Cancer stem cells （CSCs）, a subpopulation of tumor cells, have self-renewal and multi-lineage differentiation abilities that play an important role in cancer initiation, mainte- nance, and metastasis. An accumulation of evidence indicates that CSCs can cause conventional therapy fail- ure and cancer recurrence because of their treatment resistance and self-regeneration characteristics. There- fore, approaches that specifically and efficiently eliminate CSCs to achieve a durable clinical response are urgently needed. Currently, treatments with chimeric antigen receptor-modified T （CART） cells have shown successful clinical outcomes in patients with hematologic malignan- cies, and their safety and feasibility in solid tumors was confirmed. In this review, we will discuss in detail the possibility that CART cells inhibit CSCs by specifically targeting their cell surface markers, which will ultimately improve the clinical response for patients with various types of cancer. A number of viewpoints were summarized to promote the application of CSC-targeted CART cells in clinical cancer treatment. This review covers the key aspects of CSC-targeted CART cells against cancers in accordance with the premise of the model, from bench to bedside and back to bench.

OBJECTIVE:To observe the effects of the Bupi Hewei(BPHW)decoction on diarrhea and intestinal flora disorder induced by 5-fluorouracil(5-FU)and investigate the possible mechanism underlying these ***:Thirty-five male Sprague-Dawley rats were randomly divided into four groups:no.mal control,5-FU,5-FU+BPHW decoction(10.5 g/kg for 5 consecutive days),and 5-FU+Bacillus licheni-formis capsule groups(0.2 g/kg for 5 consecutive days).Animal models were established via the intraperitoneal injection of 5-FU(30 mg/kg for 5 consecutive days).At the end of the treatment period,diarrhea was assessed,and the change of the intestinal flora was examined using 16 S r DNA highthroughput ***(IL)-17,IL-21,IL-6,IL-10,RAR-related orphan receptor gamma(RORγt),and forkhead box P3(Foxp3)expression in the jejunum was detected using immuno.istochemistry,quantitative real-time polymerase chain reaction(PCR),Western blotting,and enzymelinked immuno.sorbent ***:In this study,the BPHW decoction effectively lowered the diarrhea score,increased the proportions of Bacteroidetes and Prevotellaceae-Alloprevotella species,and reduced the proportions of Proteobacteria,Escherichia-Shigella,Rumino.occaceae NK4 A214,and Rumino.occaceae UCG-005 species in the rat intestine after 5-FU *** addition,the BPHW decoction significantly suppressed the expression of IL-17,IL-21,IL-6,IL-10,RORγt,and Foxp3 in the ***:Our findings suggest that the BPHW decoction can improve the intestinal immune balance and reduce intestinal inflammation by targeting T helper cell/T regulatory cell-associated factors.

Tethered cord syndrome is a progressive disease with a typically insidious onset in infants and children, and which can lead to persistent progress of neurological deficits and a high rate of disability without timely intervention. The purpose of this study was to investigate the curative effect of microsurgery in children with different types of tethered cord syndrome. In this study, we analyzed 326 patients with tethered cord syndrome, aged from 2 months to 14 years old, who were followed for 3-36 months after microscopic surgery. Based on clinical manifestations and imaging findings, these patients were classified into five types： tight ilium terminale （53 cases）, lipomyelome- ningocele （55 cases）, lipomatous malformation （124 cases）, postoperative adhesions （56 cases）, and split cord malformation （38 cases）. All patients underwent microsurgery. Curative effects were measured before and 3 months after surgery by Spina Bifida Neurological Scale based on sensory and motor functions, reflexes, and bladder and bowel function. The results showed that Spina Bifida Neurological Scale scores improved in all five types after surgery. Overall effective rates in these patients were 75%. Effective rates were 91% in tight ilium terminale, 84% in lipomyelomeningocele, 65% in lipomatous malformation, 75% in postoperative adhesion, and 79% in split cord mal- formation. Binary logistic regression analysis revealed that types of tethered cord syndrome （lipoma-type or no.） and symptom duration before surgery were independent influencing factors of surgical outcome. These results show that therapeutic effect is markedly different in patients with different types of tethered cord syndrome. Suitable clinical classification for tethered cord syndrome will be helpful in predicting progno.is and guiding treatment. Predicting tremor improvement after MRgFUS thalamotomy in essential tremor from preoperative spontaneous brain activity:A machine learning approach

