AIM:To provide a comprehensive and more representative national data on the disease,especially on treatment options and outcomes,and to determine access of retinoblastoma patients from Luzon,Visayas and Mindanao to eye care,and determine if access is associated with delay in consultation,staging and outcomes.METHODS:Cohort study of retinoblastoma patients seen in eleven institutions located in the three major areas of the Philippines namely Luzon,Vizayas and Mindanao from 2010-2020.RESULTS:Totally 636 patients,involving 821 eyes,were included.Majority(57%)were from Luzon and were seen in institutions in Luzon(72%).Annually,58±10 new cases were seen with 71%having unilateral disease.Median delay of consultation remained long at 9(3,17)mo,longest in patients with unilateral disease(P<0.02)and those from the Visayas(P<0.003).Based on the International Retinoblastoma Staging System,only 35%of patients had Stage 1 while 47%already had extraocular disease.Enucleation was the most common treatment received by 484 patients while intravenous chemotherapy was received by 469.There were 250(39%)patients alive,195(31%)dead,85(13%)abandoned,17(3%)refused and 89(14%)with no data.CONCLUSION:This study presents the largest cohort of retinoblastoma patients in the Philippines in terms of patients’and participating institutions’number and geographical location and type of institution(private and public).It also presents more comprehensive data on the treatments used and outcomes(survival,globe salvage,and vision retention rates).Delay in consultation was still long among patients leading to advanced disease stage and lower survival rate.Despite increasing capacity to diagnose and manage retinoblastoma in the country,the delay of consultation remains long primarily due to accessibility issues to eye care institutions especially in the Visayas and financial concerns.The delay was still significant that overall survival rate remain low.

The 2022 Extreme Heatwave in Shanghai,Lower Reaches of the Yangtze River valley:Combined Influences of Multiscale Variabilities

In the summer of 2022,China(especially the Yangtze River valley,YRV)suffered its strongest heatwave(HW)event since 1961.In this study,we examined the influences of multiscale variabilities on the 2022 extreme HW in the lower reaches of the YRV,focusing on the city of Shanghai.We found that about 1/3 of the 2022 HW days in Shanghai can be attributed to the long-term warming trend of global warming.During mid-summer of 2022,an enhanced western Pacific subtropical high(WPSH)and anomalous double blockings over the Ural Mountains and Sea of Okhotsk,respectively,were associated with the persistently anomalous high pressure over the YRV,leading to the extreme HW.The Pacific Decadal Oscillation played a major role in the anomalous blocking pattern associated with the HW at the decadal time scale.Also,the positive phase of the Atlantic Multidecadal Oscillation may have contributed to regulating the formation of the double-blocking pattern.Anomalous warming of both the warm pool of the western Pacific and tropical north Atlantic at the interannual time scale may also have favored the persistency of the double blocking and the anomalously strong WPSH.At the subseasonal time scale,the anomalously frequent phases 2-5 of the canonical northward propagating variability of boreal summer intraseasonal oscillation associated with the anomalous propagation of a weak Madden-Julian Oscillation suppressed the convection over the YRV and also contributed to the HW.Therefore,the 2022 extreme HW originated from multiscale forcing including both the climate warming trend and air-sea interaction at multiple time scales.

Since the Bonn 2011 conference,the“water-energy-food”(WEF)nexus has aroused global concern to promote sustainable development.The WEF nexus is a complex,dynamic,and open system containing interrelated and interdependent elements.However,the nexus studies have mainly focused on natural elements based on massive earth observation data.Human elements(e.g.,society,economy,politics,culture)are described insufficiently,because traditional earth observation technologies cannot effectively perceive socioeconomic characteristics,especially human feelings,emotions,and experiences.Thus,it is difficult to simulate the complex WEF nexus.With the development of earth observation sensor technologies and human activity perception methods,geographical big data covering both human activities and natural elements offers a new opportunity for in-depth WEF nexus analysis.This study proposes a five-step framework by leveraging geographical big data mining to dig for the hidden value in the data of various natural and human elements.This framework can enable a thorough and comprehensive analysis of the WEF nexus.Some application examples of the framework,major challenges,and possible solutions are discussed.Geographical big data mining is a promising approach to enhance the analysis of the WEF nexus,strengthen the coordinated management of resources and sectors,and facilitate the progress toward sustainable development.

