Analysis of the Screening of Contact Subjects of People Living with HIV at the Antivenereal Health Center of the national institute of public hygiene of Treichville, abidjan/Côte d’Ivoire from January 2009 to December 2018

Introduction: In C?te d’Ivoire, people living with HIV are estimated to 460,000 with about 290,000 who know their HIV status. Index testing is a strategy for screening unidentified HIV carriers. This study aimed at analyzing the screening of contact subjects of people living with HIV at the anti-venereal health center of Treichville in abidjan. Methods: We carried out a cross-sectional study from January 2009 to December 2018. The target population was index patients and their sexual partners. After giving their consent, index patients and their sexual partners were interviewed during their medical visit. Data were analyzed using Epi info 3.5.4. The khi2 test was performed at 5% significance level. Results: A total of 114 index patients have been included in the study. The majority of sexual partners were men (sex ratio M/F = 1.5) with an average age of 43 years (±9.77). 87.7% of index patients lived in pairs. The analysis showed that 87.7% of index patients had disclosed their HIV status to their sexual partners and 94.3% of index patients invited their sexual partners to be tested. 85.8% of sexual partners had been tested. 19.8% of sexual partners who have been tested were HIV positive. All the 8 children of female index patients were HIV negative. HIV test acceptability was statistically associated with living in pairs (P Conclusion: Screening sexual partners from index patients has been an effective way to detect HIV carriers who did not know their status.

The Dimbokro health district has one of the highest incidences of malaria in Côte d’Ivoire, despite numerous campaigns to distribute Long-Acting Impregnated Mosquito Nets (LLINs). Given this observation, what are the population’s attitudes towards LLINs that could explain the high endemicity of malaria in Dimbokro? The aim of this study is to assess the knowledge, attitudes and practices of people in the health area of Nofou, one of the villages in the Dimbokro health district where malaria is most prevalent, with a view to strengthening malaria control strategies. A collection of historical health data (malaria cases and LLIN usage rates) and a cross-sectional questionnaire survey was carried out from July 20 to August 03, 2022 in 400 households selected by systematic random sampling. Data were entered using Epi Info 7 and multivariate statistical analyses were performed using IBM SPSS 22 software. In the Dimbokro health district, the rate of LLIN use was negatively correlated (r = -0.771) with malaria incidence over the six years of historical data collection. Households had an average of 53% good knowledge, 68.6% good practices and 28.4% good attitudes towards LLINs. The study also revealed that the main factors explaining LLIN use in the Nofou health area are mosquito bite (ORa = 5.29 (1.12 - 25.04)), age of household members (ORa = 1.07 (1.01 - 1.14)) and marital status (ORa = 8.45 (1.70 - 45.02)). Awareness-raising on the use of LLINs should be intensified in the Dimbokro health district. Other control strategies, such as vector control and environmental sanitation, should be considered to combat malaria.

Introduction: Work-aggravated asthma is pre-existing or concomitant asthma whose symptoms are aggravated by the work environment. The aim of this study was to determine the prevalence of this pathology and its associated factors among bakery workers in abidjan. Materials and Methods: This descriptive and analytical cross-sectional study was conducted over a period of four (4) months from 18 December 2019 to 18 April 2020. Two questionnaires were used, one on employees and the other on the indoor environment of bakeries. In addition, a spirometry test was performed on all bakery workers. Statistical analysis was performed using stata 15.1 software. Results: A total of 599 bakery employees, including bakers (59.73%), sales assistants/ cashiers (23.52%), cleaners (6.34%) and administrative staff (10.18%), were investigated. The mean age was 30.8 ± 8 years, with a sex ratio (M/F) = 2.2. Asthma symptoms were found in 95 (15.86%) employees, of whom 74 (77.9%) had work-related asthma and 11 (14.9%) had asthma aggravated by work. The factors associated with work-aggravated asthma were personal or family history of allergy or atopy [ORa = 3.75;CI95%: 1.56 - 8.93;p = 0.003], exposure to dust [ORa = 5.01;CI95%: 1.43 - 7.50;p = 0.011] and humidity level (60% - 70%) [ORa = 1.80;CI 95%: 0.99 - 3.28;p = 0.05]. Conclusion: Work-aggravated asthma is a reality in bakeries in abidjan, with an estimated prevalence of 14.9%. Two of the three factors associated with this condition suggest a link with indoor air pollution. Combating air pollution in these establishments must therefore be a priority for the relevant authorities, in order to provide employees with a working environment that protects their health.

Introduction: Human rabies is a major public health problem in many African countries, including Côte d’Ivoire. The present work aims at describing the eco-epidemiological characteristics of human rabies cases recorded in Côte d’Ivoire. Methods: This was a retrospective and prospective cross-sectional descriptive study based on epidemiological surveillance data that ran from september 2014 to december 2017. The clinical data were extracted from the database of the human rabies epidemiological surveillance management service of the national institute of public hygiene’s rabies centre. These data cover the period from 1 January 2012 to 31 December 2017. Results: Human rabies is endemic in Côte d’Ivoire, with most people exposed in the south and especially in the west of the country. There was no association between any of the climatological parameters (temperature, humidity, rainfall) and the occurrence of human rabies during the months of exposure to rabies infection. Conclusion: Ultimately, effective prevention and control of human rabies requires a thorough understanding of the links between climatological parameters and rabies. Health authorities must take ownership of these results if we are to achieve our goal of eliminating rabies by 2030.

