State of the art management of diabetic macular edema

Macular edema following diabetic retinopathy is one of the ocular complications associated with diabetes, and it is the leading cause of visual loss in the active young and middle aged population in developed countries. While all patients with diabetes particularly those with diabetic retinopathy are at increased risk of developing eye complications, early detection and timely intervention may prevent or delay loss of visual acuity. Systemic management of diabetes through combined control of blood sugar, hypertension, and hyperlipidemia has remained the most effective method to prevent diabetic retinopathy and its progression. Development of diabetic retinopathy and related complications require, surgical and medical interventions including photocoagulation, vitrectomy, and intravitral drug injection to preserve vision. Considering recently most popular treatment of diabetic macular edema(DME) including intravitreal anti-vascular endothelial growth factor(VEGF) agents, several issues such as ideal regimen, duration of treatment, combination therapy and long-term safety have remained unanswered yet and deserve further investigations. In this review, all the articles that had investigated such treatment modalities for DME as well as pharmacokinetic, efficacy, safety, dose and frequency of intravitreal pharmacologic agents and also the effect of macular ischemia, initial macular thickness and optical coherence tomographic patterns of DME on the final outcomes of treatment with Intravitreal drugs are reviewed. In summary, literature searches reveal that almost all studies that have been published up to now provide some evidence that support the use of intravitreal anti-VEGF agents for treatment of either nve or persistent DME in short and long term up to two years.

AIM:To compare the visual results of non-topographyguided and topography-guided photorefractive keratectomy(PRK)applying sequential and simultaneous corneal crosslinking(CXL)treatment for ***:Interventional and comparative prospective ***-nine eyes(36 patients)suffering from keratoconus(stages 1 Amsler-Krumeich classification)were divided into four groups:sequential topography-guided photorefractive keratectomy with CXL,simultaneous topography-guided photorefractive keratectomy with CXL,simultaneous nontopography guided photorefractive keratectomy with CXL,and sequential non-topography guided photorefractive keratectomy with *** main outcome measures were pre-and postoperative uncorrected distance visual acuity(UDVA),best corrected distance visual acuity(CDVA),manifest refraction,contrast sensitivity,and ***:All analyzed visual,contrast sensitivity,and refractive parameters showed a significant improvement in the four groups(all P<0.05).A noticeable improvement was seen in keratometry in all the groups,and a remarkable difference was observed between topography-guided groups in comparison to non-topography-guided groups(P<0.05).Interestingly,the improvement in all parameters showed a degree of stability to the end of the ***:The treatment priorities in all four groups are safety,efficacy,and predictability in the correction of the sphero-cylindrical errors in mild and moderate *** significant differences among groups in the recorded objective outcomes were found.

Evidence-based practice with knowledge, attitude and practice of ophthalmic nursing staffs: a cross-sectional study in south China

Background:This study aims to investigate the current status and influencing factors of evidence-based practice(EBP)with knowledge,attitude and practice(KAP)of ophthalmic nursing staffs in south ***:Using a convenient sampling method,we selected 429 ophthalmic nursing staffs from 28 ophthalmology specialist hospitals or general hospitals in south China,and investigated their general information and implemented the evidence-based practice questionnaire(EBPQ).Results:The scores of EBP and KAP of ophthalmic nursing staffs in south China from high to low were as follows:practical attitude(4.85±1.07 points),practical behavior(4.42±1.14 points),practical knowledge and skills(4.30±0.65 points).The single factor analysis results showed that the first graduation degree,technical title,scientific research achievements,whether or not participate in EBP training,the frequency of reading literature at ordinary time,and whether or not participate in EBP project were the influencing factors of EBP level;the multi-factor analysis results showed that EBP attitude,EBP knowledge and skills,whether or not participate in EBP training were independent influencing factors of ***:The ophthalmic nursing staffs in south China have a positive attitude towards EBP,however,their EBP knowledge,skills and behavioral capabilities need to be focused and *** ophthalmic nursing administrators should fully master the factors affecting the implementation of EBP,and take effective intervention measurement to improve the EBP abilities of ophthalmic nursing staffs,and promote the development of EBP in ophthalmology specialty.

