Introduction: The burden of prostatic cancer is rising in Sudan. Usually, they present late in their disease with urinary tract obstruction, hematuria, bony pain, or cachexia because there is no screening program. Here we present a patient with prostatic cancer who presented with left axillary mass as his main concern. Case Description: 82-year-old Sudanese male presented with a left axillary and left supraclavicular lymphadenopathy of a few months’ duration. He underwent a decisional biopsy which showed metastatic adenocarcinoma. Upper and lower GI endoscopy were performed but the latter was complicated by a sigmoid perforation with peritonitis. During laparatomy, multiple enlarged pelvic lymphnodes were encountered and a biopsy result suggested a metastatic prostatic neoplasm. Later, prostatic biopsy confirmed the diagnosis. The patient was treated with bilateral orchidectomy. Clinical discussion: Lymphatic metastasis to axillary lymph nodes is a very rare manifestation of prostate cancer and only a few cases have been reported in the literature. It can cause diagnostic difficulty since prostate cancer typically metastasis to the pelvic lymph node and very rarely involves he supradiaphragmatic lymph node. Conclusion: Metastatic prostatic carcinoma should be considered among the causes of supra-diaphragmatic lymph adenopathy. Careful physical and imaging examinations combined with PSA and pathological analysis are essential in the diagnosis of advanced prostate cancer with unusual presentation.

The objective of this work was to evaluate the spatial and temporal dynamics of the process of combustion of fruits of the Brazil nut tree (Bertholletia excelsa Bonpl.) using thermographic images in order to characterize the potential of this biomass as an energy source for use in ceramic kilns. Infrared thermography was used as a diagnostic tool, and the trial was conducted in November 2018, in two blocks with two repetitions in each, with the fruits organized so that the cavity was oriented upward and also downward. The thermograms were analyzed using Flir Tools (6.3 v). The temperatures were the highest in the center of the fruits (>160°C), attaining incandescence in 6 min 30 s. Heat was propagated from the center to the edge, and this process was quicker for fruits with their cavities pointed upward. A second order polynomial model explained combustion time with a maximum duration of 7.2 minutes. The thermographic diagnostic has the potential to be used in management strategies for this biomass as an energy source, thus aggregating value to the residuals that are normally left in the forest during harvest by extractivists.

Binary optics, where the optical element can be fabricated on a thin glass plate with micro-ion-etching film layer, has been widely applied in recent years. A novel optical scanning system for gene disease diagnostics described in this paper has four kinds of optical devices, including beam splitters, an array lens, an array filter and detection arrays. A software was developed to design the binary optics system using an iterative method. Two beam splitters were designed and fabricated, which can divide a beam into a 9×9 array or into a 13×13 array. The beam splitters have good diffraction efficiencies (>70%) and an even energy distribution. The gene disease diagnostic system is a portable biochip and binary optics technology. The binary optical devices in the non-confocal scanning system can raise the fluorescence detection sensitivity of the micro-array hybrid biochip.

BACKGROUND Colorectal cancer(CRC) is an important disease worldwide, accounting for the second highest number of cancer-related deaths and the third highest number of new cancer cases. The blood test is a simple and minimally invasive diagnostic test. However, there is currently no blood test that can accurately diagnose *** To develop a comprehensive, spontaneous, minimally invasive, label-free, bloodbased CRC screening technique based on Raman *** We used Raman spectra recorded using 184 serum samples obtained from patients undergoing colonoscopies. Patients with malignant tumor histories as well as those with cancers in organs other than the large intestine were excluded. Consequently, the specific diseases of 184 patients were CRC(12), rectal neuroendocrine tumor(2), colorectal adenoma(68), colorectal hyperplastic polyp(18), and others(84). We used the 1064-nm wavelength laser for excitation. The power of the laser was set to 200 *** Use of the recorded Raman spectra as training data allowed the construction of a boosted tree CRC prediction model based on machine learning. Therefore, the generalized R^2 values for CRC, adenomas, hyperplastic polyps, and neuroendocrine tumors were 0.9982, 0.9630, 0.9962, and 0.9986, *** For machine learning using Raman spectral data, a highly accurate CRC prediction model with a high R^2 value was constructed. Objectives.:A high proportion of women with persistent low levels of hCG,in the absence of pregnancy or any evidence of tumor,have received chemotherapy and hysterectomy for assumed malignancy. Such chemotherapy and surgery were ineffective and unwarranted. This study identifies the causes of persistent low level of hCG and provides guidelines for the management of these patients,preventing unnecessary treatment in the future. Methods.:The USA hCG Reference Service has consulted on 170 women with low levels of hCG persisting for 3 months or longer. Serum total hCG was measured in the diagnostic Products corporation (DPC) Immulite assay and hyperglycosylated hCG in the Nichols Advantage test. Results.:Among these 170 patients,the average persistent hCG result was 102 ±152 mIU/ml,with a range of 6.1-900 mIU/ml. Thirteen (7.6%) of the 170 patients had true malignancy,5 had placental site trophoblastic tumor,3 had other gestational trophoblastic neoplasms (GTN),and 5 had non-trophoblastic malignancies. The remaining 157 patients had false-positive hCG,quiescent gestational trophoblastic disease (quiescent GTD),or pituitary hCG(hCG of pituitary origin). Of 71 patients with false- positive hCG,47 patients received chemotherapy and 9 had surgery that had no effect on the level of hCG. Five of these patients with false-positive hCG were being monitored for hydatidiform mole or GTN. The majority of these cases were first investigated following an incidental pregnancy test. Of 69 patients who had quiescent GTD,41 received chemotherapy and 9 underwent hysterectomy. All these therapies were unnecessary and ineffective. While 21 patients with quiescent GTD followed incidental pregnancy tests,the majority were discovered while monitoring patients after treatment for hydatidiform mole or GTN/choriocarcinoma (n = 48).
