Early Morbidity and Mortality in Neonates of Mothers with Sickle Cell Disease at the Borgou/Alibori Center Departmental Teaching Hospital in Benin

Introduction: Association of sickle cell disease and pregnancy is a risky situation for both the mother and neonate. Objective: To determine the early morbidity and mortality among neonates of mothers with sickle cell disease at Borgou/Alibori Center Departmental Teaching Hospital (CHUD B/A) in Benin. Patients and Methods: This was a descriptive and analytical observational study conducted at CHUD-B/A from January 1, 2015, to August 31, 2019. It included pregnant women with sickle cell disease who gave birth to a liveborn neonate at the term of at least 28 weeks of amenorrhea. Variables studied were sociodemographic, clinical, and evolutionary. Analysis of the factors associated with early death was also carried out with a significance threshold set at p Results: Out of a total of 119 pregnant women, 95 neonates were recorded. Main morbidities were: prematurity and intrauterine growth restriction (49.5%);respiratory distress (40%), bacterial infection (30.5%), and perinatal asphyxia (21.1%). Early mortality rate was 8.4%. In bivariate analysis, the factors associated with early death were: parity (p < 0.001), the severity of maternal anemia (p < 0.008), birth weight under 1500 g (p < 0.002), the birth term under 32 weeks of amenorrhea (p < 0.001), resuscitation for at least 5 minutes (p = 0.001). In multivariate analysis, resuscitation for at least 5 minutes (p = 0.007) was mainly associated with early death. Conclusion: One out of two neonates of mothers with sickle cell disease has a low birth weight. Early mortality is high due to perinatal asphyxia. Hence the multidisciplinary care of these mothers.

Prevalence and Factors Associated with Overweight and Obesity among public Secondary school Teachers in Parakou, Benin, in 2021

Introduction: Overweight and obesity represent a public health problem in Africa due to the epidemiological transition. The objectives of this work were to determine the prevalence of overweight and obesity and to identify associated factors among public secondary school teachers in Parakou, Benin in 2021. Methods: We conducted a descriptive and analytical cross-sectional study. Teachers working in public secondary schools in Parakou during the 2020-2021 academic year, present at work and who gave their written informed consent, were included. A two-stage random sampling was carried out. Data were collected during an individual interview using a questionnaire followed by the measurement of anthropometric parameters and blood pressure. Overweight and obesity were defined by a body mass index ≥ 25 kg/m2. Multivariable logistic regression was performed to identify associated factors. Results: A sample of 325 teachers, including 88.6% of men, was recruited with an average age of 36.2 ± 6.8 years. The prevalence of overweight and obesity was 43.7% (95% CI [38.1%-44.8%]). It increased significantly with income (p Conclusion: The results show a high prevalence of overweight and obesity. Actions are necessary to prevent overweight and obesity among secondary school teachers in Parakou, in Benin.

Introduction: Work environments create the basis for arduous nursing work. This is what led us to research the factors of arduousness in the exercise of the health profession in North Benin, through this study. Methods: This is a cross-sectional and descriptive study which focused on health professionals in the health zones of Parakou-N’dali and Tchaourou. The sampling was an exhaustive census. All health workers who had given their free and informed consent were included. The data was collected using a questionnaire. The variables studied were the factors of arduous work and socio-professional characteristics. The data collected was processed and analyzed with the Epi info 7.***.0.1 software. Results: A total of 692 were surveyed, the participation rate was 85.11% and the sex ratio (M/F) equal to 0.7. The average age was 38 years old. Nurses made up 32.66% of the sample. They were 86.42% subject to night work and 82.66% to alternating work. Overall, 70.23% worked more than 40 hours per week. Between 78.12% and 96.46% of non-specialist doctors, nurses, midwives and biomedical analysis technicians were required to work night shifts. Nurses, midwives, radiology and laboratory technicians were between 89.53% and 97.35%, organized in alternating work. In the sample, 55.20% complained about the insufficient number of work materials, 26.29% handled harmful chemical substances. Among those surveyed, 58.14% had been attacked. Conclusion: All professional categories of caregivers are subject to arduous work. Measures are needed to reverse the situation.

Introduction: Work accidents (WA) have consequences for both the individual and the company. The objective of this study is to study the profile of work accidents declared to the Parakou agency of the national Social Security Fund (CNSS) from 2015 to 2021. Method: This was a descriptive cross-sectional study with retrospective data collection. It concerned workers affiliated with the CNSS in Parakou, victims of declared WA whose file was available and exploitable. The sampling was exhaustive. Data was collected from registers and worker records. The variables studied were socio-demographic and related to the accident. They were analyzed using R version 4.1 software. Results: The annual incidence rate of WA varied from 5.8% in 2015 to 1.7% in 2021. The victims were 90.6% male. There were more subjects aged 28 to 37 years (37.1%). Workers and laborers were the most concerned. Fractures (22.9%) were the major lesions. At 38.5% they sat in the thoracic limbs and at 22.5% in the pelvic limbs. The majority (63.6%) of these accidents occurred in the workplace. The processing industry sector was mainly (52.1%) the place of accidents. Conclusion: The prevalence of WA declared to the agency of the national Social Security Fund of Parakou from 2015 to 2021 is decreasing. The most affected were young people. It is important to assess their socio-economic impacts.

