Dendritic cell-mimicking scaffolds for ex vivo T cell expansion

We propose an ex vivo T cell expansion system that mimics natural antigen-presenting cells(APCs)for adoptive cell therapy(ACT).Microfiber scaffolds coated with dendritic cell(DC)membrane replicate physicochemical properties of dendritic cells specific for T cell activation such as rapid recognition by T cells,long duration of T cell tethering,and DC-specific co-stimulatory *** DC membrane-coated scaffold is first surface-immobilized with T cell stimulatory ligands,anti-CD3(αCD3)and anti-CD28(αCD28)antibodies,followed by adsorption of releasable interleukin-2(IL-2).The scaffolds present both surface and soluble cues to T cells ex vivo in the same way that these cues are presented by natural APCs in *** demonstrate that the DC-mimicking scaffold promotes greater polyclonal expansion of primary human T cells as compared toαCD3/αCD28-func-tionalized *** importantly,major histocompatibility complex molecules derived from the DC membrane of the scaffold allow antigen-specific T cell expansion with target cell-specific killing *** addition,most of the expanded T cells(~97%)can be harvested from the scaffold by density gradient ***,the DC-mimicking scaffold offers a scalable,modular,and customizable platform for rapid expansion of highly functional T cells for ACT.

Why Do We Care More About Disease than Health?

Modern Western biomedical research and clinical practice are primarily focused on *** disease-centric approach has yielded an impressive amount of knowledge around what goes wrong in ***,in comparison,researchers and physicians know little about *** is health?How do we quantify it?And how do we improve it?We currently do not have good answers to these *** lack of fundamental knowledge about health is partly driven by three main factors:(i)a lack of understanding of the dynamic processes that cause variations in health/disease states over time,(ii)an excessive focus on genes,and(iii)a pervasive psychological bias towards additive *** I briefly discuss potential reasons why scientists and funders have generally adopted a gene-and disease-centric framework,how medicine has ended up practicing“diseasecare”rather than healthcare,and present cursory evidence that points towards an alternative energetic view of *** the basis of human health with a similar degree of precision that has been deployed towards mapping disease processes could bring us to a point where we can actively support and promote human health across the lifespan,before disease shows up on a scan or in bloodwork.

Quantum dots-labeled polymeric scaffolds for in vivo tracking of degradation and tissue formation

The inevitable gap between in vitro and in vivo degradation rate of biomaterials has been a challenging factor in the optimal designing of scaffold’s degradation to be balanced with new tissue *** enable non-/minimum-invasive tracking of in vivo scaffold degradation,chemical modifications have been applied to label polymers with fluorescent ***,the previous approaches may have limited expandability due to complicated synthesis ***,we introduce a simple and efficient method to fluorescence labeling of polymeric scaffolds via blending with near-infrared(NIR)quantum dots(QDs),semiconductor nanocrystals with superior optical ***-labeled,3D-printed PCL scaffolds showed promising efficiency and reliability in quantitative measurement of degradation using a custom-built fiber-optic imaging ***,QDs-PCL scaffolds showed neither cytotoxicity nor secondary labeling of adjacent ***-PCL scaffolds also supported the engineering of fibrous,cartilaginous,and osteogenic tissues from mesenchymal stem/progenitor cells(MSCs).In addition,QDs-PCL enabled a distinction between newly forming tissue and the remaining mass of scaffolds through multi-channel ***,our findings suggest a simple and efficient QDs-labeling of PCL scaffolds and minimally invasive imaging modality that shows significant potential to enable in vivo tracking of scaffold degradation as well as new tissue formation.

Venous thromboembolism in inflammatory bowel disease

Patients with inflammatory bowel disease(IBD)are at an increased risk for venous thromboembolism(VTE).VTE events carry significant morbidity and mortality,and have been associated with worse outcomes in patients with *** have suggested that the hypercoagulable nature of the disease stems from a complex interplay of systems that include the coagulation cascade,natural coagulation inhibitors,fibrinolytic system,endothelium,immune system,and ***,clinical factors that increase the likelihood of a VTE event among IBD patients include older age(though some studies suggest younger patients have a higher relative risk of VTE,the incidence in this population is much lower as compared to the older IBD patient population),pregnancy,active disease,more extensive disease,hospitalization,the use of certain medications such as corticosteroids or tofacitinb,and IBD-related *** the increased risk of VTE among IBD patients and the safety of pharmacologic prophylaxis,adherence rates among hospitalized IBD patients appear to be ***,recent data suggests that there is a population of high risk IBD patients who may benefit from post-discharge *** review will provide an overview of patient specific factors that affect VTE risk,elucidate reasons for lack of VTE prophylaxis among hospitalized IBD patients,and focus on recent data describing those at highest risk for recurrent VTE post-hospital discharge.

Heart failure (HF), particularly in the setting of preserved systolic function, disproportionately afflicts older individuals and results in significant morbidity, mortality, and health care costs. In the United States, among the Medicare population (age > 65 years), HF is among the leading cause of hospital admissions. Optimal care for older adults with HF requires knowledge of age-related physiologic changes, complex multi-organ, multi-dimensional syndromes, interdisciplinary teamwork and palliative/end of life care.

