BACKGROUND Violence against healthcare workers(HCWs)in the Caribbean continues to prevail yet remains *** aim is to determine the cause,traits,and consequences of violence on HCWs in the *** To determine the cause,traits,and consequences of violence on HCWs in the *** This research adopted an online cross-sectional survey approach,spanning over eight weeks(between June 6th and August 9th,2022).The survey was generated using Research Electronic Data Capture forms and followed a snowballing strategy to contact individuals using emails,social media,text messages,*** regression analysis was performed to evaluate the variables that influence violence,including gender,age,years of experience,institution type,and night shift *** The survey was completed by 225 *** comprised 61%.Over 51%of respondents belonged to the 21 to 35 age ***(n=61),Haiti(n=50),and Grenada(n=31)had the most *** HCWs(49%)worked for government academic institutions,followed by community hospitals(23%).medical students(32%),followed by attending physicians(22%),and others(16%)comprised the most common cadre of *** 39%of the participants reported experiencing violence themselves,and 18%reported violence against colleague(s).Verbal violence(48%),emotional abuse(24%),and physical misconduct(14%)were the most common types of *** 63%of respondents identified patients or their relatives as the most frequent *** logistic regression analyses demonstrated that female gender(OR=2.08;95%CI:1.16-3.76,P=0.014)and higher frequency of night shifts(OR=2.22;95%CI:1.08-4.58,P=0.030)were associated with significantly higher odds of experiencing *** than 50%of HCWs felt less motivated and had decreased job satisfaction post-violent *** A large proportion of HCWS in the Caribbean are exposed to violence,yet the phenomenon remains *** a result,HCWs’job s

Vibration-controlled Transient Elastography for Assessment of Liver Fibrosis at a USA Academic medical center

Background and Aims:Vibration-controlled transient elastography(VCTE)is a noninvasive tool that uses liver stiffness measurement(LSM)to assess *** real-life data during everyday clinical practice in the USA are lacking,we describe the patterns of use and diagnostic performance of VCTE in patients at an academic medical center in new york ***:Patients who received VCTE scans were included if liver biopsy was performed within 1 *** performance of VCTE in differentiating dichotomized fibrosis stages was assessed via area under the receiver operating characteristics(AUROC).Fibrosis stage determined from VCTE LSM was compared to liver ***:Of 109 patients,49 had nonalcoholic fatty liver disease,16 chronic hepatitis C,15 congestive hepatopathy,and 22 at least two *** was 0.90 for differentiating cirrhosis(stage 4)with a positive predictive value(PPV)range of 0.28 to 0.45 and negative predictive value range of 0.96 to *** 31(32%)patients,VCTE fibrosis stage was at least two stages higher than liver biopsy fibrosis *** of thirty-five pa-tients considered to have cirrhosis by VCTE had stage 0 to 2 and 12 stage 3 fibrosis on liver ***:VCTE has reasonable diagnostic accuracy and is reliable at ruling out ***,because of its low PPV,caution must be exercised when used to diagnose cirrhosis,as misdiagnosis can lead to unnecessary health care *** routine practice,VTCE is also sometimes performed for disease etiologies for which it has not been robustly validated.

Since its inauguration in 2003, China Interventional Therapeutics （CIT） congress has witnessed the growth of percutaneous cardiovascular intervention in China. In 2003 the total number ofpercutaneous coronary intervention （PCI） in China was about 40 000 cases,1 while by CIT＇s 10th anniversary in 2012, the annual number of PCI in China had reached 340 000 cases. CIT has grown along with PCI practice in the country; last year the CIT congress hosted 6000 attendees, including 180 international faculty members and over 750 foreign participants from 42 countries or regions.

