Heparin and other anticoagulants in amniotic fluid embolism(AFE):Literature review and concept of the therapy

Aim: The objective of this study is to review all the reported outcomes of heparin application in amniotic fluid embolism (AFE) so far and to find out why, when and how heparin or other anticoagulants should be used in AFE. Material and methods: We searched Medline (from 1969 to 2011), using two key words: 1) amniotic fluid embolism;2) amniotic fluid embolism and heparin. The search for the former produced 1127 replies, of which 208 were case reports of AFE. In response to the other key word, there were 94 articles. We looked through all the articles, selecting those relevant for our study. Results: In the years 1969-2011, 208 AFE cases were reported. Heparin (unfractionated heparin) or low molecular weight heparin (LMWH) was applied in 20 cases (9.6%), being the main drug in 11 cases (5.3%) and in 6 cases as a component of spectacular treatment of AFE (surgical treatment and extracorporal membrane oxygenation). In one of these cases anithrombin (AT) with LMWH was used. In one patient heparin therapy was considered to be unsuccessful and hence recombinant plasminogen activator (rt-PA) was instituted. All the patients survived. Conclusions: 1) The attempts to use heparin in AFE could be defined as promising, although the number of treated patients is too small for conclusion;2) The postulate to use heparin at the very onset of AFE (a bolus of 10,000 U followed by monitored intravenous infusion) has serious justification: one of the pathways of AFE is the target for heparin (coagulation pathway).

Amniotic Fluid Embolism (AFE): A Review of the Literature Orientated on Two Clinical Presentations--Typical and Atypical

Background/Aim: Recently, a comparative study on the incidence of AFE has highlighted rather confusing results, showing that the complication is more than three times higher in North America than that in some European countries. In this paper, we put forward the hypothesis that this discrepancy is due to inaccurate diagnosis of non-classical form of AFE (atypical AFE). We also provide an outline of symptoms that characterize this type of AFE based on the analysis of all available case reports. Material and Methods: We searched Medline from 1969 (its inception) to 2011, using the key words “amniotic fluid embolism”. The search produced 1127 articles, including 208 case reports of AFE and other publications identified as eligible for our study (11 review articles and 6 population-based studies of the last few years). Moreover, we looked through the articles from the period before “inception of Medline” to find 178 earlier case reports. Full texts were analyzed. Results and Conclusions: (i) Worldwide, 447 cases of AFE have been reported, including 70 cases of atypical AFE (15.7%). (ii) Typical AFE is characterized by three clinical phases (cardiopulmonary collapse, clotting disorders and hemorrhages, multiorgan disturbances), whereas the atypical one shows lack of cardiopulmonary collapse as the initial presentation—the first to appear is obstetric hemorrhage and/or pulmonary and renal dysfunction. (iii) Four subclasses of atypical AFE were distinguished on the basis of case reports: uterine hemorrhage-type of AFE, ARDS as the only presentation of atypical AFE, paradoxical AFE, and cesarean section-related atypical AFEs.

Happy together？ Avoidance of conspecifics by gregarious mussels

Zebra mussel Dreissena polymorpha is a Ponto-Caspian species invasive in Europe and North America, with great environmental impact. It lives byssally attached to hard substrata in large aggregations, which is often explained by its preferences for conspecifics, though direct evidence for such preferences has been rather limited so far. We studied the reactions of zebra mussels to con- specifics, hypothesizing that they may either be attracted to one another or form aggregations only in the absence of alternative attachment sites, in Experiment 1, we tested mussel tendency to detach from existing druses depending on druse size （2-25 individuals） and substratum type （soft： sand; hard： glass）. Mussels detached significantly more often on the hard substratum and from larger druses compared to soft substratum and smaller druses, respectively. This indicates that mussels tended to avoid conspecifics at high density, particularly when alternative substratum was available. In Experiment 2, we tested the responses of single mussels to distant （3 or 15cm） conspecifics （0, 3, 15 individuals per 2.51 tank） on the sandy substratum. The presence of conspecifics, regardless of their distance and density, resulted in single unattached mussels staying more often in their initial positions. Mussels did not move preferentially towards or away from the conspecifics. Thus, even on unsuitable substratum mussels were not attracted by conspecifics and probably exhibited an avoidance reaction by reducing their movement. This suggests that dense mussel aggregations are formed due to the lack of available alternative attachment sites rather than due to their preferences for conspecifics.