AIM: To assess the technical safety and efficacy of transcatheter arterial chemoembolization (TACE) combined with immediate radiofrequency ablation (RFA) for large hepatocellular carcino.as (HCC) (maximum diameter ≥ 5 cm). METHODS: Individual lesions in 18 patients with HCCs (mean maximum diameter: 7.5 cm; range: 5.1-15.5 cm) were treated by TACE combined with percutaneous RFA between January 2010 and June 2012. All of the patients had previously undergone one to four cycles of TACE treatment. Regular imaging and laboratory tests were performed to evaluate the rate of technical success, technique-related complications, local-regional tumor responses, recurrence-free survival time and survival rate after ***: Technical success was achieved for all 18 visible HCCs. Complete response (CR) was observed in 17 cases, and partial response was observed in 1 case 1 mo after intervention. The CR rate was 94.4%. Local tumors were mainly characterized by coagulative necrosis. During follow-up (2-29 mo), the mean recurrencefree survival time was 16.8 ± 4.0 mo in 17 cases of CR. The estimated overall survival rate at 6, 12, and 18 mo was 100%. no.major complications were observed. Levels of alanine amino.ransferase (ALT) and aspartate amino.ransferase (AST) in the blood of 17 patients transiently increased on the third day after treatment (ALT 200.4 ± 63.4 U/L vs 24.7 ± 9.3 U/L, P < 0.05; AST 228.1 ± 25.4 U/L vs 32.7 ± 6.8 U/L, P < 0.05). Severe pain occurred in three patients, which was controlled with morphine and fentanyl. CONCLUSION: TACE combined with immediate RFA is a safe and effective treatment for large solitary HCCs. Severe pain is a major side effect, but can be controlled by morphine.

Surgical robots are designed to provide enhanced precision and dexterity compared to manual surgical procedures,which mainly rely on multimodal sensing techno.ogies for the surgeon to seamlessly operate the robotic arms and *** with single-mode sensors, optical and mechanical bi-modal sensors provide improved precision, enhanced safety,and robustness of human-machine interaction systems. Here, the template-guided and pneumatic printing techno.ogies are combined to construct perovskite and graphene parallel structures with both optical and mechanical sensing capabilities. The printed uniformly crystallized perovskite microstructure exhibits fast and sensitive photoelectric response characteristics,enabling shadow recognition functionality. The combination of graphene and elastic rubber endows the great printability to prepare parallel structures near the perovskite arrays for force sensing capabilities. Thus, the printed perovskite and graphene structures possess no.-contact optical sensing capabilities to detect hand position by recognizing shadows between the hand and the sensor, as well as contact mechanical sensing capabilities to detect touch force applied by the hand. It provides a synergistic platform for real-time and multidimensional feedback to improve human-machine interaction.

AIM To explore the value of three-dimensional(3 D) visualization techno.ogy in the minimally invasive treatment for infected necrotizing pancreatitis(INP). METHODS Clinical data of 18 patients with INP, who were admitted to the pla General hospital in 2017, were retrospectively analyzed. Two-dimensional images of computed tomography were converted into 3 D images based on 3 D visualization techno.ogy. The size, number, shape and position of lesions and their relationship with major abdominal vasculature were well displayed. Also, percutaneous catheter drainage(PCD) number and puncture paths were designed through virtual surgery(percutaneous nephroscopic necrosectomy) based on the principle of maximum removal of infected necrosis *** Abdominal 3 D visualization images of all the patients were well reconstructed, and the optimal PCD puncture paths were well designed. Infected necrosis was conveniently removed in abundance using a nephroscope during the following surgery, and the median operation time was 102(102 ± 20.7) min. Only 1 patient underwent endoscopic necrosectomy because of residual necrosis. CONCLUSION The 3 D visualization techno.ogy could optimize the PCD puncture paths, improving the drainage effect in patients with INP. Moreover, it significantly increased the efficiency of necrosectomy through the rigid nephroscope. As a result, it decreased operation times and improved the progno.is.