Epidemiological Profile of Stress among health Professionals in Northern Benin in 2021

Objective: To study the epidemiological profile of stress among health professionals in North Benin in 2021. Methods: It was a cross-sectional and descriptive study with health professionals in the Parakou-N’dali and Tchaourou health zones. These two areas have a population of 462,942 inhabitants served by two university hospitals, three area hospitals and 19 health centers. Sampling was exhaustive by free and informed consent. The data was collected through a questionnaire. Studied socio-professional characteristics and stress. The level of stress was assessed using the Karasek questionnaire. Results: Of 813 caregivers approached, 692 were investigated. The participation rate was 85.1%. The average age was 38 years old. Subjects aged 28 - 38 were more numerous, as were those aged between 1 and 10 years. The prevalence of stress was 73.8%. Its level was high at 17.8% and very high at 4.5%. Women were affected (76.6%) than men (70.7%). Within the professional category, midwives were the most affected (82.5%). Respondents from the denominational sector were 90.8% reached. Conclusion: Professional stress is endemic in health facilities in northern Benin, particularly faith-based ones. Action must be taken to reduce it.

Prevalence of Hypertension during Pregnancy in Benin: Results from STEPS Survey in 2015

Introduction: Hypertension disorders of pregnancy are one of the leading causes of maternal and fetal deaths. We aimed to estimate the prevalence of hypertension among pregnant women having participated in the STEPS survey in Benin in 2015. Methods: This was a database analysis. STEPS survey included consenting adults aged 18 to 69 years, living in Benin for at least 6 months prior to the survey. The participants were selected in households by a three-stage random sampling technique. The World health Organization (WHO) STEPS instrument was used for data collection. After an individual interview with the participant at home, blood pressure was measured according to WHO recommendations. Pregnant women database was extracted for analysis. A multivariable logistic regression was done for associated factors identification. Results: A total of 2800 women (out of 5127 people) participated in the STEPS study of which 240 declared to be pregnant. The mean age was 28.9 ± 6.8 years. The prevalence of hypertension was 13.3%. Severe hypertension represented 2.9%. Hypertension during pregnancy was associated with primary school education level (compared to none, adjusted OR = 0.3, 95% CI [0.1 - 0.9], p = 0.042). No significant association was found with behavioral factors. Conclusion: The prevalence of hypertension during pregnancy is high in Benin. In-depth studies and targeted interventions for prevention should be implemented.

Introduction: Undernutrition is a public health problem in the world and especially in developing countries. According to the demographic and health survey with multiple indicators of Benin IV (DHS-MICS-IV), the prevalence of acute undernutrition (AU), chronic undernutrition (CU) and that of underweight (UW) was 16%, 45% and 21%, respectively. The objective of this work was to determine the prevalence of undernutrition in children aged 6 to 59 months in the municipality of Toucountouna in 2017. Materials and Methods: This was a descriptive cross-sectional study with two-stage cluster random sampling, composed of 390 children aged 6 to 59 months, living in the municipality of Toucountouna for at least six months. The study variables were: socio-demographic, economic, behavioral, socio-sanitary and anthropometric. Data were entered and analyzed using Epi-info 7.2 software. Results: 203 out of 390 children included, were boys (sex ratio of 1.08). The mean age of the children was 28.70 ± 11.79 months. The prevalence of AU, CU and UW was 10.26%, 31.54% and 11.79%, respectively. Conclusion: The results of this study showed that efforts remain to be made in terms of undernutrition and other surveys could identify the determinants linked to this situation in the locality for the effective implementation of prevention.

Lower Limb Arteriopathy in Diabetics at the Oueme-Plateau Provincial Hospital in Porto-Novo: Frequency and Associated Factors

Introduction: The distribution of arteriopathy of the lower limbs (ALL) is not familiar in the city of Porto-Novo. The aim of this study was to determine the prevalence and associated factors of ALL in diabetic patients in treatment at the Oueme-Plateau Provincial Teaching Hospital. Methods: This was a cross-sectional, descriptive, and analytical study from July 12 to October 10, 2021, at the Oueme-Plateau Provincial Teaching Hospital in Porto-Novo. ALL was screened by performing systolic pressure index (SPI) in all diabetic patients aged at least 18 years, seen in consultation or hospitalization during the study period and who had given their consent. Results: The study registered 165 diabetics and 98.2% of them were type 2. Subjects aged between 55 and 65 years were the most represented (33.9%), with an average age of 59.4 years. Women were in the majority (61.2%), with a sex ratio of 0.6. High blood pressure was associated with diabetes in 61.2% of patients. The prevalence of ALL was 48.5% (80 patients). Obliterative arteriopathy of the lower limbs (OALL) was observed in 70 patients (42.4%), mediacalcosis in 8 patients (4.8%), and mixed arteriopathy in 2 patients (1.2%). In 88.5% of patients, the pathology was symptomless. Factors significantly associated with ALL included advanced age (p = 0.01), the age of the glycemic imbalance (p = 0.03), lack of regular physical activity (p = 0.001) and dyslipidemia (p = 0.008). Conclusion: Pelvic limb arteriopathy is usual among diabetics at Oueme-Plateau Provincial Teaching Hospital. The use of SPI, which enables diagnosis, requires simple, low-cost means, and should be encouraged.