Visual performance of four types of diffractive multifocal intraocular lenses and a review of articles

AIM: To compare the clinical outcomes of a variety of multifocal intraocular lenses(MIOLs) in patients diagnosed with presbyopia or cataracts. METHODS: This clinical trial study included 141 patients(282 eyes) with different MIOLs implantation. The Symfony(60 eyes), the Re STOR(100 eyes), the AT LISAtri(60 eyes), and the Pan Optix(62 eyes) intraocular lenses were evaluated in this prospective interventional study. The near, intermediate, and distant visual acuities, contrast sensitivity, and defocus curve were measured as valid criteria. To statistically analyze the results, we used the Statistical Package for Social Science software, the non-parametric Wilcoxon signed-rank t, the one-way analysis of variance and the Tukey’s post-hoc test in our analysis. Moreover, we conducted a detailed literature search on the Pub Med database in English about MIOLs, in total 59 studies were included in this review ***: The four approaches did not show any significant difference in the best-corrected distance visual acuity(P>0.05). The defocus curves at the contrast of 100% showed that trifocal IOLs had better intermediate performance than the bifocal IOL(P<0.05). There were no statistically significant differences between AT LISAtri and Pan Optix lenses for visual acuity at all distances. The eyes with Pan Optix, Symfony, and AT LISAtri IOL showed better contrast sensitivity than those Re STOR at spatial frequencies of 1, 3, and 6 cpd in photopic and mesopic conditions(P<0.001). CONCLUSION: All four groups of the multifocal lenses were satisfying in terms of distance and near vision. Also, the group of trifocal lenses led to satisfactory outcomes in intermediate vision, without degradation in quality of vision.

A pilot study of intraocular lens explantation in 69 eyes in Chinese patients

AIM：To study the effects of intraocular lens（IOL） explantation and demographic ***：Retrospective non-comparative case *** data recorded from patient charts included the following：demographic,preoperative and postoperative characteristics;complications;surgical methods,and changes in visual ***：A total of 69 eyes in 67 Chinese patients who received IOL explants were *** patients＇ mean age at the time of explantation was 46.1 years old [SD 22.5（6-85）],and 37 patients were female（55.2%）.Regarding employment,47.8% were farmers,23.9% were retired,16.4% were students,4.5% were unemployed,3% were workers,and 4.5% were other（including staff members,teachers and officers）.The main reasons for explantation were dislocation/decentration in 41 cases（59.4%） and retinal detachment in 10 cases（14.5%）.The third most prevalent cause was incorrect lens power in 7 eyes（10.1%）.The remaining reasons were endophthalmitis in 6 cases（8.7%）,posterior capsular opacity in 3 eyes（4.3%）,and impacting retinal surgery operation in 2 cases（2.9%）.The main comorbidities were high myopia in 18 eyes（26.1%）,trauma in 8 eyes（11.6%）,retinal detachment in 6 eyes（8.7%）,congenital cataracts in 8 eyes（11.6%）,and Marfan＇s syndrome in 2 eyes（2.9%）.The mean time from implantation to explantation was 4.0y [SD 4.2（0.005-15）].Treatment after explantation included posterior chamber IOL implantation in 44 eyes（63.8%） and aphakia in 25 eyes（36.2%）.After surgery,the best corrected visual ability（BCVA） was improved in 50 cases（72.5%）,including 28 patients（40.6%） in whom visual ability was improved by more than two ***：Dislocation/decentration is the main cause for explantation,and high myopia is a main risk *** chamber IOL implantation remains the most elected treatment after explantation.

Hemophilic articular cartilage damage presents a significant challenge for surgeons,characterized by recurrent intraarticular bleeding,a severe inflammatory microenvironment,and limited self-repair capability of cartilage ***,there is a lack of tissue engineering-based integrated therapies that address both early hemostasis,anti-inflammation,and long-lasting chondrogenesis for hemophilic articular cartilage ***,we developed an adhesive hydrogel using oxidized chondroitin sulfate and gelatin,loaded with exosomes derived from bone marrow stem cells(BMSCs)(Hydrogel-Exos).This hydrogel demonstrated favorable injectability,self-healing,biocompatibility,biodegradability,swelling,frictional and mechanical properties,providing a comprehensive approach to treating hemophilic articular cartilage *** adhesive hydrogel,featuring dynamic Schiff base bonds and hydrogen bonds,exhibited excellent wet tissue adhesiveness and hemostatic *** a pig model,the hydrogel could be smoothly injected into the knee joint cartilage defect site and gelled in situ under fluid-irrigated arthroscopic *** in vitro and in vivo experiments confirmed that the sustained release of exosomes yielded anti-inflammatory effects by modulating macrophage M2 polarization through the NF-κB *** immunoregulatory effect,coupled with the extracellular matrix components provided by the adhesive hydrogel,enhanced chondrogenesis,promoted the cartilage repair and joint function restoration after hemophilic articular cartilage *** conclusion,our results highlight the significant application potential of Hydrogel-Exos for early hemostasis,immunoregulation,and long-term chondrogenesis in hemophilic patients with cartilage *** innovative approach is well-suited for application during arthroscopic procedures,offering a promising solution for addressing the complex challenges associated with hemophilic articular cartilage damage.

