Donor-Site Morbidity Following Harvest of Autologous Costal Cartilage in Microtia Reconstruction

Objectives: The objective of this study is to evaluate donor-site morbidity after costal cartilage harvest for microtia reconstruction. Methods: A total of 70 patients who underwent autologous costal cartilage harvest for microtia reconstruction from March 2008-March 2009 were included. Anterior chest wall deformity was evaluated with chest topography, and scar quality at baseline and at 6-months follow-up, and final outcomes analyzed with SPSS. Results: In 70 patients, 52 (74%) were male, 18 (26%) were female, and altogether 40 (57%) patients developed deformity. At 6-month follow-up, the incidence of anterior chest wall deformity was highest at 80% in Block-III, and least at 0% in Block-I. The 6 - 10 years age group was the largest group at 84% (21), and also with highest incidence of deformity in association to Block-IV harvest at 83%. The incidence of donor-site deformity was higher in female gender at 66%, and 54% in males. But in the sub-group, male had higher incidence of deformity at 75% in both Block-III, and Block-IV when compared to the respective females. The 120 - 135 cm height group had the highest deformity at 67% with Block-IV costal cartilages harvested. At the three measurement points: 1) xiphisternum, 2) intersecting points between PSL and LCM, and 3) intersecting points between MCL and LCM, significant differences (mean) were observed in chest circumference from baseline to 6-month follow-up, and between the left and right chest hemi-circumference (postoperatively). Acceptable donor-site scar was observed in all but 3% (2) developed hypertrophic scar. Conclusion: The development of chest wall deformity was observed when more than one costal cartilage was harvested, particularly the 6th (complete), 7th, 8th block. Therefore, to minimize the deformity, we recommend harvesting only the necessary amount of cartilage, and at the lowest level possible to avoid injury of costochondral junction. A Comparative Study in the Outcome of Primary Asian Blepharoplasty with and without Fat Removal

Background: Asian Blepharoplasty (AB) is one of the most commonly performed aesthetic procedures today. Despite the increasing demand for AB and advancements in surgical techniques, creating an optimal upper eyelid crease remains difficult. This study aimed to determine whether removing the preaponeurotic fat pad (PAF) had any effect on the revision surgery rates following primary Asian blepharoplasty. Methods: A retrospective comparative analysis was conducted on patients who underwent primary AB between January 2016 and December 2020. Patients were divided into PAF removed and PAF not removed groups. The outcomes in the groups were compared by the frequency of revision surgery following primary AB using the chi-square test (p Results: The study included 480 patients, 442 (92%) female and 38 (8%) male, and the final outcome was evaluated after 6 months postoperatively. 412 (86%) patients were observed to have PAF pads intraoperatively, while 68 (14%) had minimal/no PAF pads. In total, 115 (24%) patients underwent revision surgery, of whom 31 (27%) had PAF removed, 66 (57%) had PAF not removed, and 18 (16%) had minimal/no PAF noted. When compared to the PAF not removed group, the PAF removed group showed a significantly lower frequency of revision surgery between the two groups of patients (p = 0.0001). Conclusion: In the final outcome, not removing the PAF in primary AB indicated a strong association with an increase in revision surgery. The removal of PAF in primary AB showed a better overall outcome and patient satisfaction.

Experiences of repairing large defect on adominal wall with artificial mesh and myocutaneous flap

Objective： To evaluate the results of repairing large defect on abdominal wall with artificial meshes（expansible polytetrafluoroethylene, e-PTFE and Composix Mesh）. Methods： Four cases with large defect of abdominal wall caused by abdominal wall tumors or injuries were repaired with artificial meshes and myocutaneous flaps. Results： The cases were followed up 7 months to 2.5 years with no complications such as incisional hernia, bowel adhesion and exposure of the prosthetic materials. Conclusion： The combined use of artificial meshes contained e-PTFE and myocutaneous flaps is a reliable and effective method for repairing large defect on abdominal wall.

