Rapid Visible Improvements in Onychomycosis: An Open-Label Clinical Investigation of Kerasal Nail<sup>TM</sup>

Traditional treatments for onychomycosis, a fungal disease primarily affecting the toenails, are associated with poor tolerability or inadequate efficacy. Moreover, because many patients find onychomycosis embarrassing, achieving rapid cosmetic improvements is desirable. Kerasal NailTM is a marketed topical treatment containing propylene glycol, urea and lactic acid. The ability of Kerasal Nail to improve the appearance of an affected nail was investigated in a 4-week open-label study. Patients aged ≥21 years and with 25% to 75% of a big toenail or thumbnail (the “target” nail) affected by clinically suspected onychomycosis (n = 65) were instructed to apply Kerasal Nail to all their affected nails once daily for 4 weeks. After 1, 2 and 4 weeks of treatment, they assessed the appearance of the target nail on a 4-point scale, with a score of ≥2 indicating a minimum of some improvement. They also assessed the nail for improvements in thickening, discoloration, brittleness and softness. Adverse events (AEs) occurring between administration of the first dose of Kerasal Nail and the end of the 4-week treatment period were recorded. 85% of patients (95% confidence interval [CI] 74 to 92) achieved some improvement in the target nail at 4 weeks. Improvement of the target nail was reported by 65% of patients (95% CI 52 to 76) after 1 week of treatment and by 82% of patients (95% CI 70 to 90) after 2 weeks. 55% of patients achieved some improvement in discoloration in the first week of treatment. After 4 weeks of treatment, discoloration, thickening, brittleness and softness were each improved in more than half of patients. Treatment-related AEs were limited to two cases of administration site pain. Overall treatment compliance was 99%. In summary, Kerasal Nail produced very rapid improvements in nail appearance in patients with onychomycosis and was well tolerated.

Effect of Heating Rates on the Formable Oxide Scale on a C-Steel Surface

Oxide scale formation on a C-steel surface has been investigated using linear heating rates ranging from 0.1℃/min to 10℃/min at high temperatures. The studies on the oxide scale formation at high temperature (650℃) at slower heating rate (0.1℃/min) shows that the kinetic regime is linear. X-ray diffraction measurements revealed that the scale constituents are significantly influenced by the heating rate. The adherence of the scale was improved by using slower heating rate (0.1℃/min-≤650℃), while above such degree the scale was susceptible to cracking and flaking out of the alloy surface. In fact, the development of oxide growth stresses can cause considerable scale cracking. As well, variation of the crystallite sizes under the aforementioned conditions might affect the scale stacking to the alloy surface. The secondary electron detector images of the oxide scale shows that the scale was imperfectly smooth and there were a number of voids and defects in the scale skin, especially at fast heating rate. This observation could be attributed to defects of the as-received alloy. In general, slower heating rate reduced the defects of the scale and improved its adherence.

Counter-regulatory phosphatases TNAP and NPP1 temporally regulate tooth root cementogenesis

Cementum is critical for anchoring the insertion of periodontal ligament fibers to the tooth root. Several aspects of cementogenesis remain unclear, including differences between acellular cementum and cellular cementum, and between cementum and bone. Biomineralization is regulated by the ratio of inorganic phosphate （Pi） to mineral inhibitor pyrophosphate （PPi）, where local Pi and PPi concentrations are controlled by phosphatases including tissue-nonspecific alkaline phosphatase （TNAP） and ectonucleotide pyrophosphatase/phosphodiesterase 1 （NPP1）. The focus of this study was to define the roles of these phosphatases in cementogenesis. TNAP was associated with earliest cementoblasts near forming acellular and cellular cementum. With loss of TNAP in the Alpl null mouse, acellular cementum was inhibited, while cellular cementum production increased, albeit as hypomineralized cementoid. In contrast, NPP1 was detected in cementoblasts after acellular cementum formation, and at low levels around cellular cementum. Loss of NPP1 in the Enppl null mouse increased acellular cementum, with little effect on cellular cementum. Developmental patterns were recapitulated in a mouse model for acellular cementum regeneration, with early TNAP expression and later NPP1 expression. In vitro, cementoblasts expressed Alpl gene/protein early, whereas Enppl gene/protein expression was significantly induced only under mineralization conditions. These patterns were confirmed in human teeth, including widespread TNAP, and NPP1 restricted to cementoblasts lining acellular cementum. These studies suggest that early TNAP expression creates a low PPi environment promoting acellular cementum initiation, while later NPP1 expression increases PPi, restricting acellular cementum apposition. Alterations in PPi have little effect on cellular cementum formation, though matrix mineralization is affected.