Fructose aggravating colon barrier dysfunction by decreasing gut bacteria metabolites indole-3-carboxaldehyde and inhibiting activation of aryl hydrocarbon receptor in vivo and in vitro

Background: Fructose may induce non-alcoholic fatty acids (NAFLD) due to the gut-liver axis interactions. The mechanism of fructose impairing colon barrier is unrevealed. Methods: Normal and DSS-induced Sprague-Dawley rats fed by 35% fructose diets were used to evaluate colon barrier functions. Microbiome and metabolome were applied to screen potential biomarker bacteria and metabolites induced by fructose. HT-29 cells were applied to validate metabolite biomarker indoleacrylic acid (IAA) and indole-3-carboxaldehyde (I3A) function in colon barrier which impaired by fructose. Results: Fructose induced colon barrier dysfunction， aggravated colon impairment in DSS-induced rats. With fructose intake， the colon length shortened， goblet numbers declined， inflammation infiltration induced， inflammatory cytokines increased， and apoptosis signals upregulated in colon tissue. Moreover， fructose induced dysbiosis of microbiota and their metabolites. Adlercreutzia， Holdemania were screened out as potential bacteria biomarkers， IAA and I3A as tryptophan metabolites were selected as metabolite biomarkers inhibited by fructose. IAA and I3A treatment alleviated the impairment induced by fructose by increasing trans epithelial electric resistance (TEER) value， tight junction proteins (TJPs)， and AhR activity in HT-29 cell. Conclusion: Fructose stimulated inflammation， apoptosis， gut bacteria alteration， and induced the reduction of IAA and I3A. Since fructose inhibited production of IAA and I3A， AhR remained inactivated and consequently induced colon barrier dysfunction.

Costs of Schizophrenia at Psychiatric Hospital of Bingerville (Ivory Coast)

Schizophrenia is classified as a priority mental disorder by the World Health Organization (WHO) and accounts for around 35% of diagnoses at the Bingerville Psychiatric Hospital (HPB). The aims of the study were to identify the cost drivers for hospitalization and to calculate the costs of managing schizophrenia in hospital, with a view to planning household expenditure on care. This pilot cross-sectional study involved 31 patients with schizophrenia who had been hospitalized in the various third-category wards at the HPB between 1st January 2019 and 31st May 2020. Sampling was accidental. The methods used to estimate costs were based on the actual costs of drugs, hospitalization and additional examinations which prices were known, and on patients’ estimations for certain expenses such as food and transport. Results: The sex ratio was 3.42, the mean age was 29.52 years. The mean length of stay was 46.19 days, and the most frequent clinical forms were paranoid schizophrenia (41.9%) and schizoaffective disorder (29%). The combination of haloperidol and chlorpromazine was the most common medications for initial treatment (67.8%) and maintenance treatment (41.9%). The average cost of hospitalization at HPB for schizophrenia was XOF 164,412 (€249.90). The average direct medical cost was XOF 105,412 (€160.226) and the average direct non-medical cost was XOF 59,000 (€89.68). The average daily cost of antipsychotic treatment was XOF 795/day (€1.2084). The high cost of drugs as a proportion of hospitalization costs suggested the need of a reflection on the simplification of prescribing practices, assistance in psychiatric emergencies and the development of other alternatives to psychiatric hospitalization in Côte d’Ivoire.

In Africa, the prevalence of diabetes is escalating and remains a concern due to the numerous complications it causes. Vascular damage associated with diabetes leads to a prothrombotic state observed in diabetic individuals. Diabetes is a complex and multifactorial disease involving genetic components. With the aim of preventing complications and contributing to an efficient management of diabetes, we investigated genes likely to lead to a risk of thrombosis, in particular the C677T of MTHFR, G20210A of prothrombin, and R506Q of factor V Leiden in type 2 diabetics in abidjan receiving ambulatory care. A descriptive cross-sectional study was carried out on consenting type 2 diabetic patients. Mutation detection was carried out using the PCR-RFLP method employing restriction enzymes. Hemostasis tests (fibrinogen, D-dimers, fibrin monomers, and von Willebrand factor) were performed using citrate tubes on the Stage? Star Max automated system. Plasminogen activator inhibitor was assayed by ELISA method, and biochemical parameters were determined using the COBAS C311. The study population consisted of 45 diabetic patients, 51.1% of whom presented vascular complications, mainly neuropathy. Disturbances in hemostasis parameters were observed, with 15.5% of patients showing an increase in fibrin monomers. Mutation analysis revealed an absence of factor V mutation (factor V Leiden) and of G20210A mutation of the prothrombin gene. However, 15.6% of subjects had a heterozygous C677T mutation of MTHFR, with 57% of them being anemic. The exploration of biological and genetic factors associated with thrombotic risk is of significant interest in the optimal management of African type 2 diabetics.

Palmitoleic acid on top of HFD ameliorates insulin resistance independent of diacylglycerols and alters gut microbiota in C57BL/6J mice