Background:The effect of transjugular intra-hepatic portosystemic shunt(TIPS)placement on renal function and the correlation of post-TIPS Cr with mortality remain *** study aimed to assess the effect of TIPS placement on renal function and to examine the relationship between post-TIPS Cr and mortality ***:A total of 593 patients who underwent de novo TIPS placement between 2004 and 2017 at a single institution were included in the *** pre-TIPS Cr level(T0;within 7 days before TIPS placement)and post-TIPS Cr levels,at 1–2 days(T1),5–12 days(T2),and 15–40 days(T3),were *** of Cr change after TIPS placement and the 1-year mortality rate were analysed using multivariable linear-regression and Cox proportional-hazards models,***:Overall,21.4%of patients(n=127)had elevated baseline Cr(≤1.5 mg/dL;mean,2.5161.49 mg/dL)and 78.6%(n=466)had normal baseline Cr(<1.5 mg/dL;mean,0.9260.26 mg/dL).Patients with elevated pre-TIPS Cr demonstrated a decrease in post-TIPS Cr(difference,-0.60 mg/dL),whereas patients with normal baseline Cr exhibited no change(difference,<0.01 mg/dL).The 30-day,90-day,and 1-year mortality rates were 13%,20%,and 32%,*** bleeding as a TIPS-placement indication(hazard ratio=1.731;P=0.036),higher T0 Cr(hazard ratio=1.834;P=0.012),and higher T3 Cr(hazard ratio=3.524;P<0.001)were associated with higher 1-year mortality ***:TIPS placement improved renal function in patients with baseline renal dysfunction and the post-TIPS Cr level was a strong predictor of 1-year mortality risk.

Introduction Patients with stroke-like symptoms may be underutilising emergency medical services and avoiding hospitalisation during the COVID-19 *** investigated a decline in admissions for stroke and transient ischaemic attack(TIA)and emergency department(ED)stroke alert *** We retrospectively compiled total weekly hospital admissions for stroke and TIA between 31 December 2018 and 21 April 2019 versus 30 December 2019 and 19 April 2020 at five US tertiary academic comprehensive stroke centres in cities with early COVID-19 outbreaks in Boston,new york City,Providence and *** collected available data on ED stroke alerts,stroke severity using the National Institutes of Health Stroke Scale(NIHSS)and time from symptom onset to hospital *** Compared with 31 December 2018 to 21 April 2019,a decline in stroke/TIA admissions and ED stroke alerts occurred during 30 December 2019 to 19 April 2020(p trend<0.001 for each).The declines coincided with state stay-at home recommendations in late *** greatest decline in hospital admissions was observed between 23 March and 19 April 2020,with a 31%decline compared with the corresponding weeks in *** of the five centres with 2019 and 2020 stroke alert data had a 46%decline in ED stroke alerts in late March and April 2020,compared with *** baseline NIHSS during these 4 weeks was 10 in 2020 and 7 in *** was no difference in time from symptom onset to hospital *** At these five large academic US hospitals,admissions for stroke and TIA declined during the COVID-19 *** was a trend for fewer ED stroke alerts at three of the five centres with available 2019 and 2020 *** stroke therapies are time-sensitive,so decreased healthcare access or utilisation may lead to more disabling or fatal strokes,or more severe non-neurological complications related to *** findings underscore the indirect effects of this *** health offici

Background and Aims:Coronavirus disease 2019(COVID-19)is a global threat,affecting more than 100 million people and causing over 2 million *** laboratory test abnormalities are an extrapulmonary manifestation of COVID-19,yet characterization of hepatic injury is *** objective was to further characterize and identify causes of liver injury in patients with ***:We conducted a retrospective cohort study of 551 patients hospitalized with COVID-19 at newyork-presbyterian Hospital/columbia university irving medical center between March 1,2020 and May 31,*** analyzed patient demographics,liver laboratory test results,vital signs,other relevant test results,and clinical outcomes(mortality and intensive care unit admission).Results:Abnormal liver laboratory tests were common on hospital admission for COVID-19 and the incidence increased during *** those with elevated serum alanine aminotransferase and/or alkaline phosphatase activities on admission,58.2%had a cholestatic injury pattern,35.2%mixed,and 6.6%*** liver disease was not associated with outcome;however,abnormal direct bilirubin or albumin on admission were associated with intensive care unit stay and *** average,patients who died had greater magnitudes of abnormalities in all liver laboratory tests than those who *** hepatitis was a mechanism of severe hepatocellular injury in some ***:Liver laboratory test abnormalities are common in hospitalized patients with COVID-19,and some are associated with increased odds of intensive care unit stay or *** hepatocellular injury is likely attributable to secondary effects such as systemic inflammatory response syndrome,sepsis,and ischemic hepatitis.
