Objective:This study examined patients with systemic lupus erythematosus(SLE)as a means to explore the impacts of chronic disease on life courses.Methods:A semi-structured in-depth interviews were conducted with nine patients in China in 2017;participants were included based on their having or having had facial butterfly erythema.Results:The study focused on both the disease's impact on the patients'health and on their daily lives.Four core themes emerged:visible changes in the patient's bodies,social dilemmas,"the encouragement of disease",and a new perspective on the relationship between disease and health.One important finding was that the occurrence of a chronic disease did not have only negative repercussions;some patients felt that there were advantages to being sick.Chronic disease resulted in a reworking of daily life.The patients developed a self-referential model of healing.Conclusion:The distinct interpretations of the same disease offered by different patients served to yield a more complete understanding of the disease.People with SLE adjust their thinking about the disease based on personal feelings as well as experiences and pursue a dialogue on their illness based on the disease pattern unique to them.The meaning that disease had for the patients was not limited to negative connotations.

Due to increasingly complex issues,challenging and acutely ill patients,advancing technology and ethical dilemmas it is important that nurses are able to think critically to improve patient outcomes.Therefore,nurse educators continually seek to identify effective learning strategies that foster critical thinking in their undergraduate students to make theoretical and clinical teaching seamless.Therefore,the purpose of this article is to describe the role of the nurse educator in helping nursing students to think critically.We will examine a case analysis and describe various teaching strategies that support critical thinking,can increase student learning,and aid in the provision of nursing care.

Constructing a caring training content for nurse managers:a Delphi study

Objective: To design a scientific, feasible caring training content for nurse managers. Methods: Training content came from review of literature;all the content were sorted out, analyzed, and compiled by the research team. Then, a Delphi method was used to collect opinions from experts on the caring training contents. Results: A 22-member multidisciplinary panel was engaged in a two-round Delphi process to assess the importance of the items and to identify any additional elements. Consensus was obtained on 23 items that belonged to four modules. There were 10 items related to caring knowledge and caring ability, six items related to caring leadership, three items related to caring education, and four items related to caring research. Conclusions: The four-module integrated training contents achieved in this study represented the core caring knowledge and skills that a nurse manager who worked in hospital needed to know. Nurse managers can choose any module to have a system learning according to their practical needs, totally or separately.

Does Obesity without Hyperglycemia Delay Wound Healing in an Obese Mouse Model Induced by a High-Fat Diet?

It was unclear that wound healing was delayed in obesity without hyperglycemia. The purpose of this study was to compare the wound healing process between obese and non-obese mice without hyperglycemia by attaching a splint. Three-week-old male mice (C57BL/6N) were fed high-fat diets (60% of calories from fat) in the obesity group, and commercial diets in the control group for 15 weeks. Two circular (4 mm in diameter) full-thickness wounds were made on the dorsal skin. Body weights and serum leptin levels were significantly higher in the obesity group than in the control group until day 15 after wounding. Fasting blood glucose levels before wounding were lower in the obesity group than in a hyperglycemic rodent model. The macrophage infiltration into subcutaneous fat before wounding in the obesity group was negligible. The ratios of the wound area were not significantly different between the two groups. No significant differences were observed in the number of neutrophils or macrophages or new blood vessels and ratio of myofibroblasts or collagen fibers between the two groups. Our results demonstrated that cutaneous wound healing was not delayed in the obesity group without hyperglycemia and macrophage infiltration into the subcutaneous fat and with high serum leptin levels.

1.Background Engel's biopsychosocial model,which was constructed in 1977,was considered a momentous breakthrough from the traditional biomedical model of medicine that regards the person as a disease-carrier requiring diagnosis and treatment[1]towards recognition of the whole person and furnish customized care.This model argues that health should not be considered solely on biological factors but on a combination of biological,psychological,and social factors[2].This idea of considering individuals as a whole and respecting their rights to self-determination has led to a shift in the care model from disease-centered care to patient-centered or person-centered care.The latter two care models are both abbreviated as PCC.Ensuring that people are involved in and central to the healthcare process is now recognized as a key component in developing high-quality care.Studies have revealed that both concepts of care can help improve people's health condition and lower health service burdens.Thus,countries and healthcare organizations have been paying close attention to the patients'concerns.The National Health Service in England has made person-centered care as one of its seven core principles[3].