Overweight and obesity versus concentrations of VEGF-A, sVEGFR-1, and sVEGFR-2 in plasma of patients with lower limb chronic ischemia

Objective： Being overweight or obese comprises a significant risk factor for atherosclerosis. Fat tissue also generates factors stimulating angiogenesis, the process by which new blood vessels form. The purpose of this paper is to assess concentrations of the vascular endothelial growth factor A （VEGF-A） and its soluble type-1 and type-2 re- ceptors （sVEGFR-1 and sVEGFR-2） in plasma of patients with peripheral arterial disease （PAD） depending on the level of nutrition according to body mass index （BMI）. Methods： The study group included patients suffering from symptomatic PAD （n=46） in Fontaine classes Ila-lV without any history of neoplastic disease and who have a normal BMI （n=15）, are overweight （n=21） or are obese （n=10）. The control group （n=30） consisted of healthy non-smoking volunteers who were neither overweight nor obese. Venous blood plasma samples were collected from both groups at rest in the morning to determine plasma concentrations of VEGF-A, sVEGFR-1, and sVEGFR-2 using the enzyme- linked immunosorbent assay （ELISA） method. Results： The group of patients with PAD co-existent with being over- weight or obese tended to have higher mean concentration levels of VEGF-A and sVEGFR-2 when compared with patients suffering from PAD with normal BMIo A statistically significant positive correlation was obtained between BMI and average plasma concentrations of sVEGFR-2 （R=0.37, P=-0.0103）. However, no significant correlation was no- ticed between BMI and VEGF-A or sVEGFR-1 concentrations. Conclusions： A positive correlation determined be- tween the level of antiangiogenic factor and BMI value may be indicative of the linearly growing prevalence of some antiangiogenic factors in patients with metabolic disorders, which may be one of numerous factors contributing to incomplete efficiency of collateral circulation development in patients with PAD.

Labile and stabile soil organic carbon fractions in surface horizons of mountain soils–relationships with vegetation and altitude

Global and local climate changes could disturb carbon sequestration and carbon stocks in forest soils. Thus, it is important to characterize the stability of soil organic matter and the dynamics of soil organic carbon(SOC) fractions in forest ecosystems. This study had two aims:(1) to evaluate the effects of altitude and vegetation on the content of labile and stabile forms of organic carbon in the mountain soils; and(2) to assess the impact of the properties of soil organic matter on the SOC pools under changing environmental conditions. The studies were conducted in the Karkonosze Mountains(SW Poland, Central Europe). The content of the most labile fraction of carbon(dissolved organic carbon,DOC) decreases with altitude, but the content of fulvic acids(FA), clearly increases in the zone above 1000 m asl, while the stabile fraction(humins, nonhydrolyzing carbon) significantly decreases. A higher contribution of stabile forms was found in soils under coniferous forests(Norway spruce), while a smaller-under deciduous forests(European beech) and on grasslands. The expected climate change and the ongoing land use transformations in the zone above1000 m asl may lead to a substantial increase in the stable humus fraction(mainly of a non-hydrolyzing carbon) and an increase in the SOC pools, even if humus acids are characterized by a lower maturity and greater mobility favorable to soil *** the lower zone(below 1000 m asl), a decrease in the most stable humus forms can be expected,accompanied by an increase of DOC contribution,which will result in a reduction in SOC pools. Overall,the expected prevailing(spatial) effect is a decreasing contribution of the most stable humus fractions,which will be associated with a reduction in the SOC pools in medium-high mountains of temperate zone of Central Europe.