Secondary causes of inflammatory bowel diseases

Inflammatory bowel diseases(IBD),conventionally consist of Crohn’s disease(CD)and ulcerative *** occur in individuals with high risk genotype for the disease in the setting of appropriate environmental *** pathogenesis of IBD involves a dysregulated autoimmune response to gut dysbiosis,which in turn is triggered due to exposure to various inciting environmental *** there is no clearly defined etiology of IBD and this type of disease is termed as“idiopathic IBD”,“classic IBD”,or“primary IBD”.We reviewed the current medical literature and found that certain etiological factors may be responsible for the development of IBD or IBD-like conditions,and we consider this form of de novo IBD as“secondary IBD”.Currently known factors that are potentially responsible for giving rise to secondary IBD are medications;bowel altering surgeries and transplantation of organs,stem cells or fecal *** associated with the development of secondary IBD include;immunomodulators,anti-tumor necrosis factor alpha agents,anti-interleukin agents,interferons,immune stimulating agents and checkpoint *** can in some cases give rise to de novo CD,pouchitis of the ileal pouch,or postcolectomy enteritis *** solid organ transplantation or hematopoietic stem cell transplantation,the recipient may develop de novo IBD or IBD *** microbiota transplantation has been widely used to treat patients suffering from recurrent Clostridium difficile infection but can also causes IBD flares.

Subtraction hemiarthoplasty in basal joint arthritis: A case report

BACKGROUND Basal joint arthritis is a common disease which is especially present in postmenopausal women. A variety of procedures have been described once conservative treatment has failed. The purpose of our study is to present a novel surgical technique based on a clinical case with three-year follow-up which is highly effective, even after failure of previous procedures. CASE SUMMARY We present a case of a 39 year-old-woman who suffered from recurring, persistent, sharp left thumb pain for several years After two operative procedures, she presented again with intolerable, sharp pain over the trapeziometacarpal joint, and wished to proceed with a pain relieving procedure or arthroplasty. Intraoperatively, the articular surfaces appeared well, without any bare areas of bone or eburnation, resulting in the decision to perform a “subtraction hemiarthroplasty”. A shortening osteotomy of the os trapezium was performed by removing a slice of 8-9 mm bone, without damaging the joint cartilages. The osteotomy was stabilized using two screws, the ligament and capsular tissue was suture imbricated, and a cut down on the translation and a dorsal radial ligament reconstruction were performed. Three years after the final procedure, a long-term follow-up demonstrates excellent results, pain free with full range-of-motion. CONCLUSION Subtraction hemiarthroplasty with ligament tightening imbrication and reconstruction of the radial ligament led to excellent results in motion and pain.

Vibration-controlled Transient Elastography for Assessment of Liver Fibrosis at a USA Academic medical center

Background and Aims:Vibration-controlled transient elastography(VCTE)is a noninvasive tool that uses liver stiffness measurement(LSM)to assess *** real-life data during everyday clinical practice in the USA are lacking,we describe the patterns of use and diagnostic performance of VCTE in patients at an academic medical center in new york ***:Patients who received VCTE scans were included if liver biopsy was performed within 1 *** performance of VCTE in differentiating dichotomized fibrosis stages was assessed via area under the receiver operating characteristics(AUROC).Fibrosis stage determined from VCTE LSM was compared to liver ***:Of 109 patients,49 had nonalcoholic fatty liver disease,16 chronic hepatitis C,15 congestive hepatopathy,and 22 at least two *** was 0.90 for differentiating cirrhosis(stage 4)with a positive predictive value(PPV)range of 0.28 to 0.45 and negative predictive value range of 0.96 to *** 31(32%)patients,VCTE fibrosis stage was at least two stages higher than liver biopsy fibrosis *** of thirty-five pa-tients considered to have cirrhosis by VCTE had stage 0 to 2 and 12 stage 3 fibrosis on liver ***:VCTE has reasonable diagnostic accuracy and is reliable at ruling out ***,because of its low PPV,caution must be exercised when used to diagnose cirrhosis,as misdiagnosis can lead to unnecessary health care *** routine practice,VTCE is also sometimes performed for disease etiologies for which it has not been robustly validated.

Microsurgical epididymal sperm aspiration： indications, techniques and outcomes

Microsurgical epididymal sperm aspiration （MESA） refers to retrieval of sperm-containing fluid from optimal areas of the epididymis that are selected and sampled using high-power optical magnification provided by an operating microscope. Retrieved sperm are subsequently used for intracytoplasmic sperm injection （ICSI） to induce fertilization and pregnancy. MESA is considered by many experts to be the gold standard technique for sperm retrieval in men with obstructive azoospermia given its high yield of quality sperm, excellent reported fertilization and pregnancy rates, and low risk of complications. However, MESA must be performed in an operating room, requires microsurgical skills and is only useful for reproduction using ICSI. Herein we present an overview of the evaluation of candidate patients for MESA, the technical performance of the procedure and the outcomes that have been reported.