The global physical and biogeochemical environment has been substantially altered in response to increased atmospheric greenhouse gases from human activities.In 2023,the sea surface temperature(SST)and upper 2000 m ocean heat content(OHC)reached record highs.The 0–2000 m OHC in 2023 exceeded that of 2022 by 15±10 ZJ(1 Zetta Joules=1021 Joules)(updated IAP/CAS data);9±5 ZJ(NCEI/NOAA data).The Tropical Atlantic Ocean,the Mediterranean Sea,and southern oceans recorded their highest OHC observed since the 1950s.Associated with the onset of a strong El Niño,the global SST reached its record high in 2023 with an annual mean of~0.23℃ higher than 2022 and an astounding>0.3℃ above 2022 values for the second half of 2023.The density stratification and spatial temperature inhomogeneity indexes reached their highest values in 2023.

Heterogeneity of clinical features, EEG and brain imaging findings in anti‑leucine‑rich glioma‑inactivated protein 1 autoimmune encephalitis: a retrospective case series study and review of the literature

Background Anti-leucine-rich glioma-inactivated 1(LGI-1)autoimmune encephalitis(AE),characterized by rapid decline of memory,seizures,and neuropsychiatric abnormalities,is a rare but devastating disorder.Early diagnosis and treatment are essential to prevent long-term sequelae.In this report,we provide a detailed description of clinical characteristics,laboratory test results,imaging,and electroencephalography(EEG)findings,as well as treatment responses of eight patients with anti-LGI-1 AE treated at our center.Case presentation At the onset,all eight patients presented with confusion/memory deterioration,seizures(including faciobrachial dystonic seizures or other types of seizure),and behavioral changes such as hallucination,paranoia,and anxiety.Four patients were found with severe hyponatremia.Anti-LGI1 antibodies were detected in the cerebrospinal fluid and/or serum of all patients.For patients with faciobrachial dystonic seizures,no discernible scalp EEG change was detected,while EEG recording of patients experiencing other types of seizure showed focal slowing,focal epileptiform discharges,and focal onset seizures.All patients showed abnormal brain magnetic resonance imaging signals,mainly involving the mesial temporal lobe and the hippocampus.In addition,one patient also experienced fulminant cerebral edema during the acute phase of the illness.All patients received immunotherapy and anti-seizure medications and achieved good seizure control.Nevertheless,these patients continued to experience cognitive impairment during their long-term follow-ups.Conclusions The care of anti-LGI1 AE patients requires rapid evaluation,prompt initiation of immunotherapy,and long-term follow-up.The long-term presence of neurocognitive complications observed in these patients underline the importance of developing reliable biomarkers that can distinguish between different subtypes of this disease with heterogeneous clinico-electrographico-radiological features.Further research is needed to understand the molecular mechanisms un

Advances of Intracranial Electroencephalography in Localizing the Epileptogenic Zone

Intracranial electroencephalography（i EEG）provides the best precision in estimating the location and boundary of an epileptogenic zone. Analysis of i EEG in the routine EEG frequency range（0.5-70 Hz） remains the basis in clinical practice. Low-voltage fast activity is the most commonly reported ictal onset pattern in neocortical epilepsy, and low-frequency high-amplitude repetitive spiking is the most commonly reported ictal onset pattern in mesial temporal lobe epilepsy. Recent studies using wideband EEG recording have demonstrated that examining higher（80-1000 Hz） and lower（0.016-0.5 Hz） EEG frequencies can provide additional diagnostic information and help to improve the surgical outcome. In addition,novel computational techniques of i EEG signal analysis have provided new insights into the epileptic network.Here, we review some of these recent advances. Although these sophisticated and advanced techniques of i EEG analysis show promise in localizing the epileptogenic zone,their utility needs to be further validated in larger studies.

