Comparison between Emergency Severity Index and Heart Failure Triage Scale in heart failure patients: A randomized clinical trial

BACKGROUND:It is not clear whether Emergency Severity Index(ESI)is valid to triage heart failure(HF)patients and if HF patients benefi t more from a customized triage scale or *** aim of study is to compare the effect of Heart Failure Triage Scale(HFTS)and ESI on mistriage among patients with HF who present to the emergency department(ED).METHODS:A randomized clinical trial was conducted from April to June *** patients with dyspnea were randomly assigned to HFTS or ESI *** level,used resources and time to electrocardiogram(ECG)were compared between both groups among HF patients who were admitted to coronary care unit(CCU),cardiac unit(CU)and discharged patients from the *** validity was examined using Kappa designating agreement on relevance(K*).Reliability of both scale was evaluated using inter-observer agreement(Kappa).RESULTS:Seventy-three and 74 HF patients were assigned to HFTS and ESI groups *** to ECG in HFTS group was signifi cantly shorter than that of ESI group(2.05 vs.16.82 minutes).Triage level between HFTS and ESI groups was signifi cantly different among patients admitted to CCU(1.0 vs.2.8),cardiac unit(2.26 vs.3.06)and discharged patients from the ED(3.53 vs.2.86).Used resources in HFTS group was significantly different among triage levels(H=25.89;df=3;P<0.001).CONCLUSION:HFTS is associated with less mistriage than ESI for triaging HF *** is recommended to make use of HFTS to triage HF patients in the ED.

Background:Quality of life is an important indicator in patients with breast *** here reported that the quality of life in patients with breast cancer is low and many factors contribute to this poor quality of ***:To examine the relationships among demographic characteristics,optimism,social support,illness related factors,appraisal of illness,coping strategies and the quality of life of Chinese women with breast cancer residing in Wuhan,***:A convenience sample of 156 Chinese women with breast cancer was recruited from five teaching hospitals in Wuhan,*** completed the Revised Life Orientation Test,the Perceived Social Support Scale,the Symptom Distress Scale,the Appraisal of Illness Scale,the Medical Coping Modes Questionnaire,and the Functional Assessment of Cancer *** analysis was used to examine factors influencing quality of ***:Significant relationships were found between optimism,symptom distress,social support,appraisal of illness,a give-in coping mode and quality of ***,social support,symptom distress,lymph node status,appraisal of illness,and a give-in coping mode accounted for 66.6%of the variance in quality of ***:The findings of this study underscore the importance of helping women reduce symptoms distress,appraise their illness positively,use less negative coping modes,and maintain optimism,maintain good social support,because all of these factors indirectly or directly affect their quality of life.

GM-CSF GENE OR B7-1 GENE MODIFIED MURINE EL-4 CELLS VACCINE

Objective: To study the vaccine potency of gene-modified tumor cells. Methods: The EL-4 lymphoma was transduced with recombinant retrovirus containing the murine GM-CSF gene or B7-1 gene. The effect of gene transduction on antitumor immunity was investigated. Results: Flow cytometry analysis showed that expression of their surface marker between wild-type EL-4 cells and gene transduced tumor cells was the same except for CD80 positive in B7-1 gene transduced cells. GM-CSF gene or B7-1 gene transduced EL-4 cells resulted in remarkable loss of tumorigenicity in syngenetic mice. The systemic protective immunity was induced against the challenge with EL-4/wt cells. Therapeutic vaccine with EL-4/GM-CSF or EL/7-1 cells could retard the growth of established early-stage EL-4/wt tumor significantly, but not retard the growth of late-stage EL-4/wt tumor. Irradiated GM-CSF gene transduced EL-4 cells showed strong vaccine effect against EL-4 cell challenge, but irradiated B7-1 gene transduced EL-4 cells showed weak vaccine effect. Remarkable cooperative antitumor effect against EL-4 cell challenge was observed when both irradiated EL-4/GM-CSF and EL-4/B7-1 were inoculated together. Conclusion: GM-CSF gene or B7-1 gene transduced BL-4 cells can be used as a good tumor vaccine. The combination of the two kinds of vaccine may have potential application value in human cancer treatment.

Baduanjin exercise improved premenstrual syndrome symptoms in Macao women

OBJECTIVE: To investigate the effect of Baduanjin,a traditional Chinese medical exercise, on improving premenstrual syndrome(PMS) symptoms in ***: Forty reproductive age women with PMS in Macao practiced standardized Baduanjin exercise for three menstrual cycles. A questionnaire,the daily record of severity of problems(DRSP), was used to measure symptom severity. DRSP was filled out every days tarting from the last cycle before exercise(i.e., the first menstrual cycle) to the third cycle after exercise initiation(i.e., the fourth menstrual cycle). The total scores and the scores of each item during the 5 premenstrual days and follicular phase(5-9 postmenstrual days)were ***: After exercise, the total DRSP scores dur-ing the 5 premenstrual days and differences in the total DRSP scores between the 5 premenstrual days and the follicular phase were both significantly reduced. Of note, the physical symptom total scores in the 5 premenstrual days and the differences between its total score in the 5 premenstrual days and in the follicular phase were both significantly reduced. The differences between the total scores of depressed mood, anxious mood, loss of interest,and reduction in social activity during the 5 premenstrual days and the follicular phase were also ***: Baduanjin exercise was able to improve the mental and especially the physical symptoms of PMS.

