Background and Aims:Acute liver failure(ALF)is a potentially fatal clinical syndrome with no effective *** study aimed to explore the role of Janus kinase 2/signal transducer and activator of transcription 3(JAK2/STAT3)pathway in modulating the phenotype and immune function of endotoxin-tolerant dendritic cells(ETDCs).In addition,we explored the use of EDTCs in an experimental model of ALF and investigated the associated ***:In the in vitro experiment,ETDCs were transfected with adenovirus to induce SOCS1^(+/+)ETDCs and SOCS1^(−/−)***,costimulatory molecules and mixed lymphocyte reaction were *** mice were randomly divided into normal control,ALF,ALF+mock-ETDCs,ALF+SOCS1^(+/+)ETDCs,ALF+AG490,and ALF+AG490+SOCS1^(+/+)ETDCs *** examined the therapeutic effect of adoptive cellular immunotherapy by tail-vein injection of target ETDCs 12 h before ALF ***490,a JAK2/STAT3 inhibitor,was used in the in vivo experiment to further explore the protective mechanism of SOCS1^(+/+)***:Compared with control ETDCs,SOCS1^(+/+)ETDCs had lower expression of costimulatory molecules,weaker allostimulatory ability,lower levels of IL-6 and TNF-αexpression and higher IL-10 ***1^(−/−)ETDCs showed the opposite *** the in vivo experiments,the ALF+SOCS1^(+/+)ETDCs and ALF+AG490+SOCS1^(+/+)ETDCs groups showed less pathological damage and suppressed activation of JAK2/STAT3 *** changes were more pronounced in the ALF+AG490+SOCS1^(+/+)ETDCs *** of SOCS1^(+/+)ETDCs had a protective effect against ALF possibly via inhibition of JAK2 and STAT3 ***:The SOCS1 gene had an important role in induction of endotoxin ***1^(+/+)ETDCs alleviated lipopolysaccharide/D-galactosamine-induced ALF by downregulating the JAK2/STAT3 signaling pathway.

Radiofrequency ablation for treating paroxysmal supraventricular tachycardia complicated by atrial fibrillation： A single-center retrospective analysis

The effect of selective radiofrequency ablation for treating paroxysmal supraventricular tachycardia （PSVT） and its associated paroxysmal atrial fibrillation （PAF） was assessed. Methods Data were collected retrospectively from patients diagnosed of PSVT and subsequently treated with radiofrequency ablation. Regular monthly follow-up by dynamic electrocardiography （ECG） was performed. Incident rates of atrial fibrillation before and after ablation were compared. Results 382 PSVT patients with 58 having atrial fibrillation were en- rolled. The order of complicated PAF from high to low in these patients was displayed as： atrial tachycardia （AT）, atrioventricular reentrant tachycardia （AVRT） and atrioventricular nodal reentrant tachycardia （AVNRT）. Among AVRT patients, PAF was more frequent in patients having accessory pathways. AVNRT patients had significant- ly lower PAF rate comparing to other patients. PAF incident rate was significantly reduced by radiofrequency ablation therapy. Conclusion We advise regular dynamic ECG for PSVT patients, especially those with atrial flutter, AT or pre-excitation syndrome. Selective radiofrequency ablation is a feasible approach for treating AF complicated PSVT patients.

Congenital choledochal cysts in adults:twenty-five-year experience

Background Choledochal cyst is rare in western countries. The relatively high incidence of coexistent hepatobiliary disease increases the difficulty of the surgical management of choledochal cyst. Here we analyze the diagnosis and treatment of congenital bile duct cyst in 122 Chinese adults. Methods The clinical data of 122 patients with congenital choledochal cysts admitted from 1981 to 2006 were analyzed. Results Clinical symptoms in most cases were nonspecific, resulting in delayed diagnosis. Sixty-one patients （50%） had coexistent pancreatobiliary disease. Among the 122 patients, 119 patients underwent ultrasonic examination; ERCP/MRCP was performed in 63 cases and CT in 102 cases. Abnormal pancreatobiliary duct junction was found in 48 patients. Sixteen patients had malignant lesions in the bile duct, arising in 11 of them from incomplete choledochal cyst that underwent various operations including cystenterostomy or cystojejunostomy There was significant difference between the patients who underwent incomplete cyst resection and complete cyst resection in malignancy rate of bile duct （Chi square test, P= 0.000; odds ratio, 7.800; 95 % confidence interval, 2.450 to 24.836）. Conclusions ERCP, CT and MRCP had proved their great values in the classification of the disease. Cyst excision with Roux-en-Y hepaticojejunostomy is recommended as the treatment of choice for patients with type Ⅰ or type Ⅳ cysts. For type Ⅴ cyst （Caroli＇s disease） with recurrent cholangitis, liver transplantation should be considered.

