epidemiology of animal bite cases in Gerash City,***:This retrospective cross-sectional study analyzed 630 cases of people with animal bites between 2011 and 2021 in Gerash *** collected data were analyzed using Chi-square ***:The mean age of victims was(30.9±17.5)years.80.54%Of victims were male,39.70%were self-employed,and 64.60%were adults(≥18 years).Most cases of bites occurred in 2019(91 cases),2020(74 cases)and 2021(87 cases),and most of the bites were related to the upper limbs(62.70%).Most of the wounds were superficial(78%),most of the biting animals were domestic animals(91.74%),and most of the victims had Iranian nationality(97.62%).Conclusions:Given the increasing trend of animal bites in Gerash City,health authorities should carry out basic measures such as education and awareness among the public,especially at-risk groups such as adult ***,since most cases of bites are due to dogs,it seems necessary to plan for vaccination of dogs,especially those with owners.

Association of Hypertension and Plant-based Dietary Patterns on Cognitive Impairment: A Nationwide Cohort Study in China

This study investigated the association of hypertension with cognitive impairment and determined whether better adherence to plant-based diet (PBD) patterns may modify the association. We analyzed 4485 elderly individuals with normal cognition at baseline from the Chinese Longitudinal Healthy Longevity Survey (2011-2018). Cognitive function was assessed by the Mini-mental State Examination (MMSE)， and cognitive impairment was defined as an MMSE score below 18 points. Diet patterns were assessed using the overall plant-based diet index (PDI)， healthful plant-based diet index (hPDI)， and unhealthful plant-based diet index (uPDI) derived from a simplified food frequency questionnaire (FFQ). Blood pressure was measured through physical examination. Cox proportional hazard regression and restricted cubic spline were performed. Participants with hypertension had an increased risk of cognitive impairment compared with normal group. The associations between hypertension and cognitive impairment were more pronounced among participants with lower PDI than those with higher PDI， among participants with lower hPDI than those with higher hPDI， and among participants with higher uPDI than those with lower uPDI. A healthful PBD pattern may attenuate hypertension-induced cognitive impairment， while an unhealthful PBD pattern may exacerbate hypertension-induced cognitive impairment in elderly. Adherence to healthful PBD pattern could be used to prevent or mitigate adverse neurological effects in the hypertensive population.

目的探索梅尼埃病患者多种症状之间的关系并通过症状网络分析将其可视化,从而帮助临床护理人员制订更加精准和个性化的干预措施。方法于2014年10月至2021年12月,选取复旦大学附属眼耳鼻喉科医院790例梅尼埃病患者进行调查。采用自行设计的症状自评量表对15个梅尼埃病相关症状进行评估,根据评估结果在R软件中构建症状网络。采用Qgraph包和Fruchterman-Reingold布局用于可视化网络。采用Bootstrapping方法评估网络的准确性和稳定性,然后采用3种中心性指标描述症状之间的关系。结果“焦虑、紧张”(98.3%)、“耳闷”(84.4%)及“耳鸣”(82.7%)是梅尼埃病患者常见症状,“耳鸣”“耳闷”及“言语识别率下降”是较严重的症状。病程越长,梅尼埃病患者各项症状的发生率及严重程度越高(P<0.05)。“单词识别能力下降”“疲劳”和“焦虑和紧张”是症状网络的核心,在整个样本中强度中心性和紧密中心性最大,对其他症状的影响最大。“言语识别率下降”“头痛”和“空间辨别和定向能力差”是整个样本中介中心性最高的症状,桥接效应最强。随着病程的发展,“跌倒发作”和“焦虑、紧张”的中心性增加,“头痛”的中心性下降。结论本研究揭示了不同病程梅尼埃病患者的症状网络。护理人员需要根据其症状严重程度和中心性的变化提供更精准的干预。当梅尼埃病患者出现多种症状时,应重点干预患者的言语识别率及其引起的不适感受。
