AIM:To study the angle between the circular smooth muscle(CSM) and longitudinal smooth muscle(LSM) fibers in the distal ***:In order to identify possible mechanisms for greater shortening in the distal compared to proximal esophagus during peristalsis,the angles between the LSM and CSM layers were measured in 9 *** outer longitudinal layer of the muscularis propria was exposed after stripping the outer *** inner circular layer of the muscularis propria was then revealed after dissection of the esophageal mucosa and the underlying muscularis *** of each specimen were taken with half of the open esophagus folded back showing both the outer longitudinal and inner circular muscle *** were measured every one cm for 10 cm proximal to the squamocolumnar junction(SCJ) by two independent *** human esophagi were obtained from organ transplant donors and the angles between the circular and longitudinal smooth muscle layers were measured using micro-computed tomography(micro CT) and Image J ***:All data are presented as mean ± *** CSM to LSM angle at the SCJ and 1 cm proximal to SCJ on the autopsy specimens was 69.3 ± 4.62 degrees vs 74.9 ± 3.09 degrees,P = *** CSM to LSM angle at SCJ were statistically significantly lower than at 2,3,4 and 5 cm proximal to the SCJ,69.3 ± 4.62 degrees vs 82.58 ± 1.34 degrees,84.04 ± 1.64 degrees,84.87 ± 1.04 degrees and 83.72 ± 1.42 degrees,P = 0.013,P = 0.008,P = 0.004,P = 0.009 *** CSM to LSM angle at SCJ was also statistically significantly lower than the angles at 6,7 and 8 cm proximal to the SCJ,69.3 ± 4.62 degrees vs 80.18 ± 2.09 degrees,81.81 ± 1.75 degrees and 80.96 ± 2.04 degrees,P = 0.05,P = 0.02,P = 0.03 *** CSM to LSM angle at 1 cm proximal to SCJ was statistically significantly lower than at 3,4 and 5 cm proximal to the SCJ,74.94 ± 3.09 degrees vs 84.04 ± 1.64 degrees,84.87 ± 1.04 degrees and 83.72 ± 1.42 degrees,P =

In the context of historical Palestine, the Palestinian and jewish demography are deeply embedded （Goldsheider, 1991; Freidlander, 2002）. The growth and structure of the populations are one of the most important tools for the control of the territory （Dahlan, 1989）. jewish immigration was and is still considered as the single most important mean for the conquest of the land （Courbage, 2005）. Yet this is only a partial truth （Fargues, 2000）. Fertility, especially when it is natural at its peak or still very high is also a very significant asset, which is likely to reverse internal equilibrium among the different forces shaping the two societies （Courbage ＆ Todd, 2011; Ettinger, 2011）. Hence, Jews and Palestinians have embarked since decades into a demographic competition （Faraoune, personal communication, December 19, 1992; Anson ＆ Avinoam, 1996）, with this extraordinary result, that in a matter of a century （1948-2048） this small piece of land will have undergone the highest population growth on earth, equivalent to sub Saharan Africa （Faitelson, 2008; Courbage ＆ Fargues, 1997）. It is likely output is not less important to define the future nature of historical Palestine： one or two states （Baskin, 2005; Courbage, 2008）？ And if the two states solution is in effect, what kind of states would it be？ After presenting the recent trends of Palestinian demography in the context of historical Palestine since the first intifada and the aftermath of the second intifada, this essay will focus on future developments, i.e. the likely population dynamics of the Palestinian territory from 2011 until 2048, and of its different components, namely fertility. Palestinian population growth during the next four decades will be compared to that of the future Israeli population and the policy components of this differential population growth will be evaluated both in the context of historical Palestine and in the context of the jewish State in its 1948 borders （Jews and Palestinians of 1

Introduction: As the U.S. population undergoes continuous shifts the population’s health profile changes dynamically resulting in more or less expression of certain psychiatric disorders and utilization of health-care resources. In this paper, we analyze national data on the psychiatric morbidity of American patients and their summated cost in different age groups. Methods: The latest data (2009) on the number of hospital discharges and national bill (hospital charges) linked with psychiatric disorders were extracted from the Nationwide Inpatient Sample (NIS). Results: national data shows that mood disorders are the largest diagnostic category in terms of percentage of psychiatri-crelated discharges in the 1 - 17 years age group. The proportion decreases gradually as age progresses while delirium, dementia, amnestic and other cognitive disorders increase exponentially after 65 years of age. Schizophrenia and other psychotic disorders as well as alcohol and substance-related disorders peak in the working age groups (18 - 64 years). From an economic point of view, mood disorders in the 18 - 44 age group has the highest national bill ($5.477 billion) followed by schizophrenic and other psychotic disorders in the same age group ($4.337 billion) and mood disorders in the 45 - 64 age group ($4.310 billion). On the third place come schizophrenic and other psychotic disorders in the 45 - 64 age group ($3.931 billion). Conclusion: This paper illustrates the high cost of psychiatric care in the U.S., especially the large fraction of healthcare money spent on working-age patients suffering from mood disorders. This underlines psychiatric cost-efficiency as a vital topic in the current healthcare debate.

Severe acute respiratory syndrome coronavirus-2(SARS-CoV-2) has become one major threat to human population *** RNA-dependent RNA polymerase(RdRp) presents an ideal target of antivirals,whereas nucleoside analogs inhibitor is hindered by the proofreading activity of ***,we report that corilagin(RAI-S-37) as a non-nucleoside inhibitor of SARS-CoV-2 RdRp,binds directly to RdRp,effectively inhibits the polymerase activity in both cell-free and cell-based assays,fully resists the proofreading activity and potently inhibits SARS-CoV-2 infection with a low 50% effective concentration(EC50) value of 0.13 μmol/*** modeling predicts that RAI-S-37 lands at the palm domain of RdRp and prevents conformational changes required for nucleotide incorporation by *** addition,combination of RAI-S-37 with remdesivir exhibits additive activity against antiSARS-CoV-2 *** with the current data available on the safety and pharmacokinetics of corilagin as a medicinal herbal agent,these results demonstrate the potential of being developed into one of the much-needed SARS-CoV-2 therapeutics.

目的:评估左氧氟沙星、加替沙星及莫西沙星等新氟喹诺酮类药物对肺结核患者的早期杀菌活性及延长早期杀菌活性。设计:随机开放临床试验。40例成年新诊断的涂阳肺结核患者(每组10例)分别每日单一接受异烟肼300mg,或左氧氟沙星1000mg,或加替沙星400mg,或莫西沙星400mg的7天治疗。治疗前2天及单药治疗7天期间,每日收集痰液进行定量培养。计量单药治疗开始的头2天(0-2天)及治疗后5天(2-7天)的菌量下降情况,以评估各药的早期及延长早期杀菌活性。对实验室工作人员关于病人治疗分组信息采用单盲法。结果:异烟肼的早期杀菌活性为0.67log10cfu/毫升/天,高于莫西沙星(0.33log10cfu/毫升/天)及加替沙星(0.35log10cfu/毫升/天)。与每日1000mg左氧氟沙星的早期杀菌活性(0.45log10cfu/毫升/天)相比,无统计学差异(P=0.14)。3种氟喹诺酮类药物的早期杀菌活性(0-2天)与延长早期杀菌活性(2-7天)相似。总体说来,氟喹诺酮类药物的延长早期杀菌活性高于异烟肼。结论:莫西沙星、加替沙星及高剂量左氧氟沙星,虽然略低于异烟肼,但均有良好的早期杀菌活性,并具有较高的延长早期杀菌活性。有理由对这些药物敏感的结核病人进行进一步的治疗研究。
