Temporal dynamics of neonatal hypoxic-ischemic encephalopathy injuries on magnetic resonance imaging

Moderate to severe perinatal hypoxic-ischemic encephalopathy occurs in~1 to 3/1000 live births in high-income countries and is associated with a significant risk of death or neurodevelopmental *** assessment is important to help identify highrisk infants,to help families,and to support appropriate interventions.A wide range of monitoring tools is available to assess changes over time,including urine and blood biomarkers,neurological examination,and *** present,magnetic resonance imaging is unique as although it is expensive and not suited to monitoring the early evolution of hypoxic-ischemic encephalopathy by a week of life it can provide direct insight into the anatomical changes in the brain after hypoxic-ischemic encephalopathy and so offers strong prognostic information on the long-term outcome after hypoxic-ischemic *** review investigated the temporal dynamics of neonatal hypoxic-ischemic encephalopathy injuries,with a particular emphasis on exploring the correlation between the prognostic implications of magnetic resonance imaging scans in the first week of life and their relationship to long-term outcome prediction,particularly for infants treated with therapeutic hypothermia.A comprehensive literature search,from 2016 to 2024,identified 20 pertinent *** review highlights that while the optimal timing of magnetic resonance imaging scans is not clear,overall,it suggests that magnetic resonance imaging within the first week of life provides strong prognostic *** challenges limit the timing consistency,particularly the need for intensive care and clinical ***,although most reports examined the prognostic value of scans taken between 4 and 10 days after birth,there is evidence from small numbers of cases that,at times,brain injury may continue to evolve for weeks after *** suggests that in the future it will be important to explore a wider range of times after hypo

Traumatic abdominal wall hernia:a rare and often missed diagnosis in blunt trauma

Dear editor,Traumatic abdominal wall hernia(TAWH)is defined as disruption of the abdominal wall musculature and fascia with herniation of intra-abdominal contents,in the absence of surgery to that area of the abdominal wall.[1]It is rare,with an incidence of 0.17%-0.90%in patients with blunt abdominal trauma.[2,3]Deceleration forces caused by falls from height or seat belt injuries are the most common causes of TAWH.[4]Low-energy blunt injuries from bicycle or motorcycles handlebars or charging animals are less frequent *** to the rare incidence and complex presentation of this condition with other distracting injuries,this diagnosis is often not considered,resulting in missed or delayed *** present a case of a 46-year-old male who suffered major abdominal trauma,and was found to have TAWH as well as intra-abdominal organ injury.

Pericytes are classically defined as contra ctile cells within the central nervous system that regulate blood flow and permeability of the blood-brain barrier(BBB).This one-sided view is gradually changing,and pericytes are now considered versatile cells that can switch their function in response to different stimuli(Uemura et al.,2020).In addition to their role as gatekeepers of the BBB and maintaining homeostasis of the brain’s microenvironment through adj usting the vascular intraluminal dia meter,pericytes are both sensors and initiators of inflammation.

service providers - from public institutions to primary care facilities - need to constantly attend toclients＇ inquiries to provide useful information and directive guidelines. Ensuring high quality serviceis challenging as it not only demands detailed domain-specific knowledge, but also the ability toquickly understand the clients＇ issues through their diverse - and often casual - descriptions. Thispaper aims to provide a framework for the development of an automated information broker agent whoperforms the task of a helper. The main task of the agent is to interact with the client and direct them toobtain further services that cater their personalized need. To do so, the agent should accomplish asequence of tasks that include natural language inquiry, knowledge gathering, reasoning, and givingfeedback; in this way, it simulates a human helper to engage in interaction with the client. Theframework combines a question-answering reasoning mechanism while utilizing domain-specificknowledge base. When the users cannot describe clearly their needs, the system tries to narrow downthe possibilities by an iterative question-answering process, until it eventually identifies the target. Inrealizing our framework, we make a proof-of-concept project, M andy, a primary care chatbot systemcreated to assist healthcare staffs by automating the patient intake process. We describe in detail thesystem functionalities and design of the system, and evaluate our proof-of-concept on benchmark casestudies.

Wahi, a discrete global grid gazetteer built using linked open data

Discrete global grid systems have become an important component of Digital Earth ***,previously there has not existed an easy way to map between named places(toponyms)and the cells of a discrete global grid *** lack of such a tool has limited the opportunities to synthesize social place-based data with the more standard Earth and environmental science data currently being analyzed in Digital Earth *** paper introduces Wāhi,the first gazetteer to map entities from the GeoNames database to multiple discrete global grid systems.A gazetteer service is presented that exposes the grid system and the associated gazetteer data as Linked Data.A set of use cases for the discrete global grid gazetteer is discussed.

Acceptability and Feasibility of YouthCHAT to Detect Psychosocial Problems in Young People with Congenital Heart Disease

Objective:Young people with congenital heart disease(CHD)are at increased risk of psychosocial issues,includ-ing anxiety and depression that can affect their medical care and quality of *** open trial investigated the acceptability,feasibility and preliminary effectiveness of YouthCHAT,a tablet-based screener,designed to identify similar issues to a face to face psychosocial assessment,with young people who have ***:Youth-CHAT was administered to 44 such young people aged 15–25 years attending a specialist outpatient clinic at a new zealand tertiary *** outcomes were(i)acceptability and(ii)feasibility of YouthCHAT,assessed via feedback from young people and clinicians and(iii)detection rates for each domain of YouthCHAT,measured by mental health or lifestyle issue being‘present’or‘absent’.Results:YouthCHAT was quick to complete(mean 9 min and 7 s)and well-received by young people(overall rating 7 out of 10)and *** of anxiety,substance use and other issues were similar to those in a previous study of young people with mixed long-term physical ***,rates of depression(41%)and life stresses(98%)were notably ***:Although further research is required,our results provide initial evidence that YouthCHAT is a viable and accep-table psychosocial screener with which to improve the holistic care of young people with CHD.

‘Planning Emergency Levels of service’(PELOS)are service delivery goals for infrastructure providers during and after an emergency *** goals could be delivered through the existing infrastructure(e.g.,pipes,lines,cables),or through other means(trucked water or the provision of generators).This paper describes how an operationalised framework of PELOS for the Wellington region,new zealand was created,alongside the key *** undertook interviews and workshops with critical infrastructure entities to create the *** this process we found five themes that informed the context and development of the PELOS framework:interdependencies between critical infrastructure,the need to consider the vulnerabilities of some community members,emergency planning considerations,stakeholders’willingness to collaborate on this re-search/project and the flexibility/adaptability of the delivery of infrastructure services following a major *** themes are all explored in this *** research finds that the understanding of the hazardscape and potential outages from hazards is critical and that co-ordination between key stakeholders is essential to create such a *** paper may be used to inform the production of PELOS frameworks in other localities.