Background: Conscious patients admitted to intensive care units(ICUs) suffer from pain for various reasons, which can affect their recovery ***: The present study compared the effects of aromatherapy with Citrus aurantium and lavender essential oils against placebo for reducing pain in conscious intensive care ***, setting, participants and interventions: This study was a parallel randomized placebo-controlled trial. The ICUs of two educational hospitals in Kerman in Southeastern Iran were the study *** hundred and fifty conscious intensive care patients were randomly divided into three groups using a stratified block randomization method. Two groups received aromatherapy with essential oils: one with lavender and the other with C. aurantium;these patients received a 30-minute therapy session using their assigned essential oil on the second day of their intensive care stay. The placebo group used 5 drops of normal saline instead of essential oil during their *** outcome measures: Patient’s pain was assessed using a visual analog scale before the aromatherapy intervention, as well as immediately after and one and three hours after ***: The mean pain score of the lavender group was 40.01 before the aromatherapy intervention and fell to 39.40, 30.60 and 23.68 immediately after the intervention, and at hour one and three postintervention, respectively. The mean pain score of the C. aurantium group was 45.48 before the intervention and was reduced to 32.34 at three hours after the intervention. The mean pain of the placebo group decreased from 42.80 before the intervention to 35.20 at three hours after the intervention. Pain scores of all groups decreased during the study(P < 0.001). Nurses’ clinical alarm-related behaviors and influencing factors in China

Objective:To explore the nurses’behaviors regarding clinical alarms,analyze the related influencing factors,and provide rationales for alarm ***:A cross-sectional survey was conducted in *** self-made questionnaire of nurses’clinical alarm-related knowledge,attitude,and behavior(NCAKAB)was ***:The valid response rate was 98.66%(n=2368).The average nurses’clinical alarm-related behaviors(NCAB)score was 65.14±7.95(out of 85).The dimension scores of NCAB from high to low were alarm learning(4.02±0.85,out of 5),alarm response(27.99±3.64,out of 35),alarm setting(19.24±3.88,out of 25),alarm recognition(7.63±1.68,out of 10)and alarm notification(6.25±1.84,out of 10).There were significant differences in alarm behavior scores between nurses of different ages(F=4.619,P=0.000),nursing stints(F=9.564,P=0.000),professional titles(F=4.425,P=0.004),departments(F=9.166,P=0.000),and hospital levels(t=2.705,P=0.007).The study showed that nurses’total alarm behavior scores were positively correlated with the total alarm knowledge score(r=0.267;Pdepartment,nursing stint,hospital level,professional title,alarm-related training,willingness to participate in alarm-related training,whether or not departments have improved alarm management over the last 3 years,and whether or not departments have formulated norms for alarm *** with higher scores for clinical alarm knowledge had higher correlating scores for alarm behavior;similarly,nurses with higher scores for clinical alarm attitude had higher scores for alarm-related behavior.

Demands of experiential training for ICU nurses in Hunan of China

Objective:To assess intensive care unit(ICU)nurses’demands for specialized experiential training and to provide inputs for developing an experiential training program for ICU ***:A questionnaire for assessing ICU nurses’demands for experiential specialty training was distributed to 360 ICU nurses,selected through purposive sampling from two secondary hospitals and six tertiary hospitals in Hunan Province,***:Of the survey participants,63.6%had undergone a specialty training program for ICU *** these individuals,53.0%were satisfied with the *** as a qualified nurse was considered an essential criterion for admission of trainees into the program by 81.8%of respondents,while 77.1%of respondents considered clinical working experience to be a critical requirement for selecting trainers.A total of 48.1%of the respondents preferred part-time training,and 36.1%considered a training cycle of 9 e12 weeks to be ***,they felt that the training methods should be tailored to different stages of the experiential learning *** for experiential training among ICU nurses were quantified,with high demand reflected in an overall score of 4.41±***“intensive care technology”experiential training module was ranked highest in terms of demand,with the top five sub-modules being specialty operating technology(4.67±0.53),care of critically ill patients(4.66±0.55),critical patient rescue procedures(4.65±0.56),assessing monitoring indexes(4.63±0.56),and the application of relevant instruments(4.61±0.57).Conclusion:Nearly half of the respondents indicated that their experiences of specialty training programs were not satisfactory,and they had high demands for experiential ***,to optimize training outcomes,continuous updating of training methods is ***,a systematic,comprehensive,and multilevel experiential training program that targets the specific needs of ICU nurses is essential.