Psoriasis is a common,chronic,and inflammatory skin disease with a high burden on individuals,health systems,and society *** the immunological pathologies and pathogenesis of psoriasis becoming gradually revealed,the therapeutic approaches for this disease have gained revolutionary ***,the mechanisms of less common forms of psoriasis remain ***,severe adverse effects and the recurrence of disease upon treatment cessation should be noted and addressed during the treatment,which,however,has been rarely explored with the integration of preliminary ***,it is crucial to have a comprehensive understanding of the mechanisms behind psoriasis pathogenesis,which might offer new insights for research and lead to more substantive progress in therapeutic approaches and expand clinical options for psoriasis *** this review,we looked to briefly introduce the epidemiology,clinical subtypes,pathophysiology,and comorbidities of psoriasis and systematically discuss the signaling pathways involving extracellular cytokines and intracellular transmission,as well as the cross-talk between *** the discussion,we also paid more attention to the potential metabolic and epigenetic mechanisms of psoriasis and the molecular mechanistic cascades related to its *** review also outlined current treatment for psoriasis,especially targeted therapies and novel therapeutic strategies,as well as the potential mechanism of disease recurrence.

Bullous eruption as a cutaneous manifestation of eosinophilic granulomatosis with polyangiitis

Cutaneous manifestations are variable in patients with eosinophilic granulomatosis with polyangiitis(EGPA).We report a case where bullous eruption was a presenting manifestation of EGPA.A 55-year-old Chinese man with a background of adult-onset asthma and chronic sinusitis was admitted for a 3-week history of lower limb weakness and a large bulla on his right calf with surrounding *** biopsy showed a subepidermal blister with significant dermal eosinophilia,without *** patient was diagnosed with EGPA with systemic involvement on the basis of a thorough medical history,eosinophilia,cardiac magnetic resonance imaging showing acute myocarditis,and nerve biopsy with eosinophilic vasculitic *** responded well to corticosteroids and *** bullous eruptions are rarely described as manifestations of *** recognition of this entity is crucial for its early diagnosis.

There are now numerous emerging flexible and wearable sensing technologies that can perform a myriad of physical and physiological *** advances in developing and implementing such sensors in the last several years have demonstrated the growing significance and potential utility of this unique class of sensing *** include wearable consumer electronics,soft robotics,medical prosthetics,electronic skin,and health *** this review,we provide a state-ofthe-art overview of the emerging flexible and wearable sensing platforms for healthcare and biomedical *** first introduce the selection of flexible and stretchable materials and the fabrication of sensors based on these *** then compare the different solid-state and liquid-state physical sensing platforms and examine the mechanical deformation-based working mechanisms of these *** also highlight some of the exciting applications of flexible and wearable physical sensors in emerging healthcare and biomedical applications,in particular for artificial electronic skins,physiological health monitoring and assessment,and therapeutic and drug ***,we conclude this review by offering some insight into the challenges and opportunities facing this field.