目的:评估可植入式Phakic角膜接触镜治疗稳定型圆锥角膜患者的疗效、安全性、稳定性和可预测性。方法:共14例患者采用植入式Phakic角膜接触镜(IPCL)矫正屈光不正,测量了未矫正视力、最佳矫正视力、离焦曲线、对比敏感度、屈光度及可能的副作用。评估结果超过6mo。结果:平均等效球镜度(SE)和散光在术后6mo末次检查时由术前-6.94±2.79 DS和-4.24±1.42 DC分别变为术后-0.23±0.43 DS和-1.05±0.49 DC。术前平均Snellen视力为0.18±0.10。6mo内未矫正视力和最佳矫正视力平均值分别为0.13±0.10和0.05±0.15。平均安全指数为1.11。所有眼视力均无降低,其中22眼视力提高超过1行。20眼(71.4%)屈光度在0.50 D以内,27眼(96.42%)在±1.00 D以内。术后1wk至6mo,屈光度变化为-0.23±0.43(范围:-1.00至+0.75)。6mo内角膜内皮细胞(ECL)丢失率小于5%。术后6mo眼压(IOP)为11.32±2.28 mmHg。结论:Toric植入式Phakic角膜接触镜在矫正与稳定圆锥角膜相关的近视和近视散光方面具有有效性、安全性和可预测性。

屈光性角膜切削术(PRK),激光原位角膜磨镶术(LASIK)和小切口微透镜取出术(SMILE)是目前临床上屈光性手术中有效和安全的方法。这三种激光视力矫正方法虽然技术不同,但成功率大致相同。至于哪种方法是最合适的取决于多种因素,包括:屈光度、个人习惯和职业因素(如运动和生活方式)。目前,LASIK屈光手术应用最广,但SMILE也在手术医生间快速推广。后者提高了术后早期的舒适感,还有视力的快速恢复,减少了伤口的愈合反应。此外,与LASIK相比,SMILE术后干眼的发生率较低,因为小切口使前基质损伤小,这意味着前部角膜神经受到影响小。与LASIK和PRK相比,SMILE同样可以降低高阶像差(HOA)的形成。术后角膜滞后量(CH)和角膜阻力因子(CRF)与SMILE、LASIK、PRK方法无明显差异。但是,PRK仍然是中度到轻度手术量的最佳选择,特别是涉及薄角膜、频繁角膜侵蚀的病例。

目的:评估活动性葡萄膜炎对屈光不正变化的影响。方法:这是一项前瞻性队列研究,纳入66例88眼活动性葡萄膜炎患者。受试者根据其疾病活动的解剖位置分为前、中、后和全葡萄膜炎。在活动期和静止期均行睫状肌麻痹验光,并对结果进行比较。结果:活动性前非肉芽肿性葡萄膜炎的球镜变化导致平均-0.25(0.77)D(95%CI:-0.5~0)的近视转变(P=0.039),在活动性中层葡萄膜炎中,平均+0.39(0.8)D(95%CI:0.09~0.69)的远视转变(P=0.003)。活动性中层葡萄膜炎的等效球镜的变化导致远视转变[+0.43(0.94)D,95%CI:0.08~0.79,P=0.005]。柱镜仅在全色素膜炎时有明显改变[-0.3(0.39)D,95%CI:-0.58^-0.02,P=0.043]。在多元分析中,我们发现疾病活动过程(急性与复发)和疾病活动的解剖位置对等效球镜变化具有统计学意义(分别为P=0.003和P=0.004)结论:活动性葡萄膜炎对屈光度有重要影响,该变化取决于具体的炎症发生部位。