Aim:Snakebite injuries of the extremities are common in tropical India among those involved in farming and outdoor *** injuries often complicated by cellulitis,gangrene,regional lymphadenopathy,compartment syndrome,bleeding abnormalities,septicemia,hypotension,and disseminated intravascular coagulation,resulting in significant morbidity and *** purpose of the study is to share our experience of hyperbaric oxygen(HBO)therapy in the management of snakebite ***:All patients who were treated for snakebite injuries in our department between October 2012 and October 2013 were included in the ***:Out of a total 395 patients,174 patients treated with anti-snake venom with a mortality of 17 ***-four out of the 174 patients was in the pediatric age *** of the patients referred to our department,23 presented with cellulitis,7 with compartment syndrome and 17 for the management of soft tissue cover over the *** the 47 patients,30 involved the lower extremity and rest involved the upper *** patients were subjected to HBO therapy as an *** patients required flap cover:cross finger flap(n=2),anterolateral thigh free tissue transfer(n=1),lateral supramalleolar flap(n=1),groin flap(n=1),and dorsal metacarpal artery flap(n=1).There was no need for fasciotomy among the patients who suffered impending compartment ***:HBO therapy may reduce the incidence of fasciotomy and increase the effectiveness of plastic surgical modalities if administered early and may be used as a useful adjunct in the management of snake envenomation injury.

Hundred plus Minimally Access Cardiac Surgery: Our Experience

Background: Minimally invasive procedures lead to less scarring resulting in better cosmetic outcomes. This has resulted in increased patient interest in such procedures and this has motivated surgeons to pursue newer and improved techniques for Minimally invasive cardiac surgery (MICS). Obviously, with the advent of MICS the techniques to achieve it also needed to be changed and upgraded which includes access for cannulation for cardiopulmonary bypass (CPB). Right internal jugular vein percutaneous cannulation, together with the direct surgical cannulation of femoral vessels with minithoracotomy/ministernotomy proves to be a safe and effective tool in patients with body weight of above 20 kg for minimally access cardiac surgery. We use this technique for Atrial septal defect (ASD) closure, aortic valve replacement (AVR), redo Tricuspid valve replacement (TVR) and mitral valve replacement (MVR). Here, we describe our experience with minimally invasive approach using total peripheral cannulation and an anterior mini-thoracotomy (6 cm or less) incision for ASD closure, AVR, TVR and MVR. Methods: The preoperative variables, intraoperative data and postoperative outcomes of patients undergoing minimally invasive ASD closure, AVR, TVR and MVR with total peripheral cannulation were collected and analyzed. Results: Between May 2014 to May 2019 we performed minimally invasive closure of atrial septal defects, AVR, TVR and MVR with total peripheral cannulation in 103 patients. There were 64 females and 39 males Mean age was 25 years (range 8 - 58 years), Spectrum of procedures include ASD closure in 81 patients (78.6%), AVR via minithoracotomy in 13 patients (12.6%) and AVR via ministernotomy in 3 patients (2.9%), redo TVR in 5 (4.8%), MVR in 1 patient (0.97%). Average cardiopulmonary bypass (CPB) time was 46 minutes (range 22 - 78 min) and average aortic cross-clamp time (AoX) 26 min (range 12 - 45 min) in ASD closure group. Facial Tightening Following a Single Acoustic Wave Treatment Using Focused and Radial Acoustic Waves

Background: Acoustic wave has been used for many medical purposes, and although it is widely applied in cosmetic procedures, it has not been conducted for facial rejuvenation and tightening. Acoustic wave has been proven to penetrate deeper than subcutaneous tissue, independent of skin color, and to be beneficial for skin tightening and body contouring. Objective: To evaluate the effectiveness of a single acoustic wave treatment session using unique focused and radial acoustic waves for facial tightening. Participants: Sixty Asian patients were treated between the years 2016 and 2018. The single acoustic wave treatment session consists of focused long planar acoustic pulses thought to have an effect on cells and their metabolism resulting in an increased release of fat, and radial short acoustic pulses thought to have an effect on tissues activating blood and lymphatic flow. Patients did not undergo any form of medical aesthetic treatment during the study. Measurements: Digital photographs and three-dimensional imaging were used to evaluate the results. Results: Objective assessments with digital photographs and superimposed three-dimensional color images showed preferable facial tightening effects and marked volumetric reduction on the lower one-third of the face. These results were obtained without the edematous reactions observed in laser and radiofrequency therapies, and were sustained up to 12 months. All of the patients reported satisfaction with the results and convenience of the procedure. Complications were minor and transitory, consisting of a slight tingly sensation to the teeth during the treatment, and mild erythema resolving within 5 hours. Side effects were not observed. Conclusions: The advantages of the single acoustic wave treatment are the high potency of facial tightening effects, reduction of discomfort and side effects.