Background Fever of unknown origin(FUO)continues to challenge clinicians to determine an etiology and the need for *** study explored the most common etiologies,characteristics,and average cost of hospitalization for FUO in a pediatric population at an urban,tertiary care hospital in Washington,*** Records from patients admitted to Children's national Health System between September 2008 and April 2014 with an admission ICD-9 code for fever(780.6)were *** charts of patients 2-18 years of age with no underlying diagnosis and a temperature greater than 38.3℃for 7 days or more at time of hospitalization were *** diagnoses,features of admission,and total hospital charges were *** 110 patients qualified for this *** majority of patients(n=42,38.2%)were discharged without a *** was followed closely by infection,accounting for 37.2%(n=41)of *** disease was next(n=16,14.5%),followed by miscellaneous(n=6,5.4%)and oncologic diagnoses(n=5,4.5%).The average cost of hospitalization was 40,295 US *** This study aligns with some of the most recent publications which report undiagnosed cases as the most common outcome in patients hospitalized with *** that,often no diagnosis is found may reassure patients,families,and *** cost associated with hospitalization for FUO may cause clinicians to reconsider inpatient admission for diagnostic work-up of fever,particularly given the evidence demonstrating that many patients are discharged without a diagnosis.

A Ten-year Retrospective Study of Invasive Candidiasis in a Tertiary Hospital in Beijing

Objective This study aimed to evaluate the epidemiological,clinical and mycological characteristics of invasive candidiasis(IC)in *** A ten-year retrospective study including 183 IC episodes was conducted in a tertiary hospital in Beijing,*** The overall incidence of IC from 2010–2019 was 0.261 episodes per 1,000 ***(71.0%)was the major infective pattern;70.3%of the patients tested positive for Candida *** before IC and the median time to develop an invasive infection after colonization was13.5 days(interquartile range:4.5–37.0 days).Candida albicans(45.8%)was the most prevalent species,followed by Candida parapsilosis(19.5%),Candida glabrata(14.2%)and Candida tropicalis(13.7%).***-albicans IC was more common in patients with severe anemia(P=0.018),long-term hospitalization(P=0.015),hematologic malignancies(P=0.002),continuous administration of broad-spectrum antibiotics(P<0.001)and mechanical ventilation(P=0.012).In vitro resistance testing showed that11.0%of the Candida isolates were resistant/non-wild type(non-WT)to fluconazole,followed by voriconazole(9.6%),micafungin(3.8%),and caspofungin(2.9%).Fluconazole was the most commonly used drug to initiate antifungal therapy both before and after the proven diagnosis(52.6%and 54.6%,respectively).The 30-day and 90-day all-cause mortality rates were 24.5%and 32.7%,*** The incidence of IC has declined in the recent five ***-albicans contributed to more than half of the IC *** can be used as first-line therapy if resistant strains are not ***,multi-center surveillance of the clinical and mycological characteristics of IC is required.

Evidence-based consensus on the diagnosis, prevention and management of hepatitis C virus disease

Hepatitis C virus(HCV) is a potent human pathogen and is one of the main causes of chronic hepatitis round the world. The present review describes the evidencebased consensus on the diagnosis, prevention and management of HCV disease. Various techniques, for the detection of anti-HCV immunoglobulin G immunoassays, detection of HCV RNA by identifying virus-specific molecules nucleic acid testings, recognition of core antigen for diagnosis of HCV, quantitative antigenassay, have been used to detect HCV RNA and core antigen. Advanced technologies such as nanoparticlebased diagnostic assays, loop-mediated isothermal amplification and aptamers and Ortho trak-C assay have also come to the front that provides best detection results with greater ease and specificity for detection of HCV. It is of immense importance to prevent this infection especially among the sexual partners, injecting drug users, mother-to-infant transmission of HCV, household contact, healthcare workers and people who get tattoos and piercing on their skin. Management of this infection is intended to eradicate it out of the body of patients. Management includes examining the treatment(efficacy and protection), assessment of hepatic condition before commencing therapy, controlling the parameters upon which dual and triple therapies work, monitoring the body after treatment and adjusting the co-factors. Examining the treatment in some special groups of people(HIV/HCV co-infected, hemodialysis patients, renal transplanted patients).