Coexistence of proangiogenic potential and increased MMP-9, TIMP-1, and TIMP-2 levels in the plasma of patients with critical limb ischemia

The objective of this study was to assess the angiogenic potential expressed as a quotient of vascular endothelial growth factor A (VEGF-A), as an indicator of proangiogenic activity, and the circulating receptors (soluble VEGF receptor protein R1 (sVEGFR-1) and sVEGFR-2), as indicators of the effect of angiogenic inhibition, depending on the concentrations of matrix metalloproteinase 2 (MMP-2) and MMP-9 and their tissue inhibitor 1 (TIMP-1) and TIMP-2 in the plasma of patients with lower extremity artery disease (LEAD). These blood parameters in patients with intermittent claudication (IC) and critical limb ischemia (CLI) were compared for select clinical and biochemical features. Stimulation of angiogenesis in the plasma of individuals with LEAD was evident as indicated by the significant increase in VEGF-A concentration along with reduced inhibition depending on circulating receptors sVEGFR-1 and sVEGFR-2. Critical ischemia was associated with higher VEGF-A, MMP-9, TIMP-1, and TIMP-2 concentrations than in the case of IC.

Acute liver failure caused by concurrent autoimmune hepatitis and hepatitis B in a 16-year old girl

A 16 year-old girl was admitted to hospital because of fatigue and somnolence,nausea,epistaxis and jaund *** examination revealed jaundice,an enlarged liver and tenderness of upper right abdom *** tests revealed an increased level of acute liver failure,bilirubin,bile acids,GGTP and a decreased prot hrombin ratio,with elevated gamma-globulin and IgG levels,and the presence of anti-mitochondrial M2 antibodies and HBV infection *** patient was diagnosed with liver failure resulting from chronic hepatitis B with an autoimmune *** treatment consisted of steroids,azathioprine,vitam in K,low-protein diet and lactulose *** undergoing a molecular test(HBV DNA 3.23 × 105 IU/mL and mutations 204 and 80),the treatment was mo tient was discharged in good clinical condition,with the recommendation of continued entecavir,prednisone and *** subsequent months,no clinical deter ioration or abnormal biochemical liver function test results were found,despite the discontinuation of immunos up pressive therapy after 10 *** patient cont inues entecavir therapy.

Metabolomic fingerprinting of porcine lung tissue during pre-clinical prolonged ex vivo lung perfusion using in vivo SPME coupled with LC-HRMS

Normothermic ex vivo lung perfusion(NEVLP)has emerged as a modernized organ preservation technique that allows for detailed assessment of donor lung function prior to *** main goal of this study was to identify potential biomarkers of lung function and/or injury during a prolonged(19 h)NEVLP procedure using in vivo solid-phase microextraction(SPME)technology followed by liquid chromatography-high resolution mass spectrometry(LC-HRMS).The use of minimally invasive in vivo SPME fibers for repeated sampling of biological tissue permits the monitoring and evaluation of biochemical changes and alterations in the metabolomic profile of the *** in vivo SPME fibers were directly introduced into the lung and were also used to extract metabolites(on-site SPME)from fresh perfusate samples collected alongside lung samplings.A subsequent goal of the study was to assess the feasibility of SPME as an in vivo method in metabolomics studies,in comparison to the traditional inlab metabolomics *** upregulated biochemical pathways involved in pro-and antiinflammatory responses,as well as lipid metabolism,were observed during extended lung perfusion,especially between the 11th and 12th hours of the procedure,in both lung and perfusate ***,several unstable and/or short-lived metabolites,such as neuroprostanes,have been extracted from lung tissue in vivo using SPME ***-site monitoring of the metabolomic profiles of both lung tissues through in vivo SPME and perfusate samples on site throughout the prolonged NEVLP procedure can be effectively performed using in vivo SPME technology.