Objective:To study the stem cell characteristics of early colon cancer tissue and their relationship with cell proliferation and ***: Colon cancer tissues and adjacent tissues surgically removed in Dongguan People's hospital between January 2010 and October 2017 were selected as the clinical samples of this study, the protein was extracted to determine the protein expression of tumor stem cell genes USP22, Nanog, Lgr5 and CD44, and RNA was extracted to determine the mRNA expression of cell proliferation genes and cell invasion ***:USP22, Nanog, Lgr5 and CD44 protein expression as well as Rab5A, TBX2, MDM2, TGF-β1, Smad2/3, Vimentin, Rac1 and VEGF mRNA expression in colon cancer tissues were significantly higher than those in adjacent tissues while Bad, Bax and Fas mRNA expression were significantly lower than those in adjacent tissues;USP22, Nanog, Lgr5 and CD44 protein expression in colon cancer tissues were positively correlated with Rab5A, TBX2, MDM2, TGF-β1, Smad2/3, Vimentin, Rac1 and VEGF mRNA expression, and negatively correlated with Bad, Bax and Fas mRNA ***: The activation of stem cell characteristics in early colon cancer can promote the proliferation and invasion of cancer cells.

SURGERY AND MULTI MODAL THERAPYIN COLON-ANGIN RECTAL CANCER

1 SummaryThe 5-year-survival rates of surgically treated patients are varying and depend on UICC stages/substages with remarkable variations in between published reports, surgical hospital units, individual surgeons, and continents. These variations may be due to surgical techniques, training status, hospital and individual case volume, but also on referral patterns, and statistical evaluation methods. Survival time and cure rates are significantly improved by modern adjuvant chemotherapy in colon cancers UICC Ⅲ and in substages of UICC Ⅱ (e.g. UICC Ⅱ B) by 5%-16%, and adjuvant radiochemotherapy in rectal cancer by 10%-14% when compared to surgical controls. In three modern colon cancer trials standard adjuvant chemotherapy was further improved by increasing the survival rates, e.g. from 59% to 71% in stage Ⅲ and IIB patients. In rectal cancer neoadjuvant radio(chemo)therapy decreases local relapse rates vs. postoperative adjuvant radio(chemo)therapy. Since surgery in rectal cancer has also been significantly improved by total mesorectal excision (TME) and better surgical training, the indications and methods for multimodal therapy have changed from UICC Ⅱ+Ⅲ to more individual criteria. Molecular and genetic factors, such as thymidylate synthase (TS), microsatellite instability (MSI) or loss of chromosome 18q/DCC might have an independent impact on prognosis in the spontaneous course, but could also help to individually select colon and rectal cancer patients and treatment protocols for multimodal therapy. Thus, surgery and multimodal therapy has become very complex, needs regularly be updated in competent reviews, and should be conducted in specialized centers of multi-and interdisciplinary excellence.

Epstein-Barr virus(EBV) is tightly associated with a variety of human tumors including nasopharyngeal carcinoma and gastric cancer of epithelial origin. The virus latently infects the host cells and express proteins and non-coding RNAs to achieve malignancy. microRNAs(miRNAs or miRs) are small RNA of 19-25 nucleotides(nt) which directly bind to the 3’ untranslational region of mRNA to promote degradation of mRNA and inhibit translation. EBV encoded miRs are generated from two portions;the transcripts from BamHI A region are present in abundance in NPC and gastric cancer. The miRs epigenetically regulate the expression of molecules which are effectors of cell cycle,migration,apoptosis,and innate immunity,playing a virtal role in supporting viral replication and occurrence of EBV associated tumors. The feasibility of using them as biomarkers for diagnosis and prognosis of EBV associated tumors is under study.

目的评价磁共振三维扰相脂肪抑制梯度回波序列(3D-FS-SPGR)在膝关节软骨损伤性病变中的诊断价值。方法 45例患者50侧膝关节镜检查病例,将每侧膝关节软骨面分为五个区,MRI检查采用3D-FS-SPGR序列,分别评价并对照关节镜检查结果进行统计分析,一致性指标用Kappa值。结果以关节镜检查为标准,3D-FS-SPGR序列诊断膝关节软骨损伤的敏感度为97.3%,特异度为93.6%,Kappa值为0.90。结论 3D-FS-SPGR序列与关节镜对膝关节软骨损伤诊断结果之间一致性极佳,具有临床应用价值,可作为临床首选。