De novo ADGRV1 variant in a patient with ictal asystole provides novel clues for increased risk of SUDEP

Background Various cardiac and autonomic manifestations are frequently reported during seizures.Among the seizure-related arrhythmia,ictal tachycardia is the most common,followed by ictal bradycardia,with ictal asystole being the rarest.The occurrence of ictal asystole may obscure the clinical presentation and delay the diagnosis,representing a life-threatening presentation of epilepsy,with an elevated risk of sudden unexpected death in epilepsy patients(SUDEP).These cardiac abnormalities are being increasingly recognized as the key to elucidating the mechanisms of SUDEP.Case presentation We present a 35-year-old man with a history of focal-onset seizures with impaired consciousness since his mid-20 s.He developed different types of seizures for 2 years,described as tonic seizure and atonic seizure(drop attack).During such clinical events,he suffered from falls and cardiac arrest.However,thorough cardiac electrophysiology and imaging workup failed to reveal a cardiac etiology.Subsequent video electroencephalograph(EEG)monitoring was performed,and ictal bradycardia and ictal asystole were discovered.A cardiac pacemaker was implanted,and at 3-year follow-up,the patient did not suffer more atonic seizures,or falls.Genetic tests discovered a de novo variant of Adhesion G Protein-Coupled Receptor V1(ADGRV1),which may provide a clue for the patient’s ictal asystole and the increased risk of SUDEP.Conclusions Considering the important impact of ictal bradycardia and asystole on the morbidity and potential mortality of epileptic patients,it is important to simultaneously utilize EEG and electrocardiogram to confirm the diagnosis.This case report highlights the link between the de novo variant of ADGRV1 and the ictal bradycardia/asystole phenotype and implicates the importance of genetic testing in adult epilepsy patients.

Analysis of Tibetan Plateau Vortex Activities Using ERA-Interim Data for the Period 1979-2013

Based on the ECMWF Interim reanalysis （ERA-Interim） 500-hPa height data for the period 1979-2013, the Tibetan Plateau vortex （TPV） activities are investigated with an objective identification and tracking algorithm. The vortex tracks over the Tibetan Plateau （TP） and its vicinity are identified, and the cold/warm nature of the TPV is revealed from the 500-hPa temperature fields. A TPV activity dataset is thus derived. Examination of the TPV frequency, intensity, persistence, geographical location, and migration from the TP suggests an average of 53 TPVs annually during the study period, with 6.7 of them shifting out of the TP. For these vortexes, the longer the lifetime, the lower the frequency, and 81% of the TPVs are initially warm in nature. The particularly high-intensity vortexes occur dominantly in the rainy period （May September）, with their origin mainly in western Nagqu and northern Ali. They disappear largely in the Tanggula maintain area on the east side of the vortex high-frequency center and the Dangqu River valley in western Qinghai Province, possibly due to the topography. It is also found that the frequency of TPVs is decreasing, with 2 fewer TPV occurrences per 10 yr. Meanwhile, it is statistically significant that 1.4 fewer TPVs move out of the TP and the percentage of TPVs moving out of the TP versus the total number of TPVs has reduced by 2.3% every 10 yr.

Burns resulting from spontaneous combustion of electronic cigarettes: a case series

Background:Electronic cigarette(e-cigarette)sales have grown rapidly in recent years,coinciding with a public perception that they are a safer alternative to traditional cigarettes.However,there have been numerous media reports of fires associated with e-cigarette spontaneous combustion.Case Presentation:Three severe burns caused by spontaneous combustion of e-cigarettes within a 6-month period were treated at the Santa Clara valley Medical center Burn Unit.Patients sustained partial and full-thickness burns.Two required hospitalization and surgical treatment.Conclusions:E-cigarettes are dangerous devices and have the potential to cause significant burns.Consumers and the general public should be made aware of these life-threatening devices.