目的本研究旨在探讨护生对交通事故创伤患者的急救知识、意愿和态度情况及相关因素。方法选择天津中医药大学护理专业475名学生作为研究对象,调查其自我效能、核心自我评价及对交通事故创伤患者急救行为的知识意愿和态度情况。结果护生急救知识、意愿和态度得分分别为(7.51±1.927)分、(15.54±5.032)分、(7.73±1.563)分。与未接受急救培训的学生相比,接受急救培训的学生对交通事故创伤患者急救态度得分更低(t=-2.345,P=0.019)。Pearson相关分析发现,自我效能与急救知识(r=0.150,P<0.001)、急救意愿(r=0.182,P<0.004)和急救态度相关(r=0.371,P<0.001)。核心自我评价与急救知识(r=0.193,P<0.001)及急救态度相关(r=0.199,P<0.001)。结论研究显示,护生急救知识水平不理想。短期和不可持续的急救培训可能带来负面影响,应采取有效的措施来保证培训质量和延续性。同时,应更多关注学生的自我效能感和核心自我评价。

Undergraduate Students’ Perceptions of the Role of the Nurse educator during Clinical Placements in Malawian

In this article we reported selected findings of a hermeneutic phenomenological study which explored the clinical learning experience of undergraduate nursing students in Malawi. The setting for the study was a university nursing college and the sample was selected purposively, consisting of thirty participants. Conversational interviews were conducted and a framework developed by modifying Colaizzi’s procedural steps guided the phenomenological analysis. The study findings reflect students’ perspectives on what the ideal role of a nurse educator in Malawi should entail. Contrary to most of the literature from western countries, the findings strongly indicate the need for nurse educators to teach students during clinical placements by engaging in hands-on-care. Additionally, the study also indicates the need for educators to view their role holistically by focusing on both academic and personal aspects of the students’ life. The study illustrates the need for connected educator-student relationships.

Objective:This review paper explores the concept,spirit,principles,and core skills of moti-vational interviewing(MI)as a psychological approach applied to CHD patients.Methods:Literature reviews based on English and Chinese articles.Results:The English and Chinese literature provided strong evidence for the effectiveness of MI in terms of behavior changes,blood pressure control,hyperglycemia and hyperlipidemia control,compliance improvement,and improving quality of life amongst chronic heart failure patients.The Happy Life Club(HLC)is the only randomized controlled trial(RCT)that examined the effects of MI on behavior changes and self-management improvements in Chinese patients with type 2 diabetes mellitus.The HLC pilot revealed positive outcomes,and RCTs will provide further evidence.Conclusion:MI has been expanded in its application to a wider range of lifestyle-related chronic diseases,including CHD.At the time of this review,MI represents a new concept and ap-proach in China for improving physical and mental health(body and mind)in patients with CHD.In order to integrate MI into medical practice,the authors suggest improving the medical education curriculum and strengthening vocational training,while conducting further research initiatives to assess the innovation and inform practice management.

Objective:Nurse practitioners(NPs)have drawn significant attention recently and played a major role in healthcare.We aim to find the evaluation of NPs through published studies and then visualize the research status and hotspots in this field.Methods:All data came from the Web of Science core Collection,and the data were counted and entered into Excel 2016.The key documents nodes were excavated by analyzing the knowledge network map using Cite Space V software.In this study,these nodes of“author,country,institution,keyword,co-citation(referencecited-authorcited-journal),and grant”were harvested for analysis and comparison.Results:A total of 4912 records,which were published between 2007 and 2018 and pertained to NPs,were retrieved from the Web of Science core Collection database(Wo SCC)from a diversity of languages.The total and the annual number of publications and citations of these continually increased over time.Most publications were in 2018(618 records).This study involved 8241 authors located in 98 countries and 4557 institutions totally.The United States(2737 records)and the University of Michigan(90 records)dominated in publication frequency.There are 902 journals and 2449 articles with funding support that have been analyzed.Most articles were from JAMA:The Journal of the American Medical Association(1386,IF=47.661),followed by the Journal of Advanced nursing(1359,IF=2.267),and The New England Journal of Medicine(1109,IF=79.258).The reference“The Role of Nurse Practitioners in Reinventing Primary Care”was co-cited most frequently,which revealed it as the highest landmark article in NP.The top-ranked keyword was“Care,”other than“Nurse practitioner,”which has an ultra-high frequency.Some of the high-frequency keywords represent the significant direction of NPs.Conclusions:NPs are at the crux of health-care delivery and play an important role in providing high-quality nursing.Publications on NPs in Wo SCC have increased notably during the recent years,and have appeared in some journals that have a high imp

Objective:Effective communication skills are one of the core competencies of nursing curricula internationally.Nurses are generally regarded as proficient.Despite our complete trust in the profession,deficiencies and gaps exist.However,it is not clear to which extent nurses use key communication skills in practice,and whether or not confident in using these skills compounds environmental issues that occur.This study explored nurse’s confidence and application of relational skills competencies in nursing practice.Methods:A 13-item online survey was used to collect data.Results:Being self-aware on key areas where there was more uncertainty.Nurses also lacked confidence in exploring the impact of their personal feelings and values on their interactions.Nurses were also less confident on responding appropriately to instances of unsafe or unprofessional practice and using information and communication systems and technology.Conclusions:Given the potential impact of poor relational skills on quality client care,an increased emphasis on caring and compassion,and the ever-expanding use of communication technologies,there is a need to explore the need for reflective practice to enhance continuous professional development for nurses to enhance their relational skills.
