To the Editor:Phosphaturic mesenchymal tumor(PMT)is a rare,slowly progressive,histomorphologically distinct entity of tumors that can emerge anywhere in the body.[1]According to World Health organization,PMT has two pathological types:benign and malignant.[2]PMT was discovered due to tumor-induced osteomalacia(TIO)and is increasingly accepted in recent years as the leading cause of TIO.[1]However,most published reports have focused on the clinicopathological,immunohistochemical,and molecular genetic studies on PMT rather than the radiological work-ups.[3,4]Hence,the aim of this study was to find the common imaging characteristics of PMT to assist clinical diagnosis.

To describe the characteristics of callers accessing the first national smoking quitline in China's Mainland. The national quitline provides residents in China's Mainland with free telephone smoking cessation services. Of 8260 callers from December 1, 2009 to May 31, 2012, the study included 1049 callers to the non-automated service who gave basic information during the call. Of the 1049 live-access callers.

The Prognostic Significance of the Preoperative Platelet-Lymphocyte Ratio in Japanese Patients with Localized Breast Cancer

Inflammation has been shown to be a factor in tumorigenesis. The circulating platelet to lymphocyte ratio (PLR) is a representative index of systemic inflammation. In this study, we analyzed the association between preoperative PLR levels and clinicopathological variables in two hundred sixty-four Japanese patients with localized breast carcinoma. We also evaluated the prognostic significance of preoperative PLR levels using the Cox proportional hazard model. Seventy-five patients (28.4%) had elevated PLR values, whereas 189 (71.6%) had depressed PLR values. The PLR correlated significantly with venous invasion (P < 0.05). Disease-free survival rates were significantly worse among patients with elevated PLR values than among those with lower PLR value (5-year survival, 81.6% vs. 90.7%, respectively;P < 0.05). In multivariate analysis, elevated PLR, nuclear grade, and lymph node involvement were significant prognostic factors for disease-free survival in patients with breast carcinoma (P < 0.05). Preoperative PLR levels may be an independent prognostic factor in patients with breast carcinoma.

Prolapsed tongue papilloma cured by administration of the traditional Japanese(Kampo) herbal medicine, Keishibukuryogan-ka-yokuinin:A case report

Squamous papilloma is a benign mass lesion of the oral mucosa. For papillomas of the tongue, surgery is recommended owing to their malignant potential;however, certain complications may be associated with surgery. A traditional Japanese(Kampo) herbal medicine, Keishibukuryogan-ka-yokuinin(KBGY),has been used to treat viral warts and various skin diseases in Japan. Therefore, the effect of KBGY on papillomas is promising. A 49-year-old Japanese man presented with a wart on his tongue that was about3 months old. He smoked 5 cigarettes per day. He did not drink alcohol. He had no history of malignant illnesses. He was taking alprazolam for panic disorder. The patient was diagnosed with a suspected papilloma of the tongue at the Department of Otolaryngology and was advised to undergo an excision biopsy to exclude malignancy. However, he refused owing to the fear of an invasive procedure. After informed consent was obtained from the patient, KBGY was prescribed. Three months later, the wart on his tongue spontaneously prolapsed. The histopathological diagnosis was squamous papilloma. There was no indication of malignancy, and the patient discontinued Kampo treatment. He has had no recurrence in the past 3 years. KBGY is a combination of Keishibukuryogan and yokuinin(adlay seeds).Keishibukuryogan may be beneficial for skin or oral mucosal remodeling, and yokuinin may have antiviral properties. The present case report suggests the use of KBGY as an appropriate complementary therapy for squamous papilloma.

Reconsideration of So-Called “Invasive” Meningioma

For the purpose of histopathological diagnosis, so-called “invasive meningioma” is defined as a meningioma that has infiltrated the cerebral parenchyma, and such cortical infiltration raises the risk of recurrence. However, as the definition of “invasive” remains far from clear, we have attempted to redefine it. Tumors that were completely removed from 32 patients between April 2005 and September 2015 and that had been diagnosed as so-called “invasive meningioma” were classified by World Health organization grade (Group I–III), and then further classified as: true invasion, involving invasion of the brain with breakdown of the pia mater;and pseudo-invasion, involving encroachment into the brain with the pia mater still intact. We then investigated recurrence rates in the brain or dura mater for each group. Rate of recurrence in the brain or dura mater was significantly higher for Group I meningioma classified as true invasion compared to that of the same group classified as pseudo-invasion. We redefined so-called “invasive meningioma” as Group I meningioma exhibiting true invasion, and considered that, when possible, wide resection of the areas of adhesion to the dura mater in addition to tumor extirpation along with the cerebral parenchyma increased the success rate for curative treatment.

Effect of Post-ERCP Pancreatitis on Perioperative Factors of Pancreaticoduodenectomy

Background:?Endoscopic retrograde cholangiopancreatography (ERCP) is commonly performed for biliary drainage and examination of the biliary and pancreatic duct before pancreaticoduodenectomy (PD). The influence of preoperative post-ERCP pancreatitis on PD is unknown. The aim of this study was to evaluate the relation between post-ERCP pancreatitis before PD and surgical outcomes of PD. Methods: We examined 38 patients who underwent PD. The relations between post-ERCP pancreatitis before PD and perioperative factors of PD such as patient characteristics, operative findings, and postoperative course were evaluated with univariate and multivariate analyses. Results: Post-ERCP pancreatitis was observed in 12 (37.5%) of the 38 patients. Univariate analyses showed operative procedure (P?= 0.034), operation time (P?= 0.004), blood loss (P?= 0.031), C-reactive protein (P?= 0.043), and delayed gastric emptying (P?= 0.035) to be significantly associated with post-ERCP pancreatitis. Multivariate analyses showed operation time (OR, 1.017;?95%CI, 1.000 - 1.034;?P?= 0.049) and delayed gastric emptying (OR, 18.72;95%CI, 1.139 - 307.6;?P?= 0.040) to be significantly associated with post-ERCP pancreatitis. Conclusions: Post-ERCP pancreatitis was associated with prolonged operation time and delayed gastric emptying in patients undergoing PD.

AIM To determine whether the presence of columnar-lined esophagus(CLE) is associated with the presence of esophageal varices(EVs) in male Japanese alcoholics. METHODS The subjects were 1614 Japanese alcohol-dependent men(≥ 40 years of age) who had undergone upper gastrointestinal endoscopic screening. Digitalized records of high-quality endoscopic images that included the squamocolumnar junction and esophagogastric junction were retrospectively jointly reviewed by four expert endoscopists for the purpose of diagnosing CLE. The authors investigated whether and to what extent there were associations between the presence of CLE and the presence of EVs, especially in the group with liver cirrhosis(LC).RESULTS CLE ≥ 5 mm in length was found in 355 subjects(≥ 30 mm in 6 of them), LC without EVs in 152 subjects, LC with EVs in 174 subjects, and EVs without LC in 6 subjects. Advanced EVs, i.e., nodular, large or coiled forms, red color sign, or post-treatment, were found in 88 subjects. The incidence of CLE ≥ 5 mm decreased in the following order(P < 0.0001): 23.3% in the group without EVs, 17.4% in the group with small and straight EVs, and 5.7% in the group with advanced EVs. The multivariate ORs(95%CI) for EVs and advanced EVs in the group with LC were lower when CLE ≥ 5 mm was present [0.46(0.23-0.93) and 0.24(0.08-0.74), respectively, vs 0-4 mm CLE]. CONCLUSION The presence of CLE in male Japanese alcoholics was negatively associated with the presence of EVs.
